Aims. The impact of a diaphyseal femoral deformity on knee alignment varies according to its severity and localization. The aims of this study were to determine a method of assessing the impact of diaphyseal femoral deformities on knee alignment for the varus knee, and to evaluate the reliability and the reproducibility of this method in a large cohort of osteoarthritic patients. Methods. All patients who underwent a knee arthroplasty from 2019 to 2021 were included. Exclusion criteria were genu valgus, flexion contracture (> 5°), previous femoral osteotomy or fracture, total hip arthroplasty, and femoral rotational disorder. A total of 205 patients met the inclusion criteria. The mean age was 62.2 years (SD 8.4). The mean BMI was 33.1 kg/m. 2. (SD 5.5). The radiological measurements were performed twice by two independent reviewers, and included hip knee ankle (HKA) angle, mechanical medial distal femoral angle (mMDFA), anatomical medial distal femoral angle (aMDFA), femoral neck shaft angle (NSA), femoral bowing angle (FBow), the distance between the knee centre and the top of the FBow (DK), and the angle representing the FBow impact on the knee (C’KS angle). Results. The FBow impact on the mMDFA can be measured by the C’KS angle. The C’KS angle took the localization (length DK) and the importance (FBow angle) of the FBow into consideration. The mean FBow angle was 4.4° (SD 2.4; 0 to 12.5). The mean C’KS angle was 1.8° (SD 1.1; 0 to 5.8). Overall, 84 knees (41%) had a severe FBow (> 5°). The radiological measurements showed very good to excellent intraobserver and interobserver agreements. The C’KS increased significantly when the length DK decreased and the FBow angle increased (p < 0.001). Conclusion. The impact of the diaphyseal femoral deformity on the mechanical femoral axis is measured by the C’KS angle, a reliable and reproducible measurement. Aims. The Coronal Plane Alignment of the Knee (CPAK) classification has been developed to predict individual variations in inherent knee alignment. The impact of preoperative and postoperative CPAK classification phenotype on the postoperative clinical outcomes of total knee arthroplasty (TKA) remains elusive. This study aimed to examine the effect of postoperative CPAK classification phenotypes (I to IX), and their pre- to postoperative changes on patient-reported outcome measures (PROMs). Methods. A questionnaire was administered to 340 patients (422 knees) who underwent primary TKA for osteoarthritis (OA) between September 2013 and June 2019. A total of 231 patients (284 knees) responded. The ­Knee Society Score 2011 (KSS 2011), Knee injury and Osteoarthritis Outcome Score-12 (KOOS-12), and Forgotten Joint Score-12 (FJS-12) were used to assess clinical outcomes. Using preoperative and postoperative anteroposterior full-leg radiographs, the arithmetic hip-knee-ankle angle (aHKA) and joint line obliquity (JLO) were calculated and classified based on the CPAK classification. To investigate the impact on PROMs, multivariable regression analyses using stepwise selection were conducted, considering factors such as age at surgery, time since surgery, BMI, sex, implant use, postoperative aHKA classification, JLO classification, and changes in aHKA and JLO classifications from preoperative to postoperative. Results. The preoperative and postoperative CPAK classifications were predominantly phenotype I (155 knees; 55%) and phenotype V (73 knees; 26%), respectively. The change in the preoperative to postoperative aHKA classification was a significant negative predictive factor for KOOS-12 and FJS-12, while postoperative apex proximal JLO was a significant negative predictive factor for KSS 2011 and KOOS-12. Conclusion. In primary TKA for OA, preoperative and postoperative CPAK phenotypes were associated with PROMs. Alteration in varus/valgus alignment from preoperative to postoperative was recognized as a negative predictive factor for both KOOS-12 and FJS-12. Moreover, the postoperative apex proximal JLO was identified as a negative factor for KSS 2011 and KOOS-12. Determining the target alignment for each preoperative phenotype with reproducibility could improve PROMs. Cite this article: Bone Joint J 2024;106-B(10):1059–1066

Abstract. Unicompartment knee replacement (UKR) has been an effective treatment of isolated medial compartment osteoarthritis (OA). There has been several studies which suggest that patellofemoral (PFJ) wear may not be a relative contraindication for UKR with no statistical difference in failure rates. There is currently conflicting evidence on the role of BMI. We will review if BMI and PFJ wear impacts on the post operative functional scores following UKR. A retrospective review of a prospectively collected database was performed. Data was collected between 26/6/2014 and 25/8/2022. 159 UKR procedures were identified. BMI and PFJ cartilage wear were collected. Oxford knee scores (OKS) were collected at > 2 years. PFJ wear was split into International Cartilage Research Society (ICRS) grades I&II and III&IV. 159 UKR procedures were identified, of these 115 had 2 year follow up. There were 77 who had OKS recorded at 2 years. For PFJ wear there was no statistical difference in the median OKS at 2 years 45 vs 43.5 (p=0.408). Assessing the BMI the median was 29kg/m. 2. , range 20–43kg/m. 2. Spearman's rank was performed to assess the correlation between BMI and >2 year OKS, this demonstrated a moderately negative correlation p(df)=−0.339 (CI 95% −0.538, −0.104) p=0.004. There is no statistically significant difference in >2 year OKS following UKR regardless of PFJ wear. There is a moderately negative correlation between BMI and >2 year OKS which was significant p=0.004. Therefore BMI is a more important consideration when counselling patients for UKR

Aims. Metaphyseal cones with cemented stems are frequently used in revision total knee arthroplasty (TKA). However, if the diaphysis has been previously violated, the resultant sclerotic canal can impair cemented stem fixation, which is vital for bone ingrowth into the cone, and long-term fixation. We report the outcomes of our solution to this problem, in which impaction grafting and a cemented stem in the diaphysis is combined with an uncemented metaphyseal cone, for revision TKA in patients with severely compromised bone. Methods. A metaphyseal cone was combined with diaphyseal impaction grafting and cemented stems for 35 revision TKAs. There were two patients with follow-up of less than two years who were excluded, leaving 33 procedures in 32 patients in the study. The mean age of the patients at the time of revision TKA was 67 years (32 to 87); 20 (60%) were male. Patients had undergone a mean of four (1 to 13) previous knee arthroplasty procedures. The indications for revision were aseptic loosening (80%) and two-stage reimplantation for prosthetic joint infection (PJI; 20%). The mean follow-up was four years (2 to 11). Results. Survival free from revision of the cone/impaction grafting construct due to aseptic loosening was 100% at five years. Survival free from any revision of the construct and free from any reoperation were 92% and 73% at five years, respectively. A total of six patients (six TKAs, 17%) required a further revision, four for infection or wound issues, and two for periprosthetic fracture. Radiologically, one unrevised TKA had evidence of loosening which was asymptomatic. In all unrevised TKAs the impacted diaphyseal bone graft appeared to be incorporated radiologically. Conclusion. When presented with a sclerotic diaphysis and substantial metaphyseal bone loss, this technique combining diaphyseal impaction grafting with a metaphyseal cone provided near universal success in relation to implant fixation. Moreover, radiographs revealed incorporation of the bone graft and biological fixation of the cone. While long-term follow-up will be important, this technique provides an excellent option for the management of complex revision TKAs. Cite this article: Bone Joint J 2020;102-B(6 Supple A):116–122

Aims

The aim of this study was to compare patient-reported outcomes (PROMs) following isolated anterior cruciate ligament reconstruction (ACLR), with those following ACLR and concomitant meniscal resection or repair.

Methods

We reviewed prospectively collected data from the UK National Ligament Registry for patients who underwent primary ACLR between January 2013 and December 2022. Patients were categorized into five groups: isolated ACLR, ACLR with medial meniscus (MM) repair, ACLR with MM resection, ACLR with lateral meniscus (LM) repair, and ACLR with LM resection. Linear regression analysis, with isolated ACLR as the reference, was performed after adjusting for confounders.

Introduction. The application of artificial intelligence (A.I) using patient reported outcomes (PROs) to predict benefits, risks, benefits and likelihood of improvement following surgery presents a new frontier in shared decision-making. The purpose of this study was to assess the impact of an A.I-enabled decision aid versus patient education alone on decision quality in patients with knee OA considering total knee replacement (TKR). Secondarily we assess impact on shared decision-making, patient satisfaction, functional outcomes, consultation time, TKR rates and treatment concordance. Methods. We performed a randomized controlled trial involving 130 new adult patients with OA-related knee pain. Patients were randomized to receive the decision aid (intervention group, n=65) or educational material only (control group, n=65) along with usual care. Both cohorts completed patient surveys including PROs at baseline and between 6–12 weeks following initial evaluation or TKR. Statistical analysis included linear mixed effect models, Mann-Whitney U tests to assess for differences between groups and Fisher's exact test to evaluate variations in surgical rates and treatment concordance. Results. The intervention group showed greater decision quality (K-DQI, Mean difference = 20%, p<0.0001), collaboration in decision-making (CollaboRATE, 12% (intervention group), 47% (control group) below median, p<0.0001), satisfaction with consultations (NRS-C, 14% (intervention group), 33% (control group) below median, p=0.008), improvement in functional outcomes from baseline up to 12 week follow-up (KOOSJR, 4.9 pts higher (intervention group), p=0.029) without significantly impacting consultation time. No differences were observed in TKR rates or treatment concordance. Conclusion. A.I-enabled decision aids incorporating PROs in predictive algorithms can improve decision quality, level of shared decision-making, satisfaction with patient-provider consultations, and functional outcomes, without extending consultation times. The combination of advanced predictive technologies and patient reported data to forecast surgical outcomes presents a paradigm shift in shared decision making and the delivery of high value care for patients with knee OA

Background. Revision total knee arthroplasty (rTKA) is a complex procedure with increased risk of blood loss and transfusions. The Musculoskeletal Infection Society has included D-dimer as a serology marker for peri-prosthetic infection. The study's intent is to understand the impact of preoperative D-dimer levels on blood loss and venous thromboembolism in revision TKA. Methods. Following IRB approval, rTKA performed by a single surgeon between January 1, 2017 and December 31, 2019 were reviewed. Inclusion criteria consisted of pre-operative D-Dimer, cemented revision TKA of one or both components under tourniquet control. 89 patients met the criteria including 37 males (41.6%) and 52 females (58.4%). Mean ages were 65 for males and 67 for females. The data revealed 54 patients (61%) had an elevated D-dimer (group 1) and 35 patients (39%) had a normal D-dimer (group 2). Sex stratification showed 21 males (57.8%) and 33 females (63.5%) with elevated D-dimer. TXA protocol included 2 grams intravenous (82 patients) or 2 grams intra-articular application (7 patients). Post-operative anticoagulation included Lovenox 40mg daily for 2 weeks followed by aspirin 325 twice daily for 4 weeks. Pre-operative and post-operative hemoglobin, transfusion rates and post-operative VTE within 90 days of surgery were recorded. Results. The mean pre-operative hemoglobin (hgb) was 13.30 and post-operative was 11.21. The mean change in hgb for males was 2.75 and for females 1.91. Both male and female cohorts had an acceptable range and the change in hgb was not statistically significant (p=0.076). Two female patients (2.25%) were transfused, both receiving IV TXA and their pre-operative hgb was lower than the cohort. No VTE events were identified in either groups of patients within the 90 day post-operative period. Conclusion. This study revealed that TXA is effective in reducing blood loss following rTKA and an elevated D-dimer is not a contraindication to its use

Introduction. Bone mineral density (BMD) is correlated with component migration and aseptic loosening after total knee arthroplasty (TKA). Older implant designs have demonstrated BMD loss up to 23% in the first 6 months after TKA, and continued to BMD decline at an average of 5% per year for as long as 2 years after TKA. The impact of component design and fixation method on BMD loss after TKA in modern implant designs has not been fully elucidated. The purpose of this study is to determine the effect of tibial tray thickness and fixation method (cemented versus cementless) on BMD loss patterns of the proximal tibia in two different modern TKA implant systems. Methods. A prospective, nonrandomized, single center study of patients undergoing primary TKA by one of two surgeons was performed with four study cohorts: cemented DePuy Attune, cementless DePuy Attune, cemented Stryker Triathlon, cementless Stryker Triathlon. Target sample size was 80, with 20 per cohort based on adhoc power analysis. Exclusion criteria included: age over 75, BMI >40, inflammatory arthritis, previous knee surgery involving the femur, tibia or tibial bone, and diagnosis of osteopenia/osteoporosis. Implant fixation type was based on surgeon intraoperative assessment of patient bone quality. Demographic data was collected preoperatively. Dual Energy X-ray Absorptiometry (DEXA) Bone Density Monitoring was performed at 6 weeks and one year postoperatively. Bone mineral density was calculated from the DEXA scans for 4 zones for the tibia relative to the keel or central peg: anterior, posterior, medial and lateral. Results were reported as BMD at 1 year postoperatively as a percentage of BMD at 6 weeks postoperatively. Results. 81 knees with complete DEXA studies at 6 weeks and 1 year postoperatively were available for analysis. The mean (SD) age at the time of surgery was 65.4 years (6.1), with 39 men and 42 women and a mean (SD) BMI of 31.2 kg/m. 2. (4.7). The patients who received cementless implants were significantly younger with a mean (SD) age of 62.1 (6.0) than those who received cemented implants with a mean age of 69.1 (3.7) (p < 0.05). There was no difference in age between implant type (p > 0.05). There was no difference in BMI or gender distribution between implant types or fixation type (p > 0.05). There was no significant difference in mean BMD loss between 6 weeks and 1 year postoperatively between the cemented and cementless Triathlon in any zone, between the cemented Triathlon and Attune in any zone, the cementless Triathlon and Attune in any zone, and the cemented and cementless attune in the lateral, anterior and posterior proximal tibia (p > 0.05). There was significantly more BMD loss seen in the medial tibia in the cemented (BMD at 1 year 88% of BMD at 6 weeks) than cementless (BMD at 1 year 103% of BMD at 6 weeks) Attune (p = 0.043). Conclusion. BMD loss at 1 year postoperatively versus 6 weeks postoperatively after TKA with two modern implant designs is not significantly affected by tibial tray thickness. BMD loss was unaffected by fixation type for the Stryker Triathlon, but was significant less for the cementless DePuy Attune than the cemented version

Aims. It remains controversial whether patellofemoral joint pathology is a contraindication to lateral unicompartmental knee arthroplasty (UKA). This study aimed to evaluate the effect of preoperative radiological degenerative changes and alignment on patient-reported outcome scores (PROMs) after lateral UKA. Secondarily, the influence of lateral UKA on the alignment of the patellofemoral joint was studied. Methods. A consecutive series of patients who underwent robotic arm-assisted fixed-bearing lateral UKA with at least two-year follow-up were retrospectively reviewed. Radiological evaluation was conducted to obtain a Kellgren Lawrence (KL) grade, an Altman score, and alignment measurements for each knee. Postoperative PROMs were assessed using the Kujala (Anterior Knee Pain Scale) score, Knee Injury and Osteoarthritis Outcome Score Joint Replacement (KOOS JR), and satisfaction levels. Results. A total of 140 knees (130 patients) were identified for analysis. At mean 4.1 years (2.0 to 8.5) follow-up, good to excellent Kujala scores were reported. The presence of mild to moderate preoperative patellofemoral joint osteoarthritis had no impact on these scores (KL grade 0 vs 1 to 3, p = 0.203; grade 0 to 1 vs 2 to 3, p = 0.674). Comparable scores were reported by patients with osteoarthritis (Altman score of ≥ 2) evident on either the medial or lateral patellofemoral joint facet (medial, p = 0.600 and lateral, p = 0.950). Patients with abnormal patellar congruence and tilt angles (≥ 17° and ≥ 14°, respectively) reported good to excellent Kujala scores. Furthermore, lateral UKA resulted in improvements to patellofemoral alignment. Conclusion. This is the first study demonstrating that mild to moderate preoperative radiological degenerative changes and malalignment of the patellofemoral joint are not associated with poor patient-reported outcomes at mid-term follow-up after lateral fixed-bearing UKA. Our data suggest that this may be explained by realignment of the patella and thereby redistribution of loads across the patellofemoral joint. Cite this article: Bone Joint J 2020;102-B(6):727–735

Clinical experience of impaction bone grafting for revision knee arthroplasty is limited, with initial stability of the tibial tray emerging as a major concern. The length of the stem and its diameter have been altered to improve stability. Our aim was to investigate the effect of the type of stem, support of the rim and graft impaction on early stability of the tray. We developed a system for impaction grafting of trays which we used with morsellised bone in artificial tibiae. Trays with short, long thick or long thin stems were implanted, with or without support of the rim. They were cyclically loaded while measuring relative movement. Long-stemmed trays migrated 4.5 times less than short-stemmed trays, regardless of diameter. Those with support migrated 2.8 times less than those without. The migration of short-stemmed trays correlated inversely with the density of the impacted groups. That of impaction-grafted tibial trays was in the range reported for uncemented primary trays. Movements of short-stemmed trays without cortical support were largest and sensitive to the degree of compaction of the graft. If support of the rim was sufficient or a long stem was used, impacted morsellised bone graft achieved adequate initial stability

Purpose. To highlight the cardiovascular responses of a trainee and supervising consultant while performing Total Knee Arthroplasty (TKA) and to demonstrate the impact that supervision has on both their responses. Methods. A third year orthopaedic trainee and his consultant underwent non-invasive, continuous cardiac monitoring while performing three primary TKAs. The consultant performed one TKA with the trainee assisting. The trainee then performed two TKAs as primary surgeon. The consultant supervised one TKA scrubbed and the other un-scrubbed. A third person noted the timing of each distinct intra-operative step. A significant peak was defined as an increase in heart rate (HR) of >10%. Results. Trainee. Significant peaks were only observed when acting as primary surgeon. Peaks occurred during patient positioning, approach, femoral cut, tibial cut, component trial, soft tissue balancing and cementation. Maximum HR was observed during cementation. Scrubbed supervision reduced the magnitude of these peaks and of the maximum HR. Consultant. Significant peaks were only observed when supervising the trainee. Timing of these peaks and the maximum HR coincided with those of the trainee. Both were of the higher magnitude when supervising un-scrubbed. Conclusion. Significant peaks in HR illustrate the cardiovascular impact of performing TKA. This impact is greatest during the seven definitive steps of the procedure that we have highlighted. This cardiovascular impact of performing TKA reduces as the experience of the surgeon increases. Supervision of a less experienced/trainee surgeon performing a TKA also has a cardiovascular impact on the supervising consultant. To reduce this impact on both trainee and consultant we suggest that supervision, when required, is best provided scrubbed rather than un-scrubbed

Abstract

Purpose

Unicompartmental knee arthroplasty (UKA) is an effective treatment for late knee osteoarthritis (OA). However, its indications remain controversial. Young age (< 60 years) has been associated with worse outcomes. The goal of this systematic review and meta-analysis is to study the effect of age on UKA outcomes.

Methods

The primary objective was to compare the UKA revision rate in young patients with that of old patients, using the age thresholds of 60 and 55 years. Secondary objectives were patient-reported outcome measures (PROMs) and implant design. PubMed, Ovid, Web of Science, Google Scholar, and Cochrane library were searched in June 2021. This review was conducted in accordance with the PRISMA guidelines (PROSPERO registration number: CRD42021248322).

Introduction. In recent years, online patient portals have been developed to offer the potential of an enhanced recovery experience. By offering videos, communication tools and patient-reported outcomes collection, online portals encourage patient's engagement in their care. In the total joint arthroplasty population, portals may also offer online physical therapy, allowing TJA patients to reach functional goals while reducing costs. Although technology may offer the potential of an enhanced recovery experience, disparities may exist between the comfort level of use and communication preferences of different patient populations. Our study aimed to analyze the utilization of an internet based patient portal, and quantify the impact of usage on patient reported outcome measures. Methods. 4,458 patients who underwent TJA across 8 major academic centers within one healthcare system were analyzed. Patients who scheduled surgery were registered for the online portal by the surgical coordinator. Upon registration, patients opt-in by signing a license agreement, and data is collected on their utilization of the portal including logins, exercise and educational videos watched, messages sent and PROs completed. Age was compared to utilization, opt-in rates, total videos watched, and messages sent. Two separate patient cohorts were identified to distinguish between active and non-active users. Anyone who opted-in and viewed over 5 preoperative videos or had at least 5 preoperative logins were considered active users. Patients’ postoperative KOOS-JR and HOOS-JR score improvements from baseline were compared between the active vs. non-active groups. Results. Of 4,458 total TJA patients, TKA and THA comprised 2670 and 1788, respectively, with an average age of 65.9 years (range 16–96 years). On average, patients logged into the portal 3.7 times preoperatively and 6.8 times postoperatively, and sent 2.29 messages. On average, 13 videos were watched preoperatively and 17 videos were watched postoperatively. Of all TJA patients, 72% opted-in to the portal. When looking solely at the TKA cohort, there was a significant difference in age between those patients that provided an email and those who didn't. The mean age of those who did provide an email was 66.3 years old, patients who didn't were on average 69.7 years old (p =.001). There was no significant impact of age on total preoperative or postoperative logins when considering both TKA and THA patients. There was a significant difference between age and preoperative or postoperative logins in the TKA group, with older patients tending to log in less (p =.001) and send fewer messages to the care team (p =.0001). Total videos watched preoperatively and postoperatively as well as preoperative logins did not impact change of HOOS JR scores at 6 weeks, 12 weeks, 6 months or 1 year. In the TKA cohort, the active user cohort correlated to greater improvement in KOOS-JR scores at 6 weeks, and 12 weeks postoperatively (p=.02, .018 respectively). Conclusion. While older TKA patients tended to be less engaged, the usage of the online portal overall was not impacted by age in the TJA population. As the average age of patients undergoing TKA continues to decrease, surgeons will be faced with a younger patient population that communicates heavily through technology. Previously published literature has proven online PT is as effective as traditional PT, making online portals a promising option to improve care while decreasing costs. As seen in our TKA population, patients experienced greater improvement in KOOS-Jr scores with greater activity and use of the portal

Introduction. Hip and knee replacements are being performed at increasing rates and currently account for one of the largest procedure expenditures in the Medicare budget. Outcomes of total knee replacement (TKR) depend on surgeon, patient and implant factors. The impact that the specific implants might have on patient-reported outcomes is unknown. The primary purpose of this study was to evaluate the patient-reported functional outcomes and satisfaction after primary total knee arthroplasty in patients with osteoarthritis who underwent TKR using five different brands of posterior-stabilized implants. Specifically, the aim was to evaluate for any difference in patient-reported outcomes based on implant brand used. The hypothesis was that there would be no difference in functional outcome that could be attributed to the implant used in primary TKR. Methods. Using our institution's total joint arthroplasty registry, we identified 4,135 patients who underwent total knee replacement (TKR) using one of the five most common implant brands used at our institution. These included Biomet Vanguard (N=211 patients), Depuy/Johnson&Johnson Sigma (N=221), Exactech OptetrakLogic (N=1,507), Smith & Nephew Genesis II (N=1,414), and Zimmer NexGen (N=779). Only posterior-stabilized primary TKRs in patients with osteoarthritis were included. Patients were evaluated preoperatively using the Knee Osteoarthritis Outcomes Score (KOOS), Lower Extremity Activity Scale (LEAS), and Short Form-12 (SF-12). Demographics including age, body mass index (BMI), Charleston Comorbidity Index (CCI), ASA physical status classification, sex, and smoking status were collected. Postoperatively, 2-year KOOS, LEAS, SF-12, and satisfaction scores were compared between implant groups. Results. At 2-year follow-up, patient-reported outcome scores were available for a total of 4,069 patients. In multiple regression analysis which separately compared each implant group to the aggregate of all others, after accounting for age, BMI, CCI, ASA status, and sex, there were no clinically significant differences in KOOS score changes from baseline to 2-year follow-up between any of the implants. In 2-year satisfaction, >80% of patients in each implant group were satisfied in all domains measured. In the multivariate regression model, patients in the NexGen group had the highest likelihood of being satisfied (OR 1.63, p=0.006) and OptetrakLogic patients had the lowest likelihood of being satisfied (OR 0.60, P<0.001) although these differences were below the minimal clinically important thresholds. Conclusion. TKR provides patient satisfaction and improvement in function regardless of implant type. While some implant systems had higher outcome scores than others, these differences were all below clinically significant thresholds. Whether these differences are a function of surgeon expertise or implant design is unknown. Future research should focus on identifying specific design elements that contribute to improved patient outcomes. Healthcare administrators may find the similarities in clinical outcomes to be a useful consideration when negotiating implant purchasing contracts

Abstract

Introduction

There is a need to develop approaches to reduce chronic pain after total knee replacement. There is an established link between disturbed sleep and pain. We tested the feasibility of a trial evaluating the clinical and cost-effectiveness of a pre-operative sleep assessment and complex intervention package for improving long-term pain after TKR.

Methodology

REST was a feasibility multi-centre randomised controlled trial with embedded qualitative study and health economics. Participants completed baseline measures and were randomised to usual care or the intervention, a tailored sleep assessment and behavioural intervention package delivered by an extended scope practitioner three months pre-operatively with a follow-up call up at four-weeks. Patient reported outcomes were assessed at baseline, one-week pre-surgery, and 3-months post-surgery.


Aims

The aim of this study was to investigate the distribution of phenotypes in Asian patients with end-stage osteoarthritis (OA) and assess whether the phenotype affected the clinical outcome and survival of mechanically aligned total knee arthroplasty (TKA). We also compared the survival of the group in which the phenotype unintentionally remained unchanged with those in which it was corrected to neutral.

Methods

The study involved 945 TKAs, which were performed in 641 patients with primary OA, between January 2000 and January 2009. These were classified into 12 phenotypes based on the combined assessment of four categories of the arithmetic hip-knee-ankle angle and three categories of actual joint line obliquity. The rates of survival were analyzed using Kaplan-Meier methods and the log-rank test. The Hospital for Special Surgery score and survival of each phenotype were compared with those of the reference phenotype with neutral alignment and a parallel joint line. We also compared long-term survival between the unchanged phenotype group and the corrected to neutral alignment-parallel joint line group in patients with Type IV-b (mild to moderate varus alignment-parallel joint line) phenotype.

A limited number of investigations with conflicting results have described perivascular lymphocytic infiltration (PVLI) in the setting of total knee arthroplasty (TKA). The purpose of this study was to determine if PVLI found in TKAs at the time of aseptic revision surgery was associated with worse clinical outcomes and survivorship.

Methods

A retrospective review was conducted on 617 patients who underwent aseptic TKA revision who had histological analysis for PVLI at the time of surgery. Clinical and radiological data were obtained pre- and postoperatively, six weeks postoperatively, and then every year thereafter.

Abstract

Introduction

Risk factors for osteoarthritis include raised BMI and female gender. Whether these two factors influenced synovial gene expression was investigated using a triangulation and modelling strategy which generated 12 datasets of gene expression in synovial tissue from three knee pathologies with matching BMI groups, obese and overweight, and gender distributions.

Methodology

Intra-operative synovial biopsies were immersed in RNAlater at 4oC before storage at -80oC. Total RNA was extracted using RNAeasy with gDNA removal. Following RT- PCR and quality assessment, cDNA was applied to Affymetrix Clariom D microarray gene chips. Bioinformatics analyses were performed. Linear models were prepared in limma with gender and BMI factors incorporated sequentially for each pathology comparison, generating 12 models of probes differentially expressed at FDR p<0.05 and Bayes number, B>0. Data analysis of differently expressed genes utilized Ingenuity Pathway Analysis and Cytoscape with Cluego and Cytohubba plug-ins.

Abstract

INTRODUCTION

Geniculate nerve blocks (GNB) and ablation (GNA) are increasing in popularity as strategies for the management of knee pain in patients unsuitable for surgical intervention. Typically these simple blocks have been performed by specialists in pain medicine. We present the results of a GNB clinic run by a surgical care practitioner (SCP).

METHODOLOGY

An SCP clinic was created where ultrasound-guided GNBs comprising local anaesthetic and steroid were administered. Patients considered unsuitable for surgery were referred with knee pain by orthopaedic knee surgeons and specialist physiotherapists.

A VAS pain score and an Oxford Knee Score (OKS) were completed prior to and immediately following blockade. Serial VAS diaries were completed. Further OKS were requested at 6 weeks and 6 months. Patients could request GNA at any point during follow-up and their follow-up ceased at this stage.
