Total hip arthroplasty has become one of the most successful orthopaedic procedures with long-term survival rate. An ever-increasing acceptance of the potential longevity of THA systems has contributed to an increasing incidence of THA in younger and more active patients. Nowadays, especially in younger patients, cementless THA is the favored method worldwide. Since the first cementless THA in late 1970s, many implant designs and modifications have been made. Despite excellent long-term results for traditional straight cementless stems, periprosthetic fractures or gluteal insufficiency are still a concern. For instance, as reported in a meta-analysis by Masonis and Bourne, the incidence of gluteal insufficiency after THA varies between 4% and 22%. In contrast, the flattened lateral profile of the SP-CL® anatomical cementless stem can protect the greater trochanter during the use of cancellous bone compressors and can avoid gluteal insufficiency after THA. Another benefit of this stem design is the rotational stability and the natural load transfer due to the anatomical concept. In this context, we report our experiences using the SP-CL® anatomical cementless stem. The study group consists of 1452 THA cases (850 male, 602 female) with an average age of 62 years (range 25–76 years). After a mean follow-up of 20 months, in seven cases (0.5%) a stem exchange was necessitated. The reason for stem revision was periprosthetic fracture in 4 cases (0.3%) and periprosthetic joint infection in three cases (0.2%). In five patients, hip dislocation and in four patients migration of the stem occurred. However, stem exchange was not required in those cases. In conclusion, the SP-CL® anatomical cementless stem has excellent short-term results.
The purpose of this study was to demonstrate the feasibility of passively collecting objective data from a commercially available smartphone-based care management platform (sbCMP) and robotic assisted total knee arthroplasty (raTKA). Secondary data analysis was performed using de-identified data from a commercial database that collected metrics from a sbCMP combined with intraoperative data collection from raTKA. Patients were included in this analysis if they underwent unilateral raTKA between July 2020 and February 2021, and were prescribed the sbCMP (n=131). The population consisted of 76 females and 55 males, with a mean age of 64 years (range, 43 – 81). Pre-operative through six-week post-operative data included step counts from the sbCMP, as well as administration of the KOOS JR. Intraoperative data included surgical times, the hip-knee-ankle angle (HKA), and medial and lateral laxity assessments from the robotic assessment. Data are presented using descriptive statistics. Comparisons were performed using a paired samples t-test, or Wilcoxon Signed-rank test, with significance assessed at p<0.05. A minimal detectable change (MDC) in the KOOS JR score was considered ½ standard deviation of the preoperative values.Introduction

Bacteriophages are natural viruses of interest in the field of PJI. A paper previously reported the PhagoDAIR procedure (use of phages during DAIR) in three patients with PJI for whom explantation was not desirable. As the need to isolate the pathogen before surgery to perform phage susceptibility testing is a strong hindrance for the development of this procedure, we developed post-operative phage injections using ultrasound, in patients infected with We performed a single center, exploratory, prospective cohort study including patients with knee PJI who received phage therapy with ultrasound after performance of a DAIR or a partial prosthesis exchange. All patients had PJI requiring conservative surgery and suppressive antimicrobial therapy (SAT) as salvage procedure. Each case was discussed in multidisciplinary meetings in agreement with French health authority, based on the clinical presentation, and the phage susceptibility testing. In 1823 J. White excised the head. In 1887 a German surgeon replaced the head with ivory. Interposition arthroplasties were common after WW1. Short-stemmed head replacing prosthesis were developed after WW2. Moores and Thompson designed a more stable intramedullary stem. Acetabular erosion was troublesome—and so replacing both surfaces started in the late 1950s using Teflon cup and metal femur. Unfortunately, these quickly became loose due to wear or sepsis. In 1960, Charnley used a polyethylene cup and stainless-steel femur and fixed both with dental cement. This ‘low friction arthroplast’ became a routine procedure after 1961. In the 1970s there were many ‘Charnley look-alike’ prosthesis with similar problems of poly-wear, granulomas and cysts causing bone loss, loosening, breakages and infection. Resurfacing with two thin shells was developed to reduce the foreign material, the bone resection and the cement used. Unfortunately, neck fractures, avascular necrosis and excessive wear of the poly shell were common. Despite operating theatres with laminar flow of sterile air, space suits and improved cementing techniques, the same problems occurred. To avoid poly and cement, Mittelmayer developed a ceramic screw cup, which did not require cement. Although some screws migrated, they did not wear. Because the un-cemented metal stem remained fixed solid to the femur, un-cemented metal cups and stems were developed. To avoid the poly-wear, ceramic liners became popular. To provide the active patients with a stable joint that requires no restriction in physical activity, a large head in a large cup is desirable. Unfortunately, the large metal-on-metal resurfacing prosthesis produce metal wear ions and nanoparticles which can form hypersensitivities, cysts and pseudotumours. Computer assisted navigation to ensure correct positioning of the prosthetic components is obviously useful for surgeons that use incisions too small to see enough to be certain of the cups position. Presently, articular cartilage research is progressing rapidly and by 2020 most arthritic hip joints will be arthroscopically debrided and resurfaced by an injection of genetically engineered articular cartilage stem cells.
Why are total knees being revised? Aseptic loosening, poly wear, and instability account for up to 59% of revision TKA procedures. Younger and more active patients are placing greater demands on total knee arthroplasty (TKA) implants and international registries have documented a much higher rate of TKA failure in this population. Implant designs utilised in the active patient population should focus on optimisation of long term wear properties and minimising interface stress. Instability after TKA, often related to technical concerns at the time of the index procedure, accounts for by far the greatest subset of failures, excluding infection, in the early revision TKA patients (<5 years). The inability to achieve a rectangular flexion gap with certain TKA techniques for certain deformities has been documented. The adverse clinical consequence of flexion gap asymmetry has also been published in peer reviewed manuscripts. Techniques should be considered that optimise flexion space balance and enhance mid-flexion stability in active, physically demanding patients. This surgical demonstration will highlight gap balancing techniques and a new rotating platform TKA system as an option for the active patient population.
Cemented unicompartmental knee replacement (UKR) has been used for many years and has excellent results in many follow-up studies. However, concerns about the quality of fixation, cementing errors, and radiolucent lines have encouraged the development of cementless prostheses. Mobile bearing UKR are probably the ideal implants for cementless fixation as the loads at the interface tend to be compressive. Prior to the widespread introduction of cementless mobile bearing UKR three studies have been performed to assess this device. In a randomised radiostereometric (RSA) study the migration of cementless and cemented devices were the same in the second year suggesting the quality of fixation achieved was similar. A clinical randomised study demonstrated a dramatic reduction in radiolucent lines with cementless components compared to cemented and no difference in clinical outcome. A prospective cohort study of 1000 patients demonstrated no overall difference in complications or revision rate and identified no contraindications for cementless fixation. There are however anecdotal reports of occasional tibial plateau fracture and early subsidence of cementless components. Data from the National Registers would suggest that the revision rate with cementless is about half that compared to cemented however this difference may be because only experienced surgeons are using the cementless device. Cementless mobile bearing UKR does seem to be a good evolutionary step in the development of UKR. It is important however that surgeons are trained in the use of the device and are careful with the technique.
The purpose of the study was to assess the outcomes of the surgical management of humeral shaft non-unions. Between 2002 and 2010, 23 patients with humeral shaft non-unions underwent revision surgery. Nine were initially treated non-operatively, 11 with a plate, 1 with screws and 2 with an intramedullary nail. In previously operated patients with aseptic non-unions single stage revision was performed, while septic non-unions underwent a two stage revision. Revision stabilisation was by single or double plate fixation +/− external fixator, depending on bone loss and bone quality. Augmentation of fixation was with iliac crest bone graft in all cases. Data collected from the case notes and radiographs included: smoking habits, type and site of non-union, bone loss, infected or not, organism isolated, definitive stabilisation, augmentation used, post operative complications, further surgical interventions and time to radiological union.Aim

Systemic levels of metal ions are surrogate markers of in-vivo wear of metal-on-metal hip resurfacings (MoMHRA). The wear-related generation of metal ions is associated with component size and positioning but also with design specific features such as coverage angle, clearance, metallurgy and surface technology. OBJECTIVES
Fretting corrosion at the Head-Neck taper interface of Large Metal on Metal (MoM), Metal on Polymer (MoP) and Ceramic on Ceramic (CoC) total hip arthroplasty (THA) remains a clinical concern. Ceramic femoral heads have gained a lot of attention more recently as a possible way to mitigate/reduce the dissolution of Cobalt Chromium ions. The objective of this study is to assess the fretting corrosion currents emanating from four material combinations for which Ti6Al4V and Co28Cr6Mo are the neck components of Co28Cr6Mo and BIOLOX®delta femoral heads at three different cyclic loads. 12/14 Ti6Al4V and Co28Cr6Mo spigots (designed to geometrically represent the stem) were impacted against Ø36mm Co28Cr6Mo and BIOLOX®delta femoral heads with a static force of 2kN as shown in Figure 1. The tapers were immersed in 25% v/v diluted Foetal Bovine Serum, PBS balance and 0.03% Sodium Azide at room temperature. In-situ electrochemistry was facilitated using a 3-eletrode cell arrangement whereby the neck components were the working electrode, Ag/AgCl was the reference electrode and a platinum counter electrode completed the cell. All combinations were held at a potential of 0V vs. Ag/AgCl and the cyclic load applied unto each couple were 1kN, 3kN and 5kN at 1Hz consecutively (see Figure 2). The fretting corrosion currents were converted into cumulative charge transferred (Q) by integrating the wear enhanced corrosion current.Introduction
Method
Algorithms for the treatment of multiply-injured patients with concomitant orthopaedic injuries have continued to evolve over the past several decades. Advances in surgical techniques and implants have revolutionised the treatment of specific musculoskeletal injuries. Improved understanding of the implications of applying these techniques to patients with compromised physiology has led to critical reevaluation of the issues surrounding definitive orthopaedic care. A discussion of these issues as they apply to the multiply-injured patient with associated femoral shaft fracture provides insight into how Damage Control Orthopaedics has evolved. As well as what questions remain unanswered in our ongoing efforts to decrease mortality and improve long-term functional recovery in this difficult and challenging patient population. While femur fracture patients in the 1950s and 1960s were often deemed ‘too sick’ for surgical treatment, the high morbidity and mortality associated with long-term traction as a primary treatment modality led to recognition of the benefits of early fracture care in the 1980s. Multiple studies demonstrating the benefit of early fixation of femur fractures in multiply-injured patients led to a dramatic shift in treatment protocols towards urgent, if not emergent, definitive stabilisation of the femur. However, weaknesses of these early studies exist and their results were often over-interpreted. In the late 1980s and early 1990s, evidence began to accumulate that early definitive treatment consisting of reamed intramedullary nailing of the femur might actually be detrimental in an at risk subgroup of patients. Early interpretation of these results led to a move toward ‘unreamed’ nails in multiply-injured patients in an effort to minimise the pulmonary impact of reaming. Our current level of understanding appears to indicate that there exists a much more complex interaction between multiple factors including patient characteristics, associated injuries, timing and mode of orthopaedic stabilisation employed. As the relative importance and influence of these multiple factors becomes clearer, our ability to appropriately select patients for early total care versus damage control orthopaedics should improve the potential for damaging physiological effects of treatment. In the meantime, application of the principles of damage control to those patients for whom it is clearly indicated, as well as to those for whom it may not be clear cut, appears to have minimal, if any, deleterious effect.
The RIPO register collects data of all primary and revision hip replacement surgeries performed in Region Emilia-Romagna, Italy since 1stJanuary 2000. The present study is aimed to analyze the survival rate of patients with Biolox®delta and Biolox®forte components, (CeramTec, Plochingen Germany) considering possible confounding factors. Only patients living in the region are considered in analysis, to avoid the bias resulting from the ‘loss’ of non-resident patients. The analysis has been conducted only on uncemented THA and monoblock, double mobility cups as well as such with pre-assembled inserts were excluded. Therefore, the finally analyzed database consists of 30’617 cases; 13’323 Biolox®forte and 17’294 Biolox®delta heads, articulating against ceramic or PE. Unadjusted survival analysis was calculated according to Kaplan Meier method; with prosthesis failure as end point, defined as the revision of any prosthetic component for any cause. To avoid a possible bias, revision of the prosthesis due to breakage of modular necks of the stem have not been considered. The results are summarized in Table 1. *survival at 6 yrs follow-up After adjusting for age and gender it resulted that Biolox®delta - XLpoly and Biolox®forte – poly increase the risk of revision (HR 1.4 and 1.2) compared to Biolox®delta - Biolox®delta; When only ceramic fractures are considered, we observed 1 fracture of a 36 mm Delta head (1/8’917 = 0.01%) and 38 fractures of 28 mm Forte heads. The 38 fractures occurred mainly in cer-cer coupling (35/3’537 implants= 1%) and occasionally in cer-poly couplings (3/4’246 implants = 0.07%). Fracture or damage of the Biolox®delta insert occurred in 10 implants out of 14’260(0.07%), while it had a higher incidence in Forte inserts (36 casesout of 6’932 implants = 0.52%). It can be concluded that the presence of a poly liner decrease the survival of the implant. Beside this Biolox®delta significantly improved the resistance to fracture of both head and insert. Simultaneous arthrodesis of the ankle and subtalar joints is an established treatment option for combined ankle and subtalar arthritis or complex hindfoot deformities. The use of a curved intra medullary nail has potential advantages in terms of stability, hindfoot alignment and avoidance of the lateral neurovascular bundle. We devised a comparative description of the results of hindfoot fusion using a curved locking nail before and after the introduction of anatomically specific modifications to the device through a retrospective review of notes and radiographs of patients undergoing simultaneous ankle and subtalar fusion by retrograde intramedullary nailing using an ACE¯ (Humeral Nail. Patients undergoing the same procedure using the Tibiotalocalcaneal [TTC] Nail System [DePuy] were recruited and studied prospectively. The outcome was assessed by a combination of notes review, clinical examination and telephone questionnaire. Between 1996 and 2004, 71 arthrodeses in 67 patients have been performed. The average follow up is 27 months [3-73] and mean age 58 years. Fifty-two arthrodeses utilised the ACE humeral nail and nineteen used the newer TTC nail. Both nailing systems are locked proximally and distally and provide a short radius laterally directed distal curve. Mean time to union is 4.3 months [3-10]. Average AOFAS hindfoot score post-operatively is 65, with a mean improvement of 40 points from the pre-operative score in the TTC nail group. Post-operative complications included deep infection, amputation and a non-union rate of 10% overall. In the humeral nail group, four symptomatic stress reactions [8%] and three fractures of the tibia [6%] occurred at the tip of the nail. No stress-riser effect has to date been seen in the TTC nail group. Prominent metalwork removal has also been significantly reduced in the TTC nail group. Our results show hindfoot fusion using a curved intramedullary nail to be an effective technique in complex cases of hindfoot arthritis and deformity. Anatomically specific alterations to the nail have resulted in a significant reduction in certain complications. Alternate proximal locking options in the TTC nail have reduced prominent metalwork and, more significantly, the incidence of stress reactions and fractures appears to have been eliminated.
