An 83-year-old woman presented with acute weakness in her right hand and wrist extensors and swelling in the proximal right forearm. Nerve conduction studies confirmed compression of posterior introsseous nerve at the level of proximal forearm. MR imaging demonstrated the characteristics of lipoma which extended on the atero-lateral aspect of the right radius neck. The lesion was parosteal lipoma of the proximal radius causing paralysis of the posterior interosseous nerve without sensory deficit. In this case report, posterior inretosseous nerve palsy due to compression of a parostel lipoma was recovered after excision of the lipoma followed by intensive rehabilitation for six month. Surgical excision should be promptly performed to ensure optimal recovery from the nerve paralysis

Fractures of the humeral diaphysis occur in a bimodal distribution and represent 3-5% of all fractures. Presently, the standard treatment of isolated humeral diaphyseal fractures is nonoperative care using splints, braces, and slings. Recent data has questioned the effectiveness of this strategy in ensuring fracture healing and optimal patient function. The primary objective of this randomized controlled trial (RCT) was to assess whether operative treatment of humeral shaft fractures with a plate and screw construct provides a better functional outcome than nonoperative treatment. Secondary objectives compared union rates and both clinical and patient-reported outcomes. Eligible patients with an isolated, closed humeral diaphyseal fracture were randomized to either nonoperative care (initial sugar-tong splint, followed by functional coaptation brace) or open reduction and internal fixation (ORIF; plate and screw construct). The primary outcome measure was the Disability Shoulder, Arm, Hand (DASH) score assessed at 2-, 6-, 16-, 24-, and 52-weeks. Secondary outcomes included the Short Musculoskeletal Functional Assessment (SMFA), the Constant Shoulder Score, range of motion (ROM), and radiographic parameters. Independent samples t-tests and Chi-squared analyses were used to compare treatment groups. The DASH, SMFA, and Constant Score were modelled over time using a multiple variable mixed effects model. A total of 180 patients were randomized, with 168 included in the final analysis. There were 84 patients treated nonoperatively and 84 treated with ORIF. There was no significant difference between the two treatment groups for age (mean = 45.4 years, SD 16.5 for nonoperative group and 41.7, SD 17.2 years for ORIF group; p=0.16), sex (38.1% female in nonoperative group and 39.3% female in ORIF group; p=0.87), body mass index (mean = 27.8, SD 8.7 for nonoperative group and 27.2, SD 6.2 for ORIF group; p=0.64), or smoking status (p=0.74). There was a significant improvement in the DASH scores at 6 weeks in the ORIF group compared to the nonoperative group (mean=33.8, SD 21.2 in the ORIF group vs. mean=56.5, SD=21.1 in the nonoperative group; p < 0 .0001). At 4 months, the DASH scores were also significantly better in the ORIF group (mean=21.6, SD=19.7 in the ORIF group vs. mean=31.6, SD=24.6 in the nonoperative group; p=0.009. However, there was no difference in DASH scores at 12-month follow-up between the groups (mean=8.8,SD=10.9 vs. mean=11.0, SD=16.9 in the nonoperative group; p=0.39). Males had improved DASH scores at all timepoints compared with females. There was significantly quicker time to union (p=0.016) and improved position (p < 0 .001) in the ORIF group. There were 13 (15.5%) nonunions in the nonoperative group and four (4.7%) combined superficial and deep infections in the ORIF group. There were seven radial nerve palsies in the nonoperative group and five (a single iatrogenic) radial nerve palsies in the ORIF group. This large RCT comparing operative and nonoperative treatment of humeral diaphyseal fractures found significantly improved functional outcome scores in patients treated surgically at 6 weeks and 4 months. However, the early functional improvement did not persist at the 12-month follow-up. There was a 15.5% nonunion rate, which required surgical intervention, in the nonoperative group and a similar radial nerve palsy rate between groups

Paediatric supracondylar fractures are the most common elbow fracture in children, and is associated with an 11% incidence of neurologic injury. The goal of this study is to investigate the natural history and outcome of motor nerve recovery following closed reduction and percutaneous pinning of this injury. A total of 246 children who underwent closed reduction and percutaneous pinning following supracondylar humerus fractures were prospectively enrolled over a two year period. Patient demographics (age, weight), Gartland fracture classification, and associated traumatic neurologic injury were collected and analyzed with descriptive statistics. Patients with neurologic palsies were separated based on nerve injury distribution, and followed long term to monitor for neurologic recovery at set time points for follow up. Of the 246 patient cohort, 46 patients (18.6%) sustained a motor nerve palsy (Group 1) and 200 patients (82.4%) did not (Group 2) following elbow injury. Forty three cases involved one nerve palsy, and three cases involved two nerve palsies. No differences were found between patient age (Group 1 – 6.6 years old, Group 2 – 6.2 years old, p = 0.11) or weight (Group 1 – 24.3kg, Group 2 – 24.5kg, p = 0.44). A significantly higher proportion of Gartland type III and IV injuries were found in those with nerve palsies (Group 1 – 93.5%, Group 2 – 59%, p < 0 .001). Thirty four Anterior Interosseous Nerve (AIN) palsies were observed, of which 22 (64.7%) made a full recovery by three month. Refractory AIN injuries requiring longer than three month recovered on average 6.8 months post injury. Ten Posterior Interosseous Nerve (PIN) palsies occurred, of which four (40%) made full recovery at three month. Refractory PIN injuries requiring longer than three month recovered on average 8.4 months post injury. Six ulnar nerve motor palsies occurred, of which zero (0%) made full recovery at three month. Ulnar nerve injuries recovered on average 5.8 months post injury. Neurologic injury occurs significantly higher in Gartland type III and IV paediatric supracondylar fractures. AIN palsies remain the most common, with an expected 65% chance of full recovery by three month. 40% of all PIN palsies are expected to fully recover by three month. Ulnar motor nerve palsies were slowest to recover at 0% by the three month mark, and had an average recovery time of approximately 5.8 months. Our study findings provide further evidence for setting clinical and parental expectations following neurologic injury in paediatric supracondylar elbow fractures

Abstract. Background. Reconstruction following resection of sarcomas of the upper extremity with methods described in the prevalent literature may not be possible in few selected cases. We describe Surgical Phocomelia or Phoco-reduction as a method of limb salvage in such cases of extensive sarcomas of the upper limb with its functional and oncological outcomes. Methods. Evaluation of functional and oncological outcomes was performed for 11 patients who underwent surgical phocomelia or phocoreduction for extensive sarcomas of the upper limb between 2010 and 2019. Results. Mean follow-up period in the study was27.8 months. Five patients required a segmental resection including the entire humerus while 6 patients underwent segmental resection around the elbow with a mean resection length of 21.5 cm. Mean MSTS 93 score was 22 depicting a good functional outcome. Mean hand grip strength on the operated side was 62% of the contralateral side with preservation of useful hand function. Mean time to humero-ulnar union was 6.7 months. Radial nerve palsy and implant failure occurred in 1 patient each. No patient developed local recurrence while 3 patients died of metastasis. Conclusion. Surgical phocomelia is a prudent alternative to severely incapacitating amputations in situations where other reconstruction methods are not feasible

Nerve palsy occurring after elective primary total hip arthroplasty (THA) is a devastating complication because of its effect on motor strength, walking ability, potential for pain, and unexpected nature. In general, the nerve distribution involved is the peroneal branch of the sciatic nerve, and the level of involvement is usually mixed motor and sensory. Prior publications have associated limb lengthening, dysplasia and use of the posterior approach to be associated with a higher incidence of nerve palsy. In the literature, the incidence of sciatic nerve palsy is estimated to be 0.2 to 1.9%. We examined the rate of sciatic nerve palsy after THA performed by the joint replacement service at Hospital for Special Surgery between the years 1998–2013. Each case was matched with 2 controls that underwent THA and did not develop postoperative neuropathy. Controls were matched by surgical date having been within 7 days of their matched case's surgery date. Patient and surgical variables were reviewed using data from patient charts and the institution's total joint replacement registry. A multivariable logistic regression model was created to identify potential risk factors for neuropathy following THA while adjusting for potential confounders. We found that, of 39,056 primary THA cases, there were 81 cases of sciatic nerve palsy, giving an incidence of 0.21%. The factors with the greatest odds ratios for nerve palsy were: history of smoking (OR=3.45); history of spinal stenosis (OR=4.45), and time of day of 1PM or later (OR=3.98). We did not find limb lengthening, dysplasia, or type of fixation to be associated with nerve palsy. In conclusion, post-surgical neuropathy has a low incidence after primary THA, but at our institution, was associated with several factors. Spine-related comorbidities, such as spinal stenosis and lumbar spine disease, and smoking history should be closely monitored to inform the patient and surgeon for the potential increased risk of postoperative neuropathy following THA

Thermal injury to the radial nerve caused by cement leakage is a rare complication after revision elbow arthroplasty. Several reports have described nerve palsy caused by cement leakage after hip arthroplasty. However, little information is available regarding whether radial nerve injury due to cement leakage after humeral stem revision will recover. In a recent study, radial nerve palsy occurred in 2 of 7 patients who had thermal injury from leaked cement during humeral component revisions. These patients did not regain function of the radial nerve after observation. We present a case of functional recovery from a radial nerve palsy caused by cement leakage after immediate nerve decompression in revision elbow arthroplasty[Fig. 1.2]

There is no consensus regarding the optimum frequency of ultrasound for monitoring the response to Pavlik harness (PH) treatment in developmental dysplasia of hip (DDH). The purpose of our study was to determine if a limited-frequency hip ultrasound (USS) assessment in children undergoing PH treatment for DDH had an adverse effect on treatment outcomes when compared to traditional comprehensive ultrasound monitoring. This study was a single-center non-inferiority randomized controlled trial. Children aged less than six months of age with dislocated, dislocatable and stable dysplastic hips undergoing a standardized treatment program with a PH were randomized, once stability had been achieved, to our current standard USS monitoring protocol (every clinic visit) or to a limited-frequency ultrasound protocol (USS only until hip stability and then end of treatment). Groups were compared based on alpha angle at the end of treatment, acetabular indices (AI) and IHDI grade on follow up radiographs at one-year post harness and complication rates. The premise was that if there were no differences in these outcomes, either protocol could be deemed safe and effective. One hundred patients were recruited to the study; after exclusions, 42 patients completed the standard protocol (SP) and 36 completed the limited protocol (LP). There was no significant difference between the mean age between both groups at follow up x-ray (SP: 17.8 months; LP: 16.6 months; p=0.26). There was no difference between the groups in mean alpha angle at the end of treatment (SP: 69°; LP: 68.1°: p=0.25). There was no significant difference in the mean right AI at follow up (SP: 23.1°; LP: 22.0°; p=0.26), nor on the left (SP:23.3°; LP 22.8°; p=0.59). All hips in both groups were IHDI grade 1 at follow up. The only complication was one femoral nerve palsy in the SP group. In addition, the LP group underwent a 60% reduction in USS use once stable. We found that once dysplastic or dislocated hips were reduced and stable on USS, a limited- frequency ultrasound protocol was not associated with an inferior complication or radiographic outcome profile compared to a standardized PH treatment pathway. Our study supports reducing the frequency of ultrasound assessment during PH treatment of hip dysplasia. Minimizing the need for expensive, time-consuming and in-person health care interventions is critical to reducing health care costs, improving patient experience and assists the move to remote care. Removing the need for USS assessment at every PH check will expand care to centers where USS is not routinely available and will facilitate the establishment of virtual care clinics where clinical examination may be performed remotely

Introduction. Correction of multiplanar bone deformities in children is indicated for prevention of secondary orthopaedic complications. Different problems related to surgical intervention were reported: non-union, delayed union, recurrent deformity, refracture, nerve palsy and pin tract infection. The aim of this study was to show the results of children femur and tibia bowing deformities by Ilizarov technique. Materials and Methods. We analysed 27 cases of children femur and tibia bowing deformities under the age of 13 yrs. Simultaneous deformity correction in femur and tibia was done with Ilizarov device in ipsilateral side. Contralateral side was operated after 14 days. Results. The duration of Ilizarov fixation was 130 days on an average. The deformity correction was achieved with a proper alignment in all the cases. Conclusions. Bowing of femur and tibia can be corrected simultaneously by Ilizarov fixation with minimum complications. There were no recurrent deformities in our cases

Introduction. Treatment for distal third shaft fractures of humerus is very challenging. They are commonly treated with plating. Plating has complications of iatrogenic radial nerve palsy. We report our case series of distal third fractures of humerus treated with retrograde Halder Humeral Nail. Materials/Methods. Since 1994 to 2010 we have 576 fractures of humerus treated with retrograde Halder Humeral nail. Of these 45 fractures were distal third of humerus. Average age of patients at the time of surgery was 30.4 years (Range 15–82 years, Median 33 years). Of 45 patients 26 were females and 19 males. Three out of 45 had non union at the time of presentation. All patients were followed until clinical and radiological signs of union. One patient was lost to follow-up. Results. Average time to fracture healing was 13.9 weeks (Range 9–36 weeks). There were two cases of non union, one healed at 9 months with 2. nd. surgery. There was one reported case of infection. Seven patients had nail removal electively. All the patients achieved full range of elbow and shoulder movement. No reported cases of iatrogenic radial nerve palsy. Conclusion. Our results of distal third fractures of humerus treated with retrograde Halder Humeral Nail are excellent. It avoids big exposure as needed in plating and complication of iatrogenic radial nerve palsy

Average age of patients at the time of surgery was 30.4 years (Range 15–82 years, Median 33 years). Of 45 patients 26 were females and 19 males.3 out of 45 had non union at the time of presentation. The nail was locked distally with one or two screws and proximally with a screw and tripwire. The entry point of the nail was roof of olecranon fossa (contrary to standard retrograde nails where it is an inch above olecranon fossa) This design of nail allows it be used for even very distal fractures. All patients were followed till clinical and radiological signs of union. 1 patient was lost to follow up. Results. Average time to fracture healing was 13.9 weeks (Range 9–36 weeks). There were 2 cases of non union, one healed at 9 months with 2. nd. surgery. The other non union was asymptomatic and did not agree for further surgery. There was one reported case of infection.7 patients had nail removal electively on request. All the patients achieved full range of elbow and shoulder movement. The patterns of nerve and associated skeletal injury were reviewed in 84 patients referred to the brachial plexus service who had damage predominantly to the infraclavicular brachial plexus and its branches. Patients fell into four categories: 1. Anterior glenohumeral dislocation (46 cases); 2. ‘Occult’ shoulder dislocation or scapular fracture (17 cases); 3. Humeral neck fracture (11 cases); 4. Arm hyperextension (9 cases). The axillary (38/46) and ulnar (36/46) nerves were most commonly injured as a result of glenohumeral dislocation. The axillary nerve was ruptured in only 2 patients who had suffered high energy trauma. Ulnar nerve recovery was often incomplete. ‘Occult’ dislocation refers to patients who had no recorded shoulder dislocation but the history was suggestive that dislocation had occurred with spontaneous reduction. These patients and those with scapular fractures had a similar pattern of nerve involvement to those with known dislocation, but the axillary nerve was ruptured in 11 of 17 cases. In cases of humeral neck fracture, nerve injury resulted from medial displacement of the humeral shaft. Surgery was performed in 7 cases to reduce and fix the fracture. Arm hyperextension cases were characterised by injury to the musculocutaneous nerve, with the nerve being ruptured in 8 of 9. Five had humeral shaft fracture or elbow dislocation. There was variable involvement of the median and radial nerves, with the ulnar nerve being least affected. Most cases of infraclavicular brachial plexus injury associated with shoulder dislocation can be managed without operation. Early nerve exploration and repair should be considered for:. Axillary nerve palsy without recorded shoulder dislocation or in association with fracture of the scapula. Musculocutaneous nerve palsy with median and/or radial nerve palsy. Urgent operation is necessary for nerve injury resulting from fracture of the humeral neck to relieve ongoing pressure on the nerves

Purpose of study:. The treatment goals in diaphyseal radius fractures are to regain and maintain length and rotational stability. Open reduction and plating remains the gold standard but carries the inherent problems of soft tissue disruption and periosteal stripping. Intramedullary nailing offers advantages of minimally invasive surgery and minimal soft tissue trauma. The purpose of this study is to describe the results of locked intramedullary nailing for adult gunshot diaphyseal radius fractures. Methods:. A retrospective review of clinical and radiological records was performed on patients with intramedullary nailing of isolated gunshot radius fractures between 2009 and 2013. Results:. Twenty-two nails were inserted in 22 patients, all males with a mean age of 28.9 years (range 19–40). Follow-up was for an average period of 11 weeks (range 6–24). One patient had a median nerve palsy and 2 a posterior interosseous nerve palsy pre-operatively. All operations were performed within 3 to 12 days of the injury. No primary bone grafting was performed. All fractures united with the index procedure. Average time to union was 10 weeks (range 8–24). Fourteen patients (64%) had their radial bow restored and maintained; these patients had minimal comminution in the middle to distal third of the radius. The bow could not be satisfactorily restored in severely comminuted fractures in the proximal third of the radius. Using the Anderson classification outcome was excellent in 14 patients (64%), satisfactory in 6 (27%) and unsatisfactory in 2 (9%). There were no complications associated with the surgery and one patient required nail removal for implant migration. Conclusion:. Intramedullary nail fixation of gunshot radius fractures has shown promising results in our hands with no significant complications. Several design principles were considered paramount when the surgical technique of performing total hip arthroplasty through an incision in the superior capsule without dislocation of the hip joint was developed. These design principles include: Preservation of the abductors; Preservation of the posterior capsule and short external rotators; Preparation of the femur in situ without dislocation of the hip; In-line access to the femoral shaft axis; Ability to perform a trial reduction; Independence from intraoperative imaging; Independence from a traction table; Applicable to at least 99% of THA procedures. Personal experience with more than 1950 THA using the superior capsulotomy technique over a 12-year period has demonstrated several observations: Dislocation rate of 0.15% (3 in 1950); Acute deep infection rate of 0% (0 in 1950); Universal applicability: used in 99.7% of primary THA; Lateral femoral cutaneous nerve palsy incidence: 0/1950; Femoral nerve palsy incidence: 0/1950; Transient peroneal palsy incidence: 2/1950; Length of stay (since 2010): 1.55 days; Discharge to home: 98%; 90-day cost (2/13 to 2/14) compared to other exposures in CMS patients in the same institution: $24,200 vs $30,100; Readmission costs (CMS 2/13 to 2/14) at 90 days: $0. Conclusion: Performing total hip arthroplasty without dislocation and with preservation of the abductors, posterior capsule and short external rotations has proven to have a low dislocation rate, a low infection rate, and wide applicability. CMS 12-month expenditure data documenting ZERO dollars spent on readmission for any reason within 90 days of surgery demonstrates the potential for simultaneously improving incomes and reducing cost, with particular benefit within the CMS BPCI and private bundled payment programs

Introduction. Total hip arthroplasty (THA) for a highly dislocated hip can be problematic and technically challenging. Our previous study on cemented THA with subtrochanteric femoral shortening osteotomy revealed a high incidence (20%) of non-union. Therefore, in 2008, we introduced reverse hybrid THA using S-ROM stem for the treatment of a highly dislocated hip. The purpose of this study was to assess the short-term clinical outcomes of this new method. Patients and methods. Between 2008 and 2014, 13 consecutive reverse hybrid THAs were performed on nine female patients with highly dislocated hips. The average age at the time of operation was 66 years (range, 55–85 years). The acetabular component was fixed in the true acetabulum with bone cement. Transverse osteotomy was performed below the lesser trochanter to shorten the femur and to prevent over-lengthening. The proximal sleeve of the S-ROM stem was then fixed within the proximal fragment, and the distal fin provided rotational stability of the distal fragment. Thus, the two fragments were fixed to each other with the S-ROM stem, and the resected segment was longitudinally cut for grafting at the junction. The postoperative follow-up period was an average of 4 years (range, 1–7 years), and no patients were lost. Preoperative and final Japanese Orthopaedic Association (JOA) hip score, operation time, bleeding amount, intraoperative and postoperative complications, bone healing at the osteotomy site, implant loosening, and revision surgery were retrospectively investigated. Results. The mean JOA hip score improved from 56 points preoperatively to 82 points postoperatively. The operation time and amount of bleeding were an average of 208 min and 643 g, respectively. The mean length of femoral resection was 4 cm (range, 2–6 cm), and the tip of the greater trochanter migrated an average distance of 7 cm (range, 5–9 cm) distally. The calculated limb lengthening was an average of 3 cm (range, 2–4 cm). Intraoperative fracture was seen in two patients, but no repeat operation was required. Two patients experienced postoperative dislocation in their hips, but additional surgery was not necessary. Postoperative nerve palsy did not occur in any patient, and all the osteotomy sites showed complete bone union. There was no implant loosening seen in any patient, and there was also no need for revision surgery. Discussion and conclusions. To achieve satisfactory outcomes with this method, resection of necessary and sufficient length of femur and accomplishment of adequate fixation between the proximal and distal fragments are necessary. In this study, dislocation occurred in two patients, and no nerve palsy was seen. Larger femoral heads may be recommended to eradicate dislocations. In cases where metaphysis of the femur is hypoplastic or the medullary canal is wide, reaming and stem insertion should be carefully performed. In this series, no additional surgery was required for the intra- and post-operative complications, and the osteotomy sites achieved bone union in all patients. Therefore, we recommend the use of this method, although longer follow-up periods are necessary

Aims. Only a small number of studies exist that report the results of EBM-produced porous coated trabecular titanium cups in primary total hip arthroplasty (THA). This study aims to investigate the patient satisfaction level, clinical function and radiographic outcomes of the patients who underwent THA using an EBM-produced porous coated titanium cup. Patients and Methods. A total of 32 patients who underwent primary THA with using an EBM-produced porous coated titanium cup from five hospitals between May and December, 2012 were retrospectively reviewed. Five patients were lost prior to the minimum 6-year follow-up. Clinical and radiographic outcomes were analyzed with an average follow-up of 81.48 (range: 77.00–87.00) months. Results. The median HHS and SF-36 scores improved significantly while the WOMAC Osteoarthritis Index decreased significantly at the latest follow-up (p<0.001). Eighteen (66.7%) patients rated their satisfaction level as very satisfied, 6 (22.2%) as satisfied, 2 (7.4%) as neutral and 1(3.7%) as dissatisfied. No intraoperative or postoperative complications were identified, including aseptic cup loosening, hip dislocation, periprosthetic joint infection, periprosthetic fracture, nerve palsy, hematoma. At the latest follow-up, all cups were considered to have achieved fixation via bone ingrowth with three or more of the five signs occurring in the most recent X-ray. However, three cups revealed radiolucent lines with a width of less than 1 mm. These radiolucent lines were distributed in the DeLee-Charnley zone 1 in 1 patient, zone 3 in another patient and the area between zone1 and zone 2 in an additional patient. The latest postoperative centers of rotation were restored nearly to the anatomic center of rotation both vertically and horizontally and the acetabular cups obtained satisfactory orientation postoperatively. Conclusions. The mid-term follow-up of patients who underwent primary THA using EBM-produced porous coated titanium cups demonstrated favorable patient satisfaction, clinical function and adequate biological fixation. Traditional risk factors for post-operative neuropathy include learning curve of surgical approach, DDH, and significant leg lengthening (>1 inch). Despite these risk factors, the most common scenario of a neuropathy is in a routine THA, by an experienced surgeon, for osteoarthritis, with no leg lengthening. Post-operative hematoma can lead to nerve compression, albeit rarely. The usual clinical presentation is of an acute event, with a previously intact nerve, sometime within the first days of surgery. Once diagnosed, immediate surgical decompression should be performed. Sciatic neuropathy is the most common, regardless of surgical approach, but the posterior approach poses the highest risk. Routine gluteus maximus tendon release may help to reduce the risk. When seen in the PACU, our approach is to immediately perform CT imaging to evaluate nerve integrity or to check on acetabular screw position. If no underlying cause is identified, the patient will be managed conservatively with foot orthotics and monitored for recovery. Femoral nerve palsy (FNP) can result in significant initial disability. Fortunately most patients recover function (although it can take over 18 months). In the early post-operative period it is often diagnosed after a patient complains of the leg giving away while attempting to walk. A knee brace will assist the patient with mobilization while the nerve recovers. The highest incidence of FNP is described for the direct lateral approach. Superior gluteal nerve (SGN) palsy is related to the direct lateral approach and may be avoided if the gluteus medius split is made within the safe zone (<5 centimeters from the tip of the greater trochanter). While early post-operative limp is common after the direct lateral approach, the true reported incidence of SGN palsy is low. Few studies showed that the persistent positive Trendelenburg test and limp is not exclusively related to the SGN damage and therefore the clinical effect of the SGN damage remains controversial. Lateral femoral cutaneous nerve can be affected during the direct anterior hip approach. One study suggests the presence of peri-incisional numbness in over 80% of patients. This is akin to numbness seen lateral to the incision after TKA. The incidence of meralgia paresthetica is extremely low (<1%)

Purpose. Crowe IV complete dislocated hips were thought to be difficult for primary THA. Correction for leg length discrepancy associated with nerve palsy or tough to reduction during surgery. Purpose of this study was to evaluate 3.5cm total leg length correction for any type of Crowe IV patients setting in anatomical positioning of acetabular cup with femoral shortening osteotomy. Materials & Methods. 24 hips were evaluated averaged 28 months after primary THA. Correction length of center of rotation, amount of femoral shortening, Harris hip score, and abductor muscle power recovery with MicroFet 2 caluculator. Bilateral cases were preformed with 6 months interval. Length of femur and tibia compared to contralateral normal side were evaluated from plain radiograms. Results. Average LLD before surgery was 7.3 (5 to 8.5) cm, but recovered within 3.5 cm (0.8 to 2.9) after surgery. One cm longer of femur in 9 patients, one cm longer of tibia in 12 patients, and 1cm longer both of femur and tibia in 5 patients compared to contralateral side before surgery. Average OR time was 92 minutes. All patients were women averaged age was 60 (34 to 75). Harris hip score improved from 45 to 92 before and latest follow-up. All patients did not need t-cane 2 years after surgery. Center of rotation was averaged 6.8 (5.2 to 8.1) cm reducted to anatomical position. Femoral shortening averaged 3.3 (1.7 to 4.5) cm, so total correction was 3.5cm (3.1–4.2). No nerve palsy and had good muscle recovery without T-cane. Patients with normal contra lateral side had averaged 2 cm (1.5∼3) heel up orthosis after surgery, but had no complaint about heel up situation in activity of daily living. Discussion & Conclusion. Amount of correction with leg length discrepancy during THA had controvertial issues especially in cases of completed dislocated hip joint. Several authors demonstrated that muscle-evoked potentials elicited by brain electric stimulation monitoring, but problems was occurred during epidural or spinal anesthesia. Averaged 3.5cm Total correction of leg length was very safe and satisfactory results for all Crowe IV patients

Technology within medicine has great potential to bring about more accessible, efficient, and a higher quality delivery of care. Paediatric supracondylar fractures are the most common elbow fracture in children and at our institution often have high rates of unnecessary long term clinical follow-up, leading to an inefficient use of healthcare and patient resources. This study aims to evaluate patient and clinical factors that significantly predict necessity for further clinical visits following closed reduction and percutaneous pinning. A total of 246 children who underwent closed reduction and percutaneous pinning following supracondylar humerus fractures were prospectively enrolled over a two year period. Patient demographics, perioperative course, goniometric measurements, functional outcome measures, clinical assessment and decision making for further follow up were assessed. Categorical and continuous variables were analyzed and screened for significance via bivariate regression. Significant covariates were used to develop a predictive model through multivariate logistical regression. A probability cut-off was determined on the Receiver Operator Characteristic (ROC) curve using the Youden index to maximize sensitivity and specificity. The regression model performance was then prospectively tested against 22 patients in a blind comparison to evaluate accuracy. 246 paediatrics patients were collected, with 29 cases requiring further follow up past the three month visit. Significant predictive factors for follow up were residual nerve palsy (p < 0 .001) and maximum active flexion angle of injured elbow (p < 0 .001). Insignificant factors included other goniometric measures, subjective evaluations, and functional outcomes scores. The probability of requiring further clinical follow up at the 3 month post-op point can be estimated with the equation: logit(follow-up) = 11.319 + 5.518(nerve palsy) − 0.108(maximum active flexion). Goodness of fit of the model was verified with Nagelkerke R2 = 0.574 and Hosmer & Lemeshow chi-square (p = 0.739). Area Under Curve of the ROC curve was C = 0.919 (SE = 0.035, 95% CI 0.850 – 0.988). Using Youden's Index, a cut-off for probability of follow up was set at 0.094 with the overall sensitivity and specificity maximized to 86.2% and 88% respectively. Using this model and cohort, 194 three month clinic visits would have been deemed medically unnecessary. Preliminary blind prospective testing against the 22 patient cohort demonstrates a model sensitivity and specificity at 100% and 75% respectively, correctly deeming 15 visits unnecessary. Virtual clinics and automated clinical decision making can improve healthcare inefficiencies, unclog clinic wait times, and ultimately enhance quality of care delivery. Our regression model is highly accurate in determining medical necessity for physician examination at the three month visit following supracondylar fracture closed reduction and percutaneous pinning. When applied correctly, there is potential for significant reductions in health care expenditures and in the economic burden on patient families by removing unnecessary visits. In light of positive patient and family receptiveness toward technology, our promising findings and predictive model may pave the way for remote health care delivery, virtual clinics, and automated clinical decision making
