When removing femoral cement in revision hip surgery, creating an anterior femoral cortical window is an attractive alternative to extended trochanteric osteotomy. We describe our experience and evolution of this technique, the clinical and radiological results, and functional outcomes. Between 2006 and 2021 we used this technique in 22 consecutive cases at Whanganui Hospital, New Zealand. The average age at surgery was 74 years (Range 44 to 89 years). 16 cases were for aseptic loosening: six cases for infection. The technique has evolved to be more precise and since 2019 the combination of CT imaging and 3-D printing technology has allowed patient-specific (PSI) jigs to be created (6 cases). This technique now facilitates cement removal by potentiating exposure through an optimally sized anterior femoral window. Bone incorporation of the cortical window and functional outcomes were assessed in 22 cases, using computer tomography and Oxford scores respectively at six months post revision surgery. Of the septic cases, five went onto successful stage two procedures, the other to a Girdlestone procedure. On average, 80% bony incorporation of the cortical window occurred (range 40 −100%). The average Oxford hip score was 37 (range 22 – 48). Functional outcome (Oxford Hip) scores were available in 11 cases (9 pre-PSI jig and 2 using PSI jig). There were two cases with femoral component subsidence (1 using the PSI jig). This case series has shown the effectiveness of removing a distal femoral cement mantle using an anterior femoral cortical window, now optimized by using a patient specific jig with subsequent reliable bony integration, and functional outcomes comparable with the mean score for revision hip procedures reported in the New Zealand Joint Registry

Introduction. The viscoelastic cervical disk prosthesis CP-ESP is an innovative one-piece deformable but cohesive interbody spacer. It is an evolution of the LP-ESP lumbar disk implanted since 2006. The implant provides 6 full degrees of freedom including shock absorption. The design allows a limitation for rotation and translation with resistance to motion (elastic return property) aimed at avoiding overload of the posterior facets. The rotation center can vary freely during motion. It thus differs substantially from current prostheses. This study reports the clinical results of a prospective observational study series of 89 patients who are representative of the current use of the ESP implant since 2012. The radiological results are focused on the evolution of the mean center of rotation (MCR) as an additional information to the range of motion (ROM) for the evaluation of the quality of spine movement. Materials and Methods. 89 patients (33 males, mean age 45 years [28–60], 107 implants) were included for an open, prospective and non-randomized study between October 2012 and December 2015. One level patients were at C3C4 (3), C4C5 (3), C5C6 (41) C6C7 (24) C7T1 (1). Two levels patients were C4C5/C5C6 (3), C5C6/C6C7 (12), C6C7/C7D1 (1) and 3 levels C4C5/C5C6/C6C7 (1). Results. Clinical data were obtained preoperatively and at 3, 6, 12 and 24 months (mean ± SD): . Neck VAS:. 5,85±2,24. 2,34±1,95. 1,42±1,54. 2,25±1,75. 2,1±1,1. Arm VAS:. 6,5±1,95. 2,22±1,95. 1,5±2,0. 3±2,78. 1±2,5. NDI (%):. 55,8±15,2. 29,5±17. 18,9±15. 31±13. 21±11. SF 36 PCS (%):. 31±22,5. 50±14,9. 65±8,4. 44±12,8. 54±12. SF 36 MCS (%):. 32±13,9. 51±10,9. 69±8,5. 54±6,7. 59±8. We did not observe local ossifications. One case of side level degeneration was observed after 12 months in a C5C6 mono-segmental disk replacement (retrospectively this patient was a good case for a double initial implantation). To date the patient has not been re-operated. Two cases were revised (one C5C6 implant for bone ingrowth failure at 6 months and one C4C5 case for painful hypermobility in a globally stiff spine). Range of motion was obtained after 6 months and maintained at 24 months. Radiological study of the location of the mean center of rotation at the prosthesis level and adjacent disks demonstrated the adaptation ability of the implant. Conclusion. The concept of the ESP prosthesis is different from that of the “first generation” articulated devices currently used in the cervical spine. This study reports encouraging clinical results about pain, function and kinematic behavior. An interesting point is the evolution of the Mean Centers of Rotation in the. post-operative course. This adaptation ability is one of the main features as we need to consider the mean and long term evolution of the global cervical posture and mobility after a cervical disc replacement. Additional studies and longer patient follow-up are needed to assess long-term reliability of this innovative implant

Introduction. Tribological studies have described a characteristic wear pattern of metal-on-metal hip resurfacings (MoMHRAs) with a run-in period followed by a ‘bedding-in’ phase minimising wear or by an increasing wear patch with edge loading. The use of metal ions as surrogate markers of in-vivo wear is now recommended as a screening tool for the performance of MoMHRAs. The aims of this retrospective, single-surgeon study were to measure ion levels in unilateral MoMHRAs at different stages during the steady-state in order to study the evolution of wear and factors affecting it. Methods. 218 consecutive patients with minimum two serum ion measurements were included. The mean age at surgery was 52.3 years, the first assessment was made at a mean of 2.5 years (11 months–8 years) and the last assessment at a mean of 4.6 years post resurfacing (2– 12 years). Ion level change was defined as Ion level at last assessment minus Ion level at first assessment. Ten different resurfacing designs were implanted, the majority being BHR (n=104), Conserve plus(n=55) and ASR (n=25). The median femoral component size was 50 mm (38–59mm). Radiological assessment of acetabular component orientation was made with EBRA. Results. For the whole cohort a significant reduction in Chromium (Cr) levels between initial [2.6 μg/ml (SD: 6.8)] and last assessment [1.9 μg/ml (SD: 8.3)] was found (p= 0.004). Cobalt (Co) levels were similar at initial [1.7 (SD: 7.7)] and last [1.8 (SD: 10.6)] follow-up (p=0.78). Cr levels increased in 31% and Co increased in 46% of patients. There was a significant ion level increase in 5% of patients. There was no significant difference between genders (p= 0.4) or implant designs (p = 0.07), but a trend of higher ions at last follow up with the ASR implants in comparison to BHR and C+. Neither component size (p=0.4) nor acetabular orientation (p=0.46) correlated with change in ion levels. However, a CPR distance (contact patch-rim) of <10mm was associated with an increase in Cr levels over time (p= 0.042). Patients with increasing ion levels had significantly lower Harris Hip Scores (p=0.038). Discussion. The analysis of the evolution of ion levels in unilateral hip resurfacing after the run-in phase demonstrates an overall decrease of Cr levels but no significant change in Co levels. This in vivo finding is consistent with tribocorrosion studies showing the formation of a passive protective film on the articulating surfaces after the initial wear-in, preventing further corrosion. From that point on, provided there is no edge loading causing increased surface wear, ions are mainly formed by corrosion of the particulate debris generated during the run-in phase. In vitro studies have shown a higher dissolution of the more soluble Co from the debris while Cr remains in solid form with less ion formation. In a number of cases in our study, ion levels continued to increase, indicating ongoing surface wear. These increasing ion levels were correlated with a lower coverage angle more prone to edge loading and with the development of clinical symptoms

Aim. To review the evolution of the orthopaedic surgical care offered at a small rural hospital in western Nepal. The United Missions Hospital of Tansen (UMHT) was established in 1954 and has progressed substantially over the past 55 years. Located in the hills 300 km west of Kathmandu, it services not only Palpa District but also the surrounding nine districts and Indian patients from across the nearby border. The hospital has progressed from a simple medical clinic to a 165-bed hospital with four operating theatres, a new emergency department and multiple outpatient clinics. Orthopaedics in particular has come a long way from solely non-operative care such as plasters, splints and traction, to plating and intra-medullary nailing with intra-operative imaging facilities. This talk will review the progression of services in the region and highlight how western intervention has drastically improved the health care and lives of people in and around Tansen. Methods. UMHT is a 165-bed hospital with medical staff covering paediatrics, general medicine, general surgery and orthopaedics. Exact specialty coverage can vary depending on expatriate cover and locally trained staff expertise. There are around 315 Nepali employees, and eight mission appointees, mainly from the UK, USA, Australia and Sweden. There are six resident medical officers who are post internship and usually completing GP training schemes. The hospital is accredited to train 10 interns per year and frequently takes local and international medical students on rotation. Surgical facilities include daily outpatient surgical clinics of 30–60 patients, two minor operation theatres and two operating theatres. Generally, these are run as one orthopaedic and one general surgical theatre. This review is based on the experiences of Dr John Bosanquet, Dr Sandy Bosanquet and Dr Lachlan Host in their outreach trips to this hospital and in particular will focus on the progression over the last 10 years of the orthopaedic services available. Major advances have been in the provision of x-ray and CT facilities, improved supply of sterile equipment, introduction of IM nailing using the SIGN nail, and the training of local staff to continue the work. Conclusions. UMHT is a work in progress and is only made possible by the contributions of the many doctors, nurses, teachers and other professionals who willingly offer their services to help those less fortunate. Even more importantly is the funding provided from multiple organisations to sustain this outreach. The challenge remains, as in any outreach offered, of providing a service that is sustainable long-term. To date, UMHT has stood the test of time and there is much to be learnt from its example in the establishment of similar outreach projects. Future challenges for Tansen include the need for improved sterile facilities, the challenges of introducing elective surgery, particularly arthroplasty, provision of spine and neurosurgical facilities, and retention of skilled personal in what can be an isolating environment

We aim to describe the microbiological spectrum and relevant antibiotic susceptibility profile of PJI in our institution over a five-year period(2009–2013) and determine its evolution considering the preceding six years(2003–2008) thus evaluating the adequacy of our empirical antibiotic regimen. We retrospectively reviewed the records of 96 consecutive PJI (51 hips:45 knees) treated from May 2009-December 2013. Demographics, microbial species and antibiotic susceptibility were recorded. These results were then compared to those previously obtained by studying the 2003–2008 time period. Infections were polymicrobial in 27 cases(28.1%) and only two cases(2.1%) were culture-negative accounting for a total of 132 different culture results. S.aureus grew in 37 samples(28.0%) being the most frequently isolated microorganism. Coagulase-negative staphylococci grew in 32 samples(24.2%) and gram negative bacteria in 35 samples(26.5%). Other Gram positive species (most commonly enterococci and streptococci) were isolated in 26 samples(19.7%). Comparing 2009–2013 to 2003–2008, there was a significant increase of polymicrobial infections – 28% vs. 8%(OR=4.6, 95%CI [1.9–11.3]) and a significant decrease of culture-negative cases – 2% vs. 18%(OR=0.1, 95%CI [0.02–0.4]). It is also noteworthy that the prevalence of gram negative isolates was significantly increased – 26.5% vs. 13.3%(OR=1.3, 95%CI [1.1–1.6]). Antibiotic susceptibilities study showed a 41.4% methicillin resistance among S.aureus and even higher among coagulase-negative staphylococci isolates(57.7%). This is a not quite significant decrease compared to the earlier period(p=0.10). We also found a high rate antibiotic resistance among gram negative: ampiciline(81.8%), amoxicilin/clavulanate(59.1%), ciprofloxacin(19.2%), aminoglycosides(17%), third generation cephalosporins(14.6%) and even carbapenems(13.6%). These results show that our sampling protocol has improved considerably as the proportion of culture-negative cases has dramatically decreased. On the other hand this may also help explain the increase in polymicrobial infections. We have no clear explanation for the increase in gram negative bacteria. Despite the downward trend we still face a very significant proportion of methicillin-resistant staphylococci infections. The antibiotic resistance profile among gram negative bacteria is also worrying. As such we believe a regimen consisting of vancomycin and gram-negative coverage such as aminoglycosides or a third generation cephalosporin is still warranted in our institution

Total hip arthroplasty has become one of the most successful orthopaedic procedures with long-term survival rate. An ever-increasing acceptance of the potential longevity of THA systems has contributed to an increasing incidence of THA in younger and more active patients. Nowadays, especially in younger patients, cementless THA is the favored method worldwide.

Since the first cementless THA in late 1970s, many implant designs and modifications have been made. Despite excellent long-term results for traditional straight cementless stems, periprosthetic fractures or gluteal insufficiency are still a concern. For instance, as reported in a meta-analysis by Masonis and Bourne, the incidence of gluteal insufficiency after THA varies between 4% and 22%.

In contrast, the flattened lateral profile of the SP-CL® anatomical cementless stem can protect the greater trochanter during the use of cancellous bone compressors and can avoid gluteal insufficiency after THA. Another benefit of this stem design is the rotational stability and the natural load transfer due to the anatomical concept. In this context, we report our experiences using the SP-CL® anatomical cementless stem. The study group consists of 1452 THA cases (850 male, 602 female) with an average age of 62 years (range 25–76 years). After a mean follow-up of 20 months, in seven cases (0.5%) a stem exchange was necessitated. The reason for stem revision was periprosthetic fracture in 4 cases (0.3%) and periprosthetic joint infection in three cases (0.2%). In five patients, hip dislocation and in four patients migration of the stem occurred. However, stem exchange was not required in those cases.

In conclusion, the SP-CL® anatomical cementless stem has excellent short-term results.

Introduction

The purpose of this study was to demonstrate the feasibility of passively collecting objective data from a commercially available smartphone-based care management platform (sbCMP) and robotic assisted total knee arthroplasty (raTKA).

Methods

Secondary data analysis was performed using de-identified data from a commercial database that collected metrics from a sbCMP combined with intraoperative data collection from raTKA. Patients were included in this analysis if they underwent unilateral raTKA between July 2020 and February 2021, and were prescribed the sbCMP (n=131). The population consisted of 76 females and 55 males, with a mean age of 64 years (range, 43 – 81). Pre-operative through six-week post-operative data included step counts from the sbCMP, as well as administration of the KOOS JR. Intraoperative data included surgical times, the hip-knee-ankle angle (HKA), and medial and lateral laxity assessments from the robotic assessment. Data are presented using descriptive statistics. Comparisons were performed using a paired samples t-test, or Wilcoxon Signed-rank test, with significance assessed at p<0.05. A minimal detectable change (MDC) in the KOOS JR score was considered ½ standard deviation of the preoperative values.

Background

Bacteriophages are natural viruses of interest in the field of PJI. A paper previously reported the PhagoDAIR procedure (use of phages during DAIR) in three patients with PJI for whom explantation was not desirable. As the need to isolate the pathogen before surgery to perform phage susceptibility testing is a strong hindrance for the development of this procedure, we developed post-operative phage injections using ultrasound, in patients infected with S. aureus and/or P. aeruginosa who were eligible for the PhagoDAIR procedure, but for whom phages were not available at the time of surgery.

Materials/Methods

We performed a single center, exploratory, prospective cohort study including patients with knee PJI who received phage therapy with ultrasound after performance of a DAIR or a partial prosthesis exchange. All patients had PJI requiring conservative surgery and suppressive antimicrobial therapy (SAT) as salvage procedure. Each case was discussed in multidisciplinary meetings in agreement with French health authority, based on the clinical presentation, and the phage susceptibility testing. The cocktail of highly concentrate active phages (5 mL; about 10e9 PFU/mL) was extemporaneous prepared and administered three times directly into the joint using sonography (1 injection per week during 3 weeks) during the postoperative period, before switching antibiotics to SAT.

In 1823 J. White excised the head. In 1887 a German surgeon replaced the head with ivory. Interposition arthroplasties were common after WW1. Short-stemmed head replacing prosthesis were developed after WW2. Moores and Thompson designed a more stable intramedullary stem. Acetabular erosion was troublesome—and so replacing both surfaces started in the late 1950s using Teflon cup and metal femur. Unfortunately, these quickly became loose due to wear or sepsis. In 1960, Charnley used a polyethylene cup and stainless-steel femur and fixed both with dental cement. This ‘low friction arthroplast’ became a routine procedure after 1961. In the 1970s there were many ‘Charnley look-alike’ prosthesis with similar problems of poly-wear, granulomas and cysts causing bone loss, loosening, breakages and infection. Resurfacing with two thin shells was developed to reduce the foreign material, the bone resection and the cement used. Unfortunately, neck fractures, avascular necrosis and excessive wear of the poly shell were common. Despite operating theatres with laminar flow of sterile air, space suits and improved cementing techniques, the same problems occurred. To avoid poly and cement, Mittelmayer developed a ceramic screw cup, which did not require cement. Although some screws migrated, they did not wear. Because the un-cemented metal stem remained fixed solid to the femur, un-cemented metal cups and stems were developed. To avoid the poly-wear, ceramic liners became popular.

To provide the active patients with a stable joint that requires no restriction in physical activity, a large head in a large cup is desirable. Unfortunately, the large metal-on-metal resurfacing prosthesis produce metal wear ions and nanoparticles which can form hypersensitivities, cysts and pseudotumours. Computer assisted navigation to ensure correct positioning of the prosthetic components is obviously useful for surgeons that use incisions too small to see enough to be certain of the cups position. Presently, articular cartilage research is progressing rapidly and by 2020 most arthritic hip joints will be arthroscopically debrided and resurfaced by an injection of genetically engineered articular cartilage stem cells.

Why are total knees being revised? Aseptic loosening, poly wear, and instability account for up to 59% of revision TKA procedures. Younger and more active patients are placing greater demands on total knee arthroplasty (TKA) implants and international registries have documented a much higher rate of TKA failure in this population. Implant designs utilised in the active patient population should focus on optimisation of long term wear properties and minimising interface stress.

Instability after TKA, often related to technical concerns at the time of the index procedure, accounts for by far the greatest subset of failures, excluding infection, in the early revision TKA patients (<5 years). The inability to achieve a rectangular flexion gap with certain TKA techniques for certain deformities has been documented. The adverse clinical consequence of flexion gap asymmetry has also been published in peer reviewed manuscripts. Techniques should be considered that optimise flexion space balance and enhance mid-flexion stability in active, physically demanding patients.

This surgical demonstration will highlight gap balancing techniques and a new rotating platform TKA system as an option for the active patient population.

Cemented unicompartmental knee replacement (UKR) has been used for many years and has excellent results in many follow-up studies. However, concerns about the quality of fixation, cementing errors, and radiolucent lines have encouraged the development of cementless prostheses. Mobile bearing UKR are probably the ideal implants for cementless fixation as the loads at the interface tend to be compressive. Prior to the widespread introduction of cementless mobile bearing UKR three studies have been performed to assess this device. In a randomised radiostereometric (RSA) study the migration of cementless and cemented devices were the same in the second year suggesting the quality of fixation achieved was similar. A clinical randomised study demonstrated a dramatic reduction in radiolucent lines with cementless components compared to cemented and no difference in clinical outcome. A prospective cohort study of 1000 patients demonstrated no overall difference in complications or revision rate and identified no contraindications for cementless fixation. There are however anecdotal reports of occasional tibial plateau fracture and early subsidence of cementless components. Data from the National Registers would suggest that the revision rate with cementless is about half that compared to cemented however this difference may be because only experienced surgeons are using the cementless device.

Cementless mobile bearing UKR does seem to be a good evolutionary step in the development of UKR. It is important however that surgeons are trained in the use of the device and are careful with the technique.

Aim

The purpose of the study was to assess the outcomes of the surgical management of humeral shaft non-unions.

Method

Between 2002 and 2010, 23 patients with humeral shaft non-unions underwent revision surgery. Nine were initially treated non-operatively, 11 with a plate, 1 with screws and 2 with an intramedullary nail.

In previously operated patients with aseptic non-unions single stage revision was performed, while septic non-unions underwent a two stage revision. Revision stabilisation was by single or double plate fixation +/− external fixator, depending on bone loss and bone quality. Augmentation of fixation was with iliac crest bone graft in all cases.

Data collected from the case notes and radiographs included: smoking habits, type and site of non-union, bone loss, infected or not, organism isolated, definitive stabilisation, augmentation used, post operative complications, further surgical interventions and time to radiological union.

INTRODUCTION

Systemic levels of metal ions are surrogate markers of in-vivo wear of metal-on-metal hip resurfacings (MoMHRA). The wear-related generation of metal ions is associated with component size and positioning but also with design specific features such as coverage angle, clearance, metallurgy and surface technology.

OBJECTIVES

The objective of the study was to investigate whether a hip resurfacing design (ACCIS) with TiNb engineered bearing surfaces would generate less chromium (Cr) and cobalt (Co) ions during and after the run-in phase of wear and whether Ti ions could be detected indicating wear of the coating.

Introduction

Fretting corrosion at the Head-Neck taper interface of Large Metal on Metal (MoM), Metal on Polymer (MoP) and Ceramic on Ceramic (CoC) total hip arthroplasty (THA) remains a clinical concern. Ceramic femoral heads have gained a lot of attention more recently as a possible way to mitigate/reduce the dissolution of Cobalt Chromium ions. The objective of this study is to assess the fretting corrosion currents emanating from four material combinations for which Ti6Al4V and Co28Cr6Mo are the neck components of Co28Cr6Mo and BIOLOX®delta femoral heads at three different cyclic loads.

Method

12/14 Ti6Al4V and Co28Cr6Mo spigots (designed to geometrically represent the stem) were impacted against Ø36mm Co28Cr6Mo and BIOLOX®delta femoral heads with a static force of 2kN as shown in Figure 1. The tapers were immersed in 25% v/v diluted Foetal Bovine Serum, PBS balance and 0.03% Sodium Azide at room temperature. In-situ electrochemistry was facilitated using a 3-eletrode cell arrangement whereby the neck components were the working electrode, Ag/AgCl was the reference electrode and a platinum counter electrode completed the cell. All combinations were held at a potential of 0V vs. Ag/AgCl and the cyclic load applied unto each couple were 1kN, 3kN and 5kN at 1Hz consecutively (see Figure 2). The fretting corrosion currents were converted into cumulative charge transferred (Q) by integrating the wear enhanced corrosion current.

Algorithms for the treatment of multiply-injured patients with concomitant orthopaedic injuries have continued to evolve over the past several decades. Advances in surgical techniques and implants have revolutionised the treatment of specific musculoskeletal injuries. Improved understanding of the implications of applying these techniques to patients with compromised physiology has led to critical reevaluation of the issues surrounding definitive orthopaedic care. A discussion of these issues as they apply to the multiply-injured patient with associated femoral shaft fracture provides insight into how Damage Control Orthopaedics has evolved. As well as what questions remain unanswered in our ongoing efforts to decrease mortality and improve long-term functional recovery in this difficult and challenging patient population.

While femur fracture patients in the 1950s and 1960s were often deemed ‘too sick’ for surgical treatment, the high morbidity and mortality associated with long-term traction as a primary treatment modality led to recognition of the benefits of early fracture care in the 1980s. Multiple studies demonstrating the benefit of early fixation of femur fractures in multiply-injured patients led to a dramatic shift in treatment protocols towards urgent, if not emergent, definitive stabilisation of the femur. However, weaknesses of these early studies exist and their results were often over-interpreted. In the late 1980s and early 1990s, evidence began to accumulate that early definitive treatment consisting of reamed intramedullary nailing of the femur might actually be detrimental in an at risk subgroup of patients. Early interpretation of these results led to a move toward ‘unreamed’ nails in multiply-injured patients in an effort to minimise the pulmonary impact of reaming. Our current level of understanding appears to indicate that there exists a much more complex interaction between multiple factors including patient characteristics, associated injuries, timing and mode of orthopaedic stabilisation employed. As the relative importance and influence of these multiple factors becomes clearer, our ability to appropriately select patients for early total care versus damage control orthopaedics should improve the potential for damaging physiological effects of treatment. In the meantime, application of the principles of damage control to those patients for whom it is clearly indicated, as well as to those for whom it may not be clear cut, appears to have minimal, if any, deleterious effect.

The RIPO register collects data of all primary and revision hip replacement surgeries performed in Region Emilia-Romagna, Italy since 1stJanuary 2000.

The present study is aimed to analyze the survival rate of patients with Biolox®delta and Biolox®forte components, (CeramTec, Plochingen Germany) considering possible confounding factors. Only patients living in the region are considered in analysis, to avoid the bias resulting from the ‘loss’ of non-resident patients. The analysis has been conducted only on uncemented THA and monoblock, double mobility cups as well as such with pre-assembled inserts were excluded. Therefore, the finally analyzed database consists of 30’617 cases; 13’323 Biolox®forte and 17’294 Biolox®delta heads, articulating against ceramic or PE.

Unadjusted survival analysis was calculated according to Kaplan Meier method; with prosthesis failure as end point, defined as the revision of any prosthetic component for any cause.

To avoid a possible bias, revision of the prosthesis due to breakage of modular necks of the stem have not been considered. The results are summarized in Table 1.

After adjusting for age and gender it resulted that Biolox®delta - XLpoly and Biolox®forte – poly increase the risk of revision (HR 1.4 and 1.2) compared to Biolox®delta - Biolox®delta;

When only ceramic fractures are considered, we observed 1 fracture of a 36 mm Delta head (1/8’917 = 0.01%) and 38 fractures of 28 mm Forte heads. The 38 fractures occurred mainly in cer-cer coupling (35/3’537 implants= 1%) and occasionally in cer-poly couplings (3/4’246 implants = 0.07%).

Fracture or damage of the Biolox®delta insert occurred in 10 implants out of 14’260(0.07%), while it had a higher incidence in Forte inserts (36 casesout of 6’932 implants = 0.52%).

It can be concluded that the presence of a poly liner decrease the survival of the implant. Beside this Biolox®delta significantly improved the resistance to fracture of both head and insert.

Simultaneous arthrodesis of the ankle and subtalar joints is an established treatment option for combined ankle and subtalar arthritis or complex hindfoot deformities. The use of a curved intra medullary nail has potential advantages in terms of stability, hindfoot alignment and avoidance of the lateral neurovascular bundle.

We devised a comparative description of the results of hindfoot fusion using a curved locking nail before and after the introduction of anatomically specific modifications to the device through a retrospective review of notes and radiographs of patients undergoing simultaneous ankle and subtalar fusion by retrograde intramedullary nailing using an ACE¯ (Humeral Nail. Patients undergoing the same procedure using the Tibiotalocalcaneal [TTC] Nail System [DePuy] were recruited and studied prospectively. The outcome was assessed by a combination of notes review, clinical examination and telephone questionnaire.

Between 1996 and 2004, 71 arthrodeses in 67 patients have been performed. The average follow up is 27 months [3-73] and mean age 58 years. Fifty-two arthrodeses utilised the ACE humeral nail and nineteen used the newer TTC nail. Both nailing systems are locked proximally and distally and provide a short radius laterally directed distal curve. Mean time to union is 4.3 months [3-10]. Average AOFAS hindfoot score post-operatively is 65, with a mean improvement of 40 points from the pre-operative score in the TTC nail group. Post-operative complications included deep infection, amputation and a non-union rate of 10% overall. In the humeral nail group, four symptomatic stress reactions [8%] and three fractures of the tibia [6%] occurred at the tip of the nail. No stress-riser effect has to date been seen in the TTC nail group. Prominent metalwork removal has also been significantly reduced in the TTC nail group.

Our results show hindfoot fusion using a curved intramedullary nail to be an effective technique in complex cases of hindfoot arthritis and deformity. Anatomically specific alterations to the nail have resulted in a significant reduction in certain complications. Alternate proximal locking options in the TTC nail have reduced prominent metalwork and, more significantly, the incidence of stress reactions and fractures appears to have been eliminated.

Aims. The standard of wide tumour-like resection for chronic osteomyelitis (COM) has been challenged recently by adequate debridement. This paper reviews the evolution of surgical debridement for long bone COM, and presents the outcome of adequate debridement in a tertiary bone infection unit. Methods. We analyzed the retrospective record review from 2014 to 2020 of patients with long bone COM. All were managed by multidisciplinary infection team (MDT) protocol. Adequate debridement was employed for all cases, and no case of wide resection was included. Results. A total of 53 patients (54 bones) with median age of 45.5 years (interquartile range 31 to 55) and mean follow-up of 29 months (12 to 59) were included. In all, ten bones were Cierny-Mader type I, 39 were type III, and five were type IV. All patients were treated with single-staged management, except for one (planned two-stage stabilization). Positive microbial cultures grew in 75%. Overall, 46 cases (85%) had resolution of COM after index procedure, and 49 (90.7%) had resolution on last follow-up. Four patients (7%) underwent second surgical procedure and six patients (11%) had complications. Conclusion. We challenge the need for wide tumour-like resection in all cases of COM. Through detailed preoperative evaluation and planning with MDT approach, adequate debridement and local delivery of high concentration of antibiotic appears to provide comparable outcomes versus radical debridement. This paper reviews the evolution of surgical debridement for long bone osteomyelitis, and presents the outcome of adequate debridement in a tertiary bone infection unit. Materials & Methods. All patients were treated with single-staged management with one planned second stage stabilization. Seventy-five percent grew positive microbial cultures. Forty-six (85%) cases had resolution of COM after index procedure and 51 (94%) had resolution at last follow up. Four (7%) patients underwent second surgical procedure and six (11%) patients had complications. Conclusions. We report high COM resolution rate through detailed pre operative evaluation and planning with multidisciplinary team approach. We challenge the need for wide tumor-like resection and the need for regenerative procedures in all cases of COM. Adequate debridement and local delivery of high concentration of antibiotic appears to provide comparable outcomes

Introduction. Frame HI is the #Days for device removal/cm. IM Nail HI is less relevant (31–45 D/cm). Albizzia HI was 33 D/cm (1991–2003). Patients felt fine approximately 1M after end of lengthening (EoL), resuming normal life and sports. This sometimes resulted in implants fractures (e.g. skying before bone fusion). Ideally, the full fusion should occur at the EoL. We decided to shorten the HI to reach this target, optimising all parameters. Materials & Methods. The evolution of care has been monitored over a 32-year clinical experience with a fully weight-bearing nails (Albizzia then G-nail). Monitoring was with X-rays, DEXA, blood bone activity, and in London with special 5G CBCT Scans. We implemented several changes in the Care of patients and measured them according to the ‘Five Principles’ (stability, function, ‘Roads-vascular supply’, ‘Materials-calories’ and ‘Workers-BFC’, with actions on food intake, activity levels and on muscle and bone vascular growths. Results. Preop: training (vascularity, muscle force). Op & Postop: spine morphine, IM sawing preserving BFC, controlled hypo-pressure, low hydration, 50 cm leg elevation, walking, resistance bike, full motion (drainage, muscle reactivation), discharge 3–4h postop (including bilateral). Postop daily intense gym training. POD07-21: Distraction increased to fight non-linear hyper-ossification (44–50 mm gain at POD30) +/- aided by NSAIDs. HI decreased to 12–20D/cm, sometimes 8D/cm with some ‘soft fusion’ during lengthening, hardening within 1W after EoL. Conclusions. The surgeon is not a passive X-rays observer, but has an active role in changing the healing speed and decreasing HI for patient safety. Electro/Magnetic nails (torque 1 Nm) may be clocked by bone fusion, which does not occur with the G-Nail (19 Nm). An holistic vision for patients and treatments at several levels is essential to accelerate bone healing, and to return fast to full normal life, after a short ‘lengthening parenthesis’
