Aims. The aim of this study is to report the implant survival and factors associated with revision of total elbow arthroplasty (TEA) using data from the Dutch national registry. Methods. All TEAs recorded in the Dutch national registry between 2014 and 2020 were included. The Kaplan-Meier method was used for survival analysis, and a logistic regression model was used to assess the factors associated with revision. Results. A total of 514 TEAs were included, of which 35 were revised. The five-year implant survival was 91%. Male sex, a higher BMI, and previous surgery to the same elbow showed a statistically significant association with revision (p < 0.036). Of the 35 revised implants, ten (29%) underwent a second revision. Conclusion. This study reports a five-year implant survival of TEA of 91%. Patient factors associated with revision are defined and can be used to optimize informed consent and shared decision-making. There was a high rate of secondary revisions. Abstract. Background/Objectives. The incidence of reverse total shoulder replacement (rTSR) implantation is increasing globally, but apprehension exists regarding complications and associated challenges. We retrospectively analysed the senior author's series of rTSR from a tertiary centre using the VAIOS shoulder system, a modular 4th generation implant. We hypothesised that the revision rTSR cohort would have less favourable outcomes and more complications. Methods. 114 patients underwent rTSR with the VAIOS system, over 7 years. The primary outcome was implant survival. Secondary outcomes were Oxford shoulder scores (OSS), radiographic analysis (scapular notching, tuberosity osteolysis, and periprosthetic radiolucent lines) and complications. Results. There were 55 Primary rTSR, 31 Revision rTSR and 28 Trauma rTSR. Implant survival: Primary rTSR- 0 revisions, average 3.35-year follow-up. Revision rTSR-1 revision (4.17%), average 3.52-year follow-up. Trauma rTSR- 1 revision (3.57%), average 4.56-year follow-up OSS: Average OSS improved from 15.39 to 33.8 (Primary rTSR) and from 15.11 to 29.1 (Revision rTSR). Average post-operative OSS for the Trauma rTSR was 31.4 Radiological analysis and complications: Low incidence of scapular notching One hairline fracture below the tip of stem, noted incidentally, which required no treatment. One periprosthetic fracture after alcohol related fall. Treated non-surgically One joint infection requiring two-stage revision to rTSR. One dislocation noted at 2 year follow up. This patient had undergone nerve grafting within 6 months of rTSR for axillary nerve injury sustained during the original fracture dislocation. One acromial fracture with tibial and distal humeral fracture after a fall. Conclusions. The 4th generation modular VAIOS implant is a reliable option for various indications. The revision rTSR cohort had favourable outcomes with low complication rates. In this series, early-to-medium term results suggest lower revision rates and good functional outcomes when compared to published reports. Background. Hip resurfacing arthroplasty (HRA) has seen a recent revival with third generation Metal-on-Metal prostheses and is now widely in use. However, safety and effectiveness of hip resurfacing are still questioned. We systematically reviewed peer-reviewed literature on hip resurfacing arthroplasty to address these issues. Objective. To evaluate implant survival and functional outcomes of hybrid Metal-on-Metal hip resurfacing arthroplasty (HRA). Method. Electronic databases and reference lists were searched from 1988 to May 2010. Identified abstracts were checked for inclusion or exclusion by two independent reviewers. Data were extracted and summarized by one reviewer and verified by a second reviewer. Main study endpoint was implant survival, which we compared with the National Institute of Clinical Excellence (NICE) benchmark. We also evaluated radiological and functional outcomes, failure modes and other adverse events. The quality of evidence was judged using the Grading of Recommendations Assessment, Development and Evaluation system (GRADE). Results. We identified 539 articles, of which 29 met the inclusion criteria. The studies included one randomised clinical trial, 27 prospective case series and one retrospective case series. Data were extracted from these 29 articles, totalling 10621 resurfaced hips, providing details on five out of 11 resurfacing devices on the market. Mean follow up ranged from 0.6 to 10.5 years and implant survival ranged from 84% to 100%. Of the 10621 hips, 370 were revised (3.5%), with aseptic loosening as most frequent failure mode. None of the HRA implants used to date met the full 10 year NICE benchmark. Thirteen studies showed satisfactory implant survival percentages compared to the three year NICE entry-benchmark. These 13 studies used the BHR implant (eight studies), the Conserve plus (two studies), the Durom implant (one study), the Cormet 2000 implant (one study) or both the McMinn and the BHR implant (one study)

Data were extracted from these 24 articles, totalling 8745 resurfaced hips, providing details on five out of 11 resurfacing devices on the market. Maximum follow up was 9 years, mean follow up ranged from 0.6 to 8 years. Implant survival ranged from 88.7% to 100%. Of the 8745 hips, 276 were revised (3.2%), with fracture of the femoral neck as most frequent failure mode. With implant survival plotted against time, 10 studies showed satisfactory implant survival percentages compared to the 3 year NICE entry-benchmark. Nine of these 10 studies used the BHR implant, the other study used the Cormet 2000 implant. Discussion. None of the HRA implants used to date meet the full 10 year NICE benchmark (≥ 90% survival at 10 years follow up). If follow up is too short for the full benchmark, implants are still recommendable if they meet the NICE three year ‘entry benchmark’ (> 3 year revision rate experience, consistent with the 10-year benchmark). Compared to the 3 year NICE entry-benchmark, 10 studies showed satisfactory implant survival percentages. Nine used the BHR implant, the other study used the Cormet 2000 implant. The quality of evidence is low according to the GRADE classification. Future research has to address the most important failure mode for HRA trying to explain the large variation in the frequency of femoral neck fractures

Aim. One of the most challenging problems in total knee arthroplasty (TKA) is periprosthetic infection. A major problem that arises in septic revision TKA (RTKA) are extended bone defects. In case of extended bone defects revision prostheses with metaphyseal sleeves are used. Only a few studies have been published on the use of metaphyseal sleeves in RTKA - none were septic exclusive. The aim of our study was to determine the implant survival, achieved osseointegration as well as the radiological mid-term outcomes of metaphyseal sleeve fixation in septic two-stage knee revision surgery. Method. Clinical and radiological follow-up examinations were performed in 49 patients (25 male and 24 female). All patients were treated with a two-stage procedure, using a temporary non-articulating bone cement spacer. The spacer was explanted after a median of 12 weeks (SD 5, min. 1 – max. 31) and reimplantation was performed, using metaphyseal sleeves in combination with stem fixation. Bone defects were classified on preoperative radiographs using the Anderson Orthopaedic Research Institute (AORI) classification. During follow-up postoperative range of motion (ROM) was measured and radiographs were performed to analyse: (i) osseointegration (radiolucent lines and spot welds), (ii) leg alignment, (iii) patella tilt and shift. Results. All types of bone defects were found on the tibial (4× type 1, 7× type 2a, 26× type 2b, 9× type 3) as well as on the femoral side (1× type 1, 4× type 2a, 20× type 2b, 6× type 3). Mean follow-up time was 4.7 years (minimum 1 year). In total 12 knees (24.5%) had to be re-revised, all due to re-infection. We did not encounter any case of aseptic loosening. In 3 patients (6.8%) we detected an insufficient osseointegration, but no patient had to be re-revised due to only minimal or to the absence of symptoms and no clinical signs of loosening. The ROM (mean 93°, SD 20.6, min. 25° max. 125°) has shown very satisfying results at the time of follow-up. Malalignment was detected in 4 patients (10.3%), a patella tilt in 7 (19.4%) and a patella shift in 14 (48.3%). Conclusions. Metaphyseal Sleeves have shown very promising mid-term results regarding osseointegration and aseptic implant survival in RTKA with compromised metaphyseal bone stock. Our results indicate that they are a reliable fixation option in septic RTKA patients

Introduction. The current investigation includes a retrospective review of the experience of five Institutions with distal femur megaprostheses for tumor over a twenty year period, to analyze the incidence and etiology of failure, using a new classification system based upon the failure modes. Methods. Between 1974 and 2008, 2174 patients underwent primary limb preservation for a benign or malignant extremity tumor using a metallic megaprosthesis at five Institutions, 951 (43.7%) were distal femur replacements. Retrospective analysis of complications according to the Letson and Ruggieri Classification was performed and Kaplan-Meier curves of implant survival were defined. Segmental megaprosthetic reconstruction failures were categorized as mechanical and non-mechanical failures. Results. A total of 951 skeletally mature patients received a segmental endoprosthesis for the treatment of an oncologic condition. Overall 261 (27%) of the primary procedures were considered failures. There were 137 mechanical failures (14.4%): 12 (1.3%) Type 1 (soft tissue failure), 65 (6.8%) Type 2 (aseptic loosening), and 60 (6.3%) Type 3 (structural failure). Non-mechanical causes accounted for 124 failures (13%): 45 (4.7%)Type 4 (tumor progression) and 79 (8.3%) Type 5 (infection). The overall implant survival to all modes of failure was 77% at 10 years and 73% at 20 years. The implant survival to aseptic loosening was over 90% at 10 years. Conclusion. Most frequent cause of failure was infection followed by aseptic loosening and structural failure. The implant survival at long term was quite satisfactory

Introduction

In recent years, many studies demonstrated the efficacy of an early switch to oral antibiotics after surgical treatment in orthopaedic related infections. However, large analyses on periprosthetic joint infections (PJIs) are lacking.

Material and Methods

We conducted a retrospective observational multicenter study in patients diagnosed with an early postoperative PJI (i.e less than 3 months after the index arthroplasty) treated with debridement, antibiotics and implant retention (DAIR). Patients from Europe and the USA were included. These two cohorts served as a quasi-randomised trial since an early oral antibiotic switch is routine practice in Europe versus a long duration of intravenous (IV) antibiotic treatment in the USA. Failure was defined as the clinical need for: i) a second DAIR, ii) implant removal, iii) suppressive antibiotic treatment or iv) infection related death.

Aim. To examine the mid-term survival, clinical and patient reported outcomes of the silastic 1. st. metatarsophalangeal joint replacement for the treatment of end stage hallux rigidus. Methods. We reviewed 83 consecutive silastic arthroplasties performed in 79 patients for end stage hallux rigidus. There were 3 men and 76 women; mean age 63 years (range 45–78 years). No patient was lost to follow up. Average follow-up was 5.3 years (1.1–11.3 years). The EQ 5D–5L Health index, Manchester-Oxford Foot Questionnaire (MOXFQ), visual analogue scale (VAS) of pain and overall satisfaction rate (Likert scale) were collected for patient reported outcomes. Results. 2 patients required revision; 1 for early infection (2 months) and 1 for stem breakage (10 years 1 month). 5 patients reported lateral metatarsalgia, 2 patients reported neuropathic pain, 6 patients developed superficial infection which fully responded to oral antibiotics, and 1 patient developed interphalangeal joint pain. 2 patients died in the cohort. Pre-operative mean MOXFQ was 44, mean EQ5D Index was 0.564 and VAS was 6.97. At mean follow-up of 5.3 years, the mean MOXFQ was 12.7 (0–57), the mean EQ5D Index was 0.851 (−0.02–1) and the mean VAS was 1.67 (0–8). The mean range of motion was 35° (30° dorsiflexion and 5° plantarflexion). The overall satisfaction rate was 90.2%. The implant survival rate was 97.6%. Conclusions. The silastic big toe arthroplasty offers excellent clinical mid term survival and functional outcomes and could be considered as an attractive alternative to traditional fusion for end stage hallux rigidus

From October 2003 to September 2007 at Rizzoli 161 GMRS. ®. prostheses were implanted after resections of the lower limb. This is a modular system with a knee rotating hinge mechanism, cemented and uncemented stems, in titanium and chromium-cobalt-molybdenum, curved and straight-fluted, with or without hydroxyapatite coating. Adaptors are available to revise HMRS. ®. prostheses with hybrid implants. This study includes 88 males and 73 females ranging in age from 9 to 80 years. Sites of reconstruction were 109 distal femurs, 19 proximal femurs, 1 total femur and 32 proximal tibias. There were 149 oncologic and 12 non oncologic diagnoses, including 96 primary reconstructions and 65 revisions for failures of previous reconstructions. Analysis of imaging and complications was performed and function assessed according to MSTS system. Kaplan-Meier curves were used to statistically evaluate implant survival. At a mean follow up of 2.5 yrs. 106 patients are continuously NED, 31 NED after treatment of relapse, 7 AWD, 5 DWD. There were 10 major complications: 8 infections (4.7%) (5 in primary and 3 in secondary implants) and 2 aseptic loosenings (1.2%) (1 each). There were 9 minor complications requiring minor revisions. Comparative statistical analysis of implant survival showed no statistically significant difference between primary and secondary implants. Functional results were good or excellent in 95% of patients, without any poor. Middle term results are promising with no breakages of implant components. Besides oncological cases, there are selected indications in non oncological settings, such as revisions of prosthetic or allografts failures. While a higher complication rate was expected in secondary implants, statistical analysis shows similar survival

Purpose was to evaluate the incidence of complication in lower limb reconstructions with modular prostheses comparing cemented versus uncemented stems in two different orthopedic tumor Centers. Methods: retrospective analysis of implant survival, complications and functional results assessed according to MSTS system of the Rizzoli and Moffitt series of modular megaprosthesis in lower limb. From 2002 and 2007, 238 modular prostheses of same design with a rotating hinge knee, were implanted in the lower limb as primary reconstruction in these two Centers. In 130 cases the prosthesis was implanted with cemented stems and in 108 cases with uncemented stems. Sites included: 120 distal femur, 61 proximal femur, 46 proximal tibia, 10 total femur and in 1 case both distal femur and proximal tibia. Histologic diagnoses included: 89 osteosarcoma, 13 Ewing sarcoma, 21 chondrosarcoma, 19 sarcoma, 55 metastasis, 17 TGC, 7 MFH, 11 other diagnoses and 6 non oncologic cases. Major prostheses-related complications were analysed and functional results according to Muscolo Skeletal Tumor Society system, at a mean follow up of 2.03 yrs. Also a statistical evaluation with Kaplan Meier curves, a comparative statistical analysis with Wilcoxon test and multivariate Cox regression analysis were performed. Results: Outcome in 223 evaluated oncologic pts, showed: 121 pts continuously disease free, 26 NED after treatment of relapse, 54 AWD, 16 DWD. Margins were wide in 94.3% (214/227 pts) of evaluated pts. Complications causing implants failure were: infections in 20/238 (8.41%), aseptic loosening in 7/238 (2.95%). No breakages of prosthetic components were observed. Infections occurred at mean time of 1.3 yrs, sites included: 9 distal femurs, 4 proximal femurs, 5 proximal tibias, 2 total femurs. Aseptic loosening occurred at mean time of 2.3 yrs, sites included: 3 proximal tibias, 2 distal femurs, 2 proximal femurs. Rate of aseptic loosening was higher for cemented stems 5/130 (3.85%) vs uncemented 2/108 (1.85%). This difference is not statistically significant. Infection rate was 10% (13/130) for cemented stems and 6.5% (7/108) for uncemented stems. This difference is statistically significant at comparison of survival curves logrank test. Average MSTS function score was 83.5%. Multivariate Cox regression analysis showed that the significant factor favourable reluted with lower incidence of complications was the use of uncemented stems. Conclusions: lower limb modular prostheses with both cemented and uncemented stems gave good results and a low complication rate. Better results were observed with uncemented stems and statistically confirmed, but this needs to be further investigate in a future study at long term

We present a review of 97 consecutive BioPro® metallic hemiarthroplasties performed in 80 patients for end-stage hallux rigidus, with a minimum of five years follow-up.

The mean age of the cohort was 55 (22 to 74) years. No patient was lost to follow-up. There were 15 revisions performed, one for infection, two for osteolysis, and 12 for pain. The all cause survival rate at five years was 85.6% (95% confidence interval (CI) 83.5 to 87.9). Younger age was a significant predictor of revision (odds ratio 1.09, 95% CI 1.02 to 1.17, p=0.014) on excluding infection and adjusting for confounding variables (Cox regression). Significant improvements were demonstrated at 5 years in the Manchester Oxford foot questionnaire (13.9, 95% CI 10.5 to 17.2) and in the physical component of the short form 12 score (6.5, 95% CI 4.1 to 8.9). The overall satisfaction rate was 72%. The cost per quality-adjusted-life-year at 5 years, accounting for a 3% per year revision rate, was £3,714.

The BioPro offers good short to mid-term functional outcome and is a cost effective intervention. The relative high revision rate is associated with younger age and the use of this implant may be limited to older patients.

The main purpose of the present study was to determine long-term implant fixation of 15 unicompartmental knee arthroplasty (UKAs) with an all-poly tibial component using Roentgen stereophotogrammetric analysis (RSA) at a mean 10-year follow-up. The secondary purpose was to investigate whether the progressive loss of implant's fixation correlates with a reduction in Knee society score (KSS). Fifteen non-consecutive patients with primary knee osteoarthritis received a UKA with an all-poly tibial component were assessed using KSS scores pre-operatively and post-operatively and RSA on day 2 after surgery, then at 3, 6, and 12 months and yearly thereafter. The mean last follow-up was 10 years. An increase in maximum total point motion (MTPM) values from 6 months to 1 year post-operatively was found respect to post-operative reference. Implants’ displacement values were always 2 mm during the first 6 months, and then, two different trends were noticed in revised and non-revised implants. MTPM increase between 1 and 2 years of follow-up in non-revised UKAs was always 0.2 mm, whereas it was [0.2 mm in revised UKAs. A linear and negative correlation with statistical significance was found between MTPM and both clinical and functional KSS scores (p 0.001). Also in a long-term follow-up evaluation, RSA is an effective tool to predict functional results after an all-poly UKA providing also a relevant predictive value at 1 year follow-up, and this can be very useful for both patients and surgeons.

Background

Roentgen Stereophotogrammetric Analysis (RSA) represents nowadays an excellent solution for high-accuracy fixation measurement of UKAs. To date, to the best of our knowledge, no previous study has investigated long-term micromotions of a UKA using RSA. For this reason, the main purpose of the present study is to determine long-term implant fixation of 15 UKAs with all-poly tibial component using RSA at a mean 10-years follow-up. The secondary purpose was to investigate whether the progressive loss of implant's fixation correlates with a reduction of KSS scores.

Methods

Fifteen non-consecutive patients with primary knee osteoarthritis received a UKA with an all-poly tibial component between January 1995 and April 2003 in the Authors' institution. Pre and post-operative KSS scores were recorded. RSA evaluation was performed on day 2 after surgery, than at 3, 6, and 12 months and yearly thereafter. The patients were evaluated with a mean follow-up of 10 years.

Introduction

National Institute for Health and Clinical Excellence (NICE) guidelines recommend the use of total hip replacement (THR) for displaced intracapsular fractured neck of femur (NOF) in cognitively intact patients who were independently mobile prior to the injury. The National Joint Registry for England and Wales (NJR) has collected data on THRs performed since 2003. This retrospective cohort study explores risk factors independently associated with implant failure and perioperative mortality.

Methods

NJR data recording a THR performed for acute fractured NOF between 2003 and 2010 were analysed. Cox proportional hazards models were used to analyse the extent to which risk of implant revision was related to specific covariates. Multivariable logistic regression was used to analyse factors affecting 90-day perioperative mortality. Significance was taken as p< 0.01.

Osteoarthritis of the hip is common and the mainstay of surgical treatment for end-stage disease is total hip replacement. There are few RCTs comparing long-term outcomes between prostheses; therefore, surgeons and patients are reliant on single-centre case-series and recently, analysis of joint registries, when making evidence-based implant choices.

We conducted a systematic review, conforming to PRISMA, of Medline and Embase in September 2017. Single-centre case-series and papers analysing registries were included. Series looking at disease-specific cohorts (other than OA), under 15 years follow-up or lacking survival analyses were excluded. Resurfacings, revisions and complex-primaries were also excluded. 2750 abstracts were screened, resulting in 299 full-text articles. Following full review 124 articles were excluded and 21 series added from references, resulting in 150 analyses of individual prostheses/constructs and 12 papers from registries. We also analysed annual reports of registries.

Registry data indicated cemented prostheses tended to better outcomes at late follow-ups, whereas case-series showed cementless prostheses tended to have better survival past 15 years with revision for any reason (of stem, cup or either component) as the end-point.

The discrepancy between results from registry data and single-centre case series is stark, and whilst the reasons for these differences may be multifactorial, single-centre case-series included in this review often lacked sufficient power to provide precise estimates of survival. This is contrasted to data from registries, which tended to have far greater numbers from multiple centres, allowing results to be generalised to the population.

The difference between these two modes of analysis suggests bias exists in selection and outcomes from single-centre series. The varied quality of reporting in case-series make it difficult for a reader to adequately assess bias, and accurately inform contemporary decision making.

Surgeons and patients should be cautious when interpreting single-centre case series and systems relying on data generated from them.

Objectives. Modern metal-on-metal (MoM) hip resurfacing arthroplasty (HRA), while achieving good results with well-orientated, well-designed components in ideal patients, is contraindicated in women, men with head size under 50 mm, or metal hypersensitivity. These patients currently have no access to the benefits of HRA. Highly crosslinked polyethylene (XLPE) has demonstrated clinical success in total hip arthroplasty (THA) and, when used in HRA, potentially reduces metal ion-related sequelae. We report the early performance of HRA using a direct-to-bone cementless mono-bloc XLPE component coupled with a cobalt-chrome femoral head, in the patient group for whom HRA is currently contraindicated. Methods. This is a cross-sectional, observational assessment of 88 consecutive metal-on-XLPE HRAs performed in 84 patients between 2015 and 2018 in three centres (three surgeons, including the designer surgeon). Mean follow-up is 1.6 years (0.7 to 3.9). Mean age at operation was 56 years (. sd. 11; 21 to 82), and 73% of implantations were in female patients. All patients were individually counselled, and a detailed informed consent was obtained prior to operation. Primary resurfacing was carried out in 85 hips, and three cases involved revision of previous MoM HRA. Clinical, radiological, and Oxford Hip Score (OHS) assessments were studied, along with implant survival. Results. There was no loss to follow-up and no actual or impending revision or reoperation. Median OHS increased from 24 (interquartile range (IQR) 20 to 28) preoperatively to 48 (IQR 46 to 48) at the latest follow-up (48 being the best possible score). Radiographs showed one patient had a head-neck junction lucency. No other radiolucency, osteolysis, component migration, or femoral neck thinning was noted. Conclusion. The results in this small consecutive cohort suggest that metal-on-monobloc-XLPE HRA is successful in the short term and merits further investigation as a conservative alternative to the current accepted standard of stemmed THA. Aims

An evidence-based radiographic Decision Aid for meniscal-bearing unicompartmental knee arthroplasty (UKA) has been developed and this study investigates its performance at an independent centre.

Patients and Methods

Pre-operative radiographs, including stress views, from a consecutive cohort of 550 knees undergoing arthroplasty (UKA or total knee arthroplasty; TKA) by a single-surgeon were assessed. Suitability for UKA was determined using the Decision Aid, with the assessor blinded to treatment received, and compared with actual treatment received, which was determined by an experienced UKA surgeon based on history, examination, radiographic assessment including stress radiographs, and intra-operative assessment in line with the recommended indications as described in the literature.

Aims. The primary objective of this registry-based study was to compare patient-reported outcomes of cementless and cemented medial unicompartmental knee arthroplasty (UKA) during the first postoperative year. The secondary objective was to assess one- and three-year implant survival of both fixation techniques. Methods. We analyzed 10,862 cementless and 7,917 cemented UKA cases enrolled in the Dutch Arthroplasty Registry, operated between 2017 and 2021. Pre- to postoperative change in outcomes at six and 12 months’ follow-up were compared using mixed model analyses. Kaplan-Meier and Cox regression models were applied to quantify differences in implant survival. Adjustments were made for patient-specific variables and annual hospital volume. Results. Change from baseline in the Oxford Knee Score (OKS) and activity-related pain was comparable between groups. Adjustment for covariates demonstrated a minimally greater decrease in rest-related pain in the cemented group (β = -0.09 (95% confidence interval (CI) -0.16 to -0.01)). Cementless fixation was associated with a higher probability of achieving an excellent OKS outcome (> 41 points) (adjusted odds ratio 1.2 (95% CI 1.1 to 1.3)). The likelihood of one-year implant survival was greater for cemented implants (adjusted hazard ratio (HR) 1.35 (95% CI 1.01 to 1.71)), with higher revision rates for periprosthetic fractures of cementless implants. During two to three years’ follow-up, the likelihood of implant survival was non-significantly greater for cementless UKA (adjusted HR 0.64 (95% CI 0.40 to 1.04)), primarily due to increased revision rates for tibial loosening of cemented implants. Conclusion. Cementless and cemented medial UKA led to comparable improvement in physical function and pain reduction during the initial postoperative year, albeit with a greater likelihood of achieving excellent OKS outcomes after cementless UKA. Anticipated differences in early physical function and pain should not be a decisive factor in the choice of fixation technique. However, surgeons should consider the differences in short- and long-term implant survival when deciding which implant to use. Cite this article: Bone Jt Open 2024;5(5):401–410

Aims. The outcomes of patients with unexpected positive cultures (UPCs) during revision total hip arthroplasty (THA) and total knee arthroplasty (TKA) remain unknown. The objectives of this study were to establish the prevalence and infection-free implant survival in UPCs during presumed aseptic single-stage revision THA and TKA at mid-term follow-up. Methods. This study included 297 patients undergoing presumed aseptic single-stage revision THA or TKA at a single treatment centre. All patients with at least three UPCs obtained during revision surgery were treated with minimum three months of oral antibiotics following revision surgery. The prevalence of UPCs and causative microorganisms, the recurrence of periprosthetic joint infections (PJIs), and the infection-free implant survival were established at minimum five years’ follow-up (5.1 to 12.3). Results. Of the 297 patients undergoing aseptic revisions, 37 (12.5%) had at least three UPCs obtained during surgery. The UPC cohort included 23 males (62.2%) and 14 females (37.8%), with a mean age of 71.2 years (47 to 82). Comorbidities included smoking (56.8%), hypertension (48.6%), diabetes mellitus (27.0%), and chronic renal impairment (13.5%). The causative microorganisms included Staphylococcus epidermidis (49.6%), Bacillus species (18.9%), Micrococcus species (16.2%), and Cutibacterium acnes (16.2%). None of the study patients with UPCs developed further PJIs or required further surgical intervention during follow-up. Conclusion. The prevalence of UPCs during presumed aseptic revision THA and TKA was 12.5%. The most common causative microorganisms were of low virulence, and included S. epidermidis, Bacillus species, Micrococcus species, and C. acnes. Microorganism-specific antibiotic treatment for minimum three months’ duration of UPCs in presumed aseptic revision arthroplasty was associated with excellent infection-free implant survival at mid-term follow-up. Abstract. Introduction. In revision knee arthroplasty, rotating hinge implants (RHK) have been considered to result in higher complication rates and lower survivorship when compared to constrained condylar implants (CCK). The aims of this study were to compare patient reported outcome measures (PROMs), complication rates and survivorship of RHK and CCK used in revision arthroplasty at a single, high volume elective orthopaedic centre with previously validated bespoke database. Methodology. One hundred and eight patients who underwent revision knee arthroplasty with either CCK or RHK and matched our inclusion criteria were identified. EQ5D, Health State and Oxford Knee Scores were collected pre-operatively and at 1 year post-operatively. Complication data was collected at 6 weeks, 6 months, 1 year and 2 years post-operatively. NJR data was interrogated, in addition to our own database, to investigate implant survival. Results. There was no statistically significant difference between RHK and CCK in implant survival at two to twelve years of follow up. In addition, we observed no statistically significant difference in the PROMs scores and complication rates of the two implants. Conclusion. This study shows that both the RHK and CCK remain viable options in revision arthroplasty, the implant survival and complication rate were comparable. We recommend prospective randomised control trials with long-term follow up to further investigate the use of CCK and RHK implants in revision knee arthroplasty
