Abstract. Introduction. Osteotomy is recognised treatment for osteoarthritis of the knee. Evidence suggests favourable outcomes when compared to arthroplasty, for younger and more active individuals[1]. Double level osteotomy (DLO) is considered when a single level is insufficient to restore both joint line obliquity and adequate realignment[2]. This paper aims to establish the functional outcomes up to two years post operatively for patients undergoing DLO, using patient reported outcome measures (PROMs). Methodology. All patients who underwent a DLO at either Lister Hospital, Stevenage, or One Hatfield Hospital, Hertfordshire, between 1st January 2018 and 1st October 2020 were identified. DLO were performed by two specialist consultants, independently or in combination. PROMs including pain scores, health score, Oxford knee score (OKS) and knee injury and osteoarthritis outcome score (KOOS) were recorded pre-operatively and at six month, one and two year post operative intervals. Anterior cruciate ligament (ACL) graft failure from rupture, attenuation, or malposition may cause recurrent subjective instability and objective laxity, and occurs in 3% to 22% of ACL reconstruction (ACLr) procedures. Revision ACLr is often indicated to restore knee stability, improve knee function, and facilitate return to cutting and pivoting activities. Prior to reconstruction, a thorough clinical and diagnostic evaluation is required to identify factors that may have predisposed an individual to recurrent ACL injury, appreciate concurrent intra-articular pathology, and select the optimal graft for revision reconstruction. Single-stage revision can be successful, although a staged approach may be used when optimal tunnel placement is not possible due to the position and/or widening of previous tunnels. Revision ACLr often involves concomitant procedures such as meniscal/chondral treatment, lateral extra-articular augmentation, and/or osteotomy. The use of high tibial osteotomy (HTO) to delay total knee arthroplasty (TKA) in young patients with osteoarthritis (OA) and constitutional deformity remains debated. The aim of this study was to compare the long-term outcomes of TKA after HTO compared to TKA without HTO, using the time from the index OA surgery as reference (HTO for the study group, TKA for the control group). This was a case-control study of consecutive patients receiving a posterior-stabilized TKA for OA between 1996 and 2010 with previous HTO. A total of 73 TKAs after HTO with minimum ten years’ follow-up were included. Cases were matched with a TKA without previous HTO for age at the time of the HTO. All revisions were recorded. Kaplan-Meier survivorship analysis was performed using revision of metal component as the endpoint. The Knee Society Score, range of motion, and patient satisfaction were assessed.Aims
Methods
There is little information about how to manage patients with a recurvatum deformity of the distal tibia and osteoarthritis (OA) of the ankle. The aim of this study was to evaluate the functional and radiological outcome of addressing this deformity using a flexion osteotomy and to assess the progression of OA after this procedure. A total of 39 patients (12 women, 27 men; mean age 47 years (28 to 72)) with a distal tibial recurvatum deformity were treated with a flexion osteotomy, between 2010 and 2015. Nine patients (23%) subsequently required conversion to either a total ankle arthroplasty (seven) or an arthrodesis (two) after a mean of 21 months (9 to 36). A total of 30 patients (77%), with a mean follow-up of 30 months (24 to 76), remained for further evaluation. Functional outcome, sagittal ankle joint OA using a modified Kellgren and Lawrence Score, tibial lateral surface (TLS) angle, and talar offset ratio (TOR) were evaluated on pre- and postoperative weight-bearing radiographs.Aims

We aimed to investigate factors related to the technique of medial
opening wedge high tibial osteotomy which might predispose to the
development of a lateral hinge fracture. A total of 71 patients with 82 osteotomies were included in the
study. Their mean age was 62.9 years (37 to 80). The classification
of the type of osteotomy was based on whether it extended beyond
the fibular head. The level of the osteotomy was classified according

In arthritis of the varus knee, a high tibial
osteotomy (HTO) redistributes load from the diseased medial compartment
to the unaffected lateral compartment. We report the outcome of 36 patients (33 men and three women)
with 42 varus, arthritic knees who underwent HTO and dynamic correction
using a Garches external fixator until they felt that normal alignment
had been restored. The mean age of the patients was 54.11 years
(34 to 68). Normal alignment was achieved at a mean 5.5 weeks (3
to 10) post-operatively. Radiographs, gait analysis and visual analogue
scores for pain were measured pre- and post-operatively, at one
year and at medium-term follow-up (mean six years; 2 to 10). Failure
was defined as conversion to knee arthroplasty. Pre-operative gait analysis divided the 42 knees into two equal
groups with high (17 patients) or low (19 patients) adductor moments.
After correction, a statistically significant (p <
0.001, At final follow-up, after a mean of 15.9 years (12 to 20), there
was a survivorship of 59% (95% CI 59.6 to 68.9) irrespective of
adductor moment group, with a mean time to conversion to knee arthroplasty
of 9.5 years (3 to 18; 95% confidence interval ± 2.5). HTO remains a useful option in the medium-term for the treatment
of medial compartment osteoarthritis of the knee but does not last

The aims of this study were to report functional
outcomes of salvage procedures for patients with cerebral palsy (CP)
who have chronic dislocation of the hip using validated scoring
systems, and to compare the results of three surgical techniques. We reviewed 37 patients retrospectively. The mean age at the
time of surgery was 12.2 years (8 to 22) and the mean follow-up
was 56 months (24 to 114). Patients were divided into three groups:
14 who underwent proximal femoral resection arthroplasty (PFRA group
1), ten who underwent subtrochanteric valgus osteotomy (SVO group 2),
and 13 who underwent subtrochanteric valgus osteotomy with resection
of the femoral head (SVO with FHR group 3). All patients were evaluated
using the Caregiver Priorities and Child Health Index of Life with
Disabilities (CPCHILD) and the Pediatric Quality of Life Inventory
(PedsQL). Significant improvements occurred in most CPCHILD and PedsQL
subsection scores following surgery in all patients, without significant
differences between the groups. There were 12 post-operative complications.
Less severe complications were seen in group 1 than in groups 2
and 3. Salvage surgery appears to provide pain relief in patients with
CP who have painful chronic dislocation of the hip. The three salvage
procedures produced similar results, however, we recommend the use
of PFRA as the complications are less severe. Take home message: Salvage surgery can be of benefit to patients
with CP with chronic painful hip dislocation, but should be limited

The objective of this study was to validate the
efficacy of Takeuchi classification for lateral hinge fractures
(LHFs) in open wedge high tibial osteotomy (OWHTO). In all 74 osteoarthritic
knees (58 females, 16 males; mean age 62.9 years, standard deviation
7.5, 42 to 77) were treated with OWHTO using a TomoFix plate. The
knees were divided into non-fracture (59 knees) and LHF (15 knees)
groups, and the LHF group was further divided into Takeuchi types
I, II, and III (seven, two, and six knees, respectively). The outcomes
were assessed pre-operatively and one year after OWHTO. Pre-operative
characteristics (age, gender and body mass index) showed no significant
difference between the two groups. The mean Japanese Orthopaedic
Association score was significantly improved one year after operation
regardless of the presence or absence of LHF (p = 0.0015, p <
0.001, respectively). However, six of seven type I cases had no
LHF-related complications; both type II cases had delayed union;
and of six type III cases, two had delayed union with correction
loss and one had overcorrection. These results suggest that Takeuchi
type II and III LHFs are structurally unstable compared with type

Between 2003 and 2007, 99 knees in 77 patients
underwent opening wedge high tibial osteotomy. We evaluated the effect
of initial stable fixation combined with an artificial bone substitute
on the mid- to long-term outcome after medial opening-wedge high
tibial osteotomy (HTO) for medial compartmental osteoarthritis or
spontaneous osteonecrosis of the knee in 78 knees in 64 patients
available for review at a minimum of five years (mean age 68 years;

Introduction:. High tibial osteotomy (HTO) is a common treatment for medial compartment arthritis of the knee in younger, more active patients. The HTO shifts load away from the degenerative medial compartment and into the lateral compartment. This change can be accomplished with either a lateral closing or a medial opening wedge HTO. An HTO also potentially affects leg length. Mathematical models predict that the osteotomy type (opening versus closing) and the magnitude of the correction determine the change in leg length, but no in vivo studies have been published. The purpose of this study is to quantify and compare leg length change following opening and closing wedge HTO. Study Design:. Retrospective cohort study – Level III evidence. Methods:. Thirty-two medial opening and 32 lateral closing HTO's were selected from patients treated at our institution between 2006 and 2009. Pre-operative and one-year post-operative full-length lower extremity radiographs were obtained along with operative reports. Pre- and post-operative coronal plane alignment and leg length were measured and surgical details were collected. Results:. The 64 osteotomies were performed in 62 patients (43 male, 19 female) at an average age of 57 years. The mean opening wedge was 9.3 mm (range: 5 to 17 mm) and the mean closing wedge was 8.0 mm (range: 6 to 10 mm). Knee alignment changed from a mean of 174 degrees pre-operatively to a mean of 183 degrees post-operatively in both groups. In the medial opening wedge group, total leg length was found to increase from 836.3 ± 63.5 mm pre-operatively to 841.8 ± 64.1 post-operatively, a change of 5.5 ± 4.4 mm (p < 0.0001). We report the outcome of 32 patients (37 knees) who underwent hemicallostasis with a dynamic external fixator for osteoarthritis of the medial compartment of the knee. There were 16 men (19 knees) and 16 women (18 knees) with a mean age at operation of 54.6 years (27 to 72). The aim was to achieve a valgus overcorrection of 2° to 8° or mechanical axis at 62.5% (± 12.5%). At a mean follow-up of 62.8 months (51 to 81) there was no change in the mean range of movement, and no statistically significant difference in the Insall-Salvati index or tibial slope (p = 0.11 and p = 0.15, respectively). The mean hip-knee-ankle angle changed from 190.6 (183° to 197°) to 176.0° (171° to 181°), with a mean final position of the mechanical axis of 58.5% (35.1% to 71.2%). The desired alignment was attained in 31 of 37 (84%) knees. There were 21 excellent, 13 good, two fair and one poor result according to the Oxford knee score with no correlation between age and final score. This score was at its best at one year with a statistically significant deterioration at two years (p = 0.001) followed by a small but not statistically significant deterioration until the final follow-up (p = 0.17). All the knees with Ahlback grade 1 osteoarthritis had excellent or good results. Complications included pin tract infections involving 16.4% of all pins used, delayed union in two, knee stiffness in four, fracture of the lateral cortex in one and ring sequestrum in one. In conclusion, hemicallostasis provides precision in attaining the desired alignment without interfering with tibial slope or patellar height, and is relatively free of serious complications.
High tibial osteotomy (HTO) is a surgical procedure that involves cutting the proximal tibia in an attempt to change the weight bearing axis from the medial to the lateral compartment of the knee. There is an increasing trend towards performing a high tibial osteotomy (HTO) with a medial opening wedge technique. Amongst the potential advantages is easier conversion to a total knee replacement (TKR), although literature regarding this is scarce. We report our early clinical and radiological results, along with the technical issues encountered. From our database, we identified thirty-one patients who had undergone thirty-two TKRs after medial opening wedge HTOs. Clinic and operative records were reviewed, and our database used for knee society scores. Those patients not seen in the last twelve months were reviewed in clinic. All pre- and postoperative radiographs were reviewed. The average age of the patients was fifty-seven years, and the average time from HTO to TKR was 4.5 years. Difficulties with patella eversion were seen in 12.5%, and the knee balanced easily in 85% of cases. The tibia had minimal deformity in most cases, and all osteotomies were healed at the time of conversion. A stem was used in ten cases, and only if metalwork was removed at the time of conversion (which was done in twenty-three of the thirty-two). Minimum one year follow-up was available for twenty-six patients, with a mean follow-up time of twenty-eight months (range twelve to ninety-three months). The average Knee Society knee score was eighty-eight, function score seventy-five, and total score one hundred and sixty-two. Compared to non-matched group of 1149 primary TKRs (of older average age), the knee score was significantly lower, and there was a higher percentage of patients in the poor category. Poor results correlated strongly to the presence of chronic pain or workers compensation issues, and also to whether the knee had undergone revision HTO prior to TKR. We conclude that the conversion of a medial opening wedge HTO to a TKR is relatively straightforward technically, and some aspects are easier than performing a TKR after a
