Aims. Unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) have both been shown to be effective treatments for osteoarthritis (OA) of the knee. Many studies have compared the outcomes of the two treatments, but less so with the use of robotics, or individualized TKA alignment techniques. Functional alignment (FA) is a novel technique for performing a TKA and shares many principles with UKA. Our aim was to compare outcomes from a case-matched series of robotic-assisted UKAs and robotic-assisted TKAs performed using FA. Methods. From a prospectively collected database between April 2015 and December 2019, patients who underwent a robotic-assisted medial UKA (RA-UKA) were case-matched with patients who had undergone a FA robotic-assisted TKA (RA-TKA) during the same time period. Patients were matched for preoperative BMI, sex, age, and Forgotten Joint Score (FJS). A total of 101 matched pairs were eligible for final review. Postoperatively the groups were then compared for differences in patient-reported outcome measures (PROMs), range of motion (ROM), ability to ascend and descend stairs, and ability to kneel. Results. Both groups had significant improvements in mean FJS (65.1 points in the TKA group and 65.3 points in the UKA group) and mean Oxford Knee Score (OKS) (20 points in the TKA group and 18.2 in the UKA group) two years following surgery. The UKA group had superior outcomes at three months in the OKS and at one year in ROM (5°), ability to kneel (0.5 points on OKS question), and ascend (1.3 points on OKS question) and descend stairs (0.8 points on OKS question), but these were not greater than the minimal clinically important difference. There were no differences seen in FJS or OKS at one year postoperatively. There were no statistically significant differences between the groups at 24 months in all the variables assessed. Conclusion. FA-RATKA and RA-UKA are both successful treatments for medial compartmental knee arthritis in this study. The UKA group showed a quicker recovery, but this study demonstrated equivalent two-year outcomes in all outcomes measured including stair ascent and descent, and kneeling. Aims. The free latissimus dorsi muscle (LDM) flap represents a workhorse procedure in the field of trauma and plastic surgery. However, only a small number of studies have examined this large group of patients with regard to the morbidity of flap harvest. The aim of this prospective study was therefore to objectively investigate the morbidity of a free LDM flap. Methods. A control group (n = 100) without surgery was recruited to assess the differences in strength and range of motion (ROM) in the shoulder joint with regard to handedness of patients. Additionally, in 40 patients with free LDM flap surgery, these parameters were assessed in an identical manner. Results. We measured higher values for all parameters assessing force in the shoulder joint on the dominant side of patients in the control group. Moreover, LDM flap harvest caused a significant reduction in strength in the glenohumeral joint in all functions of the LDM that were assessed, ranging from 9.0% to 13.8%. Equally, we found a significantly reduced ROM in the shoulder at the side of the flap harvest. For both parameters, this effect was diminished, when the flap harvest took place on the dominant side of the patient. Conclusion. LDM flap surgery leads to a significant impairment of the strength and ROM in the shoulder joint. Moreover, the donor morbidity must be differentiated with regard to handedness: harvest on the non-dominant side potentiates the already existing difference in strength and ROM. Conversely, if the harvest takes place on the dominant side of the patient, this difference is diminished. Cite this article: Bone Jt Open 2024;5(12):1114–1119

Aims. Arthroplasty has been shown to generate the most waste among all orthopaedic subspecialties, and it is estimated that hip and knee arthroplasty generate in excess of three million kg of waste annually in the UK. Infectious waste generates up to ten times more CO2 compared with recycled waste, and previous studies have shown that over 90% of waste in the infectious stream is misallocated. We assessed the effect of real-time waste segregation by an unscrubbed team member on waste generation in knee and hip arthroplasty cases, and compared this with a simple educational intervention during the ‘team brief’ at the start of the operating list across two sites. Methods. Waste was categorized into five categories: infectious, general, recycling, sharps, and linens. Each category was weighed at the end of each case using a digital weighing scale. At Site A (a tertiary orthopaedic hospital), pre-intervention data were collected for 16 total knee arthroplasy (TKA) and 15 total hip arthroplasty (THA) cases. Subsequently, for ten TKA and ten THA cases, an unscrubbed team member actively segregated waste in real-time into the correct streams. At Site B (a district general hospital), both pre- and post-intervention groups included ten TKA and ten THA cases. The intervention included reminding staff during the ‘team brief’ to segregate waste correctly. Results. Active real-time waste segregation reduced infectious waste by a mean of 2.51 kg (95% CI 1.492 to 3.542) in TKA, and 1.83 kg in THA cases (p = 0.004). Educational intervention reduced infectious waste by a mean of 3.52 kg in TKA and 2.09 kg in THA cases (p = 0.026). Total waste was significantly reduced in both groups post-intervention for TKA cases. Conclusion. Simple educational measures alone can significantly reduce the amount of infectious waste. Extrapolated nationally, our results would yield a reduction of approximately 315,004 kg to 594,577 kg of CO2 annually, which equates to 70 to 132 gasoline-powered passenger vehicles driven for a year. Aims. The involvement of long non-coding RNA (lncRNA) in bone marrow mesenchymal stem cell (MSC) osteogenic differentiation during osteoporosis (OP) development has attracted much attention. In this study, we aimed to disclose how LINC01089 functions in human mesenchymal stem cell (hMSC) osteogenic differentiation, and to study the mechanism by which LINC01089 regulates MSC osteogenesis. Methods. Quantitative reverse transcription polymerase chain reaction (RT-qPCR) and western blotting were performed to analyze LINC01089, miR-1287-5p, and heat shock protein family A (HSP70) member 4 (HSPA4) expression. The osteogenic differentiation of MSCs was assessed through alkaline phosphatase (ALP) activity, alizarin red S (ARS) staining, and by measuring the levels of osteogenic gene marker expressions using commercial kits and RT-qPCR analysis. Cell proliferative capacity was evaluated via the Cell Counting Kit-8 (CCK-8) assay. The binding of miR-1287-5p with LINC01089 and HSPA4 was verified by performing dual-luciferase reporter and RNA immunoprecipitation (RIP) experiments. Results. LINC01089 expression was reinforced in serum samples of OP patients, but it gradually diminished while hMSCs underwent osteogenic differentiation. LINC01089 knockdown facilitated hMSC osteogenic differentiation. This was substantiated by: the increase in ALP activity; ALP, runt-related transcription factor 2 (RUNX2), osteocalcin (OCN), and osteopontin (OPN) messenger RNA (mRNA) levels; and level of ARS staining. Meanwhile, LINC01089 upregulation resulted in the opposite effects. LINC01089 targeted miR-1287-5p, and the LINC01089 knockdown-induced hMSC osteogenic differentiation was repressed by miR-1287-5p depletion. HSPA4 is a downstream function molecule of the LINC01089/miR-1287-5p pathway; miR-1287-5p negatively modulated HSPA4 levels and attenuated its functional effects. Conclusion. LINC01089 negatively regulated hMSC osteogenic differentiation, at least in part, via governing miR-1287-5p/HSPA4 signalling. These findings provide new insights into hMSC osteogenesis and bone metabolism. Cite this article: Bone Joint Res 2024;13(12):779–789

Objective. We aimed to analyse the clinical outcomes and survivorship of anatomic total shoulder arthroplasty using a stemless humeral component with cemented pegged polyethylene glenoid performed with the technique of eccentric reaming to partially correct retroversion. These results were then compared with TSA using the same implant for end-stage shoulder arthritis with a normal version of the native glenoid. Design and methods. A retrospective case series was performed using a prospectively collected database of anatomic TSA patients operated at Woodend General Hospital, Aberdeen, UK. Between 2010 and 2019, 107 total shoulder arthroplasties (TSA) were done using standard anatomic stemless TSA implants (Affinis Short, Mathys Ltd, Bettlach, Switzerland) in 98 patients. Standardized preoperative and postoperative shoulder radiological imaging for glenoid retroversion was collected. Depending on the angle of native glenoid version, patients were divided into retroverted and non-retroverted glenoid as per the Walch Classification. To assess the radiological outcome at the final follow-up, radiolucency was assessed on the glenoid and humeral side using the Lazarus grading. The final clinical and radiologic outcome from the retroverted group was compared with the population with a non-retroverted glenoid. Five TSAs were excluded from the analysis as they did not have satisfactory postoperative radiographs. Hence, a total of 102 shoulders were available for analysis. Results. The mean follow-up was 3.48 years (2-10.2 years) in the retroverted group (n=44) and 3.9 years (2-8.9 years) in the non-retroverted group (n=58). The mean pre-operative retroversion of the glenoid in the retroverted group was 20.18, and the post-operative retroversion was 15.87, with a mean correction of 4.31. There was no significant difference between the two groups in the percentage of radiological loosening. The mean Oxford shoulder score was 41.4 (16-48) in the retroverted group, while it was 42.1 (20-48) in the non-retroverted group. Three patients in the retroverted group required revision surgery for rotator cuff failure. There were no revisions for aseptic loosening or instability. Conclusion. The degree of severity of retroversion of the glenoid was not associated with poor clinical outcomes, revisions, or failure in stemless TSA. At medium-term follow-up, partial correction of retroversion seems to provide comparable outcomes compared to a non-retroverted glenoid

Objectives. Our aim to study the incidence, demographics, inpatient stay, use of imaging and outcomes of patients who have non-operatively managed NOF fractures. Study Design & Methods. The data was collected retrospectively for the last 14 years (Jan 2009- Jan 2023) of all non-operatively managed NOF fractures at a level 2 trauma centre. The data was collected from the trauma board, electronic patient records, radiographs, and National Hip Fracture Database (NHFD). The data collected as demographic details, fracture classification, any reasons for non-operative management, mortality and further surgical management was done. Patients who died or transferred to other sites for specialist surgery were excluded. Results. Our results showed that 1.7% (62/4132) of NOF fractures were managed non-operatively at our institution. The reasons for non-operative management were as follows, medically unwell 45% (n = 28) in whom operative risk was thought to outweigh benefit and risk of death was high within the 48 - 72 hrs of admission. The second group patients, minimal or no pain and old fractures with comfortable mobilisation 55% (n = 34). Out of 34 /62 patients who were mobilised, 14% (5/34) of this patient subgroup subsequently required surgical intervention for failed non-operative management. In the medically unwell group (28/62) the 30-day mortality was 32% (12/28) with a 1-year mortality rate of 42% (17/28). The patients who were mobilised (34/62) the 30-day mortality was 6% (4/34) with a 1-year mortality rate of 20% (5/34). The combined average 1 year mortality for this cohort was 35% (22/62). Conclusions. Our study showed higher mortality rate for medically unwell group 42%. The patients whose fractures are stable enough to allow them to mobilise comfortably showed 20% 1-year mortality rate and 14% surgical intervention needed in 14 % (5/34) patients. We note that an analysis is necessary to assess the functional outcomes of this subgroup, as well as the potential cost implications. The combined average 1 year mortality for this cohort was 35% (22/62) which was due to the high-risk factors in medically unwell group

Objectives. Distal humerus fractures in elderly population are often comminuted and presents multiple challenges. When managed with fixation the results are often complicated with stiffness, pain, and reduced function while total elbow replacement is fraught with lifelong restrictions on load bearing which can significantly affect elderly patients who rely assistive devices for ambulation. Elbow hemiarthroplasty however, presents a viable alternative in these cases. This series evaluates the results of the distal humerus hemiarthroplasty in a single centre. Designs and methods. Data was collected retrospectively. Comprehensive case reviews were done to collect demographic, intra-operative and post-operative data. Fractures were classified as per AO classification. The primary outcome was functional outcome and range of motion (ROM). The secondary outcomes were complications and patient satisfaction. Results. 22 cases were included in this study from 2016 – 2023 majority being females (n=18). Mean age was 74years (SD −11.2). Majority fractures were AO type C3 (n=15) with two A2 type fractures managed for non-union. Pre-existing bone on bone arthritis was present in 3 cases and CT scan was done for 12 patients to assess fracture configuration. Excluding those treated for non- union the median time between fracture and surgery was 8.5 days. Follow-up of more than 6 months was available for 16 patients with median follow-up of 18 months. ROM was available for 17 patients with 13 (50%) patients having functional ROM. No patients had full extension and there was a mean lag of 20. 0. Mean flexion was 118.2. 0. Nearly 2/3. rd. patients (73%- n=16) were satisfied with the surgery with no problems in activity of daily living. Complications seen were ulnar neuropathy (n=3), heterotopic ossification(n=2) and periprosthetic fracture (n=1). Conclusion. This single centre study suggests good outcomes following distal humerus hemiarthroplasty. It is a reasonable alternative to fixation and total elbow arthroplasty in complex fractures

Objectives. Several studies have reported elevated blood cobalt (Co) and chromium (Cr) concentrations in patients with total knee replacements (TKRs). Up to 44% of tissue samples taken from patients with failed TKRs exhibit histological evidence of metal sensitivity/ALVAL. In simulated conditions, metal particles contribute approximately 12% of total wear debris in TKR. We carried out this investigation to determine the source and quantity of metal release in TKRs. Design and Methods. We analysed 225 explanted fixed-bearing TKRs (Attune, Genesis II, NexGen, PFC, and Vanguard) revised for any indication. These were analysed using peer-reviewed [coordinate measuring machine (CMM)] methodology to measure the volumetric wear of the polyethylene (PE) bearing surfaces and trays. The trays were analysed using 2D profilometry (surface roughness-Ra) and light microscopy. Histological and blood metal ion concentration analyses were performed in a sub-sample of patients. Results. The median (IQR) PE wear rate was 14 (6 to 20) mm. 3. /year. Microscopic examination of the superior surface of trays exhibited pitting on 132 (59%) of trays. There was a statistically significant (p<0.05) increase in Rvk on the pitted area of trays for each design, indicating material removal from the pits compared to the unpitted area. The inferior surface of 116(51%) of trays displayed polishing, indicative of abrasive wear. The median(range) Co and Cr concentrations were 2.5µg/l (0.2–69.4) and 1.7µg/l (0.5-12.5) respectively in 40 patients. Of the tissue samples examined in 30 patients, 6 had at least “mild”-ALVAL infiltrate. All corresponding “ALVAL” explants were found to be pitted and/or show evidence of loosening of the tray. Conclusion. This study provides further evidence that CoCr release in TKR appears to be an under-appreciated cause of adverse clinical outcomes. The generation of metal particles was predominantly from the metal tray, which may explain elevated metal ions after TKRs, despite no direct metal-on-metal contact

Introduction. Instability in ACL deficient knees can lead to medial compartment osteoarthritis. The risk of developing significant OA is 5x higher in knees with ACL deficiency. In associated Varus, there is quicker progression of the medial OA along with a varus thrust exerting strain on the ACL graft. The simultaneous valgus HTO and ACL reconstruction decompresses the medial tibiofemoral joint, corrects the mechanical-axis and reduces strain on the graft. Outcomes for this simultaneous procedure are still unclear in literature and we attempt evaluating its functional outcome. Methods. This Panel study was performed using data from 2019 to 2022 on 21 patients who had ACL insufficiency with Varus or medial OA and underwent a simultaneous Opening-wedge HTO with Arthroscopic ACL reconstruction. The mean follow-up was 2 years. The patients were evaluated with IKDC and Lysholm scores, Lachman test and ROM pre and post-operatively. The HKA was compared pre and post-operatively and the complications were evaluated. The progression of OA was evaluated with serial radiographs post-operatively. Results. There was a significant improvement in lifestyle and knee joint function post-operatively. The mechanical femorotibial angle was corrected from an average of 8.2° Varus to 0.8° Valgus. There was a significant improvement in IKDC and Lysholm scoring (IKDC score improved to 86.20 from 34.48 and the Lysholm score improved to 89 points from 37 points). There was significant improvement in the laxity which was evaluated by Lachman test. One patient had a clinical progression of medial-OA. No patients had non-union, graft or implant failure. Conclusion. Single stage HTO and ACL Reconstruction in patients with medial OA or Varus with ACL insufficiency is an option showing a satisfactory functional and radiological outcome along with activity scores

Objectives. Sacroiliac joint dysfunction is a degenerative condition that can result in low back pain and is likely underdiagnosed. Diagnosis is made clinically with the patient experiencing pain in the sacroiliac joint region. Initial management is non-operative with pain management, physiotherapy, injections, and rhizolysis. If these fail then surgical management, by sacroiliac joint fusion, can be considered. The aim of this study was to review the outcomes of all patients who underwent sacroiliac joint fusion by a single surgeon in a large district general hospital between April 2018 and April 2023. Design and Methods. A retrospective review of all patients who underwent sacroiliac joint fusion between April 2018 and April 2023 was conducted. Data was collected from clinical letters, operative notes, and the British Spinal Registry. Oswestry Disability Index (ODI) and Visual Analogue Scale (VAS) for back and leg pain were recorded as well as any post-operative complications. Results. In total 19 patients underwent sacroiliac joint fusion. Mean age was 47 years (range 27 – 69 years). Nine were right sided procedures and ten were left. The mean BMI was 32.3. ODI improved from a mean of 55 pre-operative to 26 at one year and 15 at two years post-operative. VAS for back pain improved from a mean of six pre-operative to three at one year and one at two years post-operative. VAS for leg pain improved from a mean of five pre-operative to four at one year and zero at two years post-operative. There were no surgical site complications. One patient developed trochanteric bursitis post-operatively. Two patients have since undergone sacroiliac joint fusion on the contralateral side with a further patient awaiting contralateral surgery. Conclusion. In patients with sacroiliac joint pain where non-operative measures have failed to control symptoms sacroiliac joint fusion is a reliable and effective surgical option

Introduction & Aim. During TKR it is mandatory to achieve perfect soft tissue balance and component alignment. It is necessary to access all tight structures for proper releases. We aim to analyze the results of Trivector arthrotomy approach for TKRs. Methods. It is a retrospective study of 1050 cases between 2010-2020. All cases were performed by a single surgeon. Approach includes dividing distal 30% of vastus medialis along with medial retinaculum - 1cm medial to patella and patellar tendon distally up to Tibial tuberosity and raised as a single flap. Results. 656 cases Varus + FFD, 305 Varus, 60 Valgus, 24 hyperextension deformity & 5 neutral alignment cases were included. Results showed 87% patients at postop day-1 and 96% by day-4 regained ability to perform unassisted SLR. 4% had 5-to-10-degree quadriceps lag at discharge & recovered to neutral by 4 weeks. The surgical field was adequate in all cases. KSS score improved from Pre op of 56 (38-71) to a post op of 89 (84-95). All patients by day 10 were walking unaided or with a single cane in case of Bilateral TKRs. Medial parapatellar arthrotomy divides the quadriceps tendon. The alteration in various vectors of Quadriceps can change the balance and laterally mal-track the patella. Incidence of Lateral release is higher with this. Mid and subvastus approaches are non-extensile and have poor visibility during surgery increasing risk of malalignment. Trivector arthrotomy is extensile and retains 70% strength of vastus medialis. At closure, the quads mechanism is perfectly aligned, reducing the incidence of lateral mal-tracking and lateral release. Conclusion. The extensile nature of the approach and minimal disruption of the quadriceps mechanism encourages us to use this approach for all cases. It is a true “Gateway.”

Objectives. Unicompartmental knee replacement (UKR) is a conservative option for degenerative disease, with mobile (Oxford UKR) and fixed bearing (Physica ZUK) the most commonly used devices. The primary reasons for revising UKRs include disease progression (36.9%), loosening (31.7%), and pain (7.5%). Loosening typically occurs due to osteolysis caused by wear particles from the polyethylene (PE) inserts. There is limited published literature which has quantified volumetric material loss from the PE inserts of cemented fixed-bearing UKRs. This study aimed to quantify bearing wear and backside deformation of these PE components. Design and Methods. At our national retrieval centre, we measured changes volumetric bearing wear and backside deformation of 31 explanted fixed-bearing Physica ZUK UKR PE inserts using a peer-reviewed methodology based on coordinate measuring machine analysis. These explants had been revised for any indication [Females (19) and Males (12)]. The location of the wear scars was identified and mapped. We compared the volumetric wear from the bearing surface with contemporary total knee replacement (TKR) inserts. Results. The median (IQR) total volumetric wear of the UKR PEs [96(50-152) mm. 3. ] was significantly higher (p<0.001) than contemporary TKR PEs [40 (24-83) mm. 3. ]. The median (IQR) total volumetric wear of the PEs in males [148(122-200) mm. 3. ] was significantly higher(p=0.016) than in females [56(61-119) mm. 3. ]. The wear scars were situated in the anterior third in 61% of cases, with 26% located posteriorly. Examination of the PE backside surfaces exhibited damage in the area directly inferior to the bearing surface wear scar in the majority of cases. Conclusion. In conclusion, this study highlights significant PE wear of Physica ZUK UKR inserts, with higher wear rates compared to contemporary TKR inserts, particularly in males

Objectives. To determine whether patients with scoliosis, treated with or without surgery, are at higher risk of needing caesarean section. To determine whether patients with scoliosis, treated with or without surgery, have increased intra-partum obstetric analgesic requirements. Design and Methods. Retrospective cohort study wherein obstetric outcomes were analysed in women with scoliosis in a tertiary teaching hospital. Women with scoliosis were identified using the high-risk obstetric anaesthesia register. Data was collected between May 2013 to April 2023. Results. We identified 39 women with corrected scoliosis. 38 patients in the scoliosis correction group and 112 patients in the uncorrected scoliosis group were analysed. Within the corrected group, 11 patients underwent lower segment caesarean section, for obstetric reasons. There were 5 GA's administered in the group, all in patients that had rods finishing at or below L3 level. All other patients underwent successful regional anaesthesia. BMI ranged between 17 and 30, with only two patients with a BMI more than 30. In the uncorrected group, 34 underwent LSCS, all for obstetric indications. There were 2 GA administrations for failure of epidural top up. Six patients underwent two LSCS in this period and one patient underwent three. The rest of LSCSs were under regional anaesthesia. BMI range in this group was between 18 to 30 with only two patients having a BMI between 31 to 36. In the corrected group, 16 patients had no analgesia during labour and in the uncorrected group 39 had no analgesia for labour. Epidural requirement in the corrected group was 23% and in the uncorrected group was 33%. Conclusion. In our cohort, the rate of LSCS was 28% and 30% in the corrected and uncorrected scoliosis groups respectively. Our unit's average rate of LSCS in the non-scoliosis women has increased over the decade, from 28% to 45% in line with the national data. In conclusion, the need for LSCS were comparable or even lesser in the scoliosis groups and administration of regional anaesthesia or a general anaesthesia was determined by the level of lowest instrumented vertebra

Introduction. Video recording to teach and assess both technical and non-technical skills is well-established within medical education. Trainees’ clinical and practical competencies are evaluated using Procedure-Based Assessments (PBAs). However, there is limited research describing how these PBAs truly reflect trainee performance. We sought to:. 1). assess the duration between the procedure and PBA completion. 2). assess the perceived viability of supplementing assessments using intra-operative camera footage and. 3). clarify medico-legal considerations for the use of cameras in theatre. Method. We undertook a survey of Orthopaedic trainees in the East of England Deanery, United Kingdom. A six-item questionnaire was designed and provided to trainees (paper and online) to assess the time between procedure and filling in PBA forms, level of consultant input, time to PBA sign-off and trainees’ views on current PBA methods, operative video recording and retrospective access to clinical footage. Results. The survey response rate was 75%. 60% felt current PBAs do NOT allow them to highlight their strengths and weaknesses; a higher percentage felt retrospective access to a video recording would aid reflective practice (87.5%) and completion of PBAs (70%). 49% stated they did not complete their PBAs with their Consultant. Conclusion. This paper highlights potential limitations in existing forms of trainee assessment and feedback. We suggest use of trainees’ clinical footage to evaluate skills and performance, enhance feedback & learning in PBAs which has resonated well with trainees, the intended beneficiary. We also consider the medicolegal implications of cameras in operative training, with possible limitations to their adoption in current practice

Aims. Mesenchymal stem cells (MSCs) are usually cultured in a normoxic atmosphere (21%) in vitro, while the oxygen concentrations in human tissues and organs are 1% to 10% when the cells are transplanted in vivo. However, the impact of hypoxia on MSCs has not been deeply studied, especially its translational application. Methods. In the present study, we investigated the characterizations of human umbilical cord-derived mesenchymal stem cells (hUC-MSCs) in hypoxic (1%) and normoxic (21%) atmospheres with a long-term culture from primary to 30 generations, respectively. The comparison between both atmospheres systematically analyzed the biological functions of MSCs, mainly including stemness maintenance, immune regulation, and resistance to chondrocyte apoptosis, and studied their joint function and anti-inflammatory effects in osteoarthritis (OA) rats constructed by collagenase II. Results. We observed that long-term hypoxic culture surpassed normoxic atmosphere during hUC-MSCs culture in respect of promoting proliferation, anti-tumorigenicity, maintaining normal karyotype and stemness, inhibiting senescence, and improving immunoregulatory function and the role of anti-apoptosis in chondrocytes. Furthermore, we demonstrated that the transplantation of long-term hypoxic hUC-MSCs (Hy-MSCs) had a better therapeutic effect on OA rats compared with the hUC-MSCs cultured in the normoxic atmosphere (No-MSCs) in terms of the improved function and swelling recovery in the joints, and substantially inhibited the secretion of pro-inflammatory factors, which effectively alleviated cartilage damage by reducing the expression of matrix metallopeptidase 13 (MMP-13). Conclusion. Our results demonstrate that Hy-MSCs possess immense potential for clinical applications via promoting stemness maintenance and enhancing immunoregulatory function. Objectives. Minimally invasive surgery (MIS) has gained popularity for hallux valgus, compared to the traditional scarf osteotomy (OS). Though evidence suggests similar clinical outcomes, there is paucity of randomised controlled studies. This study aimed to assess the feasibility of conducting a randomised controlled trial comparing the patient recorded and clinical outcomes for the surgical management of Hallux Valgus between OS and MIS Chevron Akin (MICA). Methods. Patients suitable for surgical correction were invited to participate. Post-op rehabilitation was standardised for both groups. Patients completed a validated questionnaire (Manchester Oxford Foot questionnaire and EQ-5D-5L) pre-operatively and post-operatively at 6 months and 1 year. Radiological parameters and range of motion were measured pre-and post-operatively. Results. 31 patients were recruited between Dec 2017 and June 2022. 17 patients were randomised to MICA (15 female, mean age 51) and 15 to OS (14 female, mean age 51). Both groups had a significant improvement in all MOXFQ parameters at 6m and 12m, as well as radiological parameters. VAS improved for OS (p=0.048) and for MICA (p=0.059) at 6m. There was no significant improvement in EQ-5D in either group at 12 months and no significant difference in operative time (p=0.53). There was a higher number of complications in the MICA group with 5 removal of metalwork (29.4%) and 2 superficial infections (11.8%) versus none in the OS group. The dorsiflexion significantly improved in the OS group at 6months (p=0.04). Recruitment rate dipped during COVID. No patients were lost at follow up. Conclusion. Both surgical options show similar clinical results, but higher complication rates were seen with MICA. This study illustrates the feasibility to conduct a randomised control trial for the comparison between the two techniques. Challenges to recruitment included surgeon equipoise, patient preference, training requirement and the pandemic which could be mitigated in a larger study


Objective. Neurological injuries are most common complication, which has refrained many surgeons from opting elbow arthroscopy for indicated surgeries. The objective of this study was to evaluate the safety of anterolateral (AL) and posterolateral (PL) portals and chances of injury to the radial nerve and posterior interosseous nerve around the elbow joint during elbow arthroscopy. Material & Methods. A cadaveric study was conducted on 16 non dissected cadavers (32 elbow specimens) between the period of January 2021 to June 2022. Four portals were established using 4 mm Steinmann pins which are Proximal AL Portal, Mid-AL Portal, Distal AL Portal and PL Portal. The measurements of each portal were taken for each nerve and compared with each other. Results. In our study, the mean age was 56.5 years. Proximal AL Portal was found to be at an average distance of 12.03 mm from radial nerve, 9.48 mm from the PIN and 9.35 mm from the PACN (Posterior Antebrachial Cutaneous Nerve). Distal AL Portal was at an average distance of 7.95 mm away from the radial nerve which closest to radial nerve amongst all portals. The radial nerve had the most risk of being injured out of all the nerves in the AL and PL portals followed by PIN and PACN in AL portal. Ulnar nerve was found safe in the PL portal. Conclusion. The PL portal was safer than the AL portal for conducting elbow arthroscopy. Care should be taken specially to protect radial nerve while performing elbow arthroscopy

Introduction & Aim. The use of All-Poly Tibia has been in practice since the early 1970's. Recently due to the reports on wear and osteolysis in other articulations, this component has generated significant interest. In the current study we aim to report early medium-term results of All-poly Tibial components in elderly (>70 years) patients. Method. Study of 455 cases done between 2005-2020. All the cases were performed by a single surgeon. All-Poly Tibial component implantations were performed using Standard mechanical jigs and the same posterior-stabilized implant was used for all cases. Results. 20 cases were lost to follow-up. 25 patients died due to natural causes. Mean age at index surgery was 74 years (70 - 91 years). Preop KSS average was 47 (31- 62). Post operative at the last follow up was 87 (71- 93). Of the 410 cases there were 8 revisions, 6 for deep sepsis and 2 for periprosthetic fractures. There were no revisions for aseptic loosening or osteolysis. All cases are performing well functionally and clinically. 18 cases had a non-progressive radiolucent line beneath the Tibial component. The combination of perfect alignment and soft tissue balance creates an environment for a successful TKR. The choice of the All-Poly Tibial component for functionally low demand age group patients reduces the chances of premature wear and osteolysis. In elderly patients the implant should outlive the patient. Here it is observed that at 5-7 years aseptic loosening and subsequent revision chances are low. The all-poly Tibial component is significantly cheaper as compared to its metal back counterpart. Conclusion. An excellent clinical result in our hands for this group of patients supports the continued use of this implant strongly

Objectives. We identified an unusual pattern of backside deformation on polyethylene (PE) inserts of contemporary total knee replacements (TKRs). The PE backside's margins were inferiorly deformed in TKRs with NexGen central-locking trays. This backside deformation was significantly associated with tray debonding. Furthermore, recent studies have shown high rate of tray debonding in PS NexGen TKRs. Subsequently, a field safety notice was issued regarding the performance of this particular device combination and the Option tray has been withdrawn from use. Therefore, we hypothesised that the backside deformation of PS inserts may be greater than that of CR inserts. Design and Methods. At our national implant retrieval centre, we used peer-reviewed techniques to analyse changes in the bearing wear rate and backside surface deformation of NexGen PE inserts using coordinate measuring machines [N=84 (CR-43 and PS-41) TKRs with non-augmented-trays]. Multiple regression was used to determine which variable had the greatest influence on backside deformation. The amount of cement cover on trays was quantified as a %of the total surface using Image-J software. Results. The median (IQR) bearing wear rate of the PS PEs [14(8-22) mm. 3. /year] was not significantly different(p=0.154) to that of the CR PEs [18(8-27)mm. 3. /year]. The median (IQR) backside deformation of the PS inserts [294(239-361) µm] was significantly greater (p<0.001) than that of the CR inserts [212(158-258)µm]. Multiple regression modelling showed that duration in-vivo (p=0.037), central-clearance between insert and tray (p<0.001) and constraint (p=0.003) were significantly associated with PE backside-deformation. 38(93%) PS and 31(72%) of CR trays exhibited ≤10% of cement cover. Non-contacting profilometry and microscopy revealed marked pitting and abrasive changes to the superior surface of the tray. Conclusion. This explant study showed the PE backside deformation was significantly higher in PS than in CR inserts and this may be one explanation for the unsatisfactory clinical performance reported with this device combination
