Introduction: The mini C arm is a surgeon operated fluoroscopic device for use in the operating theatre for extremity orthopaedic surgery. There have been no studies comparing the radiation dose of the mini C arm and the conventional C arm. The aim of this study was to determine if the exposure to patient and surgeon was decreased with use of the mini C arm. Methods: This was a case-control study. Operations performed with the mini C arm were matched for type, complexity and operator with cases performed with the conventional C arm. The number of exposures and the total time of exposure were measured, and the skin dose and scatter calculated. Results: There were 16 case-control pairs. There was a significantly greater number of exposures taken by the surgeon operated mini C arm (p=0.02), but there was still a significantly lower exposure to the surgeon with the mini C arm (p=0.004). There was no significant difference in the patient skin dose (p=0.21). Conclusions: The surgeon operated mini C arm results in a greater exposure time and number of exposures. Despite this, the mini C arm exposes the surgeon to less radiation compared to the conventional C arm in extremity orthopaedics. The radiation exposure with the mini C arm is approximately half that of the conventional C arm. The increased number of exposures may occur because surgeons are more trigger happy with the mini C arm, or because there are technical problems with achieving a useful image. The mini C arm should be used for extremity orthopaedics whenever possible to decrease the radiation exposure

Aim:. The Aim Is To Test Whether The Right Upper Arm Length Relative Overgrowth And Spinal Deformity Severity Were Associated With Right Or Left Upper Arm Length Size-For-Age. Method:. 94 Right-Handed Girls With Rt-Ais, Age 11–18 Years, (Mean Cobb Angle 46 Degrees, Range 10–102 Degrees), Were Evaluated Using A Harpenden Anthropometer For Upper Arm Length Asymmetry, Plotted Against Right And Left Upper Arm Length Standard Deviation Scores (Sds), Calculated From 378 Normal Girls, Age 11–18 Years. Results:. The Findings Show That Upper Arm Length Asymmetry Correlates Significantly With Right, But Not Left, Upper Arm Length Sds (Right Arm Spearman's Rho=0.330 P=0.001; Left Arm Rho=−0.013 P=0.902), And With No Other Limb Or Trunk Skeletal Region Sds. Cobb Angle And Avr Also Correlate Significantly With Right, But Not Left, Upper Arm Length Sds. Discussion:. The Abnormal Asymmetry Of Upper Arm Lengths Results From Relative Overgrowth Of Right Upper Arm. Introduction. Assessment of the humeral head translation with respect to the glenoid joint, termed humeral head migration (HHM), is crucial in total shoulder arthroplasty pre-operative planning. Its assessment informs current classification systems for shoulder osteoarthritis as well as the evaluation of surgical correction. In current clinical practice, HHM assessment relies on computed-tomography (CT) imaging. However, the associated supine position might undermine its functional relevance as it does not reflect the weight-bearing condition with active muscle engagement associated with the upright standing position of most daily activities. Therefore, we assessed to what extent HHM in a supine position is associated with HHM in a range of functional arm positions. Method. 26 shoulder osteoarthritis patients and 12 healthy volunteers were recruited. 3D shapes of the humerus and scapula were reconstructed from their respective CT scans using an image processing software. 3. , and their CT-scan-based HHMs were measured. Furthermore, all subjects underwent low-dose biplanar radiography . 4. in four quasi-static functional arm positions while standing: relaxed standing, followed by 45 degrees of shoulder extension, flexion, and abduction. Using a previously validated method implemented in the programming platforms. 5. , 3D shapes were registered to the pairs of biplanar images for each arm position and the corresponding functional HHM was measured. Bivariate correlations were assessed between the CT-based HHM and each functional arm position. Result. HHM in 45 degrees of flexion and extension both showed significant and strong correlations (r>0.66 and P<0.01) with HHM assessed in the supine position. However, such a high correlation was not found for relaxed standing and 45 abduction. Conclusion. Although HHM in a supine position correlates with HHM in 45-degree extension and flexion, it is poorly associated with the HHM in abduction and relaxed standing. These results may suggest the inclusion of more functionally-relevant patient positioning toward better-informed shoulder arthroplasty planning. Acknowledgement. Funding from PRosPERos-II Project

Only a few randomised, controlled studies have compared different non-operative methods of treatment of mid-shaft fractures of the clavicle. . In this prospective, randomised controlled study of 60 participants (mean age 31.6 years; 15 to 75) we compared the broad arm sling with the figure of eight bandage for the treatment of mid-shaft clavicle fractures. Our outcome measures were pain, Constant and American Shoulder and Elbow Surgeons scores and radiological union. The mean visual analogue scale (VAS) pain score on the first day after treatment was significantly higher (VAS 1 6.8; 4 to 9) in the figure of eight bandage group than the broad arm sling group (VAS 1 5.6; 3 to 8, p = 0.034). A mean shortening of 9 mm (3 to 17) was measured in the figure of eight bandage group, versus 7.5 mm (0 to 24) in the broad arm sling group (p = 0.30). The application of the figure of eight bandage is more difficult than of the broad arm sling, and patients experience more pain during the first day when treated with this option. We suggest the broad arm sling is preferable because of the reduction of early pain and ease of application. Cite this article: Bone Joint J 2015;97-B:1562–5

Abstract. Objectives. The need for gender specific knee arthroplasty is debated. This research aimed to establish whether gender differences in patellar tendon moment arm (PTMA), a composite measure that characterises function of both the patellofemoral and tibiofemoral joints, are a consequence of knee size or other variation. Methods. PTMA about the instantaneous helical axis was calculated from positional data acquired using optical tracking. First, data post-processing was optimised, comparing four smoothing techniques (raw, Butterworth filtered, generalised cross-validation cubic spline interpolated and combined filtered/interpolated) using a fabricated knee. Then PTMA was measured during open-chain extension for N=24 (11 female) fresh-frozen cadaveric knees, with physiologically based loading and extension rates (420°/s) applied. Gender differences in PTMA were assessed before and after accounting for knee size with epicondylar width. Results. Combined smoothing enabled sub-mm accuracy (root-mean-squared (RMS) error 0.16mm, max error 0.47mm), whereas large errors were measured for raw (RMS 3.61mm, max 23.71mm), filtered-only (RMS 1.19mm, max 7.38mm) and interpolated-only (RMS 0.68mm, max 1.80mm) techniques. Before scaling, average PTMA throughout knee flexion was 46mm and mean, maximum, and minimum absolute values of PTMA were larger in males (mean differences >8mm, p<0.001), as were the PTMAs at terminal extension and flexion, and the change in PTMA from peak to terminal extension (differences >4mm, p<0.05). After scaling, the PTMA in deep flexion and the change in PTMA from peak to terminal extension were still larger in male knees (differences >2mm, p<0.05). The flexion angle of peak PTMA, unaffected by scaling, was closer to terminal extension for female knee (female 15°, male 29°, p<0.05). Conclusion. Gender differences in PTMA were identified both before and after accounting for knee size, with implications for gender-specific arthroplasty and musculoskeletal models. The developed measurement framework could also be applied in vivo for accurate measurement of the PTMA. Declaration of Interest. Purpose of the study: Reversed prostheses provide improved active anterior elevation in shoulders free of cuff tears by lengthening the deltoid and increasing is lever arm. The purpose of this work was to search for a correlation between arm lengthening and postoperative active anterior elevation. Material and methods: One hundred eighty-three reversed prostheses were reviewed with minimum one year follow-up for a complete clinical and radiographic work-up. Using a previously validated protocol, arm lengthening was assessed either in comparison with the contralateral side or with preoperative measurements. A statistical analysis was performed to search for a correlation between lengthening of the humerus and the arm with active anterior elevation. Results: Considering the entire series, mean lengthening of the humerus was 0.2±1.4 cm (range −4.7 to +5.4). Postoperative active anterior elevation was 141±27 (range 30–180). There was no correlation between humerus lengthening or shortening and active anterior elevation (p=0.169). A shorter arm produced an active anterior elevation at 121 and 0 – 1 cm lengthening an active anterior elevation at 140; lengthening 1 – 2.5 cm gave active anterior elevation at 144 and beyond 2.5 cm 147. The difference in active anterior elevation was statistically significant (p< 0.001) between patients with a shortening and those with a lengthening. Discussion: Arm lengthening corresponds to a lengthening of the humerus plus a lengthening of the infra-acromial space. We found a statistically negative correlation between arm shortening (and thus deltoid shortening) and active anterior elevation and a positive trend between lengthening and active anterior elevation. Our measurement did not take into account the increased lever arm of the deltoid and thus only partially expresses the improvement in deltoid function. Nevertheless, our study shows that objective evaluation of deltoid lengthening is possible pre- intra- and postoperatively and that this measurement can be correlated with postoperative functional outcome

Introduction: The goals of TKR are restoration of the mechanical axis, joint line and Q –angle. Reproduction of the offset of the extensor mechanism during arthroplasty is less well understood. The lever arm of the extensor mechanism is primarily affected by femoral component position, patella tracking and overall patellar thickness. Changing this lever arm alters quadriceps muscle and patellofemoral joint reaction force. Some TKR designs purposefully aim to increase this offset in order to reduce PFJ contact pressures. Overstuffing the PFJ will however adversely affect outcome. The aims of this study were to measure and compare the pre and postoperative quadriceps lever arm and its effect on function in a consecutive series of patients undergoing TKR. Methods: Fifty consecutive patients who had an LCS TKR without patellar resurfacing by a single surgeon were reviewed. Patient demographics were recorded. We measured the pre and post-operative quadriceps lever arm using comparable lateral radiographs and digital imaging software. Functional outcome in these patients was determined using the American Knee Society Score pre and 6-months post-operatively. Patients were specifically questioned regarding the presence or absence of anterior knee pain. Results: Quadriceps lever arm was on average 6 mm greater post-operatively than pre-op but this difference was not significant [t-test]. The functional outcome in these patients was independent of any change in quadriceps lever arm. Conclusions Reproduction of normal biomechanics is essential in order to optimise outcome from TKR. Using the LCS system, there is a small but insignificant increase in the quadriceps moment arm. We have not found that this has any bearing on functional outcome in these patients

Purpose: Changes in the lever arm of the abductors is not always perfectly controlled during implantation of total hip arthroplasties. Its possible effect on the development of prothesis dislocation is not known. The purpose of this study was to evaluate the influence of the lever arm and its modifications on the development of prosthetic instability. Material and methods: We analysed prospectively 73 total hip arthroplasties implanted via the posterolateral approach. The study group was composed of a consecutive series of 45 dislocated prostheses and a control group of 28 stable prostheses selected at random. The following measurements were made on the anteroposterior x-ray: 1) lever arm of the abductors, 2) femoral offset. These measures were compared with the healthy contralateral hip and when this hip was diseased or had a prosthesis, with the pre-implantation x-rays. Results: None of the studied parameters was statistically different between the dislocated and stable prostheses. However, in the dislocated prostheses, the lever arm of the abductors before insertion of the prosthesis was shorter than in the control group (p = 0.04) suggesting the presence of a group of hips “at risk”. There was a correlation between the offset values and the lever arm values for the stable prostheses and for the healthy contralateral hips in both groups. Conversely, this balance was not found in the dislocated hips. The lever arm/offset ratio was calculated to determine if the ideal ratio influenced hip stability. This ratio was not directly related to the development of dislocation, but it was decreased for dislocated hips. This ratio was ideal for 75% of the stable prostheses and for only 53% of the dislocated prostheses. Conclusions: We concluded that: 1) hips “at risk” of dislocation would have a shorter lever arm, 2) the lever arm or the femoral offset do not have a direct effect on dislocation, and 3) stable hip prostheses have a balance similar to that in healthy hips identified by a correlation between the lever arm and the femoral offset. We thus emphasise the importance of respecting these parameters although they are probably not the only factors influencing prosthesis stability. Allowable variations are small, demanding careful and precise operation planning

Reverse total shoulder arthroplasty (RTSA) is an increasingly common treatment for osteoarthritic shoulders with irreparable rotator cuff tears. Although very successful in alleviating pain and restoring some function, there is little objective information relating geometric changes imposed by the reverse shoulder and arm function, particularly the moment generating capacity of the shoulder muscles. Recent modeling studies of reverse shoulders have shown significant variation in deltoid muscle moment arms over a typical range of humeral offset locations in shoulders with RTSA. The goal of this study was to investigate the sensitivity of muscle moment arms as a function of varying the joint center and humeral offset in three representative RTSA subjects that spanned the anatomical range from our previous study cohort. We hypothesized there may exist a more beneficial joint implant placement, measured by muscle moment arms, compared to the actual surgical implant configuration. A 12 degree of freedom, subject-specific model was used to represent the shoulders of three patients with RTSA for whom fluoroscopic measurements of scapular and humeral kinematics during abduction had been obtained. The computer model used subject-specific in vivo abduction kinematics and systematically varied humeral offset locations over 1521 different perturbations from the surgical placement to determine moment arms for the anterior, lateral and posterior aspects of the deltoid muscle. The humeral offset was varied from its surgical position ±4 mm in the anterior/posterior direction, ±12mm in the medial/lateral direction, and −10 mm to 14 mm in the superior/inferior direction. The anterior deltoid moment arm varied up to 20 mm with humeral offset and center of rotation variations, primarily in the medial/lateral and superior/inferior directions. Similarly, the lateral deltoid moment arm demonstrated variations up to 20 mm, primarily with humeral offset changes in the medial/lateral and anterior/posterior directions. The posterior deltoid moment arm varied up to 15mm, primarily in early abduction, and was most sensitive to changes of the humeral offset in the superior/inferior direction. The goal of this study was to assess the sensitivity of the deltoid muscle moment arms as a function of joint configuration for existing RTSA subjects. High variations were found for all three deltoid components. Variation over the entire abduction arc was greatest in the anterior and lateral deltoid, while the posterior deltoid moment arm was mostly sensitive to humeral offset changes early in the abduction arc. Moment arm changes of 15–20 mm represent a significant amount of the total deltoid moment arm. This means there is an opportunity to dramatically change the deltoid moment arms through surgical placement of the joint center of rotation and humeral stem. Computational models of the shoulder may help surgeons optimize subject-specific placement of RTSA implants to provide the best possible muscle function, and assist implant designers to configure devices for the best overall performance

Pain produced by the tourniquet is a common source of complaint for patients undergoing carpal tunnel decompression. Practice varies as to tourniquet position. There is little evidence to suggest benefit of one position over another. Our aim was to compare the experience of both the patient and the surgeon with the tourniquet placed either on the arm or the forearm. Ethical approval was granted. Following power calculation and a significance level set at 0.05, 100 patients undergoing open carpal tunnel decompression under local anaesthetic were randomised to arm or forearm group. Visual Analogue Scores (VAS) (0–100) for pain, blood pressure and heart rate were taken at 2 minute intervals. The operating surgeon provided a VAS for bloodless field achieved and obstruction caused by the tourniquet. The demographics of the groups was similar. There were no statistically significant differences in any measure between the groups. Average tourniquet times were 8.8 minutes (forearm) and 8.2 minutes (arm). The average VAS score for forearm and arm was 13 and 11 respectively for bloodless field, 9 and 2 for obstruction. Average overall VAS for pain was 27 in each group, however interval VAS scores for pain were higher in the arm group. The average change Mean Arterial Pressure was −5 mmHg (forearm) −2 mmHg (arm) pulse rate was −1 bpm (forearm) and −2 bpm (arm). Tourniquet placement on the arm does not result in significant difference in patient pain, physiological response or length of operation. Surgeons reported less obstruction and better bloodless fields with an arm tourniquet, however there was a trend for forearm tourniquet to result in less pain for the patient

Aims

The primary aim of this study was to report the radiological outcomes of patients with a dorsally displaced distal radius fracture who were randomized to a moulded cast or surgical fixation with wires following manipulation and closed reduction of their fracture. The secondary aim was to correlate radiological outcomes with patient-reported outcome measures (PROMs) in the year following injury.

Methods

Participants were recruited as part of DRAFFT2, a UK multicentre clinical trial. Participants were aged 16 years or over with a dorsally displaced distal radius fracture, and were eligible for the trial if they needed a manipulation of their fracture, as recommended by their treating surgeon. Participants were randomly allocated on a 1:1 ratio to moulded cast or Kirschner wires after manipulation of the fracture in the operating theatre. Standard posteroanterior and lateral radiographs were performed in the radiology department of participating centres at the time of the patient’s initial assessment in the emergency department and six weeks postoperatively. Intraoperative fluoroscopic images taken at the time of fracture reduction were also assessed.

External rotation of the shoulder is commonly measured in two ways, with the arm adducted or with the arm abducted to 90°. The measurement forms an important part of the assessment of shoulder function, but has been shown to be unreliable. Following the observation that, with the arm adducted, the range of external rotation alters according to the exact position of the arm in the sagittal plane, we conducted a study to quantify the effect on the range of external rotation of a small increment in forward flexion (15°). With the arm first in a vertical position and then in 15° of forward flexion, external rotation was measured in 40 asymptomatic and 20 ‘frozen’ shoulders. With forward flexion, the range of external rotation decreased by a mean of 16.9° in the asymptomatic and 13.5° in the ‘frozen’ shoulders. We postulate that some of the variation is a function of scapular positioning. With arm flexion, the scapula protracts, resulting in alteration in glenoid version. However, some difference is due also to alteration in soft tissue tension in the two arm positions. The sagittal position of the arm affects the range of external rotation of the adducted shoulder. This variation in measurement may affect the scores of certain outcome measures

The Stanford Upper Extremity Model (SUEM) (Holzbauer, Murray, Delp 2005, Ann Biomed Eng) includes the major muscles of the upper limb and has recently been described in scientific literature for various biomechanical purposes including modeling the muscle behavior after shoulder arthroplasty (Hoenecke, Flores-Hernandez, D'Lima 2014, J Shoulder Elbow Surg; Walker, Struk, Banks 2013, ISTA Proceedings). The initial publication of the SUEM compared the muscle moment arm predictions of the SUEM against various moment arm studies and all with the scapula fixed. A more recent study (Ackland, Pak, and Pandy 2008, J Anat) is now available that can be used to compare SUEM moment arm predictions to cadaver data for similar muscle sub-regions, during abduction and flexion motions, and with simulated scapular motion. SUEM muscle moment arm component vectors were calculated using the OpenSim Analyze Tool for an idealized abduction and an idealized flexion motion from 10° to 90° that corresponded to the motions described in Ackland for the cadaver arms. The normalized, averaged muscle moment arm data for the cadavers was manually digitized from the published figures and then resampled into uniform angles matching the SUEM data. Standard deviations of the muscle moment arms from the cadaver study were calculated from source data provided by the study authors. Python code was then used to calculate the differences, percent differences, and root-mean-square (RMS) values between the data sets. Of the 14 muscle groups in the SUEM, the smallest difference in predicted and measured moment arm was for the supraspinatus during the abduction task, with an RMS of the percent difference of 11.4%. In contrast, the middle latissimus dorsi had an RMS percent difference over 400% during the flexion task. The table presents the RMS difference and the RMS of the percent difference for the muscles with the largest abduction and adduction moment arms (during abduction) and the largest flexion and extension moment arms (during flexion). The moment arm data for the SUEM model and the cadaver data (with 1 standard deviation band) during the motion of the same muscles are provided in Figure 1 for the Abduction motion task and in Figure 2 for the Flexion motion task. It is challenging to simulate the three dimensional, time variant geometries of shoulder muscles while maintaining model fidelity and optimizing computational cost. Dividing muscles in to sub regions and using wrapping line segment approximations appears a reasonable strategy though more work could improve model accuracy especially during complex three dimensional motions

As treatments of knee osteoarthrosis are continually refined, increasingly sophisticated methods of evaluating their biomechanical function are required. Whilst TKA shows good preoperative pain relief and survivorship, functional outcomes are sub-optimal, and research focus has shifted towards their improvement. Restoration of physiological function is a common design goal that relies on clear, detailed descriptions of native biomechanics. Historical simplifications of true biomechanisms, for example sagittal plane approximation of knee kinematics, are becoming progressively less suitable for evaluation of new technologies. The patellar tendon moment arm (PTMA) is an example of such a metric of knee function that usefully informs design of knee arthroplasty but is not fully understood, in part due to limitations in its measurement. This research optimized PTMA measurement and identified the influence of knee size and sex on its variation. The PTMA about the instantaneous helical axis was calculated from optical tracked positional data. A fabricated knee model facilitated calculation optimization, comparing four data smoothing techniques (raw, Butterworth filtering, generalized cross-validated cubic spline-interpolation and combined filtering/interpolation). The PTMA was then measured for 24 fresh-frozen cadaveric knees, under physiologically based loading and extension rates. Sex differences in PTMA were assessed before and after size scaling. Large errors were measured for raw and interpolated-only techniques in the mid-range of extension, whilst both raw and filtered-only methods saw large inaccuracies at terminal extension and flexion. Combined filtering/interpolation enabled sub-mm PTMA calculation accuracy throughout the range of knee flexion, including at terminal extension/flexion (root-mean-squared error 0.2mm, max error 0.5mm) (Figure 1). Before scaling, mean PTMA throughout flexion was 46mm; mean, peak, and minimum PTMA values were larger in males, as was the PTMA at terminal flexion, the change in PTMA from terminal flexion to peak, and the change from peak to terminal extension (mean differences ranging from 5 to 10mm, p<0.05). Knee size was highly correlated with PTMA magnitude (r>0.8, p<0.001) (Figure 2). Scaling eliminated sex differences in PTMA magnitude, but peak PTMA occurred closer to terminal extension in females (female 15°, male 29°, p=0.01) (Figure 3). Improved measurement of the PTMA reveals previously undocumented characteristics that may help to improve the functional outcomes of knee arthroplasty. Knee size accounted for two-thirds of the variation in PTMA magnitude, but not the flexion angle at which peak PTMA occurred, which has implications for morphotype-specific arthroplasty and musculoskeletal models. The developed calculation framework is applicable both in vivo and vitro for accurate PTMA measurement and might be used to evaluate the relative performance of emerging technologies. Introduction. Total knee arthroplasty (TKA) using conventional instrumentation has been shown to be a safe and effective way of treating end stage osteoarthritis by restoring function and alleviating pain. As robotic technology is developed to assist surgeons with intra-operative decision making such as joint balancing and component positioning, the safety of these advancements must be established. Furthermore, functional recovery and clinical outcomes should achieve comparable results to the gold standard of conventional instrumentation TKA. Methods. Eighty-seven subjects (89 knees) underwent robotic arm assisted TKA by one of three investigators as part of an FDA and IRB approved Investigational Device Exemption (IDE). To achieve the primary endpoint of intra-operative patient safety using a robotic arm assisted cutting tool, the investigators completed questionnaires to assess a series of complications related to soft tissue damage associated with conventional TKA. Western Ontario and McMaster Universities Arthritis Index (WOMAC) and Knee Society Knee Scores (KSS) were collected pre-operatively and at three month follow-up. Results. The average subject age was 65.8 ± 8.5 and the average BMI was 31.4 ± 5.7. 55% of the subjects were female. No subject experienced any of the rare intra-operative complications that comprise the primary safety endpoint. The average WOMAC score improvement from pre-operative to three months was 33.1 ± 20.04 (p<0.0001). The average KSS Knee score improvement was 46.1 ± 19.7 (p<0.0001). Subjects recovered their pre-operative range of motion by three months post-operative. Conclusion. Results from this prospective robotic arm assisted trial indicated greater intra-operative safety in blood loss and ligamentous damage when compared to conventional TKA. Subjects had excellent return to function and pain relief by three months as indicated by the KSS and WOMAC scores. Robotic technologies should be considered a safe and effective method in total joint restoration

Introduction. One of the disadvantages of lateral decubitus position during arthroscopic rotator cuff repair is the difficulty to control arm rotation intraoperatively making it necessary to create additional portals for anchor placement or an additional scrubbed assistant to control arm rotation. Methods & Technique. We describe the use of commercially available TRIMANO® support arm from Arthrex as an easy device for secure positioning during arthroscopic rotator cuff repair in lateral decubitus position. Though initially marketed to be used for shoulder arthroplasties performed in beach chair position, the senior author has used TRIMANO® to perform arthroscopic rotator cuff repair in lateral decubitus position. The device is easily connected to the operating table side rail while the affected side forearm of the patient is placed in a disposable sterile foam arm holder and attached to the TRIMANO® arm. The “Click and Move” system of the TRIMANO® allows freedom to move the arm in any direction and also provides traction to distract the joint if needed. Conclusion. To conclude TRIMANO® arm is an easy to use flexible device for use in arthroscopic rotator cuff repair in lateral decubitus position decreasing the need for additional portals or an additional scrub assistant thereby decreasing the manpower required and cost of surgery

INTRODUCTION. Total hip arthroplasty (THA) is regarded as one of the most successful surgeries in medicine. However, recent studies have revealed that ideal acetabular cup implantation is achieved less frequently than previously thought, as little as 50% of the time. It is well known that malalignment of the acetabular component in THA may result in dislocation, reduced range of motion, or accelerated wear. This study reports accuracy of a tactile robotic arm system to ream the acetabulum and impact an acetabulur cup compared to manual instrumentation. METHODS. 12 fresh frozen cadaveric acetabulae were pre-operatively CT scanned and 3D templating was used to plan the center of rotation, and anteversion and inclination of the cup. Each specimen received THA, six prepared manually and six prepared with robotic arm guidance. Tactile, visual, and auditory feedback was provided through robotic guidance as well as navigated guided reaming and cup impaction. The robotic guidance constrained orientation of instruments thus constraining anteversion, inclination, and center of rotation for reaming, trialing, and final cup impaction. Post-operative CT scans were taken of each specimen to determine final cup placement for comparison to the pre-operative plans. RESULTS. In all cases, robotic arm guidance resulted in ±4° of anteversion and ±5° of inclination each relative to the pre-operative plan. Absolute RMS errors were 2.16 ± 1.35° for anteversion and 1.91 ± 1.55° for inclination. Cup placement with robotic guidance was significantly more accurate and precise than placement with manual instruments. With manual instrumentation the errors were, on average, 4.0 times higher in anteversion and 5.9 times higher in inclination compared to robotic instrumentation. CONCLUSION. This tactile robotic system substantially improved the accuracy of acetabular reaming and placement of the final cup compared to traditional manual techniques. With greater knowledge of ideal acetabular cup position, highly accurate techniques may allow surgeons to decrease the risk of dislocation, promote durability and improve the ability to restore appropriate leg length and offset. Tactile robotics has proven to be safe and effective in both knee and hip surgery and provides the potential to redefine the “instrument set” used for orthopedic procedures

We describe a method of partial limb salvage for the treatment of large primary malignant tumours of the arm. The tumour-bearing area is resected as a cylindrical segment and the distal arm is then replanted with the necessary shortening. The method is suitable for stage-IIB tumours with or without neurovascular involvement which, because of their extent, could otherwise be adequately treated only by amputation. From 1987 to 1992 we used this method in 12 patients with primary malignant bone or soft-tissue sarcomas. Wide resection margins were achieved in all, but six patients died from their disease at a mean of 21.5 months (6 to 48), none with any local recurrence. Five patients have no evidence of disease at a mean follow-up period of 52.2 months (22 to 78), and one was lost to follow-up at 48 months postoperatively when there was no evidence of disease. The results of the functional evaluation of ten patients with a follow-up of over ten months were excellent in one, good in six and fair in three, by the criteria of Enneking (1987). Recovery after nerve reconstruction was satisfactory in all cases with sensation S3 or higher and motor function M2+ or higher. Detailed evaluation of hand function on the Millesi score rated only 22% (9.6% to 33.7%) as compared with the contralateral side, but the patients were satisfied and refused further operations for the improvement of function. These oncological and functional results allow us to recommend resection-replantation as a valuable alternative to amputation for the treatment of primary malignant tumours of the arm

Introduction: To ascertain the optimal functional abductor moment arm of the hip for THA, we focused on the Trendelenburg sign. We investigate the various conditions associated with abductor moment arm to achieve a negative Trendelenburg sign postoperatively. Methods: We reviewed 30 patients (34 uncemented primary THAs; mean age, 56 years) at a minimum of two years postoperatively. The tilting angle of the pelvis while performing the Trendelenburg test (Trendelenburg angle) was measured using a magnetic sensor system. On the hip radiographs, the %FO was calculated by dividing the femoral offset, by the distance between the centers of the bilateral femoral heads, and by multiplying by 100. Results: The Trendelenburg angle averaged −4.3 degrees (−9 to −2.0) in all cases with a positive Trendelenburg sign, whereas it averaged +1.4 degrees (−2.0 to +12.0) when the Trendelenburg sign was negative. The %FO having a positive Trendelenburg sign (16.9 %; 10.0 to 22.5) were significantly shorter than those having a negative Trendelenburg sign (19.4 %; 13.5 to 24.7). The Trendelenburg angle correlated positively with %FO. Of the cases with a %FO value of more than 20%, about eighty percent of the cases had a negative Trendelenburg sign. Discussion: The optimum abductor moment-arm, when the Trendelenburg sign is negative, has not been described. This study indicates that a %FO of 20 % may be one of the factors taken into account when determining the suitable size and position for acetabular and femoral neck components
