The aims of this study was to determine the incidence of malnutrition in children with supracondylar fractures. It was hypothesised that the presence of malnutrition will increase the severity type of fractures. The study was a retrospective, cross-sectional study at a single institution. Children between 0 years and 12 years of age, who sustained documented supracondylar fracture treated surgically as a result of low velocity trauma were included in the study. Patients who sustained high velocity trauma, who had known bone disorders or had incomplete chart data, were excluded from the study Data was captured from children's’ notes who have been treated surgically for supracondylar fractures from casualty, theatre and the clinic notes. The nutritional status of children and fracture grade were identified and the two sets of data were compared against each other to try to identify a possible relation between fracture severity and malnutrition. Data was analysed in STATA and 5% level of significance was used to signify statistically significant associations. 150 patients were identified and included in the study. The majority of patients reviewed were in the normal nutritional range according to their z-scores. The severity of the fracture was not only associated with a poorer nutritional status however children with high and low z-scores (over weight as well as undernourished children) had the more severe fracture patterns, while children with normal z-scores had a fracture patterns of varying severity. Children who were malnourished were more likely to sustain more severe fracture types. The results highlighted the need for all children to have a good nutritional status as this may play a role in preventing more complex fractures

First rib fractures (FRFs) have historically been a marker for severe trauma and poor outcomes. The aim of this study was to assess whether an association still exists between a fractured first rib and global trauma scores suffered by the patient, examine mortality rate and identify other commonly associated injuries. This study examined patients who presented to the Rockhampton Hospital with a traumatic FRF between the dates of July 2015 to June 2020. Patient demographics, mortality rate and additional injuries sustained by the patients were collected. The Injury Severity Score (ISS) was utilised and calculated for each patient. Analysis was conducted to determine associations between trauma scores and FRFs, mortality rate and other injuries sustained at the time of rib fracture. In total, 545 patients had a rib fracture with 48 patients identified as having an FRF. Median age was 50 years. Thirty-seven (77%) were male. The most common mechanism of FRF was motor vehicle/motorbike accidents (71%). Fifty percent of patients with an observed FRF had the highest global ISS of very severe, with 13% severe, 22% moderate and 15% mild. No patients died from their injuries. Of those with an FRF, 79% experienced fractures other than ribs, 75% had other rib fractures and 52% had chest injuries. A larger than expected proportion of FRFs were not associated with severe trauma scores or high mortality. These findings suggest that patients with an FRF may have a greater chance of surviving their traumatic FRF than previously reported. Clinicians should be aware of the potential for severity and other associated injuries, such as chest wall fractures and thoracic injuries, when treating a patient with a fracture of their first rib

Aim. This study seeks to outline the clinical, laboratory, and imaging features of patients with pyogenic spondylitis. It aims to define a novel imaging sign that could indicate the severity of suppurative spondylitis, aiding in its early diagnosis and treatment. Method. This retrospective study included 137 patients from 2013 to 2023. Through the analysis and summary of imaging characteristics among all patients, we identified a distinct MRI sign known as ‘the Disc Penetration sign’ (DP). This sign is defined as an image finding on sagittal MRI depicting the anterior and posterior penetration of an abscess through the intervertebral disc space, affecting both the anterior margin of the vertebrae and the structures within the spinal canal. Observational parameters included WBC, ESR, CRP, hemoglobin, and albumin levels. Documentation of the study included location and segment of the lesion, presence or absence of spinal cord compression, and paravertebral abscesses. Results. 56 patients presented with the Disc Penetration sign(DP) and 81 did not. In both groups, there were no significant differences in gender ratio or age (P > 0.05). However, significant differences were observed in the presence of comorbid diabetes and chronic kidney disease (p < 0.05). The DP group had a significantly greater ESR level (74.30±33.79 mm/h vs. 51.46±30.46 mm/h, P < 0.001) and CRP level (47.28 mg/L vs. 26.18 mg/L, P = 0.003). Additionally, the DP group had a significantly lower Hb (100.66±19.82 g/L vs. 116.99±19.99g/L,P < 0.001) and the serum albumin level (28.81±6.59 g/L vs. 34.09±6.17 g/L,P < 0.001). Imaging results showed no significant differences in affected spinal segments or parts (p>0.05). Patients in the DP group showed a higher likelihood of developing paravertebral abscesses compared to those in the non-DP group (n = 54 [96.4%] vs. n = 33 [40.7%], P < 0.001), and also exhibited a higher incidence of spinal cord compression(n = 32 [57.1%] vs. n = 17 [21.0%], P < 0.001). Conclusions. The study suggests that the Disc Penetration sign in pyogenic spondylitis patients correlates with more severe inflammation and higher incidence of paraspinal abscess, pointing to worse stability of the spine, longer bone restructuring time, and potentially poorer prognosis. These findings enable clinicians to rapidly assess the severity of the disease and prognosticate outcomes more effectively We emphasize the need for early, pathogen-specific diagnosis and treatment, particularly considering surgical intervention for patients demonstrating substantial paraspinal abscesses or spinal instability

Multiple studies have shown that the symptomatology of knee osteoarthritis weakly correlate to radiographic severity of disease. Current literature however does not have much in the way of comparing functional outcomes of those with OA knees with radiographic severity. Our objective was to compare radiographic measurements of OA knees with self-reported functional outcomes and determine if radiographic severity of OA knees correlated with loss of functional ability. A retrospective review of prospectively collected registry data of 305 patients with osteoarthritis of the knee was collected. The patient's x-rays were reviewed, and radiographic measurements were taken to include medial, lateral and patellofemoral joint space distance measured in millimetres. The Kellgren and Lawrence, and Ahlback classifications of radiographic knee OA were computed. These were correlated with severity of functional limitations was measured using the SF36, Knee society score (KSS) and Oxford knee scores. Statistical analysis were conducted with SPSS V22.0 statistical software. Demographic characteristics and functional assessments were analysed using one way ANOVA test. Post-hoc test using Tukey HSD and effect size (partial-eta squared η. 2. ) was performed if one-way ANOVA was found to be statistically significant. A p-value of 0.05 or less was considered statistically significant. Pre-operative patient demographics are shown in table 1. Patients in with Grade 2 osteoarthritis were significantly younger than Grade 4 patients (post-hoc p=0.003). There were no statistically significant differences in age between the other Grades, and there were no differences in BMI or gender or operative site between all grades. There were significant differences in KSS Function scores between Grade 2 and Grade 3 patients (post-hoc p=0.017) and Grade 2 and 4 patients (post-hoc p < 0 .001). Statistically significant differences were also found between Grade 1 and Grade 4 patients for the KSS Knee score (post-hoc p=0.016). There were significant differences in Oxford knee score (post-hoc p=0.026) and SF- Physical Function (post-hoc p < 0 .001) between Grade 2 and Grade 4 patients too. The effect size η. 2. for KSS Function, KSS Knee and Oxford knee score was 0.05, 0.06 and 0.33 respectively. When comparing the loss of joint space with the functional scores, there were no statistically significant correlations. Our study show that the radiological severity of knee osteoarthritis based on the two scoring methods was able to correlate with worsening functional scores. Most notably, the differences in KSS function scores correlated strongly between Grade 2 and Grade 3 patients. Of note, there was no correlation between the loss of joint space and the severity of functional limitations across any of the scoring systems. Our study showed that although both the Kellgren and Lawrence and Ahlback radiological grading of Osteoarthritis were able to correlate with worsening functional scores, this was not due to loss of joint space alone and further studies need to be conducted on the other contributors to the scoring system such as osteophytes and subchondral sclerosis. Aims. The aim of this study was to determine the impact of the severity of anaemia on postoperative complications following total hip arthroplasty (THA) and total knee arthroplasty (TKA). Methods. A retrospective cohort study was conducted using the American College of Surgeons National Quality Improvement Program (ACS-NSQIP) database. All patients who underwent primary TKA or THA between January 2012 and December 2017 were identified and stratified based upon hematocrit level. In this analysis, we defined anaemia as packed cell volume (Hct) < 36% for women and < 39% for men, and further stratified anaemia as mild anaemia (Hct 33% to 36% for women, Hct 33% to 39% for men), and moderate to severe (Hct < 33% for both men and women). Univariate and multivariate analyses were used to evaluate the incidence of multiple adverse events within 30 days of arthroplasty. Results. Following adjustment, patients in the THA cohort with moderate to severe anaemia had an increased odds of 6.194 (95% confidence interval (CI) 5.679 to 6.756; p < 0.001) for developing any postoperative complication. Following adjustment, patients in the TKA cohort with moderate to severe anaemia had an increased odds of 5.186 (95% CI 4.811 to 5.590; p < 0.001) for developing any postoperative complication. Among both cohorts, as severity increased, there was an increased risk of postoperative complications. Conclusion. Preoperative anaemia is a risk factor for complications following primary arthroplasty. There is a significant relationship between the severity of anaemia and the odds of postoperative complications. Patients who had moderate to severe anaemia were at increased risk of developing postoperative complications relative to patients with mild anaemia. When considering elective primary THA or TKA in a moderately or severely anaemic patient, surgeons should strongly consider correcting anaemia prior to surgery if possible. Aims. To assess the impact of the declaration of the state of emergency due to the COVID-19 pandemic on the number of visits to a traumatology emergency department (ED), and on their severity. Methods. Retrospective observational study. All visits to a traumatology ED were recorded, except for consultations for genitourinary, ocular and abdominal trauma and other ailments that did not have a musculoskeletal aetiology. Visit data were collected from March 14 to April 13 2020, and were subsequently compared with the visits recorded during the same periods in the previous two years. Results. The number of visits dropped from a mean of 3,212 in 2018 to 2019 to 445 in 2020. Triage 1 to 3 level visits rose from 21.6% in 2018 to 2019% to 40.4% in 2020, meaning a reduction in minor injury visits and an increase in major ones. There was a relative reduction of 13.2% in femoral fractures in the elderly. The rate of justified visits rose from 22.3% to 48.1%. Conclusion. A marked drop in the total number of visits to our traumatology ED was observed, as well as a relative increase in major injury visits and a relative fall in the minor ones. The lower limbs of vehicle occupants are vulnerable to severe injuries during under vehicle explosions. Understanding the injury mechanism and causality of injury severity could aid in developing better protection. Therefore, we tested three different knee positions in standing occupants (standing, knee in hyper-extension, knee flexed at 20˚) of a simulated under‐vehicle explosion using cadaveric limbs in a traumatic blast injury simulator; the hypothesis was that occupant posture would affect injury severity. Skeletal injuries were minimal in the cadaveric limbs with the knees flexed at 20˚. Severe, impairing injuries were observed in the foot of standing and hyper‐extended specimens. Strain gauge measurements taken from the lateral calcaneus in the standing and hyper-extended positions were more than double the strain found in specimens with the knee flexed position. The results in this study demonstrate that a vehicle occupant whose posture incorporates knee flexion at the time of an under‐vehicle explosion is likely to reduce the severity of lower limb injuries, when compared to a knee extended position

This is the first study in the Ponseti-era to compare severity and outcomes in cases of idiopathic congenital talipes equinovarus (CTEV) diagnosed antenatally versus those diagnosed at birth. Small pre-Ponseti studies showed antenatal diagnosis to be a predictor of severity and poor prognosis. Prospective data collection was used to compare indicators of severity and outcomes for idiopathic CTEV between these two groups. These include Pirani score, number of casts, follow-up Roye score and need for surgery. 68 children with 106 affected feet were included. Antenatal diagnosis (AD) was made in 45 children (71 feet), with birth diagnosis (BD) in 23 children (35 feet). Mean follow-up age was 4.8 years (AD = 4.9, BD = 4.7), male:female ratio 2:1 (AD=BD) with bilateral CTEV in 55% (AD = 58%, BD=52%). Mean initial Pirani scores were 5.25 in the AD group vs 4.86 in the BD group (p=0.06). Mean Roye score at follow-up was 1.39/4 in the AD group vs 1.26/4 in the BD group (p=0.33) with 33% vs 30% complaining of pain respectively (p=0.80). Surgery was needed in 11/71 (15.5%) in the AD group vs 1/35 (2.9%) in the BD group (p=0.06)). There is no significant difference in severity between antenatal and birth diagnoses of idiopathic CTEV and no difference in outcomes between these groups when treated with the Ponseti regime. Although small, our sample size is greater than the largest published comparable study

Laterality and bilaterality have been reported as prognostic variables in DDH outcomes. However, there is little clarity across the literature on the reporting of laterality in developmental dysplasia of the hip (DDH) due to the variability in severity of the condition. It is widely accepted that the left hip is most frequently affected; however, the true incidence of unilateral left, unilateral right and bilateral cases can be hard to quantify and compare across studies. The purpose of this study was to examine laterality accounting for graded severity in a multicentre, international prospective observational study of infants with hip dysplasia in order to demonstrate the complexity of this issue. A multicentre, prospective database of infants diagnosed with DDH between the ages of 0 and 18 months was analysed from 2010 to April 2015. Patients less than six months were enrolled in the study if at least one hip was frankly dislocated. Patients between 6 and 18 months were enrolled if they had any form of hip dysplasia. Each hip was classified as reduced, dysplastic, dislocatable/subluxable, dislocated reducible or dislocated irreducible. Baseline diagnosis was used to classify patients into a graded laterality category accounting for hip status within the DDH spectrum. A total of 496 patients were included in the analysis; 328 were <6 months old at diagnosis and 168 were between 6 and 18 months old. Of these patients, 421 had at least one frankly dislocated hip. Unilateral left hip dislocations were most common, with 223 patients, followed by unilateral right and bilateral dislocations with 106 and 92 respectively. Stratifying these patients based on status of the contralateral hip, 54 unilateral left and 31 unilateral right dislocated patients also had a dysplastic or unstable contralateral hip. There were significantly fewer bilateral patients in the 6–18 month group (p=0.0005). When classifying laterality by affected hip, bilaterality became the predominant finding, comprising 42% of all patients. The distribution of unilateral left, unilateral right and bilateral cases was greatly impacted by the method of classification. Distinct patterns were seen when considering dislocated hips only, or when considering both dislocated and dysplastic/unstable hips. Findings from this multicentre prospective study demonstrate the necessity to account for the graded severity in hip status when reporting DDH laterality. In order to accurately compare laterality across studies, a standardised, comprehensive classification should be established, as contralateral hip status may impact prognosis and treatment outcomes

Aims. To determine whether a correlation exists between the clinical symptoms and signs of impingement, and the severity of the lesions seen at bursoscopy. Methods. Fifty-five consecutive patients who underwent arthroscopic subacromial decompression were analysed. Pre-operatively patients completed an assessment form consisting of visual analogue pain score, and shoulder satisfaction. The degree of clinical impingement was also recorded. At arthroscopy impingement was classified according to the Copeland-Levy classification. Clinical assessment and scoring was performed at 6 months post-operatively. Linear regression coefficients were calculated to determine if the degree of impingement at arthroscopy correlated with pre-operative pain, satisfaction and clinical signs of impingement. Results. Pre-op pain levels, shoulder satisfaction and the degree of clinical impingement did not correlate with severity of the lesions of the acromion and cuff. (Average Correlation co-efficient r2 = 0.018). There was no correlation between the improvement in the shoulders post-operatively and the severity of lesions (r2 = 0.008). Conclusion. There is no significant correlation between pain, clinical signs, or satisfaction and the severity of lesions seen at arthroscopy

Introduction. Geometric variations of the hip joint can give rise to repeated abnormal contact between the femur and acetabular rim, resulting in cartilage and labrum damage. Population-based geometric parameterisation can facilitate the flexible and automated in silico generation of a range of clinically relevant hip geometries, allowing the position and size of cams to be defined precisely in three dimensions. This is advantageous compared to alpha angles, which are unreliable for stratifying populations by cam type. Alpha angles provide an indication of cam size in a single two-dimensional view, and high alpha angles have been observed in asymptomatic individuals. Parametric geometries can be developed into finite element models to assess the potential effects of morphological variations in bone on soft tissue strains. The aim of this study was to demonstrate the capabilities of our parameterisation research tool by assessing impingement severity resulting from a range of parametrically varied femoral and acetabular geometries. Methods. Custom made MATLAB (MathWorks) and Python codes. [1]. were used to generate bone surfaces, which were developed into finite element models in Abaqus (SIMULIA). Parametric femoral surfaces were defined by a spherical proximal head and ellipse sections through the neck/cam region. This method produced surfaces that were well fitted to bone geometry segmented from CT scans of cam patients and capable of producing trends in results similar to those found using segmented models. A simplified spherical geometry, including the labrum and acetabular cartilage, represented the acetabulum. Femoral parameters were adjusted to define relevant variations in cam size and position. Two radii (small and large cams) and two positions (anterior and superior cams) were defined resulting in four models. Alpha angles of these parametric femurs were measured in an anterior-posterior view and a cross-table lateral view using ImageJ (NIH). A further model was developed using a femur with a medium cam size and position, and the level of acetabular coverage and labrum length were varied. Bones were modelled as rigid bodies and soft tissues were modelled as transversely isotropic linearly elastic materials. With the acetabulum fully constrained in all cases, the femurs were constrained in translation and rotated to simulate flexion followed by internal rotation to cause impingement against the labrum. Results and Discussion. Models generated using the parametric approach showed that potential for tissue damage, indicated through local strain, was not predicted by measured alpha angle, but resulted from cam extent and position as defined by the ellipses. When variations were made to the acetabular rim, an increase in bone coverage had the greatest effect on impingement severity, indicated by strain in the cartilage labral-junction. An increase in labral length increased labral displacement, but had less effect on cartilage-labral strain. Patient specific models currently require full image segmentation, but there is potential to further develop these parametric methods to assess likely impingement severity based on a series of measures of the neck and acetabulum when three-dimensional imaging of patients is available

Introduction and Aims. Clinically many factors such as variations in surgical positioning, and patients' anatomy and biomechanics can affect the occurrence and severity of edge loading which may have detrimental effect on the wear and durability of the implant. Assessing wear of hundreds of combinations of conditions would be impractical, so a preclinical testing approach was followed where the occurrence and severity of edge loading can be determined using short biomechanical tests. Then, selected conditions can be chosen under which the wear can be determined. If a wear correlation with the magnitude of dynamic separation or the severity of edge loading can be shown, then an informed decision can be made based upon the biomechanical results to only select important variables under which the tribological performance of the implant can be assessed. The aim of this study was to determine the relationship between the wear of ceramic-on-ceramic bearings and the (1) magnitude of dynamic separation, (2) the maximum force reached during edge loading and (3) the severity of edge loading resulting from component translational mismatch between the head and cup centres. Methods. The Leeds II hip joint simulator with a standard walking cycle and 36mm diameter ceramic-on-ceramic bearings (BIOLOX. ®. delta, DePuy Synthes Joint Reconstruction, Leeds, UK.) were used. The study was in two parts. Part one: a biomechanical study where the dynamic separation, the maximum load during edge loading, and the duration of edge loading alongside the magnitude of forces under edge loading (severity of edge loading) were assessed. Part two; a wear study where the wear rates of the bearing surfaces were assessed under a series of input conditions. These input testing conditions included inclining the acetabular cups at 45° and 65° cup inclination angle (in-vivo equivalent), with 2, 3, and 4mm medial-lateral component mismatch between the centres of the head and the cup. This equated to six conditions being assessed, each with three repeats for the biomechanical test, and six repeats completed for the wear study. The severity of edge loading was assessed as described in Equation 1. Severity of Edge Loading = ∫. t. t0. F(x) dx + ∫. t. t0. F(y) dy … Equation 1,. where F(x) is the axial load, F(y) is the medial-lateral load and t-t0 is the duration of edge loading. The wear of the ceramic bearings were determined using gravimetric analysis (XP205, Mettler Toledo, UK). Results. The wear rates of ceramic-on-ceramic bearings increased as the magnitude of dynamic separation (Figure 1), the maximum load at the rim during edge loading (Figure 2), and the severity of edge loading (Figure 3) increased. The magnitude of dynamic separation was found to have the highest correlation to the wear rate under the conditions tested in this study (R. 2. =0.94). Conclusions. A preclinical testing approach has been developed to understand the occurrence and severity of edge loading associated with variation of component positioning. A good correlation was found between the wear rates obtained for ceramic-on-ceramic bearings and the magnitude of parameters obtained under edge loading during a short-term biomechanical study. Introduction and Aims. There are many variables that can affect the occurrence and severity of edge loading in hip replacement. A translational mismatch between the centres of rotation of the head and cup may lead to dynamic separation, causing edge loading and increased wear. Combining a steep inclination angle with such translational mismatch in the medial-lateral axis caused a larger magnitude of separation and increased severity of edge loading. Previous studies have shown variation in the hip Swing Phase Load (SPL) during gait between different patients. The aim of this study was to apply a translational mismatch and determine the effect of varying the SPL on the occurrence and severity of edge loading under different cup inclination angles in a hip joint simulator. Methods. The Leeds II hip joint simulator with a standard gait cycle and 36mm diameter ceramic-on-ceramic bearings (BIOLOX. ®. delta) were used in this study. The study was in two stages; [1] a biomechanical study where the magnitude of dynamic separation, the duration of edge loading and the magnitude of force under edge loading (severity) were assessed under variations in component positioning and SPLs. [2] A wear study to assess edge loading with selected input conditions. For the biomechanical study, a combination of four mismatches, three cup inclination angles, and eight SPLs (Table 1) were investigated. For the wear study, three SPL conditions were selected with one cup angle and one mismatch (Table 1). Three million cycles were completed under each condition. Mean wear rates and 95% confidence limits were determined and statistical analysis (one way ANOVA) completed (significance taken at p<0.05). Table 1: Study matrix. Results. For any given translational mismatch or cup inclination angle, increasing the SPL from 50N to 450N resulted in a decrease in the magnitude of dynamic separation (Figures 1 and 2). In some scenarios when the mismatch between the centres of rotation was low and the SPL was high, no separation was observed. Under 150N SPL, the severity of edge loading was similar to that determined for the 50N SPL conditions although the magnitude of dynamic separation was lower. Higher wear rates were found for the 70N and 150N compared to 300N SPL (Figure 3). No significant difference was found between wear rate under the SPLs of 70N and 150N (p=0.05), but significant differences were found between the wear rates under 150N and 300N SPL and between 70N and 300N SPL (p<0.01 and p<0.01 respectively). Conclusion. The SPL contributed to the resistance of separation between the head and the cup, hence a lower dynamic separation was measured under higher SPL. The wear study demonstrated that edge loading was present even under a higher SPL. For figures/tables, please contact authors directly.

Introduction. Corrosion at modular junctions of total hip replacements has been identified as a potential threat to implant longevity, resulting in efforts to determine appropriate countermeasures. Visual scoring and volumetric material loss measurements have been useful tools to elucidate various clinical and design factors associated with corrosion damage. However, corrosion involves electron exchange that results in chemical changes to biomedical alloys, and electrochemical assessment may therefore be a more appropriate approach to understand the phenomenon. The purpose of this pilot study was to electrochemically distinguish the severity of corrosion in retrieved femoral heads. A secondary goal was to identify the potential of electrochemical impedance spectroscopy (EIS) as a method to identify different forms of corrosion damage. Methods. Twenty femoral heads were identified from a larger study of total hip replacements, obtained as part of an ongoing multi- center IRB-approved retrieval program. Using a previously established 4-point scoring method, components were binned by taper damage: 10 components were identified as having severe damage, 7 with moderate damage and 2 with mild damage. One (1) unimplanted control was included to represent minimal corrosion damage. All components were then characterized using electrochemical impedance spectroscopy under the frequency domain: a 10 mV sinusoidal voltage, ranging from 20 kHz to 2 mHz, was applied to the taper of a femoral head (working electrode) filled with a 1M solution of PBS, a platinum counter electrode and a chlorided silver reference electrode. Absolute impedance at 2 mHz (|Z. 0.002. |), and max phase angle (θ) were assessed relative to taper damage severity. After least-squares fitting of the EIS data to a Randles circuit with a constant phase element, circuit elements: polarization resistance (Rp), CPE-capacitance, and CPE-exponent were also evaluated. The seven (7) most severely corroded components were further examined with scanning electron microscopy to identify corrosion modes. For all statistical analyses, significance was determined at alpha=0.05. Results. Taper damage was strongly correlated with both |Z. 0.002. | (ρ = −0.857, p<0.001) and CPE-capacitance (ρ=0.913, p<0.001). Taper damage was moderately associated with max phase angle (ρ= −0.483, p=0.031), CPE-exponent (ρ= −0.653, p=0.002) and Rp (ρ=0.556, p=0.011). Log-log plots of the strongest predictors of taper damage (|Z. 0.002. | and CPE- capacitance) identified some clustering among severely corroded components. SEM analysis identified evidence of grain/phase boundary corrosion on four components, all with log CPE-capacitance ≥ −4.4. Discussion. The results of this pilot study highlight that electrochemical impedance spectroscopy is useful in determining corrosion severity in retrieved femoral heads, and may also identify intergranular corrosion attack. For an undamaged taper, the self- passivating behavior of CoCrMo creates a surface that opposes charge transfer, but greater corrosion appears to compromise this barrier. The observed trend of low impedance but high capacitance for severely corroded components with intergranular corrosion may signal charge storage at the boundaries of individual grains. Additional work is underway to characterize this behavior

Depression and anxiety are widely prevalent amongst patients suffering from chronic diseases including osteoarthritis. National Institute for Clinical Excellence (NICE) guidelines recommend vigilance and routine screening in such chronic disease patients, and a recent King's Fund report found depression causes considerable expense to the NHS, with £1 for every £8 spent on chronic disease spent on patients mental health, costing between £8-13 billion annually. Depression increases length of hospital stay, and poorer clinical outcomes: depressed patients are 3.5 x as likely to die after suffering from myocardial infarction. Despite this, depression screening is not performed widely pre operatively. To see if there is a basis to recommend routine pre operative depression screening in hip and knee arthroplasty patients, we asked all patients attending Joint School to complete a Hospital Anxiety and Depression Scale (HADS). This is a well-validated tool for assessing the presence and degree (either mild, moderate or severe if present) of anxiety and depression, using fourteen questions, and providing a score out of 21 for each. We then correlated these results to pre operative Oxford Knee and Oxford Hip Scores to see if there was any relationship between the extent of impact of disease and any mood disorder present. 190 patients completed the questionnaire pre operatively. Of those, 82 (43%) scored as anxious. 73 (38%) scored as depressed. 47 (25%) scored as both. Overall 107 (56%) were either anxious, depressed or both. The median anxiety score was 7, with 44 (54%) scoring 8–10 (mild), 35 (43%) 11–15 (moderate) and 3 (4%) 16+ (severe). The median depression score was 6, with 53 (73%) scoring 8–10 (mild), 19 (26%) 11–15 (moderate) and 1 (1%) 16+ (severe). In order to look for an association between the severity of depression or anxiety and Oxford Scores, we grouped the Oxford Scores according to the patient's score on the HADS, and performed analysis of variance (ANOVA) to look for a significant difference between the Oxford Scores in the groups. There was no significant link between increase in Oxford Score and anxiety score (p=0.173314) but there was between Oxford Score and depression score (p=0.001377). There was equally no correlation between scores in patients classified simply as anxious or not anxious (p=0.14918) but a significant difference in Oxford Score was present between patients designated as depressed or not depressed (p=0.000297). We thus conclude depression and anxiety are very common amongst pre operative arthroplasty patients and thus assessment for this should be considered routinely preoperatively. In addition, there is a link between severity of osteoarthritis and severity of depression with increasing depression score associated with increasing Oxford Score. This makes further work to assess the reasons for this link imperative: does increasing severity of osteoarthritis result in increasing severity of depression, or does depression cause patients to feel the effects of their disease more keenly, and thus score higher on Oxford Scores? If this is the case, would patients treated for depression find the effect of their joint problems severe enough to warrant undertaking arthroplasty surgery?

Purpose of study. Pin tract infection is a common complication with the use of circular external fixators. HIV infection itself is frequently quoted as a relative contra-indication for the use of circular external fixators for complex trauma and limb reconstruction in HIV seropositive individuals. Methods. Between July 2008 and December 2012, 286 patients were treated with circular external fixators at our tertiary level government hospital. A retrospective review was undertaken to compare the rate and severity of pin tract sepsis in HIV seropositive and seronegative patients. Results. Two-hundred and twenty-three patients met the inclusion and exclusion criteria. Pin tract sepsis was found in 51 patients overall (22.8%). The incidences of pin tract sepsis in the seropositive group, seronegative group, and the unknown group are 22.5%, 22.8% and 23.8% respectively, and the differences were not statistically significant. The severity of pin tract sepsis in the individual groups was also similar. Conclusion. Pin tract sepsis is a common complication with the use of circular external fixators. The incidence and severity of pin tract infection is not influenced by HIV infection, and should not in itself deter from the use of circular external fixators for complex trauma and limb reconstruction in HIV seropositive individuals. Introduction

We assessed the role of four different High Tibial osteotomies (HTOs) for medial compartment osteoarthritis of knee (MCOA): Medial Opening Wedge High Tibial Osteotomy (MOWHTO), Focal Dome Osteotomy with Ilizarov Fixator (FDO-I), intra-articular, Tibial Condylar Valgus Osteotomy with plating (TCVO-P) and intra-articular plus extra-articular osteotomy with Ilizarov(TCVO-I); in correcting three deformity categories: primary coronal plane varus measured by Mechanical Axis deviation (MAD), secondary intra-articular deformities measured by Condylar Plateau Angle (CPA) and Joint Line Convergence Angle (JLCA), and tertiary sagittal, rotational and axial plane deformities in choosing them.

Materials and Methods

We retrospectively studied HTOs in 141 knees (126 patients). There were 58, 40, 26, and 17 knees respectively in MOWHTO, FDO-I, TCVO-P and TCVO-I. We measured preoperative (bo) And postoperative (po) deformity parameters.

It has been proposed that the amount of energy transferred to the bone during a high velocity projectile injury determines the extent of bony injury. We studied the validity of this theory Fresh rear skeletally mature deer femurs were subjected to progressively increasing velocity projectile injuries within a pneumatic ballistic chamber with non-deforming steel spheres capturing the energy transferred. Analysis of fracture severity was performed including micro computer tomography analysis of micro-fractures. The effect of projectile caliber size was then analyzed. Characteristic fractures patterns were observed with fracture lines extending radially from the impact site, often propagating longitudinally along the sample. It was found that a greater energy transfer resulted in more severe fracture for a given projectile. However, fractures of differing severity were produced by different projectiles for similar energy transfer. Neither specific energy transfer nor energy density could explain this phenomenon. Although energy transfer plays a role in ballistic fracture, it is not the sole determinant. Other factors such as contact surface area, projectile mass and angle of impact may need to be considered

Introduction and Aims. In order to improve the longevity and design of an implant, a wide range of pre-clinical testing conditions should be considered including variations in surgical delivery, and patients' anatomy and biomechanics. The aim of this research study was to determine the effect of the acetabular cup inclination angle with different levels of joint centre mismatch on the magnitude of dynamic microseparation, occurrence and severity of edge loading and the resultant wear rates in a hip joint simulator. Methods. The six-station Leeds Mark II Anatomical Physiological Hip Joint Simulator and 36mm diameter ceramic-on-ceramic bearings (BIOLOX® delta) were used in this study. A standard gait cycle, with a twin-peak loading (2.5kN peak load and approximately 70N swing phase load), extension/flexion 15°/+30° and internal/external ±10° rotations, was applied. Translational mismatch in the medial-lateral axis between the centres of rotation of the head and the cup were considered. In this study, mismatches of 2, 3 and 4 (mm) were applied. Two acetabular cup inclination angles were investigated; equivalent to 45° and 65° in-vivo. These resulted in a total of six conditions [Figure 1] with n=6 for each condition. Three million cycles were completed under each condition. The lubricant used was 25% (v/v) new-born calf serum supplemented with 0.03% (w/v) sodium azide to retard bacterial growth. The wear of the ceramic bearings were determined using a microbalance (XP205, Mettler Toledo, UK) and a coordinate measuring machine (Legex 322, Mitutoyo, UK). The stripe wear was analysed using RedLux software. The dynamic microseparation displacement was measured using a linear variable differential transformer. Mean wear rates and 95% confidence limits were determined and statistical analysis (one way ANOVA) completed with significance taken at p<0.05. Results Increasing the medial-lateral joint centre mismatch from 2 to 3 to 4mm resulted in an increased dynamic microseparation [Figure 2]. A similar trend was observed for the wear. A higher level of medial-lateral mismatch increased the wear rate under both 45° and 65° cup inclination angle conditions [Figure 3]. The mean wear rates obtained under 65° were significantly higher compared to those obtained under the 45° cup inclination angle conditions for a given medial-lateral mismatch in the joint centre (p=0.02 for 2mm mismatch, p=0.02 for 3 mm mismatch, and p<0.01 for 4mm mismatch). Conclusions. The condition with the acetabular cups positioned at an inclination of 45° exhibited greater resistance to dynamic microseparation for any given medial-lateral mismatch in the centres of rotation. Higher wear rates correlated with higher levels of dynamic microseparation. These results highlight how different conditions can alter the severity of edge loading, and highlight the necessity of understanding how the surgical positioning can affect the occurrence of edge loading and wear. Future studies will look into the other factors which can influence the microseparation conditions such as joint laxity, swing phase load and version angles

Introduction. Grade IIIa/b open tibial fractures (OTF) result mostly following high energy trauma and may be accompanied by significant associated injuries. Aim of this study was to compare the characteristics and the outcome of OTF between patients of different injury severities. Materials and methods. Retrospective analysis of the records of patients admitted to a level I centre in a 4 year period with an OTF. Epidemiologic data, AO-ASIF classification, method of treatment, complication rates and time to fracture-union have been statistically analysed. An AIS98-ISS threshold (ISS 16) was used to separate OTF in 2 groups (A<16, B>16). Results. Forty-one cases of OTF have been retrieved. Group A included 28 and Group B 13 patients. Gender ratio and mechanisms of injuries did not show significant differences. Median age was 35.5 vs 41 years and median ISS 9.7 vs 26 for A and B groups respectively. AO types 42.A and 42.B were identified in 25% vs 23%, and type 42.C in 50% and 53% respectively. External fixation was used in 33% of the less severely injured and in 61% of the severely injured OTF; nailing techniques in 54% and 31% and covering flaps in 57% and 77% in that order. Secondary interventions were recorded in 38% of the B group but only in 17% of the A group. Median time to union was prolonged for over 1 month in the more severely injured group (28 vs 24 months). Complication rates had significant higher representation to the polytrauma group (39% and 17%). Conclusion. Treatment and prognosis of severe open fractures was proven to be influenced by the overall injury severity in our patients' group. The achieved healing rates for both groups were acceptable, but fracture union required more time with increasing injury severity score
