Introduction

Robotic-assisted total knee arthroplasty (TKA) has demonstrated significant benefits, including improved accuracy of component positioning compared to conventional jig-based TKA. However, previous studies have often failed to associate these findings with clinically significant improvements in patient-reported outcome measures (PROMs). Inertial measurement units (IMUs) provide a more nuanced assessment of a patient's functional recovery after TKA. This study aims to compare outcomes of patients undergoing robotic-assisted and conventional TKA in the early postoperative period using conventional PROMS and wearable sensors.

Method

100 patients with symptomatic end-stage knee osteoarthritis undergoing primary TKA were included in this study (44 robotic-assisted TKA and 56 conventional TKA). Functional outcomes were assessed using ankle-worn IMUs and PROMs. IMU- based outcomes included impact load, impact asymmetry, maximum knee flexion angle, and bone stimulus. PROMs, including Oxford Knee Score (OKS), EuroQol-Five Dimension (EQ-5D-5L), EuroQol Visual Analogue Scale (EQ-VAS), and Forgotten Joint Score (FJS-12) were evaluated at preoperative baseline, weeks 2 to 6 postoperatively, and at 3-month postoperative follow-up.

Introduction

Recent technological advancements have led to the introduction of robotic-assisted total knee arthroplasty to improve the accuracy and precision of bony resections and implant position. However, the in vivo accuracy is not widely reported. The primary objective of this study is to determine the accuracy and precision of a cut block positioning robotic arm.

Method

Seventy-seven patients underwent total knee arthroplasty with various workflows and alignment targets by three arthroplasty-trained surgeons with previous experience using the ROSA® Knee System. Accuracy and precision were determined by measuring the difference between various workflow time points, including the final pre-operative plan, validated resection angle, and post-operative radiographs. The mean difference between the measurements determined accuracy, and the standard deviation represented precision.

Re-rupture rates after rotator cuff repair remain high because of inadequate biological healing at the tendon-bone interface. Single-growth factor therapies to augment healing at the enthesis have so far yielded inconsistent results. An emerging approach is to combine multiple growth factors over a spatiotemporal distribution that mimics normal healing. We propose a novel combination treatment of insulin-like growth factor 1 (IGF-1), transforming growth factor β1 (TGF-β1) and parathyroid hormone (PTH) incorporated into a controlled-release tyraminated poly-vinyl-alcohol hydrogel to improve healing after rotator cuff repair. We aimed to evaluate this growth factor treatment in a rat chronic rotator cuff tear model.

A total of 30 male Sprague-Dawley rats underwent unilateral supraspinatus tenotomy. Delayed rotator cuff repairs were then performed after 3 weeks, to allow tendon degeneration that resembles the human clinical scenario. Animals were randomly assigned to: [1] a control group with repair alone; or [2] a treatment group in which the hydrogel was applied at the repair site. All animals were euthanized 12 weeks after rotator cuff surgery and the explanted shoulders were analyzed for biomechanical strength and histological quality of healing at the repair site.

In the treatment group had significantly higher stress at failure (73% improvement, P=0.003) and Young's modulus (56% improvement, P=0.028) compared to the control group. Histological assessment revealed improved healing with significantly higher overall histological scores (10.1 of 15 vs 6.55 of 15, P=0.032), and lower inflammation and vascularity.

This novel combination growth factor treatment improved the quality of healing and strength of the repaired enthesis in a chronic rotator cuff tear model. Further optimization and tailoring of the growth factors hydrogel is required prior to consideration for clinical use in the treatment of rotator cuff tears. This novel treatment approach holds promise for improving biological healing of this clinically challenging problem.

Obesity is associated with poor outcomes and increased risk of failure after rotator cuff (RC) repair surgery. The effect of diet-induced obesity (DIO) on enthesis healing has not been well characterised and whether its effects can be reversed with dietary intervention is unknown. We hypothesised that DIO would result in inferior enthesis healing in a rat model of RC repair and that dietary intervention in the peri-operative period would improve enthesis healing.

A total of 78 male Sprague-Dawley rats were divided into three weight-matched groups from weaning and fed either: control diet (CD), high-fat diet (HFD), or HFD until surgery, then CD thereafter (HF-CD). After 12 weeks the left supraspinatus tendon was detached, followed by immediate surgical repair. At 2 and 12 weeks post-surgery, animals were cullers and RCs harvested for biomechanical and histological evaluation. Body composition and metabolic markers were assessed via DEXA and plasma analyses, respectively.

DIO was established in the HFD and HF-CD groups prior to surgery, and subsequently reversed in the HF-CD group after surgery. At 12 weeks post-surgery, plasma leptin concentrations were higher in the HFD group compared to the CD group (5.28 vs. 2.91ng/ml, P=0.003). Histologically, the appearance of the repaired entheses was poorer in both the HFD and HF-CD compared to the CD group at 12 weeks (overall histological score 6.20 (P=0.008), 4.98 (P=0.001) and 8.68 out of 15, respectively). The repaired entheses in the HF-CD group had significantly lower (26.4 N, P=0.028) load-at-failure 12 weeks post-surgery compared to the CD group (34.4 N); while the HFD group was low, but not significantly different (28.1 N, P=0.096). Body mass at the time of surgery, plasma leptin and body fat percentage were negatively correlated with histological scores and plasma leptin with load-at-failure 12 weeks post-surgery.

DIO impaired enthesis healing in this rat RC repair model, with inferior biomechanical and histological outcomes. Restoring normal weight with dietary change after surgery did not improve healing outcomes. Exploring interventions that improve the metabolic state of obese patients and counselling patients appropriately about their modest expectations after repair should be considered.

The introduction of robotics for total knee arthroplasty (TKA) into the operating theatre is often associated with a learning curve and is potentially associated with additional complications. The purpose of this study was to determine the learning curve of robotic-assisted (RA) TKA within a multi-surgeon team.

This prospective cohort study included 83 consecutive conventional jig-based TKAs compared with 53 RA TKAs using the Robotic Surgical Assistant (ROSA) system (Zimmer Biomet, Warsaw, Indiana, USA) for knee osteoarthritis performed by three high-volume (> 100 TKA per year) orthopaedic surgeons. Baseline characteristics including age, BMI, sex and pre-operative Kellgren-Lawrence grade were well-matched between the conventional and RA TKA groups. Cumulative summation (CUSUM) analysis was used to assess learning curves for operative times for each surgeon. Peri-operative and delayed complications were reviewed.

The CUSUM analysis for operative time demonstrated an inflexion point after 5, 6 and 15 cases for each of the three surgeons, or 8.7 cases on average. There were no significant differences (p = 0.53) in operative times between the RA TKA learning (before inflexion point) and proficiency (after inflexion point) phases. Similarly, the operative times of the RA TKA group did not differ significantly (p = 0.92) from the conventional TKA group. There was no discernible learning curve for the accuracy of component planning using the RA TKA system. The average length of post-operative follow-up was 21.3 ± 9.0 months. There was no significant difference (p > 0.99) in post-operative complication rates between the groups.

The introduction of the RA TKA system was associated with a learning curve for operative time of 8.7 cases. Operative times between the RA TKA and conventional TKA group were similar. The short learning curve implies this RA TKA system can be adopted relatively quickly into a surgical team with minimal risks to patients.

The optimal timing of when to perform manipulation under anesthesia (MUA) for stiffness following total knee arthroplasty (TKA) is unclear. This study aimed to identify the risk factors for MUA following primary TKA and whether performing an “early” MUA within 3 months results in a greater improvement in range of motion.

Primary TKAs performed between January 2013 and December 2018 at three tertiary New Zealand hospitals were reviewed. International Classification of Diseases discharge coding was used to identify patients who underwent an MUA. Multivariate Cox regression was performed to identify patient and surgical risk factors for MUA. Pre- and post-MUA knee flexion angles were identified through manual review of operation notes. Multivariate linear regression was performed to compare the mean flexion angles pre- and post-MUA, as well as the mean gain in flexion, between patients undergoing “early” (<3 months) versus “late” MUA (>3 months).

7386 primary TKAs were analyzed in which 131 underwent subsequent MUA (1.8%). Patients aged <65 years were two times more likely to undergo MUA compared to patients aged ≥65 years (2.5% versus 1.3%, adjusted hazard ratio = 2.1, p<0.001). Gender, body mass index, patient comorbidities or a history of cancer were not associated with the risk of MUA. There was no difference in the final post-MUA flexion angle between patients who underwent early versus late MUA (104.7 versus 104.1 degrees, p = 0.819). However, patients who underwent early MUA had poorer pre-MUA flexion (72.3 versus 79.6 degrees, p = 0.012), and subsequently had a greater overall gain in flexion compared to patients who underwent late MUA (mean gain 33.1 versus 24.3 degrees, p<0.001).

Younger age was the only patient risk factor for MUA. A greater overall gain in flexion was achieved in patients who underwent early MUA within 3 months.

Traumatic rotator cuff injuries can be a leading cause of prolonged shoulder pain and disability, and contribute to significant morbidity and healthcare costs. Previous studies have shown evidence of socio-demographic disparities with these injuries. The purpose of this nationwide study was to better understand these disparities based on ethnicity, sex, and socio-economic status, in order to inform future healthcare strategies.

Accident Compensation Corporation (ACC) is a no-fault comprehensive compensation scheme encompassing all of Aotearoa/New Zealand (population in 2018, 4.7 million). Using the ACC database, traumatic rotator cuff injuries were identified between January 2010 and December 2018. Injuries were categorized by sex, ethnicity, age and socioeconomic deprivation index of the claimant.

During the 9-year study period, there were 351,554 claims accepted for traumatic rotator cuff injury, which totalled over $960 million New Zealand Dollars. The greatest proportion of costs was spent on vocational support (49.8%), then surgery (26.3%), rehabilitation (13.1%), radiology (8.1%), general practitioner (1.6%) and “Other” (1.1%). Asian, Māori (Indigenous New Zealanders), and Pacific peoples were under-represented in the age-standardized proportion of total claims and had lower rates of surgery than Europeans. Māori had higher proportion of costs spent on vocational support and lower proportions spent on radiology, rehabilitation and surgery than Europeans. Males had higher number and costs of claims and were more likely to have surgery than females. There were considerably fewer claims from areas of high socio-economic deprivation.

This large nation-wide study demonstrates the important and growing economic burden of rotator cuff injuries. Indirect costs, such as vocational supports, are a major contributor to the cost suggesting improving treatment and rehabilitation protocols would have the greatest economic impact. This study has also identified socio-demographic disparities which need to be addressed in order to achieve equity in health outcomes.

Unicompartmental knee arthroplasty (UKA) is effective for patients with isolated compartment osteoarthritis, however the procedure has higher revision rates. Long-term survivorship and accurate characterisation of revision reasons are limited by a lack of long-term data and standardised revision definitions. We aimed to identify survivorship, risk factors and revision reasons in a large UKA cohort with up to 20 years follow-up.

Patient, implant and revision details were recorded through clinical and radiological review for 2,137 consecutive patients undergoing primary medial UKA across Auckland, Canterbury, Counties Manukau and Waitematā DHB between 2000 and 2017. Revision reasons were determined from review of clinical, laboratory, and radiological records for each patient using a standardised protocol. To ensure complete follow-up data was cross-referenced with the New Zealand Joint Registry to identify patients undergoing subsequent revision outside the hospitals. Implant survival, revision risk and revision reasons were analysed using Cox proportional-hazards and competing risk analyses.

Implant survivorship at 15 years was comparable for cemented fixed-bearing (cemFB; 91%) and uncemented mobile-bearing (uncemMB; 91%), but lower for cemented mobile-bearing (cemMB; 80%) implants. There was higher incidence of aseptic loosening with cemented implants (3–4% vs. 0.4% uncemented, p<0.01), osteoarthritis (OA) progression with cemMB implants (9% vs. 3% cemFB/uncemMB; p<0.05) and bearing dislocations with uncemMB implants (3% vs. 2% cemMB, p=0.02). Compared with the oldest patients (≥75 years), there was a nearly two-fold increase in risk for those aged 55–64 (hazard ratio 1.9; confidence interval 1.1-3.3, p=0.03). No association was found with gender, BMI or ASA.

Cemented mobile-bearing implants and younger age were linked to lower implant survivorship. These were associated with disease progression and bearing dislocations. The use of cemented fixed-bearing and uncemented mobile-bearing designs have superior comparable long-term survivorship.

The repaired entheses in the HF-CD group had a significantly lower load to failure (P=0.03) at 12 weeks after surgery compared with the CD group, while the load to failure in the HFD group was low but not significantly different (P=0.10). Plasma leptin were negatively correlated with histology scores and load to failure at 12 weeks after surgery.

DIO impaired enthesis healing in this rat RC repair model, with inferior biomechanical and histological outcomes. Restoring normal weight with dietary change after surgery did not improve healing outcomes. Circulating levels of leptin significantly correlated with poor healing outcomes. This pre-clinical rodent model demonstrates that obesity is a potentially modifiable factor that impairs RC healing and increases the risk of failure after RC surgery.

Patient-reported outcome measures (PROMs) have failed to highlight differences in function or outcome when comparing knee replacement designs and implantation techniques. Ankle-worn inertial measurement units (IMUs) can be used to remotely measure and monitor the bi-lateral impact load of patients, augmenting traditional PROMs with objective data. The aim of this study was to compare IMU-based impact loads with PROMs in patients who had undergone conventional total knee arthroplasty (TKA), unicompartmental knee arthroplasty (UKA), and robotic-assisted TKA (RA-TKA).

77 patients undergoing primary knee arthroplasty (29 RA-TKA, 37 TKA, and 11 UKA) for osteoarthritis were prospectively enrolled. Remote patient monitoring was performed pre-operatively, then weekly from post-operative weeks two to six using ankle-worn IMUs and PROMs. IMU-based outcomes included: cumulative impact load, bone stimulus, and impact load asymmetry. PROMs scores included: Oxford Knee Score (OKS), EuroQol Five-dimension with EuroQol visual analogue scale, and the Forgotten Joint Score.

On average, patients showed improved impact load asymmetry by 67% (p=0.001), bone stimulus by 41% (p<0.001), and cumulative impact load by 121% (p=0.035) between post-operative week two and six. Differences in IMU-based outcomes were observed in the initial six weeks post-operatively between surgical procedures. The mean change scores for OKS were 7.5 (RA-TKA), 11.4 (TKA), and 11.2 (UKA) over the early post-operative period (p=0.144). Improvements in OKS were consistent with IMU outcomes in the RA-TKA group, however, conventional TKA and UKA groups did not reflect the same trend in improvement as OKS, demonstrating a functional decline.

Our data illustrate that PROMs do not necessarily align with patient function, with some patients reporting good PROMs, yet show a decline in cumulative impact load or load asymmetry. These data also provide evidence for a difference in the functional outcome of TKA and UKA patients that might be overlooked by using PROMs alone.

Little information exists regarding optimal tibial stem usage in revision total knee arthroplasty (rTKA) utilising a tibial trabecular metal (TM) cone. The purpose of this study was to compare 1) functional outcomes, 2) radiographic outcomes, and 3) implant survivorship in rTKA utilising TM cones combined with either short stems (SS) or long stems (LS) at minimum two-years clinical follow-up.

In this retrospective, multi-centre study, patients undergoing TM cone utilising rTKA between 2008 and 2019 were included. Patients were divided into: SS group (no diaphyseal engagement), and LS group (diaphyseal engagement). All relevant clinical charts and post-operative radiographs were examined. Oxford Knee Score (OKS) and EuroQol-5D (EQ-5D-5L) data were collected at most recent follow-up.

In total, 44 patients were included: 18 in the SS group and 26 in the LS group. The mean time of follow-up was 4.0 years. Failure free survival was 94.5% for the SS group and 92.3% for the LS group. All failures were for prosthetic joint infections managed with debridement, antibiotics, and implant retention. At most recent follow-up, 3 patients demonstrated radiographic signs of lucency (1 SS 2 LS, p = 1) and the mean OKS were 37 ± 4 and 36 ± 6 (p = 0.73) in the SS and LS groups, respectively.

Tibial SS combined with TM cones performed as well as LS in rTKA at minimum two-years follow-up. A tibial SS in combination with a TM cone is a reliable technique to achieve stable and durable fixation in rTKA.

No animal model currently exists for hip abductor tendon tears. We aimed to 1. Develop a large animal model of delayed abductor tendon repair and 2. To compare the results of acute and delayed tendon repair using this model.

Fourteen adult Romney ewes underwent detachment of gluteus medius tendon using diathermy. The detached tendons were protected using silicone tubing. Relook was performed at six and 16 weeks following detachment, histological analysis of the muscle and tendon were performed. We then attempted repair of the tendon in six animals in the six weeks group and compared the results to four acute repairs (tendon detachment and repair performed at the same time). At 12 weeks, all animals were culled and the tendon–bone block taken for histological and mechanical analysis.

Histology grading using the modified Movin score confirmed similar tendon degenerative changes at both six and 16 weeks following detachment. Biomechanical testing demonstrated inferior mechanical properties in both the 6 and 16 weeks groups compared to healthy controls.

At 12 weeks post repair, the acute repair group had a lower Movin's score (6.9 vs 9.4, p=0.064), and better muscle coverage (79.4% of normal vs 59.8%). On mechanical testing, the acute group had a significantly improved Young's Modulus compared to the delayed repair model (57.5MPa vs 39.4MPa, p=0.032)

A six week delay between detachment and repair is sufficient to produce significant degenerative changes in the gluteus medius tendon. There are significant histological and mechanical differences in the acute and delayed repair groups at 12 weeks post op, suggesting that a delayed repair model should be used to study the clinical problem.

Proximal tibial bone mineral density (BMD) has been shown to decrease following Total Knee Arthroplasty (TKA) by both dual-energy x-ray absorbtiometry (DEXA) and quantitative computed tomography (qCT)-assisted osteodensitometry. Little is known about changes in BMD following unicompartmental knee arthroplasty (UKA). Additionally, there are proposed differences in stress transmission between cemented metal and polyethylene (PE) components. We proposed two hypotheses. First, that proximal tibial BMD decreases following UKA. Second, that BMD loss would be greater below metal tibial components.

We performed a prospective clinical trial of 50 consecutive UKAs in 49 patients performed by two surgeons at one institution. There were 25 mobile bearing Oxford and 25 fixed bearing Accuris arthroplasties, all were medial. BMD was assessed with qCT-assisted osteodensitometry scans prior to discharge and then at 1 and 2 years post surgery. Each CT slice was divided into medial and lateral halves and cortical and cancellous bone was analysed separately. The six 2mm slices immediately beneath the tibial implant were analysed using previously validated software to create a three-dimensional assessment of BMD. The lateral half was used as a control.

There were a total of 30 females (60%), with an average age of 70 (49–84). One patient was lost to follow-up and another was unable to be analysed due to failure requiring revision before follow-up was complete. Preliminary results showed no significant change in BMD at either 1 or 2 years follow-up. There was no difference in BMD change between the mobile and fixed bearing prostheses, between the medial and lateral halves nor between cortical and cancellous bone. Final results will be presented at the AONZOA conference.

This trial shows that UKA does not result in significant change to BMD at 2 years. The preservation of BMD may indicate that UKA is better at maintaining physiologic stress transfer than a TKA, which has been shown to be associated with a reduction in BMD.

Aseptic loosening is the leading cause for revision in total hip arthroplasty. Retro-acetabular lysis is often a silent process until severe bone loss causes catastrophic failure. This presents a technically difficult problem for the surgeon and a poorer result for the patient compared to primary arthroplasty. While the major cause of osteolysis is reaction to polyethylene particles, there is little data on the initiation and progression of such lesions. Further, alterations in the mechanical environment caused by such pathology is unclear. We present our use of 3D, finite element (FE) models of retro-acetabular pathology to investigate the biomechanical effects of osteolysis in total hip arthroplasty. Axial CT scan slices from a patient with cystic osteolysis were selected. Areas of cortical bone, cancellous bone, the cup and the cyst are accurately identified. The axial images are matched to a predetermined grid and used to build a complex finte element model. In this way complex anatomy can be built into the FE model and used to map cystic lesions. Force is then applied to the acetabulum.

Initial analysis shows similar stress transmission in cystic disease compared to the post operative pelvis. Pelvic bone still behaves as a sandwich construct with transmission from the acetabulum to the SI joints, pubic symphysis and medial wall. In the setting of pelvic medial wall deficiency, stress transmission is altered with areas of low stress around the defect.

The FE models containing pathology can be compared to models with generic bone density values immediately after total hip arthroplasty. The presence of a cyst in cancellous bone with intact cortical bone, demonstrates strain patterns similar to the post operative pelvis. Once cortical bone loss occurs strain patterns begin to change. This may mark a critical point in osteolytic progression. We present a developing new tool to be used in the assessment of a patient population with retroacetabular cystic disease.

The management of peri-prosthetic fracture following total hip arthroplasty is difficult, requiring expertise in both trauma and revision surgery. With rising numbers of patients in the population living with hip prostheses in situ, their frequency is increasing, and controversy remains over their ideal management. The objective of this study was to review all peri-prosthetic fractures at a single institution to identify injury and treatment patterns, and their associated clinical outcomes.

Fifty-four peri-prosthetic fractures in 50 patients were reviewed to determine the relative frequency of fracture types, complication rates, and clinical outcomes. Patient data were obtained through review of the clinical notes and individual patient follow up. Clinical outcomes were evaluated using the Oxford Hip Score (OHS) and Harris Hip Score (HHS).

The fractures were classified using the Vancouver system, the majority of which were type B 1 (20) or type B2 (10). The mean time to union for all fracture types was 4.6 months. A high non-union rate was seen amongst fractures fixed operatively. Fifteen percent of fractures went on to develop loosening following treatment, suggesting under-recognition at the time of injury. The average HHS was 73.1 and OHS 30.3 for all fracture types, at a mean follow up of 3.3 years. Of the 15 patients treated with revision surgery, the most common complication was dislocation (27%).

Treatment of patients with peri-prosthetic fractures requires recognition of the challenging nature of these injuries, their associated poor prognosis, and high complication rate.

Proximal tibial bone density (BD) is a key determinant in the migration of implants following total knee arthroplasty (TKA). CT-osteodensitometry offers three-dimensional, volumetric analysis of both cortical and cancellous regions and has been shown to be both accurate and reliable about the hip.

The primary objective of this study is to assess differences in the bone remodelling pattern (tibial segment) of patients operated on using fixed or rotating platform, using CT-osteodensitometry. The hypothesis is that the presence of a rotating platform optimises stress distribution along the proximal tibia, resulting in reduced proximal bone remodelling of tibia. The secondary objective is to correlate the osteodensitometry and clinical data at follow-up.

Following completion of the initial assessment and after obtaining written informed consent, the patients were randomly allocated to either a PFC Sigma fixed bearing or a PFC Sigma rotating platform (mobile bearing) total knee system (DePuy, Leeds, UK). Clinical follow-ups were performed at three months, one year, and two years after the index operation. CT scans were performed immediately post-operatively and at one and two years respectively. 52 knees (47 patients) were randomised for cemented TKR management using rotating or fixed tibia platform and followed up at one and two years.

At the two year follow-up, the knee function and patellar scores were comparable in both groups. There were no radiographic signs of loosening, and no revision surgery was performed. Sequential BD measurement from post-operative baseline to two year follow-up showed minimal differences between the fixed and rotating platform implants. Bone density assessment showed a 20% mean loss of cancellous BD and minimal changes of cortical BD, with little differences between the two groups. Loss of cancellous BD appears progressive.

In conclusion, this study shows that the mode of fixation of the TKR polyethylene liner does not influence bone remodelling of the proximal tibia after surgery. This is the first study performed to assess separately cortical and cancellous BD changes in THR. There are no data in the literature for comparison.

Long term clinical follow-up of total hip arthroplasty (THA) has identified problems associated with cyst formation. Such cysts are formed as a result of expansile osteolysis, which starts on a small area of the skeleton and spreads into the bone away from the surface of the prosthesis. Since large areas of the prosthesis are still in immediate contact with the skeleton the prosthesis is not loose and the patients are usually without pain. However this form of osteolysis may destroy large areas of the skeleton before it is detected and result in a sudden fracture due to a weakened skeleton. While there are some short term prospective trials that have shown changes in bone density in the periacetabular region, one needs a biomechanical model to understand factors that influence bone remodeling leading to cyst formation. This study aims to develop a mathematical model for studying the mechanical effects of bone cysts in the acetabulum of THA patients.

2D finite element (FE) models of patients with known restroacetabular cystic disease were generated using coronal CT images from the central region of the acetabulum. The boundary between bone and soft tissue was segmented and an FE model generated. Mesh convergence tests were performed to identify a suitable level of mesh refinement. Three material zones representing– cortical bone (E=17GPa), cancellous bone (E=1GPa) and a titanium cup (E=120GPa) – were included in the model. A series of simulations were run to investigate how cysts affect stress distribution as well as the mechanical consequence of medial wall deficiency.

The presence of a cyst did not alter the pattern of stress distribution in the lateral and medial wall. But the strain energy function increased significantly at the inferior margin of the cyst within its cancellous bone. This may encourage bone formation at the cyst margin and help to explain the sclerotic walls seen in some cysts. Models with absent medial walls showed that both compressive and tensile stresses lowered in the cortical wall and the strain energy function reduced almost to zero. This suggests that a medial wall defect has a high risk of progression.

The current 2D model cannot incorporate complex acetabular geometry or complex forces acting on the hip. Therefore the current model will be further developed into a 3D FE model of the whole pelvis that also represents the pelvic ring structure more adequately. Physiologically meaningful boundary conditions as well as patient specific geometry and material properties will be used to investigate mechanical effects of bone cysts realistically.

The aim of this prospective study was to assess safety and effectiveness of Spacer-K, a pre-formed articulating spacer made of gentamicin-impregnated acrylic bone cement. used in two-stage exchange technique for the infected total knee arthroplasty.

32 consecutive patients infected total knee arthroplasty were operated on in two centres. Infection was caused by Coagulase neg. Staphylococcus (21), Staphylococcus aureus (4), and others (4). In 3 case microorganisms were not identified. The mean implantation time of the spacer was 11 weeks (range, 7 to 28 weeks). The rehabilitation program between stages consisted in early range of motion exercises and partial weight-bearing. In one patient removal of the spacer and insertion of a molded cement block with vancomycin was necessary because of insurgence of methicillin-resistant Staphylococcus aureus.

The mean follow-up of 32 knees after removal of the spacer and insertion of the new prosthesis was 40 months (range, 24 to 75 months. The mean Knee Society functional score during spacer management was rated 75 points (range, 41 to 91 points) and was rated 87 points (range, 77 to 97 points) at latest follow-up. Preoperative range of motion improved markedly after removal of the infected prosthesis and insertion of the spacer. Neither breakage nor clinically relevant surface wear were detected. No implant-related complications were observed. Bone stock remained unchanged during the whole management process.

The Spacer-K is safe and effective for the management of the infected total knee arthroplasty.

Considerable advances have recently been made in improving cementing techniques in THA. It has been proposed that certain benefits in terms in reduction of curing time and increased bonding at the cement-stem interface, can be attained by the preheating of the femoral component prior to insertion. The aim of this study was to assess the clinical and radiological 5 year follow-up results of the preheating technique used for fixation of the femoral stem (Lubinus, Link, Germany).

100 consecutive patients (100 hips) with OA and an average age of 72 years (65–85) were operated on by a single surgeon in a single institution. two cohorts of 50 hips were randomised to either conventional cementing techniques, or with a preheated femoral component (40–50 degrees). The same cement was used in both cohorts (Cemex, Tecres, Italy). Follow up was clinical (Harris Hip Score) and radiological.

A reduction in curing time provides significant time savings without compromising implant performance. We recommend preheating of the femoral component for suregons experienecd in joint replacement and with a skilled surgical team.

The purpose of this study was to evaluate the clinical outcome of a hydroxyapatite (HA)-coated tapered stem and to assess bone remodelling of the proximal femur using quantitative computed tomography osteodensitometry.

Fifty consecutive hips were managed with total hip replacement using the Cerafit Multicone H-A.C. stem with HA coating and the Cerafit Triradius-M press-fit cup (Ceraver Osteal, Paris, France). The mean follow-up was 3 years (range, 2.9 to 4 years). Current criteria were used for clinical and radiological assessment.

Forty-nine hips (98%) were clinically rated good or excellent. The mean preoperative Harris Hip Score was rated 57, and it has improved to 96 at the time of follow-up. The radiographs showed stable fixation by bone ingrowth in all hips. Fifteen patients (15 hips) were eligible for osteodensitometry. The mean decrease of the overall bone density (BD) in the metaphyseal portion of the femur 3 years after insertion of the stem was rated 14.21%, and the mean decrease of the cortical BD was rated 15.52%. The mean decrease of the overall BD in the diaphyseal portion of the femoral component was rated 10.00%, and the mean decrease of cortical BD was rated 7.76%. Little changes were observed underneath the tip of the stem.

The clinical and radiological outcomes of the tapered stem with HA coating at a mean follow-up of 3 years compares favourably with other reports. Results of osteodensitometry show less proximal femur BD loss in comparison to similar investigations performed using uncemented stems.
