Aims. In the treatment of basal thumb osteoarthritis (OA), intra-articular autologous fat transplantation has become of great interest within recent years as a minimally invasive and effective alternative to surgical intervention with regard to pain reduction. This study aims to assess its long-term effectiveness. Methods. Patients diagnosed with stage one to three OA received a single intra-articular autologous fat transplantation. Fat tissue was harvested from the abdomen and injected into the trapeziometacarpal (TMC) joint under radiological guidance, followed by one week of immobilization. Patients with a minimum three-year post-procedure period were assessed for pain level (numerical rating scale), quality of life (Mental Health Quotient (MHQ)), the abbreviated version of the Disabilities of Arm, Shoulder and Hand questionnaire (QuickDASH)), and grip and pinch strength, as well as their overall impression of the treatment.
A huge commitment is required from patients and families who undergo a limb reconstruction procedure using the hexapod frame. This includes turning the struts on the frame, pin site care and intensive rehabilitation. Montpetit et al (2009) discovered that function, participation, engagement in regular activities of daily living is severely impacted during the hexapod lengthening period. Due to the long duration and burden for families, it is imperative that healthcare professionals understand the impact that the hexapod frame has on functional abilities and health related quality of life (HRQL). This project involved a retrospective review of prospectively collected data on function and HRQL during two periods of time: (1) when the hexapod frame is applied on the child's lower extremity and (2) when the lengthening phase is completed, and the hexapod frame is removed. This study investigates those at risk for recurrent dislocation after revision THA for instability at a single institution. Vacuum orthoses are being applied in the care of patients with foot and lower leg conditions, as ankle fractures or sprains. The lower leg is protected and immobilized, which increases mobility. Due to the design, the orthoses lead to a difference in leg length, i.e. the side with the orthosis becomes longer, which changes the gait kinematics. To prevent or mitigate the unfavourable effects of altered gait kinematics, leg length-evening devices (shoe lifts) are offered that are worn under the shoe on the healthy side. Tenotomy of the iliopsoas tendon has been described as an effective procedure to treat refractive groin pain induced by iliopsoas tendinitis. However, the procedure forces the rectus femoris to act as the primary hip flexor and little is known about the long-term effects of this procedure on the peri-articular muscle envelope (PAME). Studies suggest that iliopsoas tenotomy results in atrophy of the iliopsoas and decreased hip flexion strength with poorer outcomes, increasing the susceptibility for secondary tendinopathy. The aim of this study is to describe changes in the PAME following psoas release. All patients who presented for clinical examination at our hospital between 2016 and 2021 were retrospectively reviewed. Patients who presented after psoas tenotomy with groin pain and who were unable to actively lift the leg against gravity, were included. Pelvic MRI was taken. Qualitative muscle evaluation was done with the Quartile classification system. Quantitative muscle evaluation was done by establishing the cross-sectional area (CSA). Two independent observers evaluated the ipsi- and contralateral PAME twice. The muscles were evaluated on the level: iliacus, psoas, gluteus minimus-medius-maximus, rectus femoris, tensor fasciae lata, piriformis, obturator externus and internus. Introduction. Survival after contemporary solid organ transplant (SOT) is increasing, and demand for total hip arthroplasty (THA) among SOT recipients is rising accordingly. The purpose of this study is to compare the perioperative outcomes and short-term implant- and patient-survivorship of contemporary THA following the most common types of SOT. Methods. Among SOT recipients, 119 primary THAs (92 patients, 39% female) were performed at a single institution from 2000–2020 and were retrospectively reviewed at a mean follow-up of 3.6yrs. INTRODUCTION. Measurement of range of motion is a critical item of any knee scoring system. Conventional measurements used in the clinical settings are not as precise as required. Smartphone technology using either inclinometer application or photographic technology may be more precise with virtually no additional cost when compared to more sophisticated techniques such as gait analysis or image analysis. No comparative analysis between these two techniques has been previously performed. The goal of the study was to compare these two technologies to the navigated measurement considered as the gold standard. MATERIAL. Ten patients were consecutively included. Inclusion criterion was implantation of a TKA with a navigation system. METHODS. Two free angle measurement applications were downloaded to the Smartphone: one using inclinometer technology, the other using camera technology. After navigation assisted TKA and just before wound closure, the operated knee was positioned at full extension, 30±2°, 60±2°, 90±2° and 120±2° according to the navigated measurement. At each step, the knee flexion angle was measured with both Smartphone applications: inclinometer application (figure 1) and camera application (figure 2). For each of the ten patients, 5 navigated, 5 inclinometer and 5 camera measurements were obtained for each patient, giving three sets of 50 repeat measurements. The sample size was calculated to get a significance level of 0.05 and a power of 0.8 to detect a 10° difference. The treatment of medial knee osteoarthritis (OA) in conjunction with anterior knee laxity is an issue of debate. Current treatment options include knee joint distraction, unicompartmental knee replacement (UKR) or high tibial osteotomy with anterior cruciate ligament (ACL) reconstruction or total knee replacement. Bone-conserving options are preferred for younger and active patients with intact lateral and patello-femoral compartment. However, still limited experience exists in the field of combining medial UKR and ACL reconstruction. The aim of this study is to retrospectively evaluate the results of combined fixed-bearing UKR and ACL reconstruction, specifically with regard to patient satisfaction, activity level, and postoperative functional outcomes. The hypothesis was that this represents a safe and viable procedure leading to improved stability and functional outcome in patients affected by isolated unicompartmental OA and concomitant ACL deficiency. Fourteen patients with ACL deficiency and concomitant medial compartment symptomatic osteoarthritis were treated from 2006 to 2010. Twelve of them were followed up for an average time of 7.8 year (range 6–10 years). Assessment included Knee Osteoarthritis Outcome Score (KOOS), Oxford Knee score (OKS), American Knee Society scores (AKSS), WOMAC index of osteoarthritis, Tegner activity level, objective examination including instrumented laxity test with KT-1000 arthrometer and standard X-rays.
Introduction. Component position and overall limb alignment following total knee arthroplasty (TKA) have been shown to influence prosthetic survivorship and clinical outcomes. Robotic-assisted (RA) total knee arthroplasty has demonstrated improved accuracy to plan in cadaver studies compared to conventionally instrumented (manual) TKA, but less clinical evidence has been reported. The objective of this study was to compare the three-dimensional accuracy to plan of RATKA with manual TKA for overall limb alignment and component position. Methods. The risk of postoperative complications after resection of soft-tissue sarcoma in the medial thigh is higher than in other locations. This study investigated whether a vessel sealing system (VSS) could help reduce the risk of postoperative complications after wide resection of soft-tissue sarcoma in the medial thigh. Of 285 patients who underwent wide resection for soft-tissue sarcoma between 2014 and 2021 at our institution, 78 patients with tumours in the medial thigh were extracted from our database. Information on clinicopathological characteristics, preoperative treatment, surgical treatment (use of VSS, blood loss volume, operating time), and postoperative course (complications, postoperative haemoglobin changes, total drainage volume, and drainage and hospitalization durations) were obtained from medical records. Bi-cruciate stabilized (BCS) TKA is the prosthesis that aims to substitute bi-cruciate ligament with post-cam engagement. We estimated to describe the in vivo kinematics during deep knee bending in BCS and Cruciate retaining (CR) TKA with the same articular geometry. We analyzed 26 knees who agreed to the current investigation under institutional review board approval. 17 knees were implanted with BCS (Journey ∥BCS, Smith & Nephew. Closed suction intraarticular drain (CSD) use after total knee arthroplasty (TKA) has been studied with regards to wound healing and range of motion, however, no data exist on how CSD use impacts knee joint effusion and quadriceps strength. The primary purpose of this study was to determine whether CSD use influences recovery of quadriceps strength. Secondary outcomes examined effects of CSD on intraarticular effusion, lower limb swelling, knee range of motion (ROM), pain and wound healing complications. Methods. Twenty-nine patients undergoing same-day bilateral TKA were enrolled in a prospective, randomized blinded study. Subjects were randomized to receive a CSD on one lower extremity while the contralateral limb had the use of a subcutaneous drain (SCDRN) without the use of suction. In the past decades, titanium-based biomaterials have been broadly used in maxillofacial and periodontology surgery. The main aetiological agents related to complications in this procedures are Porphyromonas gingivalis, a Gram-negative anaerobic bacteria that is also responsible for the development of chronic gingivitis, and Streptococcus oralis, a Gram positive facultative anaerobic bacteria. In previous studies, we have demonstrated that the fluorine doping of titanium-based alloys reduces bacterial adherence. The aim of this study is to evaluate the bacterial adherence on fluorine-doped titanium (TiF) probes compared to chemical polishing titanium (Ti) probes. The P. gingivalis ATCC 33277 and S. oralis ATCC 9811 adherence study was performed by introducing each probe in a well of 6-well plate with 5 ml containing 106 colony forming units (CFU/ml) in sterile 0.9% NaCl and was incubated 37°C 5% CO2 for 90 minutes, in anaerobiosis in the case of P. gingivalis. Introduction. Periprosthetic joint infection (PJI) remains the main cause of failure in primary and revision total knee arthroplasties (TKAs). Local delivery of antibiotics, mainly antibiotic-loaded bone cement (ALBC), is commonly employed to prevent PJI. Over the past decade, tantalum and porous titanium have been successfully utilized as metaphyseal fixation devices to address bone loss and improve biologic fixation during revision TKA. However, no study has examined the antimicrobial properties compared to bone cement. The purpose of this study was to compare the ability of tantalum, 3D porous titanium, antibiotic-loaded bone cement (ALBC) and smooth titanium alloy (STA) to inhibit Staphylococci bacterial agents in an in vitro medium environment, based on the evaluation of the zone of inhibition (ZOI) and the antibacterial activity duration. Our study hypothesis was that we will found no significant difference between groups to inhibit Methicillin-Sensitive or Methicillin-Resistant Staphylococcus aureus (MSSA/MRSA) agents. Methods. Thirty beads made of 3 different materials (tantalum/ 3D porous titanium/ STA) were bathed during 1hour inside of a solution made of 1g vancomycin with 20-mL of sterile water for injection (bath concentration: 50 mg/mL). Introduction. A careful evaluation of new technologies such as robotic-arm assisted total knee arthroplasty (RATKA) is important to understand the reduction in variability among users. While there is data reviewing the use of RATKA, the data is typically presented for experienced TKA surgeons. Therefore, the purpose of this cadaveric study was to compare the variability for several surgical factors between RATKA and manual TKA (MTKA) for surgeons undergoing orthopaedic fellowship training. Methods. Two operating surgeons undergoing orthopaedic fellowship training, each prepared six cadaveric legs for cruciate retaining TKA, with MTKA on one side (3 knees) and RATKA on the other (3 knees). These surgeons were instructed to execute a full RATKA or MTKA procedure through trialing and achieve a balanced knee. The number of recuts and final poly thickness was intra-operatively recorded. After completion of bone cuts, the operating surgeons were asked if they would perform a cementless knee based on their perception of final bone cut quality as well as rank the amount of mental effort exerted for required surgical tasks. Introduction. Trabecular Titanium is a biomaterial characterized by a regular three-dimensional hexagonal cell structure imitating trabecular bone morphology. Components are built via Electron Beam Melting technology in aone- step additive manufacturing process. This biomaterial combines the proven mechanical properties of Titanium with the elastic modulus provided by its cellular solid structure (Regis 2015 MRS Bulletin). Several in vitro studies reported promising outcomes on its osteoinductive and osteoconductive properties: Trabecular Titanium showed to significantly affect osteoblast attachment and proliferation while inhibiting osteoclastogenesis (Gastaldi 2010 J Biomed Mater Res A, Sollazzo 2011 ISRN Mater Sci); human adipose stem cells were able to adhere, proliferate and differentiate into an osteoblast-like phenotype in absence of osteogenic factors (Benazzo 2014 J Biomed Mater Res A). An open-label prospective clinical study was conducted to compare 3D modeling with manual TKA (non-randomized) at 4 U.S. centers between July 2016 and August 2018. Men and women aged > 18 with body mass index < 40kg/m. 2. scheduled for unilateral primary TKA were recruited for the study. 144 3DTKA and 86 manual TKA (230 patients) were included in the analysis of accuracy outcomes. INTRODUCTION. Navigation systems have proved allowing performing measurement of the lower limb axis with a good accuracy, but the mandatory use of reference pins or screws limit their use to the operating room. The use of non-invasive navigation systems has been suggested to overcome this limitation. We conducted a prospective study to assess the validity of such a measurement system with non-invasive fixation of the reference arrays. The main goal was to compare this method with a standard, invasive navigation system requiring bony fixation of the arrays. The following hypothesis was tested: there will be a significant difference between the simultaneous measurement of the mechanical femoro-tibial angle by a standard navigation system and by the non-invasive navigation system. MATERIAL AND METHODS. 20 patients scheduled for total or partial knee arthroplasty were included after giving their informed consent. There were 7 men and 13 women with a median age of 65 years (range, 55 to 90). The median coronal deformation measured by X-rays was 8° of varus (range, 5° valgus to 22 ° varus). This study aimed to define the histopathology of degenerated humeral head cartilage and synovial inflammation of the glenohumeral joint in patients with omarthrosis (OmA) and cuff tear arthropathy (CTA). Additionally, the potential of immunohistochemical tissue biomarkers in reflecting the degeneration status of humeral head cartilage was evaluated. Specimens of the humeral head and synovial tissue from 12 patients with OmA, seven patients with CTA, and four body donors were processed histologically for examination using different histopathological scores. Prosthetic joint infections (PJI) occur infrequently, but they represent the most devastating complication with high morbidity and substantial cost. Staphylococcus aureus and coagulase-negative S. epidermidis are the most commonly infecting agents associated with PJI. Nowadays, Gram-negative species like Escherichia coli and Pseudomonas aeruginosa are gaining relevance. The use of TiO2 conical nanotubular doped with fluorine and phosphorous (FP-cNT) surfaces is an interesting approach to prevent surface bacterial colonization during surgery and favouring the osseointegration. Despite of there are serum markers related with PJI, to date there is described no biomaterial-related marker that allows detecting PJI. Here we describe the adherence and the bactericidal effect of FP-cNT and its capacity of marking the non-fermenting bacteria that have been in contact with it by Al. This metal is delivered by FP-cNT in non-toxic concentrations (between 25 and 29 ng/mL). F-P-cNT layers on Ti6Al4V alloy were produced as described previously by our group. Ti6Al4V chemical polishing (CP) samples without nanostructure were used as control and produced as described previously.
