In a prospective randomised study we compared the results of arthroscopic subacromial bursectomy alone with debridement of the subacromial bursa followed by acromioplasty. A total of 57 patients with a mean age of 47 years (31 to 60) suffering from primary subacromial impingement without a rupture of the rotator cuff who had failed previous conservative treatment were entered into the trial. The type of acromion was classified according to Bigliani. Patients were assessed at follow-up using the Constant score, the simple shoulder test and visual analogue scores for pain and functional impairment. One patient was lost to follow-up. At a mean follow-up of 2.5 years (1 to 5) both bursectomy and acromioplasty gave good clinical results. No statistically significant differences were found between the two treatments. The type of acromion and severity of symptoms had a greater influence on the clinical outcome than the type of treatment. As a result, we believe that primary subacromial impingement syndrome is largely an intrinsic degenerative condition rather than an extrinsic mechanical disorder

We report a randomised controlled trial to examine the effectiveness and cost-effectiveness of arthroscopic acromioplasty in the treatment of stage II shoulder impingement syndrome. A total of 140 patients were randomly divided into two treatment groups: supervised exercise programme (n = 70, exercise group) and arthroscopic acromioplasty followed by a similar exercise programme (n = 70, combined treatment group). The main outcome measure was self-reported pain on a visual analogue scale of 0 to 10 at 24 months, measured on the 134 patients (66 in the exercise group and 68 in the combined treatment group) for whom endpoint data were available. An intention-to-treat analysis disclosed an improvement in both groups but without statistically significant difference in outcome between the groups (p = 0.65). The combined treatment was considerably more costly. Arthroscopic acromioplasty provides no clinically important effects over a structured and supervised exercise programme alone in terms of subjective outcome or cost-effectiveness when measured at 24 months. Structured exercise treatment should be the basis for treatment of shoulder impingement syndrome, with operative treatment offered judiciously until its true merit is proven

Purpose of the study: Palliative treatment for unreparable rotator cuff tears by intra-articular resection of the long head of the biceps provides demonstrated satisfactory results. We hypothesized that associated acromioplasty could be deleterious. Material and method: We conducted a case-control study comparing 24 tenotomies or tenodeses of the long head of the biceps with 24 tenotomies or tenodeses of the long head of the biceps associated with acromioplasty. All patients had an unreparable tear of the rotator cuff. Full-thickness tear of the subscapularis or Hamada and Fukuda radiological stage 3 or 4 tear were exclusion criteria. The two groups were matched for weighted Constant score, Hamada and Fukuda stage, type of tear of the infraspinatus and subscapularis, overall fatty infiltration grade, and follow-up. After the physical examination, we compared the four items of the Constant score, the absolute and weighted score, active anterior elevation, pain score on a visual analogue scale (VAS), and the subjective shoulder score. All patients were examined by a clinician unaware of the group at last follow-up (mean 51 months). Results: The two groups were comparable preoperatively except for the fatty infiltration index which was higher in the acromioplasty group. At 51 months follow-up, active anterior elevation was statistically lower in the acromioplasty group (145 versus 170). The absolute and weighted Constant scores were lower in the acromioplasty group (p< 0.05). There was no difference for pain and the subjective shoulder score (67 versus 71). Four patients in the acromioplasty group required revision total shoulder arthroplasty versus two in the group without acromioplasty. Discussion: Association of acromioplasty with intra-articular resection of the long head of the biceps for palliative treatment of unreparable rotator cuff tears is deleterious for elevation and reduces overall shoulder function. Even though acromioplasty provides good short-term results for average-sized tears, outcome is less satisfactory for massive tears and deteriorates over time. In addition, as described by Wiley, acromioplasty can induce a pseudoparalytic shoulder in patients with unreparable tears

Purpose: Failure is still observed after 20% of acromioplasties which can be explained by acromioclavicular osteoarthritis. The purpose of this study was to demonstrate the deleterious effect of this degeneration on outcome. Material and methods: We reviewed 103 arthroscopic acromioplasties performed in 100 patients who presented non-torn non-calcified tenopathies. Seven patients were excluded so 96 patients, 63 women and 33 men were retained for analysis. Mean age at operation was 48.2 years and mean follow-up was 3.8 years. Patients were divided into two groups on the basis of the sonographic findings: group 1 had no computed tomography (CT) signs of acromioclavicular osteoarthritis (66 patients), such signs were found in group 2 (30 patients). Each patient was reviewed clinically and CT-scan was used to diagnosis osteoarthritis classed as stage 0 to 3. Subjective outcome was assessed in terms of patient satisfaction and objectively with the Constant score. Results: Subjectively, three-quarters of the patients in group 1 were satisfied versus one-third in group 2. The Constant score confirmed this finding with a mean 76 points in group 1 versus 68 in group 2 (the weighted score was 93.5% and 83% respectively). The weighted score showed that good or excellent results were achieved in 84.84% of the patients in group 1 and in 43% in group 2. Discussion: Our results are in agreement with data in the literature and provide scientific evidence of the influence of acromioclavicular osteoarthritis on the failure of acromioplasty. The results in group 1 were clearly better than in group 2, proving statistically a widely accepted notion: acromioclavicular osteoarthritis compromises significantly outcome of acromioplasty. Furthermore, it is interesting to note that among the seven cases excluded (for resection of the articulation), six had satisfactory outcomes. Conclusion: These results confirm the unfavourable influence of acromioclavicular osteoarthritis on the outcome of acromioplasty. A prospective study designed to determine the effect of simultaneous acromioclavicular resection would be useful to propose a coherent therapeutic approach

Purpose: We evaluated the respective roles of acromioplasty and curettage of calcifications in arthroscopic treatment of calcifying tendinopathy of the rotator cuff. Material and methods: We reviewed 41 cases of calcifying tendinopathy at mean 42 months. We retained for analysis only true calcifications identified at least 12 months after arthroscopy. All patients underwent acromioplasty and 13 underwent calcification curettage. The physical examination searched for subacromial impingement and cuff tendon suffering. The weighted Constant score was determined to assess outcome as excellent, good, fair, or poor. Patient satisfaction was assessed using three subjective questions. We searched for persistent calcification on the AP and Lamy lateral x-rays and quantified acromial resection by measuring the height of the subacromial space, the acromial arrow, and the type of acromion (Bigliani). Ultrasonography was performed to search for cuff lesions. Cuffs were classed as normal, atrophic or torn. Results: After statistical analysis, the mean Constant score was found to have increased from 55 points to 80 points, with 88% excellent and good results (weighted Constant score > 85%). There was no significant difference between patients with and without calcification curettage (p> 0.1). Patients who were mobilised rapidly had a better outcome (p< 0.005). Subjectively, 88% of the patients were satisfied or very satisfied.These results were not correlated with duration of follow-up. The degree of preoperative calcification did not affect outcome, but persistent calcification (nine cases) had an unfavourable effect on outcome. Nevertheless, 80% of the calcifications without curettage did not resorb after acromioplasty. The type of acromion had an effect on outcome. Acromions which were not flat (type II or III) had an unfavourable influence. The degree of acromial correction had a significant effect on outcome, the Constant score increased proportionally with the height of the subacromial space and inversely with acromial arrow. Ultrasonography disclosed two cuff tears but in elderly subjects, probably due to degeneration. Conclusion: Curettage of calcifications does not improve outcome of good quality acromioplasty. The stage of the calcification is not an indication for curettage. Furthermore, it appears that the impingement is partly the cause of persistent calcifications since 80% of them disappeared after acromioplasty alone

Chronic rotator-cuff syndrome with impingement is satisfactorily treated by acromioplasty by both open and arthroscopic techniques, but the Neer operation with deltoid detachment requires a prolonged rehabilitation. Arthroscopic acromioplasty reduces recovery time but is a difficult procedure. McShane, Leinberry and Fenlin (1987) described a conservative open anterior technique with good results and shortened rehabilitation. A similar technique has been used in 25 patients with an average age of 46 years. After a mean follow-up of 24 months, 80% were very satisfied and none was dissatisfied. The average recovery times were 2.7 weeks for activities of daily living, 1.9 months for return to work and 3.7 months for all activities including overhead manual labour

The purpose of this prospective randomised clinical trial is to examine the effect of acromioplasty on the outcome of arthroscopic rotator cuff repair. Patients included individuals that were referred for assessment after six months of failed conservative management. Following informed consent patients were randomly assigned to receive arthroscopic rotator cuff repair with or without acromioplasty. The surgeon was not blinded to the type of procedure; however, the researcher who performed the follow-up evaluations and the patient was blinded to the surgical protocol. Subacromial decompression (acromioplasty) was performed with release of the coracoacromial ligament off the anterior undersurface of the acromion. The procedure for arthroscopic cuff repair without acromioplasty followed the protocol of arthroscopic cuff repair with acromioplasty, without division of the coracoacromial ligament or resection of the acromion. Both groups experienced the same post-operative rehabilitation protocol. Wound healing and active and passive range of motion were assessed and recorded at six to eight weeks post-operatively. Subsequent post-operative visits occurred at three, six, twelve, eighteen and twenty-four months and included documentation of patient range of motion, patient derived WORC scores (1) and complete ASES scores. Preliminary results suggest, based on a one-tailed t-test, patients that receive a rotator cuff repair with acromioplasty demonstrate a statistically significant improvement (< 0.05) in Quality of Life, based on WORC and ASES scores, compared to the non-acromioplasty group. To date, three patients in the non-acromioplasty group required a revision surgery; two of these patients had a Type III acromion. Arthroscopic rotator cuff repair with arthroscopic acromioplasty in the treatment of full thickness rotator cuff tears is recommended for patients with a Type III acromion

This is a retrospective review of 108 patients who underwent decompressive anterior acromioplasty for chronic impingement in the absence of a full thickness rotator cuff tear. Before operation, all the patients had had shoulder pain for at least one year despite conservative treatment. At operation, the rotator cuff tendons were explored and were intact. Anterior acromioplasty, followed by rehabilitation was successful in 87% of patients. The operation was less successful in women, in those who had diminished movement before operation, who were involved in worker's compensation claims, and whose pain followed direct trauma. Appropriate selection of patients is considered the key to success

We performed open acromioplasty for intractable impingement syndrome on 96 shoulders (93 patients) with an intact rotator cuff. All the shoulders were examined by ultrasound after a mean interval of nine years. Those showing pathological findings, a poor or fair subjective result, or deterioration of the primary excellent outcome had MRI and/or arthrography. The mean Constant score for the affected shoulders was 70 points and that for 48 non-involved, symptom-free shoulders, 84 points. The subjective outcome was excellent in 45, good in 24, fair in 18 and poor in 9 shoulders. Complete tears were found in 12 shoulders and partial tears in seven. A total of 14 shoulders was symptom-free after acromioplasty, but after an average of five years became painful again and showed deterioration. Of these, six had complete tears and four partial tears of the cuff. The tear rate was 4% in shoulders initially judged to be excellent, 25% in good, 33% in fair and 55% in poor shoulders. The tear rate was 71% in shoulders which subsequently deteriorated. The incidence was higher in men (25%) than in women (11%). We conclude that a tear of the rotator cuff may appear after acromioplasty, although there was no evidence of a tear at the time of operation. This is usually the reason for deterioration in a shoulder with an initially good operative outcome

The assessment of the arthroscopic findings in patients suffering from impingement syndrome and partial tear of the supraspinatus tendon, staged as type 1& 2 under Ellman, Gartsman, Snyder, that were treated by acromioplasty, debridment and repair of the supraspinatus tear. Thirty four patients,(20 male, 14 female), with a median age of 52 years, (48–64 years old), underwent shoulder joint arthroscopy. All were evaluated by two physicians and subjected to plain films (AP& Y views) as well as to MRI. Clinically, the differencies in the evaluation of patients with type 1& 2a lesion were insignificant. Type 2b had a better outcome, but in comparison to type 1, that were subjected only to acromioplasty, variations involving range of motion, pain and scoring (Constant & Oxford scores), were observed, from the first stages of rehabilitation, with no further improvement. The comparative method in relation to the clinical trials was in favor of those patients treated with repair of the tear. After a 29 m follow up, we suggest only acromioplasty, in stages 1& 2a, although the daily activities of each patient can change the approach. In type 2b lesions, irrespective of age, we suggest the repair of the tear

The aim of this study was to evaluate and compare the results of acromioplasty in two groups of patients operated upon for impingement syndrome using two different techniques, In one group the insertion of the devoid was partially divided (deltoid off strategy) while in the other the insertion of the deltoid was preserved (deltoid on strategy). Twenty-one patients, suffering from impingement syndrome of the shoulder, were operated during the period 1996–2001. Preoperatively all patients presented with positive impingement test and they were complaining of night pain as well as pain during activity. Two different techniques were used. In 2 group of 10 patients. the “deltoid off’”strategy was applied and the acromioplasty was performed with the use of an osteotome. In a second group of 11 patients the “deltoid on” strategy was applied and the acrormioplasty was performed by using a high speed burr. Eight out of 10 patients of the first group were satisfied with the results of the operation, whereas all the 11 patients of the second group were satisfied. The return to full activity in patients with the “deltoid off” strategy was 10 weeks in average, while in the “deltoid on” group it was 8 weeks. Night pain subsided in ail patients in both groups. Two patients of the first group complained of mild pain with daily living activities, while 10 out of the 11 patients of the second group had no pain at all. Finally 2 patients of the first group and none of the second group presented residual painful arc. We conclude that the “deltoid on” technique for treatment of the impingement syndrome of the shoulder appears more simple and reliable, has less morbidity and gives better clinical results, compared to the “deltoid off” technique

Purpose: The purpose of this study was to assess long-term functional outcome after arthroscopic acromioplasty for full-thickness rotator cuff tears, to evaluate the efficacy of complementary procedures (biceps tenotomy, extended acromioclavicular resection), and to examine the course of anatomic lesions. Material and methods: From 1988 to 1994, 141 full-thickness rotator cuff tears were treated by arthroscopic acromioplasty. Ninety-eight patients, mean age 60 years, were reviewed clinically, radiographically, and sonographically at more than eight years. The mean preoperative Constant score was 48.5 points. The tear involved the supraspinatus in 18 cases, the supraspinatus and infraspinatus in 40, the supraspinatus and the subscapularis in ten, and all three tendons in 20. Coronal extension showed a distal tear in four, an intermediate tear in 52, and a retracted stump in 32. Systemic acromioplasty was associated in 36 patients with tenotomy of the long head of the brachial biceps and in 44 with acromioclavicular extension. Results: At mean follow-up of 10.7 years (8–13.5), the mean Constant score was 60 points. The clinical outcome was excellent or good in 39.7%, fair in 45.5%, and poor in 14.8. 62.5% of the patients were satisfied or very satisfied. The height of the subacromial space was 5.19 mm at last follow-up compared with 4.3 mm preoperatively. The antalgesic effect of biceps tenotomy was significant when the initial acromiohumeral space was less than 5 mm. This procedure did not produce any significant change in the subacromial height or development of osteoarthritis at last follow-up. Sonography showed stability of the size of the tear in 83.8% of the cases. The results were significantly less satisfactory when the initial tear involved the subscapularis or all three tendons. Conclusion: The clinical results of arthroscopic acropmioplasty for full-thickness tears show long-term stability. Biceps tenotomy improves the antalgesic effect significantly, particularly if the subachromial space measures less than 5 mm preoperatively, without causing significant radiological degradation. Extension to the acromioclavicular level should be systematic in patients with acromioclavicular pain preoperatively and/or radiological anomalies


Aim:. To assess the long term MRI pathoanatomical changes of unrepaired, isolated full thickness supraspinatus tears in a population of patients that had acromioplasty done for symptomatic impingement syndrome. Background:. To date there are no studies assessing the effect of acromioplasty on rotator cuff tear progression in impingement syndrome. The natural evolution of unrepaired tears suggests that small isolated tears may heal, and not all tears progress onto significant fatty change and atrophy. Which tears heal and which tears progress and the effect of acromioplasty on tear progression is still not known. Results:. There were 17 shoulders examined. Average age of our patients was 65 years at time of surgery and 73 years at follow up. The patients were divided into three groups depending on the size of the tear at the time of surgery: C1 tears (< 1cm) n = 5, C2 tears (1–2 cm) n = 8, C3 tears (2–3 cm) n = 4. The C1 tear group did the best clinically with a Constant score mean 86, 4(range 83–96) and mean Oxford score of 47 (range 46–48). The MRI findings in this group showed only one patient (20%) with significant fatty infiltration and atrophy. The C2 group had mean Constant scores of 72(range 65–98), mean Oxford score 45(range 42–48). On MRI evaluation there were 2 patients (11.8%) that had tears that had regressed in size (CI 1.4‐36.4). In the C3 group the mean Constant score was 75(range 65–85) and Oxford score 45(range 42–48). MRI evaluation revealed that all (100%) the rotator cuff tears went on to significant degeneration and atrophy in this group. No statistical difference was found between these groups when assessing Oxford score (p=0.75) and Constant scores(p=0.69). Significant associations were found between tear size and fatty infiltration (p=0.028), and tear size and atrophy (p=0.054. Conclusion:. Acromioplasty in the management of impingement syndrome may be protective when small isolated tears are noted during surgery as two tears regressed in size. Large tears all progress onto fatty infiltration and atrophy, but all three groups despite tear size had good clinical and functional outcomes. More research is needed to further understand the evolution of tears in impingement syndrome

Purpose: Several treatments can be proposed for calcified tenopathy of the rotator cuff. Corticosteroid infiltration, radioscopic trituration-aspiration, and arthroscopy are the most widely used modalities. Over the last decade, we have come to refer our cases of well-circumscribed calcified tenopathy easily accessible to radioscopy to our radiology colleagues since radioscopic treatment has appeared to be quite cost-effective. This trend has continued despite the new interest of the arthroscopists in this disease. We have nevertheless had a certain number of failures (25%) and at this time have decided to prefer arthroscopy. The purpose of this work was to present our results with arthroscopy used after failure of tirturation-aspiration or for patients with calcifications we considered to have contraindications for trituration-aspiration (poorly circumscribed chain of calcifications). Material and methods: Between 1990 and 1997, we performed 28 arthroscopic procedures in 28 patients. There were 18 women and ten men, mean age 47.5 years (28–71 years). All suffered pain at night and painful blockage during certain motions, particularly anterolateal elevation and forced internal rotation. We did not use the preoperative Constant score because we considered that the pain always gives a false score in these patients, particularly for muscle force. Nevertheless, the mean pain score preoperatively was 4.5 (0–10), daily activity was 14 (8–18) and active motion was 32 (20–40). All calcifications were located in the supraspinatus and the anterior part of the infraspinatus. Acromial morphology was type III in seven cases. All the patients underwent arthroscopy with resection of the coracoacromial ligament and anterior acromioplasty without touching the residual calcification. Results: All patients were reviewed by an independent surgeon different than the operator. Mean follow-up was 54 months (18–108 months). Subjectively, 89% of the patients were cured or improved, 11% were unchanged. Objectively, the Constant score weighted for age and sex was a mean 91.4% (50–100%) with a median 100%. We had 20 shoulders with excellent outcome (weighted Constant score 85–94%), two with fair outcome (65–84%), and three with poor outcome (< 65%), giving 82% satisfactory outcome. Muscle force was very satisfactory (mean 7.5%) and close to the contralateral shoulder (8.25 kg). Radiologically, 17 of the 29 shoulders were cleared of calcifications (61%). Conclusion: Arthroscopic acromioplasty after failure of trituration-aspiration gives quite satisfactory results, including for calcifications we had considered to by “untriturables”

We evaluated 67 shoulders in 65 patients who had pain and dysfunction for more than two years after an initial acromioplasty for impingement syndrome without a rotator cuff tear. In addition to a thorough history, physical examination, local anaesthesia injection and any other necessary investigations all patients had arthroscopic evaluation of the shoulder. In 27 shoulders there had been diagnostic errors, and in 28 operative errors; only in 12 had the diagnosis and the operative procedure both been correct. Subsequent operative intervention in patients not receiving worker's compensation benefit had a 75% success rate, whereas in those receiving such benefits the success rate was only 46%


Purpose: We propose a simple surgical treatment with sustained efficacy for chronic symptomatic acromioclavicular instability. Material and methods: Dissection of ten cadaver specimens (20 shoulders) enabled a detailed biometry of the acromiocoracoid ligament (ACCL) with measures of motion and clavicular fixation. We were also able to measure tear resistance with a dynamometer. The proposed surgical technique was designed from Weaver-Dunn reconstruction as modified by Bircher. Briefly, acromioplasty is used to dis-insert the ACCL which is folded back on the clavicular resection border then fixed by screws or wires. Results: The size of the acromioplasty section varied from 18 to 25 mm in diameter. The ACCL had a quadrilateral aspect: mean anterior length 37.7 mm, mean posterior length 25.6 mm, mean middle (coracoid) width 16 mm, mean lateral width 22.3 mm and mean thickness 1.55 mm. The fold angle was 68° on average giving 10.8 mm in height and 21.8 thickness. The coracoclavicular distance varied from 15.7 mm to 50.1 mm. Mean tear force was 11.5 daN. Discussion: Posttraumatic osteoarthritis of the unstable acromioclavicular joint requires resection of the lateral quarter of the clavicle using the Baccarani technique which should be completed by a stabilisation fixation. The Weaver-Dunn technique appears to be insufficient for Rockwood stage IV and B chronic instability. The distance between the anterior border of the coracoid and the posterior border of the clavicle is within the reach of the length of the acromiocoracoid ligament. The available length allows tension adjustments before fixation, which should be performed preferably using a wire fixation or a 3.5 compression screw set on a washer to achieve satisfactory stability and good pull-out resistance. Conclusion: Biometrics of the ACCL and the width of the acromioplasty and clavicular osteotomy demonstrate several possibilities for regulating the fixation which can thus be adapted to the morphology of each case while assuring good compression and solid fixation necessary for excellent long-term results

The acromion is a bony process that juts out from the lateral end of the scapular spine. It is continuous with the blade and the spinous process. The process is rectangular, and carries a facet for the clavicle. Inferiorly is sited the subacromial bursa. Inferior encroachment or displacement of the acromion can result in impingement.

The aim of this osteological study was to assess the presence of acromial displacement and variations predisposing to compaction of the subacromial space. Using the method described by Morrison and Bigliana, we assessed the scapulae of 128 men and women ranging from 35 to 92 years of age. We found a flat acromion in 30%, no hook in 48%, a small hook in 18% and a large hook in 4%. The presence of a hook was associated with a subacromial facet and a large hook with glenoid erosion.

This study confirms the presence of four types of acromion.

Frozen shoulder is a recognised complication following simple arthroscopic shoulder procedures, but its exact incidence has not been reported. Our aim was to analyse a single-surgeon series of patients undergoing arthroscopic subacromial decompression (ASD; group 1) or ASD in combination with arthroscopic acromioclavicular joint (ACJ) excision (group 2), to establish the incidence of frozen shoulder post-operatively. Our secondary aim was to identify associated risk factors and to compare this cohort with a group of patients with primary frozen shoulder.

We undertook a retrospective analysis of 200 consecutive procedures performed between August 2011 and November 2013. Group 1 included 96 procedures and group 2 104 procedures. Frozen shoulder was diagnosed post-operatively using the British Elbow and Shoulder Society criteria. A comparative group from the same institution involved 136 patients undergoing arthroscopic capsular release for primary idiopathic frozen shoulder.

The incidence of frozen shoulder was 5.21% in group 1 and 5.71% in group 2. Age between 46 and 60 years (p = 0.002) and a previous idiopathic contralateral frozen shoulder (p < 0.001) were statistically significant risk factors for the development of secondary frozen shoulder. Comparison of baseline characteristics against the comparator groups showed no statistically significant differences for age, gender, diabetes and previous contralateral frozen shoulder.

These results suggest that the risk of frozen shoulder following simple arthroscopic procedures is just over 5%, with no increased risk if the ACJ is also excised. Patients aged between 46 and 60 years and a previous history of frozen shoulder increase the relative risk of secondary frozen shoulder by 7.8 (95% confidence interval (CI) 2.1 to 28.3)and 18.5 (95% CI 7.4 to 46.3) respectively.
