Aims. A variety of surgical methods and strategies have been demonstrated for Andersson lesion (AL) therapy. In 2011, we proposed and identified the feasibility of stabilizing the spine without curettaging the vertebral or discovertebral lesion to cure non-kyphotic AL. Additionally, due to the excellent reunion ability of ankylosing spondylitis, we further came up with minimally invasive spinal surgery (MIS) to avoid the need for both bone graft and lesion curettage in AL surgery. However, there is a paucity of research into the comparison between open spinal fusion (OSF) and early MIS in the treatment of AL. The purpose of this study was to investigate and compare the clinical outcomes and radiological evaluation of our early MIS approach and OSF for AL. Methods. A total of 39 patients diagnosed with AL who underwent surgery from January 2004 to December 2022 were retrospectively screened for eligibility. Patients with AL were divided into an MIS group and an OSF group. The primary outcomes were union of the lesion on radiograph and CT, as well as the visual analogue scale (VAS) and Oswestry Disability Index (ODI) scores immediately after surgery, and at the follow-up (mean 29 months (standard error (SE) 9)). The secondary outcomes were total blood loss during surgery, operating time, and improvement in the radiological parameters: global and local kyphosis, sagittal vertical axis, sagittal alignment, and chin-brow vertical angle immediately after surgery and at the follow-up. Results. Data for 30 patients with AL were evaluated: 14 in the MIS group and 16 in the OSF group. All patients were followed up after surgery; no nonunion complications or instrumentation failures were observed in either group. No significant differences in the VAS and ODI scores were identified between the two groups. Mean ODI improved from 51 (SE 5) to 17 (SE 5) in the MIS group and from 52 (SE 6) to 19 (SE 5) in the OSF group at the follow-up. There were significant improvements in total blood loss (p = 0.025) and operating time (p < 0.001) between the groups. There was also no significant difference in local kyphosis six months postoperatively (p = 0.119). Conclusion. Early MIS is an effective treatment for AL. MIS provides comparable clinical outcomes to those treated with OSF, with less total blood loss and shorter operating time. Our results support and identify the feasibility of solid immobilization achieved by posterior instrumentation without bone graft via MIS for the treatment of AL. Aims. To investigate the correlations among cytokines and regulatory T cells (T-regs) in ankylosing spondylitis (AS) patients, and their changes after anti-tumour necrosis factor-α (TNF-α) treatment. Methods. We included 72 AS patients with detailed medical records, disease activity score (Bath Ankylosing Spondylitis Disease Activity Index), functional index (Bath Ankylosing Spondylitis Functional Index), and laboratory data (interleukin (IL)-2, IL-4, IL-10, TNF-α, interferon (IFN)-γ, transforming growth factor (TGF)-β, ESR, and CRP). Their peripheral blood mononuclear cells (PBMCs) were marked with anti-CD4, anti-CD25, and anti-FoxP3 antibodies, and triple positive T cells were gated by flow cytometry as T-regs. Their correlations were calculated and the changes after anti-TNF-α therapy were compared. Results. The frequency of T-regs in PBMCs was positively correlated to ESR and CRP in AS (r = 0.35 and 0.43; p = 0.032 and 0.027, respectively), and there was also a significant correlation between serum level of TNF-α and CRP (p = 0.041). The frequency of T-regs in PBMCs positively correlated to serum levels of TNF-α, IL-10, and TGF-β, while IL-2, IL-4, and IFN-γ showed opposite results. After anti-TNF-α treatment, there were significantly lower serum levels of TNF-α, IL-10, TGF-β, and frequency of T-regs in PBMCs among these AS patients (p = 0.026, 0.032, 0.029, and 0.037, respectively). Conclusion. In AS patients, proinflammatory cytokine may give positive feedback to induce more T-reg production and anti-inflammatory cytokine secretion to suppress this inflammatory status, and they can be reversed by anti-TNF-α therapy. However, the detailed interactions among T-regs and complex cytokine networks in autoinflammatory diseases still need more studies and further functional assay. Introduction. Pedicle subtraction osteotomy is a powerful technique for correcting sagittal imbalance in ankylosing spondylitis. There has been significant perioperative morbidity associated with this technique in the peer review literature. We present the Royal National Orthopaedic Hospital experience with a single surgeon retrospective study that was conducted to evaluate the outcomes in patients who underwent lumbar pedicle subtraction osteotomy for the correction of thoracolumbar kyphotic deformity in ankylosing spondylitis. Method. Twenty seven patients underwent a lumbar pedicle subtraction osteotomy and adjacent level posterior instrumentation between 1995 and 2010. There were 18 males and 9 females in the study. Events during the peri-operative course and post-operative complications were recorded. The radiological outcome and patient satisfaction were analysed with mean follow-up of one and a half years. Results. The mean operative time was three and half hours and the mean blood loss was 2290mls. Final follow-up radiograph showed an increase in lumbar lordosis angle from 17 degrees to 45 degrees. The sagittal imbalance improved by 85mm with the operation. Complications included loosening in two patients, one transient neurologic deficit and one infective non-union occurred overall. There were no mortalities from the surgery. Two patients developed junctional kyphosis and required a repeat operation. There was an improvement in the Oswestry Disability Score from a mean of 29 to 16 after the surgey. All (100%) of the patients were satisfied with the results of the procedure and would recommend the surgey to others. Conclusion. The study shows that pedicle subtraction extension osteotomy is effective for the correction of kyphotic deformity in ankylosing spondylitis

Aims. The aim of this study was to evaluate the survival of a collarless, straight, hydroxyapatite-coated femoral stem in total hip arthroplasty (THA) at a minimum follow-up of 20 years. Methods. We reviewed the results of 165 THAs using the Omnifit HA system in 138 patients, performed between August 1993 and December 1999. The mean age of the patients at the time of surgery was 46 years (20 to 77). Avascular necrosis was the most common indication for THA, followed by ankylosing spondylitis and primary osteoarthritis. The mean follow-up was 22 years (20 to 31). At 20 and 25 years, 113 THAs in 91 patients and 63 THAs in 55 patients were available for review, respectively, while others died or were lost to follow-up. Kaplan-Meier analysis was performed to evaluate the survival of the stem. Radiographs were reviewed regularly, and the stability of the stem was evaluated using the Engh classification. Results. A total of seven stems (4.2%) were revised during the study period: one for aseptic loosening, three for periprosthetic fracture, two for infection, and one for recurrent dislocation. At 20 years, survival with revision of the stem for any indication and for aseptic loosening as the endpoint was 96.0% (95% confidence interval (CI) 92.6 to 99.5) and 98.4% (95% CI 96.2 to 100), respectively. At 25 years, the corresponding rates of survival were 94.5% (95% CI 89.9 to 99.3) and 98.1% (95% CI 95.7 to 99.6), respectively. There was radiological evidence of stable bony fixation in 86 stems (76.1%) and evidence of loosening in four (3.5%) at 20 years. All patients with radiological evidence of loosening were asymptomatic. Conclusion. The Omnifit HA femoral stem offered promising long-term survival into the third decade. Introduction: The occurrence of bony ankylosis in ankylosing spondylitis (AS) is not precisely known. Bony ankylosis, especially in stiff spine may present several exclusive challenges in its management. The current study is an endeavor to evaluate the clinical and the radiological results of cementless THA in patients with bony ankylosis of hip due to ankylosing spondylitis. Materials and Methods: We retrospectively reviewed 54 patients (92 hips) who underwent cementless total hip arthroplasty for bony ankylosis in ankylosing spondylitis between September 1988 and 2002. Clinical assessment was done at follow-up, which envisages assessment of the pain, function, deformities and range of motion using the Harris Hip Score. Radiographic analysis was done. Kaplan-Meier survivorship analysis was done at 5 and 8.5 years using the revision for the removal of femoral component, acetabular component or both due to any cause as the end point. Results: The mean age of the patients was 25.5 years. The mean duration of follow up was 8.5 years. The average preoperative Harris Hip Score of 49.5 improved to 82.6 post operatively. Post operatively 10 hips had mild to moderate pain. Anterior dislocation occurred in four hips (4.3 %) and sciatic nerve palsy in one hip. Heterotopic ossification was seen in 12 patients, reankylosis rate was 0%. Thirteen arthroplasties were revised due to aseptic loosening. Kaplan-Meier survivorship analysis with revision as end point revealed 98.8% survival at 5 years and 85.8% survival at 8.5 years 11 follow up. Discussion: Cementless THA in osseous ankylosis in ankylosing spondylitis is a worthwhile surgical intervention in bony ankylosis. Newfound mobility, maneuverability and improved ability to sit comfortably were the outcomes, which alleviated the patients’ daunted morale. However, the technically demanding nature of the procedure should not be underestimated

Objective: To emphasize the need to provide a controlled method of intra-operative reduction to correct fixed cervical flexion deformities in ankylosing spondylitis and to describe the technique involved. Design: The treatment of severe fixed cervical flexion deformity in ankylosing spondylitis represents a challenging problem that is traditionally managed by a corrective cervicothoracic osteotomy. The authors describe a method of controlled surgical reduction of the deformity, which eliminates saggital translation and reduces the risk of neurological injury. Subjects: 2 male patients aged 39 and 45 years old with ankylosing spondylitis presented with severe fixed flexion deformity of the cervical spine. Both patients had previously undergone a lumbar extension osteotomy to correct a severe thoracolumbar kyphotic deformity. As a result of the fixed cervical flexion deformity, marked restriction in forward gaze with ‘chin on chest’ deformity, feeding difficulties and personal hygiene were encountered in both. Their respective chin-brow to vertical angle was 60 and 72°. Somatosensory and motor evoked potentials were used throughout surgery. A combination of cervical lateral mass screws and thoracic pedicle screws were used. Interconnecting malleable rods were then fixed at the cervical end, thereby allowing them to slide through the thoracic clamps thus achieving a safe method of controlled closure of the cericothoracic osteotomy. When reduction was achieved, definitive pre-contoured titanium rods were interchanged. Halo-jacket was not considered necessary in view of the segmental fixation used. Results: Good anatomical reduction was achieved, with near complete correction of the deformities, restoration of saggital balances and forward gazes. There were no neurological deficits in either patient and the postoperative recoveries were uneventful. Both osteotomies united with no deterioration noted at 2 years. Conclusions: We illustrate a controlled method of surgical reduction during corrective cervicothoracic osteotomy of fixed cervical kyphosis in ankylosing spondylitis. This has been achieved with the use of a combination of cervical lateral mass screws and thoracic pedicle screws with interconnecting malleable rods that were later replaced with titanium rods. The authors believe that the unique technique described remains a technically demanding but adequate and safe approach for correcting such challenging deformities

Purpose: A retrospective study of 17 fractures of the cervical spine in patients with ankylosing spondylitis is reported. The purpose of this study was to search for risk factors of fracture in ankylosing spondylitis an to assess treatment outcome. Material and methods: Seventeen patients treated between 1982 and 2201 were reviewed with a mean follow-up of five years. There were three women and fourteen men, mean age 60 years at trauma. Fifteen patients underwent surgery and two were treated orthopaedically. Results: This group of patients with ankylosing spondylitis with fracture of the cervical spine was homogeneous: age 60 years, disease duration 30 years, fracture due to fall. The fracture was at the C6/C7 level in 47% of the patients where the lever arm is the greatest and also a level that is difficult to explore, explaining the late diagnosis in 35% of the patients. Sixty percent of the patients were in Frankel classes D or E and 23% in classes A or B. Anterior fixation was used for 14 patients, posterior fixation in one. A long osteosynthesis involving several levels was used in all cases. Major kyphosis had developed in three patients after fracture which was not recognised initially; at fixation, an anterior wedge graft was inserted in the fracture line for correction. Mean correction was 20° with good restoration of the lordosis and rehorizontalization. Bone healing was achieved in all operated patients without loss of the reduction of the kyphosis at last follow-up. The neurological status did not worsen in any patient. Anterior fixation was insufficient to reduced an old fracture-dislocation in one patient who required posterior decompensation. Orthopaedic treatment was used in two patients: the first (Frankel A) died at two months and the second healed with a 10° aggravation of the cervical kyphosis. All the Frankel A and B patients in this series died. Conclusion: All patients with severe neurological involvement died. The anterior approach, used alone, provided good stabilisation of the cervical spine. For the patients without neurological involvement, reduction of the cervical kyphosis should be associated with a stabilisation procedure in case of fracture with kyphosis

Uncemented femoral components have been used in total hip arthroplasties (THAs) for over three decades. Data on long-term performance of hydroxyapatite-coated femoral stems is however limited. This study reports the survivorship of a collarless, straight, hydroxyapatite-coated femoral stem (Omnifit HA, Osteonic) with a minimum of 20 years of follow-up. We reviewed the results of 165 THAs using Omnifit HA in 138 patients performed between August 1993 and December 1999. The mean age at surgery was 46 years (range 20 – 77 years). Avascular necrosis was the most common cause of THA, followed by ankylosing spondylitis and primary osteoarthritis. The mean follow-up was 22 years (range 20–31 years). At 20 and 25 years, 113 arthroplasties (91 patients) and 63 arthroplasties (55 patients) were available for follow-up respectively, while others had deceased or were lost to follow-up. Kaplan-Meier survivorship analysis was performed to evaluate the survival of the femoral component. Radiographs were reviewed at regular intervals, and implant stability was evaluated using Engh classification. Seven out of 165 stems were revised upon the latest follow-up; that included one case of aseptic loosening with revision done at 15 years, three peri-prosthetic fractures, two infections, and one recurrent dislocation. At 20 years, survivorship with revision of the femoral stem for any cause and aseptic loosening as the endpoint was 96.0% (95% CI, 92.6 – 99.5%) and 98.4% (95% CI, 96.2 – 100%) respectively. At 25 years, the corresponding survivorship rates were 94.5% (95% CI, 91.9 – 97.3%) and 98.1% (95% CI, 95.7 – 99.6%) respectively. Radiographic findings of stable bony fixation were seen in 86 stems (76.1%) and those of loosening in 4 stems (3.5%) at 20 years. All patients with radiographic signs of loosening were asymptomatic and did not require revision. The Omnifit HA femoral stem offered promising long-term survivorship into the third decade

Intraoperative fractures although rare are one of the complications known to occur while performing a total hip arthroplasty (THA). However, due to lower incidence rates there is currently a gap in this area of literature that systematically reviews this important issue of complications associated with THA. Method: We looked into Electronic databases including PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), the archives of meetings of orthopaedic associations and the bibliographies of included articles and asked experts to identify prospective studies, published in any language that evaluated intra-operative fractures occurring during total hip arthroplasty from the year 1950-2020. The screening, data extraction and quality assessment were carried out by two researchers and if there was any discrepancy, a third reviewer was involved. Fourteen studies were identified. The reported range of occurrence of fracture while performing hip replacement surgery was found to be 0.4-7.6%. Major risk factors identified were surgical approaches, Elderly age, less Metaphyseal-Diaphyseal Index score, change in resistance while insertion of the femur implants, inexperienced surgeons, uncemented femoral components, use of monoblock elliptical components, implantation of the acetabular components, patients with ankylosing spondylitis, female gender, uncemented stems in patients with abnormal proximal femoral anatomy and with cortices, different stem designs, heterogeneous fracture patterns and toothed design. Intraoperative fractures during THA were managed with cerclage wire, femoral revision, intramedullary nail and cerclage wires, use of internal fixation plates and screws for management of intra operative femur and acetabular fractures. The main reason for intraoperative fracture was found to be usage of cementless implants but planning and timely recognition of risk factors and evaluating them is important in management of intraoperative fractures. Adequate surgical site exposure is critical especially during dislocation of hip, reaming of acetabulum, impaction of implant and preparing the femoral canal for stem insertion. Eccentric and increased reaming of acetabulum to accommodate a larger cup is to be avoided, especially in females and elderly patients as the acetabulum is thinner. However, this area requires more research in order to obtain more evidence on effectiveness, safety and management of intraoperative fractures during THA

Percutaneous vertebroplasty is an effective procedure for the treatment of osteoporotic vertebral compression fractures, spinal metastasis and other pathologic spinal diseases. However, there has been no mention in the relevant literature of the use of percutaneous vertebroplasty for the treatment of spinal pseudarthrosis in ankylosing sponyloarthritis. A 58-year-old male with a long standing ankylosing spondylitis presented with increasing, intolerable and non-intractable back pain. There was a 16- month-old history of a non-significant minor fall. Various radiological imaging technicques showed spinal pseudarthrosis with extensive discovertebral destruction and fracture of the posterior elements at the level T11–T12. Under local anaesthesia, and through a transpedicular approach with the guidance of CT, the cannula of a large bore needle was introduced into the level of spinal pseudarthrosis. Bone cement was then instilled into the affected spinal level. Results were documented by spiral CT and with sagittal reconstructions. Extraosseous cement leakage was seen at the puncture site of the vertebra and in the epidural veins and the paravertebral vessels. However, the patient did not present any immediate or late neurological and systemic complications. Percutaneous vertebroplasty of spinal pseudarthrosis in patients with ankylosing spondylitis is an effective procedure for stabilization of the affected spine segments and pain management

To investigate the effect of bilateral total hip replacement for patients with ankylosed hip joints caused by late ankylosing spondylitis (AS) and to discuss its related pre- and post-operation rehabilitation problems. Data of 20 patients with ankylosed hip joints caused by late AS undergone total hip replacement (40 hips) were reviewed. Among the total 14 patients (28 hips) undergone bilateral total hip replacement, other 6 patients (12 hips) undergone twice operations. We used Harris score, assessment of the joint pain, range of motion to make sure the curative effect of the operative strategy. The mean duration of follow-up was 3. 8 years, all hip joints function was improved, and the flexion deformity of the involved hips were disappeared. The range of hip flexion were 75°–105°(average 86. 2°), and the range of hip extension were 5°–15°(average 8. 7°), the average Harris score was from 32.8 pre-operation improved to 88.2 post-operation, the patients experienced no pain on their hips, the pain of the knee and the lower back complained before the treatment were obviously relieved. Bilateral total hip replacement is an effective treatment for ankylosed hip joint caused by late ankylosing spondylitis, early rehabilitation intervention is useful for the functional recovery of the joints. Protrusio acetabuli can be either primary or secondary. Primary or idiopathic protrusio is a rare condition of unknown etiology. Secondary protrusio may be associated with rheumatoid arthritis, ankylosing spondylitis, osteoarthritis, osteomalacia, trauma and Paget's disease. Challenges in surgery include: lack of bone stock, deficient medial support to the cup, difficulty in dislocating the femoral head, and medialization of the hip joint center. Several surgical techniques have been described: use of cement alone without bone graft; morselised impacted autograft or allograft with a cemented cup; metal cages, reinforcement rings, and solid grafts. We describe our technique of impaction grafting using autologous bone and a cementless porous-coated hemispherical cup without the use of acetabular rings or cages in patients with an average age of 46 years. Protrusion was graded depending on distance of medial wall from Kohler's line as mild (1–5 mm medial), moderate (6–15 mm medial) and severe if it was more than 15 mm medial to the Kohler's line. All patients were operated in the lateral position using a modified Hardinge's anterolateral approach. Adductor tenotomy may be required in cases of severely stiff hips. After careful dislocation of the femoral head, it was sectioned in situ into slivers to facilitate obtaining the graft. The periphery was reamed and care was taken to preserve the membrane lining the floor of the defect. Morselised graft was impacted with hemispherical impactors and the trial cup 1–2 mm larger than the last reamer placed in the desired position. The final socket was then inserted. Femoral preparation was performed in routine fashion. The mean pre-operative Harris Hip Score of 52 improved to 85 points at a mean follow up of 4 years. The average acetabular inclination angle was 42 degrees. Our results have shown incorporation of the graft in all cases. There was no evidence of progression of the protrusio or cup loosening in any of the cases. Thus far, our hips have not shown osteolytic lesions. The technique described is a satisfactory biological solution of restoring bone stock particularly in young and middle-aged patients

Background. Heterotopic ossification (HO) is a known complication following total hip arthroplasty, with increased incidence in certain patient populations. Current prophylaxis options include oral non-steroidal anti-inflammatory drugs (NSAIDs) and radiation therapy, but an optimal radiation protocol has yet to be clearly defined. We performed a randomized, double-blinded clinical trial in high-risk total hip arthroplasty patients to determine the efficacy of 400 cGy versus 700 cGy doses of radiation. Methods. 147 patients at high risk for HO undergoing total hip arthroplasty (THA) at Rush-St. Luke's- Presbyterian medical center were randomized to either a single 400 cGy or 700 cGy dose of radiation. High risk was defined as diagnosis of diffuse idiopathic skeletal hyperostosis (DISH), hypertrophic osteoarthritis, ankylosing spondylitis, or history of previous heterotopic ossification. Radiation was administered over a 14 × 6 cm area of soft tissue and given on the first or second post-operative day. A blinded reviewer graded anterior-posterior (AP) and lateral radiographs taken immediately post-operatively and at a minimum of 6 months post-operatively. Progression was defined as an increase in Brooker classification from the immediate post-operative to the long-term post-operative radiograph. Operative data including surgical approach, use of cemented implants, revision surgery, and post-operative range of motion data were also collected. Results. A significantly greater portion of patients who received the 400 cGy dose demonstrated progression of HO than patients who received the 700 cGy dose. No pre-operative factors were associated with a higher rate of progression. Patients who progressed had less flexion on physical exam than patients who did not progress, but this was not clinically significant. Conclusion. To the authors' knowledge this is the largest randomized prospective study to date comparing two single-dosages of radiation treatment. 700 cGy demonstrates superiority over 400 cGy in preventing HO formation following total hip arthroplasty in high-risk patients and may be the more effective treatment in this population

Introduction. The purpose of this retrospective study was to review the outcome of THA in the treatment of bilateral hip ankylosis of different causes; surgical, septic or spontaneous. Methods & Material. 20 THA procedures in 10 patients were included in the study, 5 males and 5 females all had bilateral fusion. Previous pathologies included: ankylosing spondylitis, AVN, septic arthritis and surgical arthrodesis. Flexion deformity ranged (10°-45°). Shortening as compared to normal anatomy was up to 6 cm and leg length discrepancy (LLD) ranged from 1 cm to 2.5 cm. Most unified X-ray finding was massive osteophytes formation with 3 patients showing severe narrowing of the femoral canal. Operative time averaged 147 minutes (70–210) and lateral approach was used in all patients, anesthesia was general with only 3 undergoing spinal anesthesia. Results. Serious complications were reported and were related to the correction of LLD; 1 incidence of sciatic nerve injury that recovered in one year, and another incidence of femoral nerve injury (sensory > motor) that recovered within 3 months, and one case of incomplete correction of LLD. At 5 years follow up (minimum 6 month), there is no loosening or revision. Discussion. The conversion of bilateral fused hip joints to THA is a very rewarding surgery but with higher risk of complications

Hip resurfacing offers an attractive alternative to conventional total hip arthroplasty in young active patients. It is particularly advantageous for bone preservation for future revisions. Articular Surface Replacement (ASR) is a hip resurfacing prosthesis manufactured by DePuy Orthopaedics Inc. (Warsaw, IN). The manufacturer voluntarily recalled the ASR system in 2010 after an increasing number of product failures. The present study aimed to determine the long-term results in a large cohort of patients who received the ASR prosthesis. Between February 2004 and August 2010, 592 consecutive hip resurfacings using the ASR (DePuy Orthopaedics Inc., Warsaw, IN) resurfacing implant were performed in 496 patients (391 males and 105 females). The mean age of the patients at the time of the surgery was 54 (range: 25 to 74) years. Osteoarthritis was the most common diagnosis in 575 hips (97.1%). The remaining patients (2.9%) developed secondary degenerative disease from ankylosing spondylitis, avascular necrosis, developmental hip dysplasia, and rheumatoid arthritis. Clinical and radiographic information was available for all patients at the last follow up. Cobalt (Co) and chromium (Cr) levels were measured in 265 patients (298 hips) by inductively coupled plasma-mass spectrometry (ICP-MS). The average follow up of the study was 8.6 years (range: 5.2 to 11.6 years). The mean Harris hip and UCLA scores significantly improved from 44 and 2 pre-operatively to 85.3 and 7.1 respectively. The median Co and Cr ion level was 3.81 microgram per liter and 2.15 microgram per liter respectively. Twenty-seven patients (5.4%) were found to have blood levels of both Co and Cr ions that were greater than 7 microgram per liter. Fifty-four patients (9.1%) were revised to a total hip arthroplasty. Kaplan-Meier survival analysis showed a survival rate of 87.1% at 8.6 years with revision for any cause and 87.9% if infection is removed. A significantly higher survival rate was observed for the male patients (90.2%, p <0.0001) and for the patients with ASRs with femoral heads diameters larger than 52 mm (90.1%, p=0.0003). This study confirms that patient selection criteria are of great importance to the overall survivorship of hip resurfacing arthroplasty. Improved clinical results have been reconfirmed with the use of larger diameter femoral heads

Objectives: We investigated the correlation between the laboratory results of the synovial fluid and the clinical features of the affected joints to find which clinical findings are suggestive of septic arthritis. Patients and methods: Retrospective study of 267 joint fluid tests which have been done during the period 2000–2001 from knee hydrathrosis of 256 patients (86 men and 170 women, 19–81 years of age). We found clinical evidence for 183 hydrathrosis (176 patients) due to rheumatoid arthritis 63, psoriatic arthritis 24, reactive arthritis 36, uric acid arthritis 9, ankylosing spondylitis 6 and osteoarthritis 45. We correlated the clinical and the macroscopic and microscopic findings of joint fluid. The statistical analysis was done by x-square test. Results: Septic arthritis supposed to 86 hydrathrosis (46,9%) but the analysis of synovial fluid showed only at 8 (4,37%) p< 0.001. Conclucions: The fever, the macroscopic image of puriform joint fluid, and the combination of warmth with redness are strong evidence for septic arthritis. So if the above findings are present we must wait for Gram stain and culture before the cortisone intraarticular infusion

Introduction. Total hip arthroplasty (THA) is becoming a common procedure because it dramatically relieves the patient's pain. Accordingly, young patients who hope to take THAs are also increasing. The results of THAs in the young patients have historically shown relatively poor survivorship. However several improvements of components may promise good long term results in active young patients. There was no consensus about the appropriate time for performing THA. The purpose of this study is to review the results of THAs younger than 40 years. Material and methods. A retrospective review of 14 primary THAs in 12 patients was conducted. The mean age was 27.0 (range 17–30). Ten were female (83%) and 2 were male (17%). The average follow –up period was 38.3 months (range 18–64). All operations were performed using direct anterior approach (DAA). All patients were allowed full weight bearing as tolerated in the immediate postoperative periods. All were cementless THA and the articulation bearing were ceramic/ceramic in 5 hips (33.3%), metal/highly cross linked polyethylene in 8 hips (53.3%), and ceramic/ highly cross linked polyethylene in 2 hips (13.3%). All patients were followed clinically and radiographically. Patients were evaluated at 4,6,12 weeks and each year postoperatively. Patients were allowed sports at 3 months postoperatively without limitations. Results. The underlying etiology was steroid-induced osteonecrosis in 5 patients, alcohol-induced osteonecrosis in 2 patients, developmental dysplasia in 2 patients (1 post osteotomy and 2 conservative therapy), infection in 1 patients, ankylosing spondylitis in 1 patients, and epiphyseal dysplasia in 1 patients. All patients failed conservative treatment. Two patients had prior osteotomy surgeries. Preoperative average JOA score, which is 100 points in full score, was 45 points. Those scores improved to 99 points post operatively. All patients became to walk without any assistive devices. There were no infections and dislocations. Their radiographs showed no loosening, subsidence, and osteolysis. All of their radiographs showed well fixed osseous integrations. Discussion and conclusion. Although our patients were very small numbers and follow up periods were not long, all of our patients showed good clinical and radiological results. When conservative treatments failed to relieve their pain, surgical treatments are required to restore their highly active lives. The modern THA has been showed numerous good results in older generations. With modern implants and surgical technique, we can also expect promising results in young patients under the condition with careful and long term follow-up

Protrusio acetabuli can be either primary or secondary. Primary or idiopathic protrusio is a rare condition of unknown etiology. Secondary protrusio may be associated with rheumatoid arthritis, ankylosing spondylitis, osteoarthritis, osteomalacia, trauma and Paget's disease. Challenges in surgery include lack of bone stock, deficient medial support to the cup, difficulty in dislocating the femoral head, and medialisation of the hip joint center. Several surgical techniques have been described: use of cement alone without bone graft; morsellised impacted autograft or allograft with a cemented cup; metal cages, reinforcement rings, and solid grafts. We describe our technique of impaction grafting using autologous bone and a cementless porous-coated hemispherical cup without the use of acetabular rings or cages in patients with an average age of 46 years. Protrusion was graded depending on distance of medial wall from Kohler's line as mild (1–5mm medial), moderate (6–15mm medial) and severe if it was more than 15mm medial to the Kohler's line. All patients were operated in the lateral position using a modified Hardinge's anterolateral approach. Adductor tenotomy may be required in cases of severely stiff hips. After careful dislocation of the femoral head, it was sectioned in situ into slivers to facilitate obtaining the graft. The periphery was reamed and care was taken to preserve the membrane lining the floor of the defect. Morsellised graft was impacted with hemispherical impactors and the trial cup 1–2mm larger than the last reamer placed in the desired position. The final socket was then inserted. Femoral preparation was performed in routine fashion. The mean preoperative Harris hip score of 52 improved to 85 points at a mean follow up of 4 years. The average acetabular inclination angle was 42 degrees. Our results have shown incorporation of the graft in all cases. There was no evidence of progression of the protrusio or cup loosening in any of the cases. Thus far, our hips have not shown osteolytic lesions. The technique described is a satisfactory biological solution of restoring bone stock particularly in young and middle-aged patients
