Background. Tension band epiphysiodesis for lower limb length discrepancy in children Planned physeal growth arrest (epiphysiodesis) for the treatment of limb length discrepancy (LLD) in growing children is a well described treatment modality in the literature. We describe our experience of temporary epiphysiodesis using a tension band technique with the “8-plate” in the treatment of LLD in growing children. Aim. The main objective of this study was to confirm whether bilateral 8-plates achieve an epiphysiodesis or not?. Methods and results. This is a prospective study of 27 patients who were treated with 8-plate epiphysiodesis for limb length discrepancy with a mean follow up of 28 months. Perthes disease was the most common underlying pathology for the LLD. The average preoperative LLD was 25.9 mm (15–49 mm). 17 patients successfully corrected to < 15 mm LLD, 5 patients corrected to between 15–20 mm and 5 patients did not correct to with in 15 mm LLD (22.2%). In those patients whom have corrected, the average correction length was 25.6 months with an average correction rate of 1.52 mm per month. There was a trend for insufficient equalisation if the procedure was performed < 1.5 years prior to skeletal maturity. The was also a trend for insufficient equalisation if performed at single physis only (femur or tibia). Complications included one superficial infection and one deep infection following plate removal at the end of treatment. Screw breakage was noticed in one patient. No long term complications were reported. No angular deformity was reported. Conclusion. This study has confirmed that bilateral 8 plates produce an epiphysiodesis. Failures are mainly due to late insertion or single physis usage. Future application depends on demonstrating reversibility when applied to younger children

Aims. Temporary epiphysiodesis (ED) is commonly applied in children and adolescents to treat leg length discrepancies (LLDs) and tall stature. Traditional Blount staples or modern two-hole plates are used in clinical practice. However, they require accurate planning, precise surgical techniques, and attentive follow-up to achieve the desired outcome without complications. This study reports the results of ED using a novel rigid staple (RigidTack) incorporating safety, as well as technical and procedural success according to the idea, development, evaluation, assessment, long-term (IDEAL) study framework. Methods. A cohort of 56 patients, including 45 unilateral EDs for LLD and 11 bilateral EDs for tall stature, were prospectively analyzed. ED was performed with 222 rigid staples with a mean follow-up of 24.4 months (8 to 49). Patients with a predicted LLD of ≥ 2 cm at skeletal maturity were included. Mean age at surgery was 12.1 years (8 to 14). Correction and complication rates including implant-associated problems, and secondary deformities as well as perioperative parameters, were recorded (IDEAL stage 2a). These results were compared to historical cohorts treated for correction of LLD with two-hole plates or Blount staples. Results. The mean LLD was reduced from 25.2 mm (15 to 45) before surgery to 9.3 mm (6 to 25) at skeletal maturity. Implant-associated complications occurred in 4/56 treatments (7%), and secondary frontal plane deformities were detected in 5/45 legs (11%) of the LLD cohort. Including tall stature patients, the rate increased to 12/67 legs (18%). Sagittal plane deformities were observed during 1/45 LLD treatments (2%). Compared to two-hole plates and Blount staples, similar correction rates were observed in all devices. Lower rates of frontal and sagittal plane deformities were observed using rigid staples. Conclusion. Treatment of LLD using novel rigid staples appears a feasible and promising strategy. Secondary frontal and sagittal plane deformities remain a potential complication, although the rate seems to be lower in patients treated with rigid staples. Introduction. Epiphysiodesis, defined as the process of closing the growth plate (physis), have been used for several years as a treatment option of cases where the predicted leg-length discrepancy (LLD) falls between 2 to 5 cm. The aim of this study was to systematically review the existing literature on the effectiveness of three different epiphysiodesis techniques with implant usage for the treatment of leg-length discrepancy in the pediatric population. The secondary aim was to address the reported complications of staples, tension-band plates (TBP) and percutaneous epiphysiodesis screws (PETS). Method. This systematic review was performed according to PRISMA guidelines. We searched MEDLINE (PubMed), Embase, Cochrane Library, Web of Science and Scopus for studies on skeletally immature patients with LLD treated with epiphysiodesis with an implant. The extracted outcome categories were effectiveness of epiphysiodesis (LLD measurements pre/post-operatively, successful/unsuccessful) and complications that were graded on severity. Result. Forty-four studies (2184 patients) were included, from whom 578 underwent TBP, 455 PETS and 1048 staples. From pooled analysis of the studies reporting success rate, 64% (150/234) successful TBP surgeries (10 studies), 78% (222/284) successful PETS (9 studies) and 52% (212/407) successful Blount staples (8 studies). Severe complications rate was 7% for PETS, 17% for TBP and 16% for Blount staples. TBP had 43 cases of angular deformity (10%), Blount staples 184 (17%) while PETS only 18 cases (4%). Conclusion. Our results highlighted that PETS seems to be the most successful type of epiphysiodesis surgery with an implant, with higher success rate and lower severe complications than TBP or Blount staples

Introduction. Legg-Calvé-Perthes disease (LCPD) often results in femoral head deformity and leg length discrepancy (LLD). Objective of this study was to analyse femoral morphology in LCPD patients at skeletal maturity to assess where the LLD originates, and evaluate the effect of contralateral epiphysiodesis for length equalisation on proximal and subtrochanteric femoral lengths. Materials and Methods. All patients treated for LCPD in our institution between January 2013 and June 2020 were retrospectively reviewed. Patients with unilateral LCPD, LLD of ≥5mm and long leg standing radiographs at skeletal maturity were included. Total leg length, femoral and tibial length, articulotrochanteric distance (ATD) and subtrochanteric femoral length were compared between LCPD side and unaffected side. Furthermore, we compared leg length measurements between patients who did and who did not have a contralateral epiphysiodesis. Results. 79 patients were included, 21/79 underwent contralateral epiphysiodesis for leg length correction. In the complete cohort the average LLD was 1.8cm (95% CI 1.5 – 2.0), average ATD difference was 1.8cm (95% CI −2.1 – −1.9) and average subtrochanteric difference was −0.2cm (95% CI −0.4 – 0.1). In the epiphysiodesis group the average LLD before epiphysiodesis was 2.7 (1.3 – 3.4) cm and 1.3 (−0.5 – 3.8) cm at skeletal maturity. In the non-epiphysiodesis group the average LLD was 2.0 (0.5 – 5.1), p=0.016. The subtrochanteric region on the LCPD side was significantly longer at skeletal maturity in the epiphysiodesis group compared to the non-epiphysiodesis group: −1.0 (−2.4 – 0.6) versus 0.1 (−1.0 – 2.1), p<0.001. Conclusions. This study concludes that LLD after LCPD originates from the proximal segment only. In patients who had had a contralateral epiphysiodesis, the subtrochanteric femoral region was significantly longer on the LCPD side. These anatomical changes need to be considered by paediatric surgeons when advising leg length equalisation procedures, and by arthroplasty surgeons when LCPD patients present for hip arthroplasty

Introduction. To evaluate the results of correction of knee deformities based on deformity analysis in Achondroplasia, the commonest skeletal dysplasia as some have concomitant ligamentous deformities. Materials and Methods. Retrospective study from a prospective database (2007–2020) of achondroplasts who underwent growth modulation. Analysis of medical records with objective measurement of mechanical axis radiographs was done (Traumacad). Satisfactory alignment was defined as neutral to slightly varus (0–15 mm MAD) so that the MCL/LCL laxity is not revealed. Results. 23 patients, 41 limbs, 34 bilateral, 6 unilateral underwent multiple growth modulation procedures. 2 had valgus knees. 15 patients underwent proximal fibular epiphysiodesis in addition for LCL laxity with one isolated fibular epiphysiodesis. Mechanical axis deviation (MAD) improved or normalised in 16 patients (70%). 4 patients were still undergoing correction. 4 patients needed further surgery out of which 2 patients were over 13 years when growth modulation was attempted and 2 needed correction of ankle varus. JLCA improved/ normalised in 12 patients (75%) with evidence of indirect LCL tightening and no improvement was seen in 4. The rate of correction was MAD 0.61mm/month, LDFA 0.29°/month and MPTA 0.13°/month; expectedly lower in achondroplasia due to lower growth velocity. Conclusions. This study highlights the pathology, application of growth modulation as per deformity analysis unlike previous studies. Proximal fibular epiphysiodesis improves LCL laxity in a majority of these children and is a simple procedure compared to our published series with indirect LCL tightening with frames

Aim. To investigate the effect of the eight plate position in sagittal plane on tibial slope in temporary epiphysiodesis technique applied to the proximal tibia and whether there is a rebound effect after removing the plate. Method. Forty New Zealand rabbits (6 weeks old) were divided into four groups. In all groups, two 1.3 mm mini plates and cortical screws implantation were placed on both medial and lateral side of the proximal epiphysis of the right tibia. In Group 1 and 3, the plates were placed on anterior of the proximal tibial anatomical axis in the sagittal plane, and placed posteriorly in Group 2 and 4. The left tibia was examined as control in all groups. Group 1 and Group 2 were sacrificed after four week-follow-up. In Group 3 and Group 4, the implants were removed four weeks after index surgery and the rabbits were followed four more weeks to investigate the rebound effect. The tibial slope was measured on lateral X-rays every two weeks. Both medial and lateral plateau slopes were evaluated on photos of the dissected tibia. Results. In Group 1, right MTPA (medial tibial plateau angle) and left MTPA, right LTPA (lateral tibial plateau angle) and left LTPA, and right 4wTPPA (the tibial proximal posterior angle at 4th week) and left 4wTPPA values were compared with each other. There was a significant difference in MTPA, LTPA, and 4wTPPA in Group 1 (p: 0.003, 0.006, 0.004). In Group 1, the medial and lateral slope significantly decreased after 4 weeks. There was no significant difference in MTPA, LTP and 4wTPPA measurements in Group 2 (p= 0.719, 0.306, 0.446, respectively). In Group 2, the slope did not change in four weeks. There was a significant difference in MTPA, LTPA, 4wTPPA, and 8wTPPA (tibial proximal posterior angle at 8th week) in Group 3 (p= 0.005, 0.002, <0.001, <0.001, respectively). In Group 3, the slope decreased at 4th week and remained stabile during the next four week-follow up and no rebound effect was observed. There was no significant difference in MTPA, LTPA, 4wTPPA, and 8wTPPA measurements in Group 4 (p= 0.791, 0.116, 0.232, 0.924), respectively. In group 4, slope did not change at 4th week of index surgery and no rebound effect was observed in the next four week-follow up. Conclusion. If eight plates were placed on anterior of lateral proximal tibia axis on both medial and lateral side, the tibial slope would reduce, and remain stabile after implant removal. Care should be taken to place the plates on the line of proximal tibial axis in sagittal plane in temporary epiphysiodesis technique performed due to angular knee deformities. Changing the slope due to plate placement can be used as a secondary gain for patients who will benefit from slope change, such as adolescent ACL surgery

Aim: To review our experience with epiphysiodesis using three different methods to correct LLD and to establish the efficacy of these procedures. Method: A retrospective review of 42 patients from 1999 to 2008 with at least one year follow-up recorded type and location of the epiphysiodesis, average operating time and hospital stay, complications, method of prediction, timing and the final LLD. CT scanograms and mechanical axis view with grids were used to assess LLD. Results: Epiphysiodesis was as per Canale for 26, by Metaizeau screw in 14 and by staples in 2. Average operation time was 42 minutes for Canale type, 45 minutes for the screws and 56 minutes for the staple cases. The pre operative LLD of 3.7 cms In the Canale group, improved to 1.2 cms over an average follow-up of 2.1 yrs. There were 4 minor and 2 major complications with a 92% success rate. For the screw group, the mean change was 1.8 cms over 2.2 yrs with 2 minor and 2 major complications giving a success rate of 85%. With staples the success rate was 100% and the mean change was 1.8 cms at an average of 2.3 yrs. In 14 cases where bone age reports were available, the multiplier method seemed better at predicting estimated LLD at skeletal maturity and timing of epiphysiodesis than the Moseley chart. Conclusions: Percutaneous epiphysiodesis by any method is reliable, minimally invasive and with acceptable complication rate when compared to a corrective osteotomy or open Phemister-type epiphysiodesis. Our experience suggests that the Canale method has the least complications and best success rate. Paleys multiplier method was better at predicting LLD and timing of epiphysiodesis than the Moseley Chart

Introduction: Percutaneous epiphysiodesis is a well established procedure in the treatment of leg length discrepancy. Many techniques have been described ranging from an open technique to the more recently described percutaneous technique. This study assesses the percutaneous single portal technique, in combined distal femoral and proximal tibial lower limb epiphysiodesis, performed by a single surgeon. Methods: We performed a retrospective review of cases performed in a single institution by a single surgeon from 1994 to present. A total of 45 combined epiphysiodesis were performed. 40 patients qualified for the study group with at least 2 years follow up. There were 19 female and 21 male patients, with the operative side equally shared between left and right. Results: The mean predicted leg length discrepancy using the Mosley Straight Line Graph was 2.43 cm. The mean final leg length discrepancy, at an average follow up of 31 months, was 1.5 cm with a range of 0 to 2.81 cm. There were no angular deformities at follow up. One female patient had a knee effusion which resolved spontaneously. One male patient complained of anterior knee pain initially post surgery however this resolved at final follow up without treatment. The majority of patients (n=34) were inpatients, however more recently this procedure has been successfully carried out as a day case (n=6). Discussion: Percutaneous epiphysiodesis has been accepted as a standard technique to treat leg length discrepancy of 2 cm to 5 cm. Several techniques have been described in the literature with varying complication rates. This study shows that single portal combined epiphysiodesis is successful and has a relatively low complication rate

Purpose: Partial epiphysiodesis of the growth plate due to physeal aggression is a common problem in paediatric patients. Surgical management requires precise imaging. We recall other imaging techniques currently employed and describe a novel method for studying the characteristic features of epiphysiodesis bridges of the growth plate: 3D-magnetic resonance imaging (3D-MRI). Material and methods: We analysed retrospectively MRI series of 27 epiphysiodesis bridges in 23 children (ten boys and thirteen girls) aged 11.3 years (range 2.5 – 15). We recorded information concerning the cause of the physeal aggression, the joint involved, the type of bony bridge (Ogden classification), the clinical deformation, and the proposed treatment. The 27 bridges were studied on coronal MRI acquired with echo-gradient and fat suppression sequences. Data were processed with a manual 3D reconstruction program in 15 minutes to precisely define the localisation, the volume, and the morphology of the bony bridge and the active physis. Results: The epiphysiodeses were caused by trauma (65%), iatrogenic aggression (17%), ischemia-infection (purpura fulminans) (9%), juxta-physeal essential cyst (4.5%), and unknown causes (4.5%). Eighty-seven percent involved a lower limb joint, 75% of which involved the tibia. The surface of the epiphysiodesis bridge covered 20% of the physis. The bridges were peripheral (46.5%), central (46.5%), and linear (7%). Discussion: It is difficult to determine the position and the 3D relations of an epiphysiodesis bridge in a healthy active physis with imaging techniques such as plain x-rays, scintigraphy, tomography and computed tomography. The 3D-MRI method described here provides a sure way to distinguish the active growth plate which gives a high intensity signal and the epiphyseal bridge which gives a low intensity signal. Morphological (size, form) and topographic characteristics of the bony bridge and the physis can be described with precision facilitating therapeutic decision making and guiding surgery. The lack of radiation risk is also an advantage of MRI. Conclusion: The quality of the images obtained, the safety of MRI and the easy interpretation of 3D reconstructions makes this imaging technique an excellent method for pre-therapeutic analysis of epiphysiodesis bridges

Summary Statement. Bilaretal epiphysiodesis of he neurocentral cartilages causes shortening of the sagittal length of the pedicles and a subsequent spinal stenosis at the operated segments, resembling that found in patients with achrondroplasia. Introduction. The introduction of pedicle screws in the immature spine may have implications for the growth of the vertebra. The effect of blocking the growth of neurocentral cartilage (NC) is not yet fully defined. Block hypothetically leads to a bilateral symmetrical alteration of the vertebral growth. Using an experimental animal model, our goal is to analyze if a bilateral epiphysiodesis of the NC using pedicle screws is able to induce narrowing of the spinal canal in the thoracolumbar spine. Experimental animals and Methods. A total of 24 domestic pigs were operated on by bilateral blocking of the NC using pedicle screws. The animals were divided into 4 groups depending on the level of blockage: A, low thoracic levels; B, thoracolumbar transitional hinge; C, upper lumbar spine; and D, blocking of the caudal lumbar level below L5 segment. Different morphological, morphometric and standard radiological parameters were analyzed at the thoracic and lumbar vertebrae of the animals. The deviation from the physiological parameters was established by comparing all parameters obtained in the NC-blocked animals with those acquired in 14 pigs without NC blocking. These animals were considered as the control group. Results. None of the animals that underwent NC epiphysiodesis showed asymmetrical spinal growth inducing deformities in the coronal plane. There was neither rotation nor wedging of the vertebral bodies. Whatever the level involved, NC epiphysiodesis caused shortening of the sagittal length of the pedicles and a subsequent decreasing of the antero-posterior diameter of the spinal canal. These features resulted in a frank spinal stenosis at the operated levels. However, the transverse diameter of the spinal canal was conserved in the coronal plane. In the sagittal plane, blocking of the neurocentral cartilage conditioned a lumbar hyperlordosis with compensatory kyphosis of the upper level to the operated vertebra. Conclusions. Symmetrical growth arresting of neurocentral cartilages induces a narrow spinal canal by decreasing the sagittal diameter similar to that observed in patients with achondroplasia. The most affected structure was the development of the vertebral pedicles

Purpose: Physeal distraction can be used for the treatment of the consequences of epiphysiodesis bridges, correcting simultaneously angular deformations and length discrepancy. Material and methods: Chondrodiastasis was performed in six children aged 13.1 years (range 10.4–15.7). The cause of the epiphysiodesis was trauma in three children (2 distal tibia, 1 distal radius), osteomyelitis in two (distal femur), and surgical sequela of a clubfoot (distal tibia). Mean follow-up was two years (18 months – 4 years). An Ilizarov device was used in four cases and an Orthofix in two. Results: Limb length discrepancy was corrected in all cases. Angular correction was insufficient in two. Distraction was continued for four months (1–9) and total duration of treatment was 7.5 months (4–13). Minor complications were pin track infection (n=2) and joint stiffness (n=3). Major complications were one fracture of the femur on a pin site and premature closure of the growth cartilage treated by callotasis and one fracture after removal of the external fixator, treated by plaster cast immobilisation. The final outcome was good in both of these children. Discussion: De-epiphysiodesis with surgical resection of the bony bridge can only be performed before a certain age (10–11 years) and the outcome in uncertain. The principle advantages of physeal distraction applied for angular deformation in growing patients is that it avoids the need for osteotomy and allows progressive correction. This noninvasive method allows angular correction in the upright patient, concomitant lengthening is also achieved. Chondrodiastasis makes an exact correction of the deformation without resection of the bony bride which can be ruptured by simple distraction. The fertility of the growth cartilage after distraction must be considered as lost so the amount of correction must be calculated on the basis of a complete and definitive postoperative epiphysiodesis. Conclusion: Chondrodiastasis allows correction of acquired and predictable epiphysiodesis bridges at the apex of the deformation and without direct access. This method can be used for partial epiphysiodesis (less than 50%) in children who have not reached maturity. After distraction, the growth cartilage must be considered as definitively closed

Introduction. The objective of this study is to report the first cases of femoral lengthening in children using Ilizarov fixator. Materials and Methods. We carried out a retrospective study about the cases of femoral lengthening done in 2010 to 2020 in our BARI-ILIZAROV Orthopaedic centre Dhaka. Results. 48 lengthening were done during this period using Ilizarov fixator. The procedure was done incongenital bone diseases in 20 cases and after a distal femoral epiphysiodesis in 10 cases. The mean age at surgery was 12.8 years. Lengthening was required in all patients and an axis correction was required in 16 of 26 cases. The mean lengthening was 5.9 cm. The healing index was 45.5 day/cm (25.5–62). We noticed 8 knee stiffness and 5 broken wires. Knee Stiffness were corrected by Judet'squadricepsplasty and 6 broken wires were replaced by new wires. The goal of lengthening was reached in all cases. The goal of axis correction was reached in 98.5% of cases. Conclusions. Ilizarov technique allows to do accurate lengthening and axis correction and it is a unique reliable external fixator for femoral lengthening in children

Introduction. Leg length discrepancy (LLD) in patients with unilateral developmental dysplasia of the hip (DDH) can be problematic for both patients and surgeons. Patients can acquire gait asymmetry, back pain, and arthritis. Surgical considerations include timing of correction and arthroplasty planning. This study audits standing long leg films performed at skeletal maturity in our patients. The aim of this study is to identify if surgical procedure or AVN type could predict the odds of needing an LLD Intervention (LLDI) and influence our surveillance. Materials and Methods. Hospital database was searched for all patients diagnosed with DDH. Inclusion criteria were patients with appropriately performed long leg films at skeletal maturity. Exclusion criteria were patients with non DDH pathology, skeletally immature and inadequate radiographs. All data was tabulated in excel and SPSS was used for analysis. Traumacad was used for measurements and AVN and radiologic outcome grades were independently classified in duplicate. Results. 110 patients were identified. The mean age of follow-up was 15 years with final average LLD of 1mm(±5mm). The DDH leg tended to be longer and length primarily in the femur. 31(28.2%) patients required an LLDI. 19 Patients had a final LLD >1.5cm. There was no statistical significant difference in the odds of needing an LLDI by type of surgical procedure or AVN. AVN type 4 was associated with greatest odds of intervention. The DDH leg was more likely to require ipsilateral epiphysiodesis or contralateral lengthening in Type 1 and 2 AVN. Conclusions. The DDH leg tends to be longer, leg lengths should be monitored, and leg length interventions are frequently required irrespective of previous DDH surgical procedure or the presence of AVN

The 8-plate (Orthofix, SRL, Italy) is a titanium extraperiosteal plate with 2 screws which acts as a hinge at the outer limits of the physis. It has been used for correction of both angular and sagittal deformity around the knee. To our knowledge this is the first study describing the use of 8-plates in leg length discrepancy (LLD) correction. We aimed to evaluate outcomes of temporary 8-plate epiphysiodesis in LLD, and to assess the complications associated with its usage. This retrospective study included 30 patients between 2007 and 2010 whom underwent 8-plate epiphysiodesis to address LLD. Leg length measurements were recorded using erect full leg length scanograms and comparison made between pre-operative, interval and final scanograms. Any deviations of the mechanical axis were also recorded. During the study period 34 epiphysiodeses were performed on 30 patients. There were 17 males and 14 females. The average age at the time of procedure was 10.7 years (range 3–15). Average time to final follow-up was 24 months (range 52–10). The average pre-operative LLD was 2.5 cm (range 1.5–6 cm). The mean overall rate of correction was 1.0 cm per year. The mean residual LLD at end of treatment was 1.1 cm (range 0–4.5 cm). Two patients experienced genu recurvatum deformity. This was associated with placement of distal femoral plates anterior to the mid-lateral line. Based on our experience 8-plate epiphysiodesis is a reversible, minimally invasive procedure with reliable results in length correction. However, careful device placement is required to prevent deformity

Purpose: Surgical epiphysiodesis is one technique used to correct lower limb length discrepancy. Methods described include: in situ graft (Phemister, 1993), stapling (Blount, 1949), percutaneous curettage (Bowen, 1984). The purpose of this work was to evaluate a new technique described in 1998 (Metaizeau) which uses two percutaneous transphyseal screws. Material and methods: Forty-two patients (29 boys, 13 girls), mean age 13.1 years underwent the procedure. The cause of leg length discrepancy was unknown (n=12), fracture (n=16), congenital (n=7), other (n=7). Epiphysiodesis using two percutaneous screws was performed on the distal femur (n=24), the proximal tibia (n=7), both (n=11). Stance radiograms were obtained of the lower limbs before the intervention and at last follow-up to measure length of the lower limb, the tibia, and the femur. Difference with the healthy limb was determined as well as the percentage of growth comparing the healthy and epiphysiodesis sides. The operative time, duration of hospital stay and complications were studied. Results: Preoperatively, mean limb length discrepancy was 22.3 mm (10 to 70); at skeletal maturity, the difference measured 11 mm (28 to −20). Mean percent growth from epiphysiodesis to last follow-up was 3.15% for the epiphysidesis side and 6.26% for the contralateral side. Mean operative time was 20 min per bone (15–40). Complication rate was 16% including 7% stiff knee postoperatively with total recovery in two weeks, and 9% discomfort due to the presence of the screws. The growth curves showed that the epiphysiodesis was effective before three months. Mean hospital stay was 1.3 days (1–4). Discussion: The final outcome in terms of leg length discrepancy were comparable with other techniques. The rate of complications appears to be more favourable since there were no infections, no frontal or sagittal deviations, no vascular or nerve injuries, and since all complications resolved without sequelae. This intervention can be proposed as an outpatient procedure. Epithysiodesis is always obtained within three months. Conclusion: Epiphysiodesis using a percutaneous transphyseal screw is a simple method with minimal complications which provides reliable results and many advantages compared with other methods

The aims of this study were to compare the outcome of epiphysiodesis in patients with limb length discrepancy (LLD) as a result of cerebral palsy with those as a result of other causes in order to test our hypothesis that the hemiplegic / monoplegic limb may respond differently to epiphysiodesis, to evaluate the accuracy of the Moseley method and evaluate whether there is any difference between the outcomes of left or right hemiplegic limbs with LLD bearing in mind that the left hand is used for bone age calculations. We reviewed the case notes and radiographs of 34 children who had undergone epiphysiodesis for the management of LLD by the same surgeon, using the Moseley method between February 1999 and May 2005 to final follow up at skeletal maturity. Of the 34 patients, 9 had a LLD as a result of cerebral palsy (4-Left, 5-Right) and 25 as a result of other causes. In the cerebral palsy group the mean residual LLD was 0.59cm and in the other group it was 1.18cm. Both groups were similar in terms of age and sex distribution. There was no demonstrable statistically significant difference in outcome between the 2 groups (unpaired T test, P=0.734). The Moseley method appeared accurate and there was no difference demonstrated in the outcome between left and right hemiplegic LLD. We conclude that the Moseley method is reliable. We have not found any evidence that the hemiplegic limb behaves any differently. We have not demonstrated any difference in the outcome of left or right hemiplegic limbs

We evaluated the use of percutaneous screw epiphysiodesis to treat genu valgum deformity in adolescents, and the possibilities of extending its use to younger patients with different causes of angular deformities or leg length discrepancies. To date, the surgical options for adolescent idiopathic genu valgum have been medial physeal retardation by stapling, growth arrest by epiphysiodesis of the distal femur and/or tibia, or osteotomy. From September 1999, we prospectively studied 16 patients, 11 of whom had angular knee deformities (20 legs) and five limb length inequality. From a preoperative mean of 12.25( the tibiofemoral angle reduced to 6.4° at the latest assessment. Percutaneous epiphysiodesis using transphyseal screws proved to be a reliable method with few complications and the advantages of simplicity, short operating times, rapid postoperative rehabilitation and reversibility

The results of 34 knees with stage IV to VI Blount’s disease were reviewed. 24 patients were treated over the seven-year period from 1994 to 2000. The surgical technique addressed the medial joint line depression with an elevating osteotomy maintained with a tricortical wedge from the iliac crest. The tibial varus and intorsion was corrected with an osteotomy proximal to the apophysis. In the more recent patients a proximal lateral tibial and fibular epiphyseodesis was done concomitantly. The average preoperative mechanical varus angle of 30.6°(range 14° to 60°) was corrected to 0–4° mechanical valgus in 29 knees. In five knees an undercorrection of 2–4° mechanical varus occurred. At follow-up a further 8 knees developed residual varus due to a delayed epiphyseodesis. The tibial varus angle (ie. angle subtended by the mechanical axis of the tibia with the lateral tibial joint line) increased at an average of 1°/ month due to the inevitable medial growth plate fusion. The average pre operative joint depression angle of 49° (range 40° to 60°) was corrected to an average of 26°(mean 20°–30°). There was no significant preoperative frontal plane f emoral deformity to warrant a femoral correction. At long term follow up of 3.5 years (range 2 to 5 years) all knees had a full range of movement without any varus instability. However in eight cases a delay of more than six months occurred before a lateral epiphysiodesis was performed, and in these patient’s mechanical axis varus recurred although the joint line correction was maintained. Neutral or valgus mechanical axis was maintained in all patients who underwent an epiphysiodesis within six months. We concluded that although the joint elevation correction was maintained in our series, mechanical axis varus recurs if lateral epiphysiodesis is not performed early
