Introduction. Direct lateral approaches to the hip require detachment and repair of the anterior part of the gluteus medius and minimus tendon attachments. Limping may occur postoperatively due to nerve injury or failure of muscle re-attachment. The aim of this study was to assess the integrity of abductor muscle repairs using a braided wire suture marker. Methods. Total hip arthroplasties were inserted using a modified Freeman approach. After repair of the abductor tendons using a 1 PDS suture with interlocking Kessler stitches, a 3–0 braided wire suture marker was stitched into the lower end of the flap. The suture was easily visible on postoperative radiographs and its movement could be measured. Patients were assessed using radiographs and Oxford hip scores collected prospectively. Results. 56 joint replacements were performed in 51 patients with no major surgical complications. Mean age was 65 yr and 80% (n=41) were female. It proved possible to reproducibly classify repairs based on radiographic measurements as: no wire movement (43%); repair stretched, moving cephalad a short distance (46%); repair detached, moving cephalad a significant distance (11%). Risk of failure of the repair had no relationship with age, preoperative Oxford hip score, or postoperative Oxford hip score. The mean Oxford hip score drop in the no movement group was 36%, the stretched group 37%, and the detached group 33%. These differences were not significant. Discussion/Conclusion. The wire marker proved to be a revealing method of auditing abductor repair following hip arthroplasty. No difference was demonstrated in outcome in relation to wire movement, therefore this study did not show any advantage from a careful repair technique. Due to the small number of failed repairs there may be a type II error. This method may also be useful in assessing the integrity of other large tendon repairs, such as quadriceps tendons splits in total knee replacements

Background:. Potential injury to the common peroneal nerve at the level of the fibula head/neck junction during fine wire insertion in stabilization of proximal fibula, is a recognised complication. This study aims to relate the course of the common peroneal nerve to fibula head transfixion wire. Methods:. Standard 1.8mm Ilizarov wires were inserted percutaneously in the fibula head of cadaveric knees. The course of the common peroneal nerve was dissected calculating wire to nerve distance. Results:. The common peroneal nerve was seen to cross the neck of fibula at a mean distance of 34.8mm from the tip of fibula (range 21.5–44.3mm). Wire placement was found to be on average, 52% of the maximal AP diameter of the fibula head and 64% of the distance from tip of fibula to the point of nerve crossing fibula neck. Conclusion:. When inserting a fibula head transfixion wire, care must be taken not to place wire entry point too distal or posterior on the fibula head. Observing a safe zone in the anterior half of the proximal 20mm of the fibula head would avoid injury to the nerve. In cases where palpation of fibula is difficult, we recommend wire insertion under fluoroscopic guidance

The aim of this prospective randomized controlled trial was to compare patient reported and functional outcomes, complications and costs for displaced olecranon fractures managed with either tension band wire (TBW) or plate fixation. We performed a registered prospective randomized, single blind, single centre trial in 67 patients aged between 16–74 years with an acute isolated displaced fracture of the olecranon. Patients were randomised to either TBW (n=34) or plate fixation (n=33). The primary outcome measure was the Disability Arm Shoulder and Hand (DASH) score at one-year. The baseline demographic and fracture characteristics of the two groups were overall comparable. The one-year follow-up was 85percnt;. There was a significant improvement in elbow function over the 12 months following injury in both groups (p<0.001). At one-year following surgery the DASH for the TBW group was not statistically different to the plate fixation group (12.8 vs 8.5; p=0.315). There was no significant difference between groups in terms of range of movement, Broberg and Morrey Score, Mayo Elbow Score or the DASH at all assessment points over the one-year following injury (all p≥0.05). Complication rates were significantly higher in the TBW group (63percnt;vs38percnt;; p=0.042), predominantly due to a significantly higher rate of symptomatic metalwork removal (50percnt;vs22percnt;; p=0.021). In active patients with an isolated displaced fracture of the olecranon, no difference was found in the patient reported outcome between TBW and plate fixation at one year following surgery. The complication rate is higher following TBW fixation due to a high rate of symptomatic metalwork removal

Background:. Septic arthritis following intra-capsular penetration of the knee by external fixation devices is a complication of traction/fixation devices. This study aimed to demonstrate the capsular attachments and reflections of the distal femur to determine safe placements of wires. Methods:. The attachments of the capsule to the distal femur were measured in cadaveric knees. Medially and laterally measurements were expressed as percentages related to the maximal AP diameter of the distal femur. Results:. Mean distance of the anterior attachment was 79.5mm (Range 48.1–120.7mm). The medial capsular reflections were attached an average of 57% back from the anterior edge (Range 41–74%). Laterally the capsular reflections were attached an average of 48% from the anterior reference point (Range 33–57%). Discussion:. Capsular reflections varied. Medially the capsule attachment was up to 74% of diameter of distal femur at the level of the adductor tubercle. Therefore, the insertion of distal femoral traction pins or similar should be placed proximal to the adductor tubercle and no further than 25% of the distance to the anterior cortex. Care is needed to ensure pins do not travel to exit too anteriorly on the lateral side as capsular attachments were found to be up to a distance 48% of the diameter of the femur from anterior reference point

Background. Many patients undergo frame removal in the outpatient setting and nitrous oxide is frequently used, but has varying effects. The aim of the study was to ascertain whether pain levels during frame removal are improved with local infiltration of local anaesthetic (LA) and to assess the effect of LA and nitrous oxide compared to nitrous oxide alone. Methodology. This was a small single centre study using patient reported questions to assess pain levels during frame removal. The test group received 5–20ml 2% lidocaine infiltrated into tissues surrounding half pins and olive wire exit sites. All patients were asked to complete a questionnaire to assess pain levels and patient satisfaction following the procedure. Patients were asked to mark their pain level on a 100mm visual analogue scale giving a final pain score out of 100. Results. There were twenty three patients in the LA group but due to observed levels of increased distress without LA, the control group was restricted to 7 patients. Patient satisfaction was high and there were no complications across both groups. The LA group (N=23) had a mean pain score of 35, which was significantly lower than the mean of 62 in the N=7 control patients (unpaired t-test: p=0.010). Conclusion. Frame removal in clinic has been shown to be a safe and well tolerated procedure and this study has shown that LA does improve pain levels during frame removal. Further work to compare pain levels and patient satisfaction with alternative frame removal methods at other centres is being carried out

Introduction:

Bicondylar tibial plateau fractures are serious periarticular injuries. We investigated outcomes in injuries managed with Ilizarov external fixators.

Methods:

We retrospectively reviewed bicondylar tibial plateau fractures treated with Ilizarov fixators in a major trauma centre from 2008–2012. Radiological parameters were measured from standardised weight-bearing radiographs. A subset (n=34) had patient-related outcome measures.

Lateral-entry wiring (LEW) for displaced supracondylar humeral fractures (SHFs) has been popularised internationally. BOAST guidance suggests either LEW or crossed wires; the latter has reported lower risk of loss of fracture reduction –we explore technical reasons why. We reviewed 8 years of displaced SHFs in two regional centres. Injuries were grouped using the Gartland Classification, with posterolateral or posteromedial displacement assessment for Gartland 3 injuries. We identified any loss of fracture reduction, and reviewed intra-operative imaging to identify learning points that may contribute to early rotational displacement (ERD). 345 SHFs were included, between 2012 and 2020. Gartland 2 (n=117) injuries had a 3.42% risk. ERD. Gartland 3 crossed wirings (n=114) had a 6.14% risk of ERD, with those moving all being posterolaterally displaced. Gartland 3, posterolaterally displaced LEW (n=56) had a 35.7% risk of ERD. Gartland 3, posteromedially displaced LEW (n=58) had a 22.4% risk of ERD. All injuries with ERD except 3 had identifiable learning points, the commonest being non-divergence of wires, or wires not passing through both fracture fragments. LEW requires divergent spread and bicolumnar fixation. Achieving a solid construct through this method appears more challenging than crossed wiring, with rates of ERD 3–5× higher. Low-volume surgeons should adhere to BOAST guidelines and choose a wiring construct that works best in their hands. They can also be reassured that should a loss of position occur, the risk of requirement for revision surgery is extremely low in our study (0.3%), and it is unlikely to affect long term outcomes

Our aim was to explore factors associated with early post operative infection for surgically managed base of 4th/5th metacarpal fractures. We hypothesised that K-wires crossing the 4th and 5th carpometacarpal joint (CMCJ) would be associated with an increased risk of post-operative infection. Data from consecutive patients requiring surgical fixation for a base of 4th/5th metacarpal fracture from October 2016 to May 2021 were collected. Patient demographics, time to surgery, length of surgery, operator experience, use of tourniquet, intra-operative antibiotics, number and thickness of K-wire used, as well as whether or not the K-wires crossed CMCJ joints were recorded. Factors associated with post operative infection were assessed using Chi Squared test and univariable logistic regression using R studio. Of 107 patients, 10 (9.3%) suffered post operative infection. Time to surgery (p 0.006) and length of operation (p=0.005) were higher in those experiencing infection. There was a trend towards higher risk of infection seen in those who had K-wires crossed (p=0.06). On univariable analysis, patients who had wires crossed were >7 times more likely to experience infection than those who didn't (OR 7.79 (95% CI, 1.39 - 146.0, p=0.056). Age, smoking, K-wire size, number of K-wires used, intraoperative antibiotics, tourniquet use and operator experience were not associated with infection. In patients with a base of 4th/5th metacarpal fractures requiring surgical fixation, we find an increased risk of post-operative infection associated with K-wires crossing the CMCJ, which has implications for surgical technique. Larger prospective studies would be useful in further delineating these findings

Aims. The management of completely displaced fractures of the distal radius in children remains controversial. This study evaluates the outcomes of surgical and non-surgical management of ‘off-ended’ fractures in children with at least two years of potential growth remaining. Methods. A total of 34 boys and 22 girls aged 0 to ten years with a closed, completely displaced metaphyseal distal radial fracture presented between 1 November 2015 and 1 January 2020. After 2018, children aged ten or under were offered treatment in a straight plaster or manipulation under anaesthesia with Kirschner (K-)wire stabilization. Case notes and radiographs were reviewed to evaluate outcomes. In all, 16 underwent treatment in a straight cast and 40 had manipulation under anaesthesia, including 37 stabilized with K-wires. Results. Of the children treated in a straight cast, all were discharged with good range of mo (ROM). Five children were discharged at six to 12 weeks with no functional limitations at six-month follow-up. A total of 11 children were discharged between 12 and 50 weeks with a normal ROM and radiological evidence of remodelling. One child had a subsequent diaphyseal fracture proximal to the original injury four years after the initial fracture. Re-displacement with angulation greater than 10° occurred for 17 children who had manipulation under anaesthesia. Four had a visible cosmetic deformity at discharge and nine had restriction of movement, with four requiring physiotherapy. One child developed over- granulation at the pin site and one wire became buried, resulting in a difficult retrieval in clinic. No children had pin site infections. Conclusion. Nonoperative management of completely displaced distal radial fractures in appropriately selected cases results in excellent outcomes without exposing the child to the risks of surgery. This study suggests that nonoperative management of these injuries is a viable and potentially underused strategy. Cite this article: Bone Joint J 2021;103-B(5):902–907

INTRODUCTION. Surgical correction of spinal deformities in the growing child can be applied with or without fusion. Sublaminar wiring, first described by Luque, allows continuation of growth of the non-fused spine after correction of the deformity. Neurological complications and wire breakage are the main clinical problems during the introduction and removal of currently used sublaminar wires. In this pilot study a posterior hybrid construction with the use of a medical-grade UHMWPE (Dyneema Purity®) sublaminar wire was assessed in an ovine model. We hypothesized that such a hybrid construction can safely replace current titanium laminar wires, while providing sufficient stability of the non-fused spinal column with preservation of growth. MATERIALS AND METHODS. This study included 6 Tesselaar sheep, age 7±2months. Two pedicle screws (Legacy system, Medtronic) were placed at lumbar level. Four consecutive laminae were attached to two titanium bars (4.5 mm) using 3 mm diameter UHMWPE (Dyneema Purity®) on the left side and 5 mm diameter on the right side. The sublaminar wires were fixed with a double loop sliding knot and tightened with a tensioning device. As a control, in one animal titanium sublaminar wires (Atlas cable, Medtronic) were applied. After sacrifice the spine of the animals was harvested. Radiographs were taken and CT scans were performed. The vertebrae were dissected and placed in formaldehyde for macroscopic and histological evaluation. RESULTS. The animals were sacrificed after a (minimal) postoperative period of 15 weeks. One animal developed a wire fistula and one animal died the first postoperative day due to complications of the anesthesia. None of the 3 or 5 mm knots loosened and no neurological complications occurred. An average of 8.7 mm growth was seen over the segment operated on. Computed tomography confirmed the preserved stability. Even though no decortication was performed, variable bone bridges with fused levels were seen on CT. Macroscopic and histological analysis showed no inflammation at lamina and dura levels containing Dyneema Purity®, with the exception of the case with the fistula where it was observed locally. DISCUSSION. This pilot animal model study shows that the UHMWPE laminar wire made by Dyneema Purity® has good handling and tensioning properties and can provide sufficient stability in fusionless spinal instrumentation while allowing substantial growth. The examined model showed to be a feasible spinal study model, without occurrence of neurological problems. Reactive periostal bone formation with fusion levels led to some restrictions in this model. In the future it will be necessary to test the described construction in a large animal scoliosis model

We performed a prospective, randomised study on 57 patients older than 60 years of age with unstable, extra-articular fractures of the distal radius to compare the outcome of immobilisation in a cast alone with that using supplementary, percutaneous pinning. Patients treated by percutaneous wires had a statistically significant improvement in dorsal angulation (mean 7°), radial length (mean 3 mm) and radial inclination (mean 3 mm) at one year. However, there was no significant difference in functional outcome in terms of pain, range of movement, grip strength, activities of daily living and the SF-36 score except for an improved range of movement in ulnar deviation in the percutaneous wire group. One patient developed a pin-track infection which required removal of the wires at two weeks. We conclude that percutaneous pinning of unstable, extra-articular fractures of the distal radius provides only a marginal improvement in the radiological parameters compared with immobilisation in a cast alone. This does not correlate with an improved functional outcome in a low-demand, elderly population

Background. In a systematic review of 1125 distal tibia fractures treated with an intramedullary nail, the reported incidence of malalignment was 14%. The purpose of our study is to assess whether the addition of blocking screws during intramedullary nailing of a distal tibia fracture improved radiological outcomes. As a secondary outcome, the time to radiographic union was compared to see if a more rigid bone-implant construct had an effect on healing. Methods. We searched computerised records at a UK level 1 major trauma centre. The joint alignment was measured on the immediate post-operative radiograph and compared to the most recent radiograph. We used a difference of 2 degrees to indicate a progressive deformity and a RUST score greater or equal to 10, to indicate radiographic fracture union. Results. Twenty-seven patients were included. Nineteen patients had no blocking screw and 8 patients had a blocking screw. Five patients had a difference in their coronal plane alignment of 2 degrees or more (3/5 had no blocking screw). The results were analysed and found to be not statistically significant (p=0.88). The addition of a blocking screw has also been shown not to have an effect on the time to union. Conclusion. We have changed our surgical practice. We use a 2.5mm blocking wire to aid in fracture reduction prior to reaming / nail insertion and then remove the wire when the nail has been adequately locked. Implications. This could save surgical procedure time, radiation exposure for the patient, implant costs and potential complications

We reviewed our results and complications of using a pre-bent 1.6mm Kirschner wire (K-wire) for extra-articular metacarpal fractures. The surgical procedure was indicated for angulation at the fracture site in a true lateral radiograph of at least 30 degrees and/or in the presence of a rotatory deformity. A single K-wire is pre-bent in a lazy-S fashion with a sharp bend at approximately 5 millimetres and a longer smooth curve bent in the opposite direction. An initial entry point is made at the base of the metacarpal using a 2.5mm drill by hand. The K-wire is inserted blunt end first in an antegrade manner and the fracture reduced as the wire is passed across the fracture site. With the wire acting as three-point fixation, early mobilisation is commenced at the metacarpo-phalangeal joint in a Futuro hand splint. The wire is usually removed with pliers post-operatively at four weeks in the fracture clinic. We studied internal fixation of 18 little finger and 2 ring finger metacarpal fractures from November 2007 to August 2009. The average age of the cohort was 25 years with 3 women and 17 men. The predominant mechanism was a punch injury with 5 diaphyseal and 15 metacarpal neck fractures. The time to surgical intervention was a mean 13 days (range 4 to 28 days). All fractures proceeded to bony union. The wire was extracted at an average of 4.4 weeks (range three to six weeks). At an average follow up of 8 weeks, one fracture had to be revised for failed fixation and three superficial wound infections needed antibiotic treatment. With this simple and minimally invasive technique performed as day-case surgery, all patients were able to start mobilisation immediately. The general outcome was good hand function with few complications

Aims

Despite limited clinical scientific backing, an additional trochanteric stabilizing plate (TSP) has been advocated when treating unstable trochanteric fractures with a sliding hip screw (SHS). We aimed to explore whether the TSP would result in less post operative fracture motion, compared to SHS alone.

Methods

Overall, 31 patients with AO/OTA 31-A2 trochanteric fractures were randomized to either a SHS alone or a SHS with an additional TSP. To compare postoperative fracture motion, radiostereometric analysis (RSA) was performed before and after weightbearing, and then at four, eight, 12, 26, and 52 weeks. With the “after weightbearing” images as baseline, we calculated translations and rotations, including shortening and medialization of the femoral shaft.

We treated 110 fractures of the middle third of the clavicle in 108 patients aged between 14 and 66 years, by fixation with 2 mm Kirschner wires. The wire was introduced, using an air drill and a telescoping guide, from the medial into the lateral fragment. The medial end of the wire which perforated the anterior cortex of the medial fragment was bent backwards to prevent migration into the lung or mediastinum. In cases of nonunion and acute fracture in which a bone gap was present, bone graft was laid around the fracture site after internal fixation. All the fractures united. No wires broke but in three patients one migrated before union was achieved. Twenty-one patients failed to return for removal of the wire; nine of them had radiological evidence of union before they were lost to follow-up

Aims

Ankle fractures are common injuries and the third most common fragility fracture. In all, 40% of ankle fractures in the frail are open and represent a complex clinical scenario, with morbidity and mortality rates similar to hip fracture patients. They have a higher risk of complications, such as wound infections, malunion, hospital-acquired infections, pressure sores, veno-thromboembolic events, and significant sarcopaenia from prolonged bed rest.

Methods

A modified Delphi method was used and a group of experts with a vested interest in best practice were invited from the British Foot and Ankle Society (BOFAS), British Orthopaedic Association (BOA), Orthopaedic Trauma Society (OTS), British Association of Plastic & Reconstructive Surgeons (BAPRAS), British Geriatric Society (BGS), and the British Limb Reconstruction Society (BLRS).

This study aimed to determine the long-term functional, clinical and radiological outcomes in patients with Schatzker IV to VI fractures of the tibial plateau treated with an Ilizarov frame. Clinical, functional and radiological assessment was carried out at a minimum of one year post-operatively. A cohort of 105 patients (62 men, 43 women) with a mean age of 49 years (15 to 87) and a mean follow-up of 7.8 years (1 to 19) were reviewed. There were 18 type IV, 10 type V and 77 type VI fractures. All fractures united with a mean time to union of 20.1 weeks (10.6 to 42.3). No patient developed a deep infection. The median range of movement (ROM) of the knee was 110. o. and the median Iowa score was 85. . Our study demonstrates good long-term functional outcome with no deep infection; spanning the knee had no detrimental effect on the ROM or functional outcome. . High-energy fractures of the tibial plateau may be treated effectively with a fine wire Ilizarov fixator. Cite this article: Bone Joint J 2015;97-B:1693–7

Aims and objectives. The aim of this study was to assess the radiographic outcome by using Lindstrom grading for the management of Malone 2A and 2B fractures by 3 different methods of treatment. The three different methods included manipulation under anaesthesia, k-wire fixation and open reduction internal fixation. Methods and materials. Between March 2006 and February 2007, 62 intra-articular distal radius fractures were retrospectively selected. 31 patients including 21 females and 9 males with an average age of 62 years were classified as Malone 2A fractures and 32 patients including 18 females and 14 males with an average age of 64.5 years were classified as Malone 2B fractures. 18 patients had manipulation under anaesthetic (MUA) and immobilisation in a plaster of paris (POP) cast, 27 patients had manipulation and k-wire fixation and 17 patients had open reduction internal fixation (ORIF) of the fractures. All the patients had radiographic assessment at 6 weeks post op using Lindstrom grading. Grade 1 and 2 were considered acceptable, grade 3 and 4 were considered unacceptable. Results. In Malone 2A fractures the acceptable number of fractures were as follows −5/10 (50%) of the MUA group, 6/13(46.1%) in the k wire fixation group and 6/7 (85.7%) in the ORIF group. In Malone 2B fractures, the acceptable numbers were - nil in MUA group, 4/14 (28.6%) in k wire fixation group and 8/10 (80%) in the ORIF group. Conclusion. MUA and k wire fixation were not a good option for Malone 2B fractures as these fractures showed the best outcome with 80% acceptable results after ORIF. Malone classification can be used as a reliable predictor for deciding the treatment method for intra articular distal radius fractures as seen by the high acceptable results after ORIF for both type 2A and 2B fractures

Distal radius fractures are typical and frequent fracture of elderly woman with reduced bone density. Thus implant fixation is more difficult. Dorsal and radial comminution are frequent in these patients and so reduction and angle stable osteosynthesis needed. The angle stable plate, often also multidirectional is today the most common stabilisation device. Because of the introduction of bulky and bended implants as the Micronail or Targon DR wich require difficult opening of the bone with awles we decided to test the XS radius nail witch is a 4,5mm or 3,5mm straight nail and witch is introduced after guide wire placement and over drilling with a canulated drill of the same diameter. It is locked parallel to the joint in 3 different directions with angular stability with threaded wires. Methods 16 radius sawbones were osteotomised corresponding to a A3 Fracture and stabilised with a angle stable plate (8) and XS nail (8). 1000 alternating load cycles from 20–200N were performed and the deformation was registered. Also a FE analysis with the MSC Patran/Marc softwere were performed. Also the calculated deformation in the FE study was 20% lower. Also deformation amplitude was lower with 0.31mm compared to 0.42mm in the plate group. The differences however were not significant. Both devices show good biomechanical results. The XS nail has the advantage of mainly intraosseus position, simple operation technique with introduction over a guide wire from the proc. Styloideus radii and over drilling with a canulated drill of the same size. The exposure of the N rad. superf. must be performed. First clinical evaluation is presented. Due to the results we developed a anatomically adapted XS radius nail. The results of the first 100 patients are presented. Conclusions. Both angular stable plate and XSR nail can be used in unstable distal radius fracture fixation. The mainly intraosseus position of the nail and saving of the pronator quadratus as well as lower deformation are in favour of the XSR nail. However frontal plane fractures and very comminuted fractures are better treated with a multidirectional locking plate due to technical reasones so that we use the XSR nail mainly in A3 and C1 fractures

Aims

Patients with A1 and A2 trochanteric hip fractures represent a substantial proportion of trauma caseload, and national guidelines recommend that sliding hip screws (SHS) should be used for these injuries. Despite this, intramedullary nails (IMNs) are routinely implanted in many hospitals, at extra cost and with unproven patient outcome benefit. We have used data from the National Hip Fracture Database (NHFD) to examine the use of SHS and IMN for A1 and A2 hip fractures at a national level, and to define the cost implications of management decisions that run counter to national guidelines.

Methods

We used the NHFD to identify all operations for fixation of trochanteric fractures in England and Wales between 1 January 2021 and 31 December 2021. A uniform price band from each of three hip fracture implant manufacturers was used to set cost implications alongside variation in implant use.
