Aims. Dual mobility (DM) bearings are an attractive treatment option to obtain hip stability during challenging primary and revision total hip arthroplasty (THA) cases. The purpose of this study was to analyze data submitted to the American Joint Replacement Registry (AJRR) to characterize utilization trends of DM bearings in the USA. Methods. All primary and revision THA procedures reported to AJRR from 2012 to 2018 were analyzed. Patients of all ages were included and subdivided into DM and traditional bearing surface cohorts. Patient demographics, geographical region, hospital size, and teaching affiliation were assessed. Associations were determined by chi-squared analysis and logistic regression was performed to assess outcome variables. Results. A total of 406,900 primary and 34,745 revision THAs were identified, of which 35,455 (8.7%) and 8,031 (23.1%) received DM implants respectively. For primary THA, DM usage increased from 6.7% in 2012 to 12.0% in 2018. Among revision THA, DM use increased from 19.5% in 2012 to 30.6% in 2018. Patients < 50 years of age had the highest rates of DM implantation in every year examined. For each year of increase in age, there was a 0.4% decrease in the rate of DM utilization (odds ratio (OR) 0.996 (95% confidence interval (CI) 0.995 to 0.997); p < 0.001). Females were more likely to receive a DM implant compared to males (OR 1.077 (95% CI 1.054 to 1.100); p < 0.001). Major teaching institutions and smaller hospitals were associated with higher rates of utilization. DM articulations were used more commonly for dysplasia compared with osteoarthritis (OR 2.448 (95% CI 2.032 to 2.949); p < 0.001) during primary THA and for instability (OR 3.130 (95% CI 2.751 to 3.562) vs poly-wear; p < 0.001) in the revision setting. Conclusion. DM articulations showed a marked increase in utilization during the period examined. Younger patient age, female sex, and hospital characteristics such as teaching status, smaller size, and geographical location were associated with increased utilization. DM articulations were used more frequently for primary THA in patients with dysplasia and for revision THA in patients being treated for instability. Primary total hip replacement (THR) is a successful and common operation which orthopaedic trainees must demonstrate competence in prior to completion of training. This study aimed to determine the impact of operating surgeon grade and level of supervision on the incidence of 1-year patient mortality and all-cause revision following elective primary THR in a large UK training centre. National Joint Registry (NJR) data for all elective primary THR performed in a single University Teaching Hospital from 2005–2020 were used, with analysis performed on the 15-year dataset divided into 5-year temporal periods (B1 2005–2010, B2 2010–2015, B3 2015–2020). Outcome measures were mortality and revision surgery at one year, in relation to lead surgeon grade, and level of supervision for trainee-led operations. 9999 eligible primary THR were undertaken, of which 5526 (55.3%) were consultant led (CL), and 4473 (44.7%) trainees led (TL). Of TL, 2404 (53.7%) were non-consultant supervised (TU), and 2069 (46.3%) consultant supervised (TS). The incidence of 1-year patient mortality was 2.05% (n=205), and all-cause revision was 1.11% (n=111). There was no difference in 1-year mortality between TL (n=82, 1.8%) and CL (n=123, 2.2%) operations (p=0.20, OR 0.78, CI 0.55–1.10). The incidence of 1-year revision was not different for TL (n=56, 1.3%) and CL (n=55, 1.0%) operations (p=0.15, OR 1.37, CI 0.89–2.09). Overall, there was no temporal change for either outcome measure between TL or CL operations. A significant increase in revision within 1-year was observed in B3 between TU (n=17, 2.7%) compared to CL (n=17, 1.0%) operations (p=0.005, OR 2.81, CI 1.35–5.87). We found no difference in 1-year mortality or 1-year all-cause revision rate between trainee-led primary THR and consultant-led operations over the entire fifteen-year period. However, unsupervised trainee led THR in the most recent 5-year block (2015–2020) has a significantly increased risk of early revision, mainly due to instability and prosthetic joint infection. This suggests that modern surgical training is having a detrimental effect on THR patient outcomes. More research is needed to understand the reasons if this trend is to be reversed

Introduction. Total joint arthroplasty rates have increased dramatically in recent decades. However, a comprehensive analysis of trends in revision total hip arthroplasty has not been performed recently to address the changing volume, costs, and location of these complex cases. We sought to identify trends in volume of these procedures, geographic distribution changes, and cost trends using a national sample. Materials and methods. The National Inpatient Sample, a representative sample of all hospital discharges within the United States, was used to determine the volume of revision total hip arthroplasty (THA) from 1993 to 2014. Procedures were identified by ICD-9 codes corresponding to revision THA. Annual incidence of revision THA was compared to annual incidence of primary THA to determine whether relative growth of revisions differed proportionally from the primary procedure. State-specific data was analyzed where available to develop geographic trend maps in the incidence of revision THA procedures using the estimated state population for years under review. Trends were also reviewed for hospital location (urban versus rural; teaching versus non-teaching) and total hospital charges. Analysis of trends was performed using linear regression models. Results. Volume of revision THA increased slower than primary THA from 1993 to 2014. Revision THA volume increased from 28,429 in 1993 to 48,295 in 2014. However, the relative proportion of revision compared to primary THA during this time period decreased from 21.3% in 1993 to 13.0% in 2014. While revision THA grew significantly at 925 additional procedures per year (p <0.001), the revision/primary proportion declined at a rate of −0.4 percentage points annually (p <0.001). Nationally, only one state demonstrated statewide incidence rate of revision THA >100 / 100,000 residents in 2001, while 31 of 36 states reporting in 2014 demonstrated statewide incidence rate of revision THA >100 / 100,000 residents. Revision THA shifted dramatically away from rural and urban non-teaching hospitals to urban teaching hospitals. In the year 2000, 52.3% of revision THA was performed in urban teaching hospitals, which had increased to 71.2% by 2014. Only 5.2% of revision THA was performed in rural hospitals in 2014. Similarly, hospital charges for revision THA increased significantly from a mean of $32,007 in 2000 to $83,927 in 2014. Discussion. Dramatic changes in the geographic distribution, hospital type, and cost of revision THA have occurred over the past two decades. The importance of this cannot be overstated, especially within the context of shifting payment models including mandated and voluntary bundles that have been applied to total joint arthroplasty procedures. With the burden of these costly, complex surgical cases falling predominantly on urban teaching hospitals, there must be a mechanism for adequately valuing these procedures to allow surgeons to be able to properly care for these patients. Aims. In UK there are around 76,000 hip fractures occur each year 10% to 15% of which are undisplaced intracapsular. There is considerable debate whether internal fixation is the most appropriate treatment for undisplaced fractures in older patients. This study describes cannulated hip screws survivorship analysis for patients aged ≥ 60 years with undisplaced intra-capsular fractures. Methods. This was a retrospective cohort study of consecutive patients aged ≥ 60 years who had cannulated screws fixation for Garden I and II fractures in a teaching hospital between March 2013 and March 2016. The primary outcome was further same-side hip surgery. Descriptive statistics were used and Kaplan-Meier estimates calculated for implant survival. Results. A total of 114 operations were performed on 112 patients with a mean age of 80.2 years (SD 8.9). The 30-day and one-year mortality were 1% (n = 1) and 13% (n = 15), respectively. Median follow-up was 6.6 years (interquartile range 6.0 to 7.3). Kaplan-Meier estimates showed a survivorship of 95% at one year and 90% at five years (95% confidence interval 84% to 95%) for cannulated screws. Nine patients underwent further hip surgery: four revision to total hip arthroplasty, one revision to hemiarthroplasty, three removals of screws, and one haematoma washout. Posterior tilt was assessable in 106 patients; subsequent surgery was required in two of the six patients identified with a posterior angle > 20° (p = 0.035 vs angle < 20°). Of the 100 patients with angle < 20°, five-year survivorship was 91%, with seven patients requiring further surgery. Conclusion. This study of cannulated hip screw fixation for undisplaced fractures in patients aged ≥ 60 years reveals a construct survivorship without further operation of 90% at five years. Cannulated screws can be considered a safe reliable treatment option for Garden I and II fractures. Caution should be taken if posterior tilt angle on lateral view exceeds 20°, due to a higher failure rate and reoperation, and considered for similar management to Garden III and IV injuries. Introduction. Meaningful clinical improvement as demonstrated through patient-reported outcome measures (PROMs) are increasingly used to evaluate success of total hip arthroplasty (THA) procedures. This patient perspective can provide a full picture when used with clinical data to best evaluate surgical outcomes. Methods. All primary THA procedures reported to the American Joint Replacement Registry from 2012–2018 with linked pre-operative and 1-year post-operative functional or anatomical PROMs were included. The achievement of minimal clinically-important difference (MCID) was calculated using the distribution method. Logistic regression models with covariate adjustment for patient demographics, American Society of Anesthesiologists (ASA) score, and body mass index (BMI) were constructed to identify associations with PROMs. Results were analyzed based on hospital size (small, medium and large) and teaching type (non-teaching, minor and major) based on the American Hospital Association Survey (2015). Results. There were 3,952 THA with pre-operative and 1-year post-operative PROMs. The five types of PROMs collected include: HOOS (n=731), HOOS Jr. (n=295), PROMIS-10 (n=1,074), SF-36 (n=976), VR-12 (1,262). The average age was 66.3±10.5 years, and the majority were female (54.7%). 53.1% of THA patients achieved MCID. Age and gender were statistically significant, while ASA score and BMI classification were not. As age increased by 1 year, the odds of achieving MCID increased 0.8% (OR 0.992, 95%CI 0.984, 0.999) and a minor versus major teaching hospital was 20.8% less likely to achieve MCID (p<0.04). While small hospital sizes had significantly fewer linked PROMs (6.5% of all linked PROMs), only 44.5% achieved MCID compared to medium (52.3%) and large (54.5%) hospitals (p<0.02). Conclusion. Older patient age, major teaching hospitals, and large hospitals achieved higher levels of MCID after THA. Identifying patients that are less likely to achieve MCID can aid physicians by determining patients at risk for poor outcomes, then guiding patient expectations and providing patient-centered care. The purpose of this study is to analyze data submitted to the American Joint Replacement Registry (AJRR) to characterize utilization trends of dual mobility bearings in the United States. Methods. All primary and revision THA procedures reported to AJRR from 2012–2018 were analyzed. Patients of all ages were included and subdivided into dual mobility and traditional bearing surface cohorts. Independent variables included patient demographics, geographic region, hospital size, and teaching affiliation. Associations were determined by chi-square analysis and a logistic regression was performed to assess the association between dual mobility and independent variables. Results. The West was associated with the highest rate of dual mobility usage compared to the other regions of the United States. Dual mobility articulations were used most commonly for dysplasia (OR 2.448 vs osteoarthritis, 95% CI 1.143–1.285, p<0.001) during primary THA and for instability (OR 3.130 vs poly-wear, 95% CI 2.751–3.562, p<0.001) in the revision setting. (Table 2). Conclusion. Dual mobility articulations showed a marked increase in utilization during the period examined. Thru purpose of this study was to evaluate the clinical outcomes of a consecutive series of conversion total hip arthroplasty (cTHA) following previous hip fractures. A retrospective chart review of patients who underwent cTHAs from 2008–2017 at an urban academic teaching institution was performed. Eighty-eight patients were included in this study. The mean age at the cTHA was 66 years (range 27 to 89). 67% of the patients wre women. The mean BMI was 28 kg/m. 2. (range 17 to 41). The mean Charlson Comorbidity Index was 3 (range 0 to 9). The mean follow-up was 49 months (range 24 to 131). The mean duration from the hip fracture fixation to the cTHA was 51 months (range 10 to 144). The mean operating time was 188 minutes, (range 71 to 423) with a mean estimated blood loss of 780 ml (range 300 to 2500). Revision-type (long-stem) designs were used in 65% of the cases. The mean length of hospital stay was 8 days (range 2 to 61). The readmission rate was 37% within 90 days after the CTHAs. Of these, 57% were due to non-orthopaedic complications. There were 10 orthopaedic complications: 7 PJIs, all of which required I&D and 3 required staged revision. There were 2 dislocations treated with closed reduction and 1 case of intraoperative periprosthetic femur fracture during femoral component insertion. There was no revision for aseptic loosening within the follow-up period. The one-year mortality rate was 0%. cTHAs were associated with longer operating time, more blood loss, longer length of hospital stay, and higher readmission rates than the primary THAs in our institutional database. We believe utilizing a multi-disciplinary care protocol to optimize these patients is needed to reduce the high rate of readmissions, and the complications in this patient population

Introduction. Patients with FNF may be treated by either total hip arthroplasty (THA) or hemiarthroplasty (HA). Utilizing American Joint Replacement Registry (AJRR) data, we aimed to evaluate outcomes in FNF treatment. Methods. Medicare patients with FNF treated with HA or THA reported to the AJRR database from 2012–2019 and CMS claims data from 2012–2017 were analyzed in this retrospective cohort study. “Early” was defined as less than 90 days from index procedure. A logistic regression model, including index arthroplasty, age, sex, stem fixation method, hospital size. 1. , hospital teaching affiliation. 1. , and Charlson comorbidity index (CCI), was utilized to determine associations between index procedure and revision rates. Results. Of 75,333 FNF procedures analyzed, 82.2% had HA. 8.4% had cemented fixation. 36.9% had cementless fixation. Fixation was unknown for 41,225 (54.7%) patients. 90-day readmissions rates were 1.3% for both cohorts. Both the early revision rate (0.9% HA vs. 1.3% THA, p<0.0001) as well as the overall revision rate (1.5% HA vs. 2.3% THA, p<0.0001) were higher in the THA cohort. The three most common reasons for any revision were instability (26%), infection (21%), and periprosthetic fracture (15%). Higher rates of any revision were associated with cementless fixation (OR=1.37, 95% CI 1.20–1.57) and increased age (OR=0.96, 95% CI 0.96–0.97). THA and increased age were risk factors for early and any revision for instability. Cementless fixation, female sex, and decreased age were associated with lower revision rates for infection. Conclusion. The most common surgical treatment for FNF reported to the AJRR was cementless stem fixation and hemiarthroplasty. The higher revision rates for FNF treatment with THA warrant further investigation. The use of cemented femoral fixation in this patient population may be under-utilized. These data provide a benchmark of US practice for reference and comparison to other practices throughout US and the world


Introduction. In the United States, cementless femoral fixation remains the dominant mode of fixation for femoral neck fractures, despite strong worldwide registry data that supports cemented fixation. The reason for this discrepancy remains unknown, controversial and often difficult to compare due to multiple variables. The purpose of this study was to evaluate a matched cohort of patients undergoing arthroplasty for femoral neck fractures and assess outcomes of revisions, periprosthetic fractures and mortality. Methods. This is an exact matched cohort study. Cemented fixation cases were exact matched to cementless fixation cases in a 1:1 fashion based on age, sex and Charlson Comorbidity Index (CCI). Outcome variables included: revision for periprosthetic fracture; all cause revision and mortality at any time point; all cause revision and mortality within 1-year and within 90-days. The primary independent variable was femoral fixation (cemented, cementless) and covariates included race (black, white, other), ethnicity (hispanic, non-hispanic), teaching status (minor, major, nonteaching) and bedsize (1–99, 100–399, >=400). Chi-square tests and multivariable logistic regression models were used for statistical analysis. Results. A total of 64,283 femoral neck fractures were evaluated. 17,138 cementless femoral stems were matched exactly to cemented femoral stems based on age, gender and Charleston comorbidity index (CCI). In the multivariate logistic regression analysis, compared to cementless femoral fixation, cemented fixation was associated with a 20% reduction in overall revision (OR 0.796, 0.675–0.939), a 30% reduction in revision at 1 year (OR: 0.709, 0.589–0.854) and a 86% reduction in revision for periprosthetic fracture (OR: 0.144, 0.07–0.294). However, cemented stem fixation was associated with a 23% (OR: 1.33, 1.134–1.338) and 16% (OR: 1.232, 1.134–1.338) increase in mortality at 90 days and 1 year post op respectively. Conclusions. In this exact matched cohort study, risk of early revision and revision for PPFx was dramatically reduced at all time points with the use of cemented stem fixation for elective THA and FNF. However, the increased risk of 90d and 1 year mortality following cemented stem fixation in the elective population warrants further investigation

Introduction. The number of total hip replacements taking place across the UK continues to grow. In an ageing population, with people living longer and placing greater strain on their prostheses, the number of peri-prosthetic femoral fractures is increasing. We studied the economic impact this has on a large university teaching hospital. Method. All patients with a peri-prosthetic femoral fracture between 24/11/2006 and 31/5/2014 were identified using theatre databases. Radiographic and case note analysis was performed for each case. Costings from finance department for implants and in-patient stay were obtained. Results. 90 cases were identified, 58 female and 32 male, with a mean age of 76 (range: 38–91). 86 fractures occurred around total hip replacements, 4 around hemi-arthroplasties. 89 of the cases were managed surgically, 66% undergoing revision and 33% receiving open reduction and internal fixation (ORIF). According to the Vancouver Classification, 3% were Type A, 79% Type B and 18% Type C. The mean length of stay for all fractures was 43 days, ranging from 5 – 179. The mean overall cost of management was £31,370 (range: £6885 – £112,327). Patients with type C fractures had the highest mean length of stay at 53 days and highest overall mean cost of £33,417. Conclusion. When including rehabilitation costs, our study illustrated a mean cost of £31370, roughly four times the NHS tariff of £8552 and significantly greater than the £3702 per case recovered by one UK teaching hospital. This equates to an average £22000 shortfall per case for peri-prosthetic fractures alone at our institution. Although implant costs are greater, treatment with revision where appropriate allows earlier weight bearing, reduced length of stay and rehabilitation costs and lower overall cost

Aims

Implant waste during total hip arthroplasty (THA) represents a significant cost to the USA healthcare system. While studies have explored methods to improve THA cost-effectiveness, the literature comparing the proportions of implant waste by intraoperative technology used during THA is limited. The aims of this study were to: 1) examine whether the use of enabling technologies during THA results in a smaller proportion of wasted implants compared to navigation-guided and conventional manual THA; 2) determine the proportion of wasted implants by implant type; and 3) examine the effects of surgeon experience on rates of implant waste by technology used.

Methods

We identified 104,420 implants either implanted or wasted during 18,329 primary THAs performed on 16,724 patients between January 2018 and June 2022 at our institution. THAs were separated by technology used: robotic-assisted (n = 4,171), imageless navigation (n = 6,887), and manual (n = 7,721). The primary outcome of interest was the rate of implant waste during primary THA.

Aims

Hip fractures are some of the most common fractures encountered in orthopaedic practice. We aimed to identify whether perioperative hypotension is a predictor of 30-day mortality, and to stratify patient groups that would benefit from closer monitoring and early intervention. While there is literature on intraoperative blood pressure, there are limited studies examining pre- and postoperative blood pressure.

Methods

We conducted a prospective observational cohort study over a one-year period from December 2021 to December 2022. Patient demographic details, biochemical results, and haemodynamic observations were taken from electronic medical records. Statistical analysis was conducted with the Cox proportional hazards model, and the effects of independent variables estimated with the Wald statistic. Kaplan-Meier survival curves were estimated with the log-rank test.

Background. Isolated fracture of the greater trochanter is an uncommon presentation of hip fracture. Traditional teaching has been to manage these injuries nonoperatively, but modern imaging techniques have made it possible to detect occult intertrochanteric extension of the fracture in up to 90% of cases. This study aims to review the investigation and management of greater trochanter fractures in a single major trauma centre. Methods. A retrospective review was completed of patients admitted with greater trochanter fractures. These were matched to cases with 2-part extracapsular fractures. Initial management and clinical outcome was established using electronic notes and radiographs. Mortality and length of stay was calculated for both groups. Results. 85 isolated greater trochanter fractures in 84 patients were identified from 2006–2017. 81/85 patients were treated non-operatively. 78 were mobilised full weight bearing. None required readmission or operation due to fracture displacement. 58 of these patients had cross-sectional imaging with MRI or CT and 15 of those scanned had intertrochanteric extension of the fracture. In the same time period, 998 2-part extra-capsular fractures were treated, using a sliding hip screw. Length of stay was shorter in patients with greater trochanter fractures than 2-part extracapsular fractures (median 7 days vs 14 days, P<0.0001). 30-day mortality was 11.9%, with no significant difference to patients with 2-part extracapsular fractures. Discussion. Cross sectional imaging rarely changed the treatment protocol for isolated greater trochanter fractures. The outcome following non-operative treatment is good even in the presence of occult fractures identified on CT or MRI. We advocate a treatment protocol that encourages early mobilisation and repeat plain radiographs if patients fail to progress. This will reduce unnecessary morbidity from fixation of stable occult fractures

The piriformis muscle is an important landmark in the surgical anatomy of the hip, particularly the posterior approach for total hip replacement (THR). Standard orthopaedic teaching dictates that the tendon must be cut in to allow adequate access to the superior part of the acetabulum and the femoral medullary canal. However, in our experience a routine THR can be performed through a posterior approach without sacrificing this tendon. We dissected the proximal femora of 15 cadavers in order to clarify the morphological anatomy of the piriformis tendon. We confirmed that the tendon attaches on the crest of the greater trochanter, in a position superior to the trochanteric fossa, away from the entry point for broaching the intramedullary canal during THR. The tendon attachment site encompassed the summit and medial aspect of the greater trochanter as well as a variable attachment to the fibrous capsule of the hip joint. In addition we dissected seven cadavers resecting all posterior attachments except the piriformis muscle and tendon in order to study their relations to the hip joint, as the joint was flexed. At flexion of 90° the piriformis muscle lay directly posterior to the hip joint. The piriform fossa is a term used by orthopaedic surgeons to refer the trochanteric fossa and normally has no relation to the attachment site of the piriformis tendon. In hip flexion the piriformis lies directly behind the hip joint and might reasonably be considered to contribute to the stability of the joint. We conclude that the anatomy of the piriformis muscle is often inaccurately described in the current surgical literature and terms are used and interchanged inappropriately. Aims

Adult patients with history of childhood infection pose a surgical challenge for total hip arthroplasty (THA) due to distorted bony anatomy, soft-tissue contractures, risk of reinfection, and relatively younger age. Therefore, the purpose of the present study was to determine clinical outcome, reinfection rate, and complications in patients with septic sequelae after THA.

Methods

A retrospective analysis was conducted of 91 cementless THAs (57 male and 34 female) performed between 2008 and 2017 in patients who had history of hip infection during childhood. Clinical outcome was measured using Harris Hip Score (HHS) and Modified Merle d’Aubigne and Postel (MAP) score, and quality of life (QOL) using 12-Item Short Form Health Survey Questionnaire (SF-12) components: Physical Component Score (PCS) and Mental Component Score (MCS); limb length discrepancy (LLD) and radiological assessment of the prosthesis was performed at the latest follow-up. Reinfection and revision surgery after THA for any reason was documented.

Aims

This study aims to assess the feasibility of conducting a pragmatic, multicentre randomized controlled trial (RCT) to test the clinical and cost-effectiveness of an informal caregiver training programme to support the recovery of people following hip fracture surgery.

Methods

This will be a mixed-methods feasibility RCT, recruiting 60 patients following hip fracture surgery and their informal caregivers. Patients will be randomized to usual NHS care, versus usual NHS care plus a caregiver-patient dyad training programme (HIP HELPER). This programme will comprise of three, one-hour, one-to-one training sessions for the patient and caregiver, delivered by a nurse, physiotherapist, or occupational therapist. Training will be delivered in the hospital setting pre-patient discharge. It will include practical skills for rehabilitation such as: transfers and walking; recovery goal setting and expectations; pacing and stress management techniques; and introduction to the HIP HELPER Caregiver Workbook, which provides information on recovery, exercises, worksheets, and goal-setting plans to facilitate a ‘good’ recovery. After discharge, patients and caregivers will be supported in delivering rehabilitation through three telephone coaching sessions. Data, collected at baseline and four months post-randomization, will include: screening logs, intervention logs, fidelity checklists, quality assurance monitoring visit data, and clinical outcomes assessing quality of life, physical, emotional, adverse events, and resource use outcomes. The acceptability of the study intervention and RCT design will be explored through qualitative methods with 20 participants (patients and informal caregivers) and 12 health professionals.

Aims

The current study aimed to compare robotic arm-assisted (RA-THA), computer-assisted (CA-THA), and manual (M-THA) total hip arthroplasty regarding in-hospital metrics including length of stay (LOS), discharge disposition, in-hospital complications, and cost of RA-THA versus M-THA and CA-THA versus M-THA, as well as trends in use and uptake over a ten-year period, and future projections of uptake and use of RA-THA and CA-THA.

Methods

The National Inpatient Sample was queried for primary THAs (2008 to 2017) which were categorized into RA-THA, CA-THA, and M-THA. Past and projected use, demographic characteristics distribution, income, type of insurance, location, and healthcare setting were compared among the three cohorts. In-hospital complications, LOS, discharge disposition, and in-hospital costs were compared between propensity score-matched cohorts of M-THA versus RA-THA and M-THA versus CA-THA to adjust for baseline characteristics and comorbidities.

We have compared the survival and radiological outcome at ten years after total hip replacement using two techniques for preparing the femoral canal. The same prosthesis was used throughout and all operations were performed by the same surgical team. In technique 1 the canal was over-reamed by 2 mm and in technique 2 it was reamed to the same size as the prosthesis. Technique 1 was performed on 92 patients and technique 2 on 97 patients. The survival at ten years was 97.2% (90.6 to 99.2) for technique 1 and 98.8% (92.9 to 99.8) for technique 2. Vertical migration was greater in technique 1 (1.8 mm versus 1.0 mm at five years; p = 0.36). There were significantly more lytic lesions and radiolucent lines at five years (p = 0.0061) with technique 1. We conclude that technique 2 is not worse and may produce better long- term results than current teaching suggests

From 1982 to 1987, we randomised prospectively 413 patients requiring primary total hip replacements to receive either a Stanmore or Charnley prosthesis. They were reviewed by an independent observer in an attempt to correlate a number of factors including femoral head size with longevity. There were 213 Stanmore hips and 200 Charnley prostheses. At five to ten years (mean 6.5) 76 patients had died and 16 arthroplasties had required revision. Seven were radiologically loose in asymptomatic patients. There was only one case of deep infection. We found no difference statistically in the clinical outcome or in the revision rate of 4% in the two types of prosthesis. The revision rate was greater for trainees than for senior operating surgeons, and there were recognisable technical errors in seven of the nine Stanmore, and four of the seven Charnley replacements which required revision. Retrospective radiological analysis of a random subset of 51 Charnley and 57 Stanmore femoral components showed no difference in femoral subsidence, but in 14 patients who had had bilateral replacements with one femoral component of each type, there was greater early subsidence of the Stanmore prosthesis. Our results confirm that conventional cemented total hip replacements give acceptable results in a general teaching unit, and we found no evidence of any effect of the size of the femoral head on wear or loosening at five to ten years

Aims

Postoperative delirium (POD) and postoperative cognitive decline (POCD) are common surgical complications. In the UK, the Best Practice Tariff incentivizes the screening of delirium in patients with hip fracture. Further, a National Hip Fracture Database (NHFD) performance indicator is the reduction in the incidence of POD. To aid in its recognition, we sought to determine factors associated with POD and POCD in patients with hip fractures.

Methods

We interrogated the NHFD data on patients presenting with hip fractures to our institution from 2016 to 2018. POD was determined using the 4AT score, as recommended by the NHFD and UK Department of Health. POCD was defined as a decline in Abbreviated Mental Test Score (AMTS) of two or greater. Using logistic regression, we adjusted for covariates to identify factors associated with POD and POCD.
