Aims

The aim of this study was to evaluate the suitability of the tapered cone stem in total hip arthroplasty (THA) in patients with excessive femoral anteversion and after femoral osteotomy.

Methods

We included patients who underwent THA using Wagner Cone due to proximal femur anatomical abnormalities between August 2014 and January 2019 at a single institution. We investigated implant survival time using the endpoint of dislocation and revision, and compared the prevalence of prosthetic impingements between the Wagner Cone, a tapered cone stem, and the Taperloc, a tapered wedge stem, through simulation. We also collected Oxford Hip Score (OHS), visual analogue scale (VAS) satisfaction, and VAS pain by postal survey in August 2023 and explored variables associated with those scores.

Aims

The primary objective of this study was to develop a validated classification system for assessing iatrogenic bone trauma and soft-tissue injury during total hip arthroplasty (THA). The secondary objective was to compare macroscopic bone trauma and soft-tissues injury in conventional THA (CO THA) versus robotic arm-assisted THA (RO THA) using this classification system.

Methods

This study included 30 CO THAs versus 30 RO THAs performed by a single surgeon. Intraoperative photographs of the osseous acetabulum and periacetabular soft-tissues were obtained prior to implantation of the acetabular component, which were used to develop the proposed classification system. Interobserver and intraobserver variabilities of the proposed classification system were assessed.

Total hip arthroplasty (THA) for congenital hip dysplasia (CDH) presents a challenge. In high-grade CDH, key surgical targets include cup placement in the anatomical position and leg length equality. Lengthening of more than 4 cm is associated with sciatic nerve injury, therefore shortening osteotomies are necessary. We present our experience of different shortening osteotomies including advantages and disadvantages of each technique. 89 hips, in 61 pts (28 bilateral cases), for high CDH were performed by a single surgeon from 1997 to 2022. 67 patients were female and 22 were male. Age ranged from 38 to 68 yrs. In all patients 5–8cm of leg length discrepancy (LLD) was present, requiring shortening femoral osteotomy. 12 patients underwent sequential proximal femoral resection with trochanteric osteotomy, 46 subtrochanteric, 6 midshaft, and 25 distal femoral osteotomies with simultaneous valgus correction were performed. All acetabular prostheses were placed in the true anatomical position. We used uncemented high porosity cups. Patients were followed up for a minimum of 12 months. All osteotomies healed uneventfully except 3 non-unions of the greater trochanter in the proximal femur resection group. No femoral shaft fractures in proximally based osteotomies. No significant LLD compared to the unaffected or reconstructed side. 2 patients suffered 3 and 5 degrees malrotation of the femur in the oblique sub-trochanteric group. 3 patients suffered transient sciatic nerve palsies. Shortening femoral osteotomies in the treatment of DDH are necessary to avoid injury to the sciatic nerve. In our series, we found transverse subtrochanteric osteotomies to be the most technically efficient, versatile and predictable in their clinical outcome, due to the ability to correct rotation and preserve the metaphyseal bone integrity, allowing for better initial stem stability. Distal femoral osteotomies allowed for controllable correction of valgus knee deformity

Hip precautions following total hip arthroplasty (THA) limits flexion, adduction and internal rotation, yet these precautions cause unnecessary psychological stress. This study aims to assess bony and implant impingement using virtual models from actual patient's bony morphology and spinopelvic parameters to deduce whether hip precautions are necessary with precise implant positioning in the Asian population. Individualized sitting and standing sacral slope data of robotic THAs performed at two tertiary referral centers in Hong Kong was inputted into the simulation system based on patients’ pre-operative sitting and standing lumbar spine X-rays. Three-dimensional dynamic models were reconstructed using the Stryker Mako THA 4.0 software to assess bony and implant impingement both anteriorly and posteriorly, with default cup placement at 40° inclination and 20° anteversion. Femoral anteversion followed individual patient's native version. A 36mm hip ball was chosen for all cups equal or above 48mm and 32mm for those below. Anterior impingement was assessed by hip flexion and posterior impingement was assessed by hip extension. 113 patients were included. At neutral rotation and adduction, no patients had anterior implant impingement at hip flexion of 100°. 1.7% had impingement at 110°, 3.5% had impingement at 120°, 9.7% had impingement at 130°. With 20° of internal rotation and adduction, 0.8% had anterior implant impingement at hip flexion of 90°, 7.1% had impingement at 100° and 18.5% had impingement at 110°. With the hip externally rotated by 20°, 0.8% of patients had posterior implant impingement, and 8.8% bony impingement at 0° extension. With enabling technology allowing accurate component positioning, hip precautions without limiting forward flexion in neutral position is safe given precise implant positioning and adequate osteophyte removal. Patients should only be cautioned about combined internal rotation, adduction with flexion

Dual Mobility (DM) Total Hip Replacements (THRs), are becoming widely used but function in-vivo is not fully understood. The aim of this study was to compare the incidence of impingement of a modular dual mobility with that of a standard cup. A geometrical model of one subject's bony anatomy \[1\] was developed, a THR was implanted with the cup at a range of inclination and anteversion positions (Corail® stem, Pinnacle® cup (DePuy Synthes)). Two DM variants and one STD acetabular cup were modelled. Joint motions were taken from kinematic data of activities of daily living associated with dislocation \[2\] and walking. The occurrence of impingement was assessed for each component combination, orientation and activity. Implant-implant impingement can occur between the femoral neck and the metal or PE liner (DM or STD constructs respectively) or neck-PE mobile liner (DM only). The results comprise a colour coded matrix which sums the number of impingement events for each cup position and activity and for each implant variant. Neck-PE mobile liner impingement, occurred for both DM sizes, for all activities, and most cup placement positions indicating that the PE mobile liner is likely to move at the start of all activities including walking. For all constructs no placement positions avoided neck-metal (DM) or neck-PE liner (STD) impingementevents in all activities. The least number of events occurred at higher inclination and anteversion component positions. In addition to implant-implant impingement, some instances of bone-bone and implant-bone impingement were also observed. Consistent with DM philosophy, neck-PE mobile liner impingement and liner motion occurred for all activities including walking. Neck-liner impingement frequency was comparable between both DM sizes (metal liner) and a standard cup (PE liner)

Aims

Custom triflange acetabular components (CTACs) play an important role in reconstructive orthopaedic surgery, particularly in revision total hip arthroplasty (rTHA) and pelvic tumour resection procedures. Accurate CTAC positioning is essential to successful surgical outcomes. While prior studies have explored CTAC positioning in rTHA, research focusing on tumour cases and implant flange positioning precision remains limited. Additionally, the impact of intraoperative navigation on positioning accuracy warrants further investigation. This study assesses CTAC positioning accuracy in tumour resection and rTHA cases, focusing on the differences between preoperative planning and postoperative implant positions.

Methods

A multicentre observational cohort study in Australia between February 2017 and March 2021 included consecutive patients undergoing acetabular reconstruction with CTACs in rTHA (Paprosky 3A/3B defects) or tumour resection (including Enneking P2 peri-acetabular area). Of 103 eligible patients (104 hips), 34 patients (35 hips) were analyzed.

Aims

The aim of this study was to identify the optimal lip position for total hip arthroplasties (THAs) using a lipped liner. There is a lack of consensus on the optimal position, with substantial variability in surgeon practice.

Methods

A model of a THA was developed using a 20° lipped liner. Kinematic analyses included a physiological range of motion (ROM) analysis and a provocative dislocation manoeuvre analysis. ROM prior to impingement was calculated and, in impingement scenarios, the travel distance prior to dislocation was assessed. The combinations analyzed included nine cup positions (inclination 30-40-50°, anteversion 5-15-25°), three stem positions (anteversion 0-15-30°), and five lip orientations (right hip 7 to 11 o’clock).

Accurate cup placement in total hip arthroplasty (THA) for the patients with developmental dysplasia of the hip (DDH) is one of the challenges due to distinctive bone deformity. Robotic-arm assisted system have been developed to improve the accuracy of implant placement. This study aimed to compare the accuracy of robotic-arm assisted (Robo-THA), CT-based navigated (Navi-THA), and manual (M-THA) cup position and orientation in THA for DDH. A total of 285 patients (335 hips) including 202 M-THAs, 45 Navi-THAs, and 88 Robo-THA were analyzed. The choice of procedure followed the patient's preferences. Horizontal and vertical center of rotation (HCOR and VCOR) were measured for cup position, and radiographic inclination (RI) and anteversion (RA) were measured for cup orientation. The propensity score-matching was performed among three groups to compare the absolute error from the preoperative target position and angle. Navi-THA showed significantly smaller absolute errors than M-THA in RI (3.6° and 5.4°) and RA (3.8° and 6.0°), however, there were no significant differences between them in HCOR (2.5 mm and 3.0 mm) or VCOR (2.2 mm and 2.6 mm). In contrast, Robo-THA showed significantly smaller absolute errors of cup position than both M-THA and Navi-THA (HCOR: 1.7 mm and 2.9 mm, vs. M-THA, 1.6 mm and 2.5 mm vs. Navi-THA, VCOR:1.7 mm and 2.4 mm, vs. M-THA, 1.4 mm and 2.2 mm vs. Navi-THA). Robo-THA also showed significantly smaller absolute errors of cup orientation than both M-THA and Navi-THA (RI: 1.4° and 5.7°, vs. M-THA, 1.5° and 3.6°, vs. Navi-THA, RA: 1.9° and 5.8° vs. M-THA, 2.1° and 3.8° vs. Navi-THA). Robotic-arm assisted system showed more accurate cup position and orientation compared to manual and CT-based navigation in THA for DDH. CT-based navigation increased the accuracy of cup orientation compared to manual procedures, but not cup position

Iliopsoas tendonitis occurs in up to 30% of patients after hip resurfacing arthroplasty (HRA) and is a common reason for revision. The primary purpose of this study was to validate our novel computational model for quantifying iliopsoas impingement in HRA patients using a case-controlled investigation. Secondary purpose was to compare these results with previously measured THA patients. We conducted a retrospective search in an experienced surgeon's database for HRA patients with iliopsoas tendonitis, confirmed via the active hip flexion test in supine, and control patients without iliopsoas tendonitis, resulting in two cohorts of 12 patients. The CT scans were segmented, landmarked, and used to simulate the iliopsoas impingement in supine and standing pelvic positions. Three discrete impingement values were output for each pelvic position, and the mean and maximum of these values were reported. Cup prominence was measured using a novel, nearest-neighbour algorithm. The mean cup prominence for the symptomatic cohort was 10.7mm and 5.1mm for the asymptomatic cohort (p << 0.01). The average standing mean impingement for the symptomatic cohort was 0.1mm and 0.0mm for the asymptomatic cohort (p << 0.01). The average standing maximum impingement for the symptomatic cohort was 0.2mm and 0.0mm for the asymptomatic cohort (p << 0.01). Impingement significantly predicted the probability of pain in logistic regression models and the simulation had a sensitivity of 92%, specificity of 91%, and an AUC ROC curve of 0.95. Using a case-controlled investigation, we demonstrated that our novel simulation could detect iliopsoas impingement and differentiate between the symptomatic and asymptomatic cohorts. Interestingly, the HRA patients demonstrated less impingement than the THA patients, despite greater cup prominence. In conclusion, this tool has the potential to be used preoperatively, to guide decisions about optimal cup placement, and postoperatively, to assist in the diagnosis of iliopsoas tendonitis

Aims. Iliopsoas impingement occurs in 4% to 30% of patients after undergoing total hip arthroplasty (THA). Despite a relatively high incidence, there are few attempts at modelling impingement between the iliopsoas and acetabular component, and no attempts at modelling this in a representative cohort of subjects. The purpose of this study was to develop a novel computational model for quantifying the impingement between the iliopsoas and acetabular component and validate its utility in a case-controlled investigation. Methods. This was a retrospective cohort study of patients who underwent THA surgery that included 23 symptomatic patients diagnosed with iliopsoas tendonitis, and 23 patients not diagnosed with iliopsoas tendonitis. All patients received postoperative CT imaging, postoperative standing radiography, and had minimum six months’ follow-up. 3D models of each patient’s prosthetic and bony anatomy were generated, landmarked, and simulated in a novel iliopsoas impingement detection model in supine and standing pelvic positions. Logistic regression models were implemented to determine if the probability of pain could be significantly predicted. Receiver operating characteristic curves were generated to determine the model’s sensitivity, specificity, and area under the curve (AUC). Results. Highly significant differences between the symptomatic and asymptomatic cohorts were observed for iliopsoas impingement. Logistic regression models determined that the impingement values significantly predicted the probability of groin pain. The simulation had a sensitivity of 74%, specificity of 100%, and an AUC of 0.86. Conclusion. We developed a computational model that can quantify iliopsoas impingement and verified its accuracy in a case-controlled investigation. This tool has the potential to be used preoperatively, to guide decisions about optimal cup placement, and postoperatively, to assist in the diagnosis of iliopsoas tendonitis. Aims

Navigation devices are designed to improve a surgeon’s accuracy in positioning the acetabular and femoral components in total hip arthroplasty (THA). The purpose of this study was to both evaluate the accuracy of an optical computer-assisted surgery (CAS) navigation system and determine whether preoperative spinopelvic mobility (categorized as hypermobile, normal, or stiff) increased the risk of acetabular component placement error.

Methods

A total of 356 patients undergoing primary THA were prospectively enrolled from November 2016 to March 2018. Clinically relevant error using the CAS system was defined as a difference of > 5° between CAS and 3D radiological reconstruction measurements for acetabular component inclination and anteversion. Univariate and multiple logistic regression analyses were conducted to determine whether hypermobile (Δsacral slope(SS)stand-sit > 30°), or stiff (SSstand-sit < 10°) spinopelvic mobility contributed to increased error rates.

Aims

This study aimed to evaluate the accuracy of implant placement with robotic-arm assisted total hip arthroplasty (THA) in patients with developmental dysplasia of the hip (DDH).

Methods

The study analyzed a consecutive series of 69 patients who underwent robotic-arm assisted THA between September 2018 and December 2019. Of these, 30 patients had DDH and were classified according to the Crowe type. Acetabular component alignment and 3D positions were measured using pre- and postoperative CT data. The absolute differences of cup alignment and 3D position were compared between DDH and non-DDH patients. Moreover, these differences were analyzed in relation to the severity of DDH. The discrepancy of leg length and combined offset compared with contralateral hip were measured.

Aims

Hip arthroplasty does not always restore normal anatomy. This is due to inaccurate surgery or lack of stem sizes. We evaluated the aptitude of four total hip arthroplasty systems to restore an anatomical and medialized hip rotation centre.

Methods

Using 3D templating software in 49 CT scans of non-deformed femora, we virtually implanted: 1) small uncemented calcar-guided stems with two offset options (Optimys, Mathys), 2) uncemented straight stems with two offset options (Summit, DePuy Synthes), 3) cemented undersized stems (Exeter philosophy) with three offset options (CPT, ZimmerBiomet), and 4) cemented line-to-line stems (Kerboul philosophy) with proportional offsets (Centris, Mathys). We measured the distance between the templated and the anatomical and 5 mm medialized hip rotation centre.

Introduction. Lumbar spine fusion in patients undergoing THA (total hip arthroplasty) is a known risk factor for hip dislocation with some studies showing a 400% increased incidence compared to the overall THA population. Reduced spine flexibility can effectively narrow the cup anteversion safe zone while alterations in pelvic tilt can alter the center of the anteversion safe zone. The use of precision cup alignment technology combined with patient-specific cup alignment goals based on preoperative assessment has been suggested as a method of addressing this problem. The current study assess the dislocation rate of THA patients with stiff or fused lumbar spines treated using surgical navigation with patient-specific cup orientation goals. Methods. Seventy-five THA were performed in 54 patients with a diagnosis of lumbar fusion, lumbar disc replacement, and scoliosis with Cobb angles greater than 40 degrees were treated by the senior author (SM) as part of a prospective, non-randomized study of surgical navigation in total hip arthroplasty. All patients were treated using a smart mechanical navigation tool for cup alignment (HipXpert System, Surgical Planning Associates, Inc., Boston, MA). Cup orientation goals were set on a patient-specific basis using supine pelvic tilt as measured using CT. Patients with increased pelvic tilt had a goal for increased cup anteversion and patients with decreased pelvic tilt had a goal for decreased cup anteversion (relative to the anterior pelvic plane coordinate system). Each patient's more recent outpatient records were assessed for history of dislocation, instability, mechanical symptoms, decreased range of motion or progressive pain. Additionally, last clinic radiographs were reviewed to confirm lumbar pathology in the form of spinal surgical hardware. Results. Seventy-five total hip arthroplasties with stiff lumbar spine were reviewed with and average follow up of 6.04 years. The average number of levels of lumbar fusion was 2.3 levels. Since the most recent follow up on all patients in this cohort no hip dislocations had occurred. Discussion and Conclusion. Fusion or stiffness of the lumbar spine is a known risk factor for instability following elective THA. The current study demonstrates that patient-specific planning of cup placement taking abnormal pelvic tilt into consideration combined with the use of accurate intra-operative cup alignment technology can be used to address this problem

Introduction. Primary robotic-arm assisted total hip arthroplasty (THA) yields more accurate and reproducible acetabular cup placement, nonetheless, data is scarce in terms of outcomes. The purpose of the present study was to report on patient-reported outcomes (PROMs) in a large group of patients who underwent robotic-arm assisted THA. The authors hypothesized that (1) patients who underwent robotic-arm assisted primary THA would achieve favorable and significant improvement in PROMs, (2) an accurate and reproducible acetabular cup placement with respect to the defined SafeZones would be obtained, and (3) a low rate of THA dislocation would be observed. Methods. Prospectively collected data were retrospectively reviewed between April 2012 to May 2017. Primary THA using Mako Robotic-Arm [Mako Surgical Corp. (Stryker), Fort Lauderdale, FL, USA] with minimum two-year follow-up for the Harris Hip Score (HHS) and the Forgotten Joint Score-12 (FJS-12) were included. Exclusion criteria were: bodymass index (BMI) > 40 kg/m2, age < 21-year old, worker's compensation, or unwilling to participate. Visual analog scale (VAS) for pain and patient satisfaction were obtained. Intraoperative measurements for leg-length, global offset, acetabular inclination and version were documented. Results. 501 hips were included (57.29% females), follow-up was 43.99 ± 15.59 months. Average age was 58.70 ± 9.41 years, and the BMI was 28.41 ± 4.55 kg/m2. The group reported HHS of 90.87 ± 13.45, FJS-12 of 79.97 ± 25.87, VAS of 1.20 ± 2.06, and patient satisfaction of 8.85 ± 2.08. Intraoperative values for acetabular inclination and version were 40.0° ± 2.2 ° and 20.5° ± 2.4° respectively. Revision due to instability was 0.2%. Conclusions. Patients who received primary robotic-arm assisted THA reported excellent results at 44-month follow-up for multiple PROMs. Consistency in acetabular cup placement accuracy was achieved in regard to the Lewinnek and Callanan safe-zones

Aims

Appropriate acetabular component placement has been proposed for prevention of postoperative dislocation in total hip arthroplasty (THA). Manual placements often cause outliers in spite of attempts to insert the component within the intended safe zone; therefore, some surgeons routinely evaluate intraoperative pelvic radiographs to exclude excessive acetabular component malposition. However, their evaluation is often ambiguous in case of the tilted or rotated pelvic position. The purpose of this study was to develop the computational analysis to digitalize the acetabular component orientation regardless of the pelvic tilt or rotation.

Methods

Intraoperative pelvic radiographs of 50 patients who underwent THA were collected retrospectively. The 3D pelvic bone model and the acetabular component were image-matched to the intraoperative pelvic radiograph. The radiological anteversion (RA) and radiological inclination (RI) of the acetabular component were calculated and those measurement errors from the postoperative CT data were compared relative to those of the 2D measurements. In addition, the intra- and interobserver differences of the image-matching analysis were evaluated.

Introduction. Previous research defines the existence of a “safe zone” (SZ) pertaining to acetabular cup implantation during total hip arthroplasty (THA). It is believed that if the cup is implanted at 40°±10° inclination and 15°±10° anteversion, risk of dislocation is reduced. However, recent studies have documented that even when the acetabular cup is placed within the SZ, high incidence dislocation and instability remains due to the combination of patient-specific configuration, cup diameter, head size, and surgical approach. The SZ only investigates the angular orientation of the cup, ignoring translational location. Translational location of the cup can cause a mismatch between anatomical hip center and implanted cup center, which has not been widely explored. Objective. The objective of this study is to define a zone within which the implanted joint center can be altered with respect to the anatomical joint center but will not increase the likelihood of post-operative hip separation or dislocation. Methods. A theoretical forward solution hip model, previously validated by telemetric devices and fluoroscopy data of existing implants, was used for analysis. The model allows for modifications of implant geometries/placement and soft tissue resection to simulate various surgical conditions. For the baseline simulation, the cup center was matched to the anatomical hip joint center, calculated as the center of the best fit sphere mapping the acetabulum, and the orientation of the cup was 40°/15° (inclination/anteversion). Keeping cup orientation the same, the location of the cup was moved in 1 mm increments in all directions to identify the region where a mismatch between the two centers did not lead to separation or instability in the joint. Results. During both swing and stance phase, when the acetabular cup was placed within the optimal conic with a slant height of 5±1 mm, no hip instability or dislocation risk occurred. As the acetabular cup was translated to the boundary of the optimal conic, hip instability increased. When the acetabular cup was placed at the boundary of the optimal conic, up to 2 mm of hip separation in the lateral direction occurred during swing phase, resulting in a decrease in contact area and an increase in contact stress. As the cup was placed outside the optimal conic, severe edge loading and hip separation up to 3.5 mm occurred during swing phase. In general, this resulted in large increases in cup stress, resulting in increased risk of wear leading to early complications. Discussion. This study introduces the concept of an optimal conic in the hip joint space to reduce the incidence of dislocation and hip instability after THA. Placing the cup center within the optimal conic reduces hip instability. Moving the cup further from the anatomical hip center increases the occurrence of hip instability. Introduction. Component positioning is of great importance in total hip arthroplasty (THA) and navigation systems can help guide surgeons in the optimal placement of the implants. We report on a newly developed navigation system which employs an inertial measurement unit (IMU) to measure acetabular cup inclination and anteversion. Aims. To assess the accuracy of the IMU when used for acetabular cup placement and compare this with an established optical navigation system (ONS). Methods. At the time of acetabular cup impaction, the IMU and ONS were separately mounted on the impactor handle. Cup inclination and anteversion as measured by each device were recorded. Post-operative CTs were acquired for all patients and used to determine the final cup position. Results. Data were recorded for a total of 100 patients undergoing THA; 51 had a direct anterior approach (DAA) and 49 had a posterior approach (PA). In the DAA group, the mean difference in IMU versus CT measured cup inclination was −0.7°(range −6 to 8º) compared with mean difference of ONS versus CT of −2° (range −8 to 5º). Mean difference in IMU versus CT measured anteversion was − 1.3° (range −10 to 10º) compared with a mean difference of −1.1° (range −23 to 20º) between ONS and CT. In the PA group, mean difference in IMU versus CT inclination was 1.3º (range −8 to 6º) compared with mean difference between ONS versus CT of 1.6° (range −5 to 7º). Mean difference in anteversion was 3.7° (range −7 to 16º) between IMU and CT and 7.3° (range −3 to 19º) between ONS and CT. Conclusion. The novel IMU can be used to accurately determine the position of the acetabular cup at the point of impaction, demonstrating comparable accuracy with an established navigation system in the direct anterior approach, and even greater accuracy in the posterior approach

Introduction. While THA is associated with positive results and long-term improvement in patient quality of life, outcomes are nonetheless associated with adverse events and post-procedural deficits related to discrepancies in leg length (LLD), offset and cup placement. Post-THA errors in these parameters are associated with gait alteration, low back pain and patient dissatisfaction. Such discrepancies often necessitate revision and increasingly lead to medical malpractice litigation. Maintaining accuracy in post-surgical leg length, offset and cup placement during THA is difficult and subject to error. The sensitivity of these factors is highlighted in studies that have shown that a change of as little as 5 degrees of flexion or abduction can induce alterations in leg length of up to several millimeters. Similarly, positioning of implants can alter global and femoral offset, affecting abductor strength, range of motion and overall physical function. Compounding the biochemical issues associated with inaccurate leg length are the costs associated with these deficits. Traditional freehand techniques of managing intra-operative parameters rely on surgeon experience and tissue tensioning to manually place components accurately. These methods, however, are only able to assess leg length and are subject to inaccuracies associated with patient movement or orientation changes during surgery. Mechanical methods of minimizing post-surgical discrepancies have been developed, such as outrigger or caliper devices, although these methods also address leg length only and provide poor feedback regarding offset and center of rotation, therefore providing insufficient data to accurately achieve appropriate post-surgical leg length. Computer-assisted navigation methods provide more data regarding leg length, offset and center of rotation, but are limited by their cumbersome nature and the large capital costs associated with the systems. The Intellijoint HIP. ®. surgical smart tool (Intellijoint Surgical, Inc., Waterloo, ON) is an intra-operative guidance tool that provides surgeons with real time data on leg length, offset and center of rotation, thereby allowing for confident selection of the correct implant in order to ensure appropriate post-surgical biomechanics. The early clinical results from an initial cohort of patients indicate that Intellijoint HIP. ®. is safe and effective. No adverse events were reported in the initial cohort, and the smart tool was able to measure surgical parameters to within 1mm when compared to radiographic measurements. With training cases removed, 100% of cases had a post-procedure leg length discrepancy of less than 5mm. This paper describes the indications, procedural technique and early clinical results of the Intellijoint HIP. ®. smart tool, which offers a safe, accurate and easy-to-use option for hip surgeons to manage leg length, offset and cup position intra-operatively

Pre-operative knowledge. Knowledge-based total hip arthroplasty is becoming increasingly recognised for improved safety, efficiency, and accuracy. Pre-operative knowledge of native and planned femoral anteversion, the exact size of implants, neck length and offset, and head lengths can serve to safely accelerate surgery and reduce the need for intra-operative imaging. Pre-operative knowledge of the effect on change in leg length and offset effected by specific implant combinations can serve to minimise undesired changes. The use of a smart mechanical navigation tool superimposed on this knowledge, can serve to easily and swiftly achieve optimal component position. Cost savings. Economic data from Q1 2013 to Q2 2016 demonstrate that CMS-insured patients treated by knowledge-based surgery using the HipXpert mechanical navigation system combined with the superior hip approach have the lowest cost of all patients treated in Massachusetts by an average of more than $7,000 over 90 days for Medicare Part A expenditure (HipXpert System, Surgical Planning Associates, Boston, MA). The data show that these combined techniques outpace all other technology/technique combinations including robotics. Accuracy. The system has been proven to be robust, with repeated studies showing accurate cup placement in 100% of cases including an independent study. This compares to a recent study of robotic methods that showed only 88% accuracy in inclination and 84% for anteversion. Summary. Knowledge-based surgery with smart mechanical navigation has shown the potential to accelerate surgery, improve safety, lower cost and facilitate recovery
