Twenty-five patients with 30 chondral lesions of the knee were treated with an autogenous strip of costal perichondrium. The graft was fixed to the subchondral bone with Tissucol (Immuno, Vienna), a human fibrin glue. The leg was then immobilised for two weeks followed by two weeks of continuous passive motion. Weight-bearing was permitted after three months. The mean knee score (Ranawat, Insall and Shine 1976) changed from 73 before operation to 90 one year after; in 14 patients evaluated after two years there was no decrease. In 28 cases the defect was completely filled with tissue resembling articular cartilage. We conclude that in most cases perichondral arthroplasty of cartilage defects of the knee gives excellent results.

Purpose: As a one-step surgical procedure, microfracture is frequently considered to be technically easier and associated with less postoperative morbidity than autologous chondrocyte implantation (ACI), which involves both arthrotomy and arthroscopy and therefore safety was assessed in patients with symptomatic cartilage lesions of the knee treated with characterized chondrocyte implantation (CCI) or microfracture. Methods: CCI (n=57) was compared to microfracture (n=61) in patients with grade III–IV symptomatic cartilage defects of the femoral condyles in a Phase III, prospective, multi-center, randomized, controlled trial. Safety assessments included adverse events (AEs), physical examination, vital signs, hematology and clinical chemistry. Results: At 18 months post-surgery, similar proportions of patients experienced AEs in the CCI (88%) and microfracture (82%) groups; 67% and 59%, respectively, experienced AEs considered treatment related. The AE profile was generally similar between groups, with no significant difference for hypertrophy, although significantly more CCI-treated patients had joint swelling (19% versus 4.9%; p=0.022) and treatment-related joint crepitation (12% versus 1.6%; p=0.028). Although the proportion of patients with severe AEs was similar for CCI (12%) and microfracture (13%), slightly more microfracture-treated patients experienced serious (life-threatening or requiring hospitalization) AEs (13% versus 8.8%). No patients discontinued due to AEs or died during the study. Conclusion: Contrary to general opinion, the two-step CCI procedure, involving arthroscopy followed by arthrotomy, has a similar safety profile to that of microfracture, a one-step arthroscopic procedure, for treating cartilage lesions of the knee

Objectives. Matrix-assisted autologous chondrocyte transplantation (MACT) has been developed and applied in the clinical practice in the last decade to overcome most of the disadvantages of the first generation procedures. The purpose of this systematic review is to document and analyse the available literature on the results of MACT in the treatment of chondral and osteochondral lesions of the knee. Methods. All studies published in English addressing MACT procedures were identified, including those that fulfilled the following criteria: 1) level I-IV evidence, 2) measures of functional or clinical outcome, 3) outcome related to cartilage lesions of the knee cartilage. Results. The literature analysis showed a progressively increasing number of articles per year. A total of 51 articles were selected: three randomised studies, ten comparative studies, 33 case series and five case reports. Several scaffolds have been developed and studied, with good results reported at short to medium follow-up. Conclusions. MACT procedures are a therapeutic option for the treatment of chondral lesions that can offer a positive outcome over time for specific patient categories, but high-level studies are lacking. Systematic long-term evaluation of these techniques and randomised controlled trials are necessary to confirm the potential of this treatment approach, especially when comparing against less ambitious traditional treatments

Conservative management of osteoarthritis is boring, boring, boring! After all, we are surgeons. We operate, we cut! We all know that to retain respectability we have to go through the motions of ‘conservative management’, just so that we don't appear too anxious to apply a ‘real’ solution to the problem. However, the statistics are overwhelming. An estimated 43 million Americans have ‘arthritis’, but only 400,000 are coming forward each year for TKR. That means that in one way or another 42,600,000 are being treated conservatively. Most of those are self treating by self medication, use of external support, but mostly by decreasing their activities to a level where they can tolerate symptoms. They come to us when these measures stop working. We know what to do. 1. Weight loss – patients don't do it, 2. Physical therapy – very limited effectiveness 3. NSAIDS – patients have already tried OTC NSAIDS and have heard scary stories about therapeutic NSAIDS, 4. Hyaluronans – expensive, labour intensive, modest effectiveness, 5. Glucosamine/Chondroitin – might work, won't hurt, mixed evidence, 6. SAM-e, MSM – limited evidence – who knows?. What's on the horizon? Could OA of the knee go the way of RA, i.e. dramatically disappear from the population seeking TKR? It could happen. Electrical stimulation – it does good things for chondrocytes, circulation, suppresses destructive enzymes and in controlled studies reduces symptoms and improves function, deferring TKR. Cell therapy – possibly an effective solution to early cartilage lesions in the knee

The present study analysed the clinical outcome and the histological characteristics of membrane-seeded autolo-gous chondrocytes implantation at 24 month after surgery for chondral defects. A prospectic study was performed on fifteen patients (8 males and 7 females, mean age 38 years) suffering from cartilage lesions of the knee (12 cases) and the ankle (3 case). The patients underwent matrix-induced autologous chondrocyte implantation (MACI). Clinical outcomes were assessed by revised IKDC form and Knee Osteoarthritis and Injury Outcome Score (KOOS). At 12 months after implantation biopsy samples were obtained from 7 patients. The specimens were analysed by histochemistry, immunohistochemistry (ICRS visual histological assessment scale) and histomorphometry (Quantimet 500+). Improvement 12 months after operation was found subjectively (39.7 to 57.9) and in articular function levels. IKDC scores showed marked improvement at 12 months (88% A/B). 90% of biopsies showed: smooth articular surface, hyaline-like matrix, columnar cell distribution, viable cells, normal subchondral bone, tide-mark. All sections were clearly stained with safranin-O, alcian blue, and revealed immunoreaction for S-100 protein, chondroitin-S and type II collagen. Clinical improvement and hyaline-like appearance of the repair tissue indicate that MACI implantation is an effective technique for the treatment of cartilage lesions

Single focal grade IV cartilage lesion in the knee has a poor healing capacity. Instead these lesions often progress to severe and generalized osteoarthritis that may result in total knee replacement. Current treatment modalities aim at biological repair and, although theoretically appealing, the newly formed tissue is at the best cartilage-like, often fibrous or fibrocartilaginous. This at the expense of sophisticated laboratory resources, delicate surgery and strict compliance from patients. An alternative may be small implants of biomaterial inserted to replace the damaged cartilage. We investigated the response of the opposing tibia cartilage to a metallic implant inserted at different depth into the surrounding cartilage level. Methods. The medial femoral condyle of both knees of 12 sheep, 70–90kg, 2 year of age and from the same breeder, was operated. A metallic implant with an articulating surface of 316L stainless steel, diameter of 7mm, HA plasma sprayed press-fit peg and a tailored radius and contour to the sheep femoral condyle was placed at the most weight-bearing position. The level of the implant was aimed flush, 0,3 and 0,8 mm below surrounding cartilage. The animals were stabled indoors, allowed to move freely and euthanized after 6 and 12 weeks. Postoperatively the knees were high resolution photographed for macroscopic evaluation. The position and depth of the implant were analysed using a laser scan device. Tibial and femoral condyles specimen were decalcified and slices were prepared for microscopic evaluation. Implant position and cartilage damage was assessed from two independent observers using a macroscopic ICRS score and a modified histologic score according to Mankin. Results. 22 tibia condyles showed a variety of cartilage damage ranging from severe damage down to subchondral bone to an almost pristine condition. There was a strong correlation between implant position and damage to opposing cartilage surface. Mankin score correlated significantly with implant position (p<0.001 regression analysis, r. 2. =.45) as did the ICRS score (p<0.001, regression analysis, r. 2. =.67). Implants sitting proud were associated with poor Mankin score. There was no difference between 6-week and 3-months knees. Conclusion. By precise postoperative measurement we have shown that significant imprecision occur; this has never before been studied. We found a distinct correlation between implant position and cartilage damage. These results suggest that further studies of metallic implants, inserted into cartilage defects with the utmost precision regarding the surrounding cartilage, may be warranted

Purpose. The prevalence of focal chondral lesions reported inthe literature during knee arhroscopy can be as high as 63%. Of these, more than half are either grade III or grade IV lesions (Outerbridge). Full thickness cartilage lesions ranging from 2cm2 to 10cm2 are the most challenging to treat. To goal of this study was to evaluate clinical outcomes of pain, function and quality of life, along with radiological outcomes of cartilage repair using microfracture, autologous minced cartilage and polymeric scaffold. Method. A cohort of thirty-eight patients with Outerbridge grade III or IV cartilage injuries larger than 2cm2 in the knee's femoral condyle, trochlea or patella were prospectively folowed since 2008. They were all treated with microfracture, fresh minced autologous cartilage grafting and a polymeric scaffold technique through mini-arthrotomy of the knee. Autografts and scaffolds were secured to subchondral bone using fibrin glue and tran-sosseous resorbable sutures. Patients were evaluated pre and postoperatively using VAS scores for pain, WOMAC and IKDC scores for knee function and SF-36 questionnaire for quality of life. Clinical evaluations were done by physical examination, and imaging was done using X-Rays, MRI and arthro-CT. Results. Mean follow-up time was14.64.6 months. Mean age was 48.39.3 years old. Pre-op lesions averaged 3.51.5 cm2. VAS pain scores were significantly reduced after surgery (7,62 to 2,52.3, p<. 0001). Improvement in knee function using IKDC score improved from 26,717.5 to 55,415.3, p<. 001). In addition, WOMAC total scores showed significant reduction from 55,520.3 to 27,517.6. SF-36 quality of life Physical Component Summary improved from 26,411.4 to 45,812.3, p<. 01; Mental Component Summary improved from 41,916.8 to 49,411.2, p<. 048). Imaging results indicate sustained cartilage thickness from 6 to 18 months. One patient was an early failure due to scaffold loosening, and two patients had no clinical improvement and no significant cartilage regeneration on MRI and Arthro-CT imaging at 6 months post-op. Conclusion. The combination of microfractures, fresh minced autologous cartilage grafting and polymeric scaffold fixation seems to be an effective treatment option for post-traumatic and focal cartilage lesions of the knee in the short term. A longer-term follow-up to evaluate the sustainability of these results is ongoing. Shortcomings of this study are its short term, the lack of second look arthroscopies and cartilage biopsies to evaluate cartilage microstructure, and the absence of a gold standard treatment for full-thickness cartilage lesions larger than 2cm2 that could be used as a control

Aims

Orthopaedic surgery requires grafts with sufficient mechanical strength. For this purpose, decellularized tissue is an available option that lacks the complications of autologous tissue. However, it is not widely used in orthopaedic surgeries. This study investigated clinical trials of the use of decellularized tissue grafts in orthopaedic surgery.

Methods

Using the ClinicalTrials.gov (CTG) and the International Clinical Trials Registry Platform (ICTRP) databases, we comprehensively surveyed clinical trials of decellularized tissue use in orthopaedic surgeries registered before 1 September 2022. We evaluated the clinical results, tissue processing methods, and commercial availability of the identified products using academic literature databases and manufacturers’ websites.

Purpose: Historically, there have been few surgical options for patients with focal full-thickness cartilage lesions in the knee who have failed or are too advanced in age for biologic resurfacing treatments, yet are also relatively too young or unwilling to undergo conventional total or unicompartmental knee arthroplasty. The UniCAp knee resurfacing arthroplasty provides an option for these patients that is minimally invasive, preserves the menisci and cruciate ligaments, and retains the bony architecture of the knee joint, thereby providing the potential for a rapid recovery to more vigorous activities than might be permitted after conventional knee arthroplasty, while preserving range of motion. The objective of this study was to examine the clinical results of a patient cohort undergoing the UniCAP knee resurfacing in the medial compartment of the knee. Method: Prospective patients were screened with history and clinical examination, weight-bearing radiographs, and MRI. Patients were offered UniCap knee resurfacing arthroplasty if they had symptomatic full-thickness cartilage lesions in the medial and/or patellofemoral compartments. The cohort included 38 cases in 35 patients who underwent the UniCAP knee resurfacing procedure in the knee with focal medial compartment (with or without patellofemoral) osteoarthritis in the knee joint. In addition, patients were assessed with validated and established outcome scales including the International Knee Documentation Committee Subjective Form, the Knee Injury and Osteoarthritis Outcome Scale, which includes the WOMAC Osteoarthritis Index. Results: The average age of patients undergoing knee resurfacing was 48.25 years (Range: 23 to 80). There were 24 males and 12 females. Thirty-one patients underwent isolated medial compartment resurfacing and 7 patients received both a medial compartment resurfacing and trochlear resurfacing. Three patients underwent a concomitant ACL reconstruction and 1 patient underwent a concomitant high tibial osteotomy. The mean duration of follow up was 18 months (Range: 12 to 26 months). There was an overall mean improvement from the pre-operative to post-operative scores in the IKDS-SF (P < 0.01), KOOS (all domains, P < 0.01) and WOMAC Index (P < 0.01). There were no major complications such as deep infection, DVT, or implant failure. In addition, there was no evidence of mechanical symptoms/signs or radiographic evidence of loosening at any time point post-operatively. Conclusion: The short-term results demonstrate that the UniCAP resurfacing arthroplasty in the knee is a viable treatment option for focal full thickness cartilage lesions in the medial compartment of the knee in patients who are no longer candidates for biologic resurfacing procedures and who are also relatively too young or unwilling to undergo conventional total or unicompartmental knee arthroplasty

Aims: Tissue engineering is an increasingly popular method of addressing pathological disorders of cartilage. Recent studies have demonstrated the clinical efficacy of autologous chondrocytes implantation in cartilage defects, but there is little information on the use of a solid scaffold and on the composition of the repair tissue. The present study analysed the clinical outcome and the histological characteristics of membrane-seeded autologous chondrocyte implantation at 12–24 month after operation. Materials and methods: Eleven patients (7 males and 4 females, mean age 37 years) suffering from cartilage lesions of the knee (10 cases) and the ankle (1 case), underwent autologous chondrocyte implantation procedure in which the expanded cells were seeded on type I/III collagen membrane before transplantation (MACI – Verigen, D). Clinical outcomes were assessed by ICRS evaluation package: revised IKDC form and Knee Osteoarthritis and Injury Outcome Score (KOOS). At least 12 months after implantation biopsy samples were arthroscopically obtained from 7 patients previous informed consent. The regenerated tissue were taken according to the ICRS standardized procedure. The specimens were stained with safranin-O and alcian blue, polyclonal antibodies anti S-100 protein and monoclonal antibodies anti chondroitin sulphate, anti-collagen type I and II. Moreover the number of cells/area was quantitatively assessed by histomorphometric method (Quantimet 500+). Ultrastructural analysis was also performed by transmission electron microscopy (TEM). The specimens were evaluated by the ICRS visual histological assessment scale. Results: Improvement 12 months after operation was found subjectively (39.7 to 57.9) and in knee function levels. The International Knee Documentation Committee (IKDC) scores showed marked improvement at 12 months (87% A/B). 90% of biopsies showed: smooth articular surface (I:3), hyaline-like matrix cartilage (II:3), cell distribution (columnar-clusters III:2), predominantly viable cells (IV:3), normal subchondral bone (V:3), normal cartilage mineralization and tide-mark (VI:3). All sections were clearly stained with safranin-O and alcian blue. In all the specimens the cells revealed a strong immunoreaction for S-100 protein and showed a positive reaction for chondroitin-S and type II collagen. Type I collagen was immuno-detected in the more superficial layers of the biopsies. TEM analysis revealed a defined chondral cell phenotype within a chondroid matrix. Tissue heterogeneity and irregularities of the surface were observed in two cases. Conclusions: Clinical improvement and hyaline-like appearance of the repair tissue indicate that membrane-seeded autologous chondrocyte implantation is an effective technique for the treatment of cartilage lesions

Perilesional changes of chronic focal osteochondral defects were assessed in the knees of 23 sheep. An osteochondral defect was created in the main load-bearing region of the medial condyle of the knees in a controlled, standardised manner. The perilesional cartilage was evaluated macroscopically and biopsies were taken at the time of production of the defect (T0), during a second operation one month later (T1), and after killing animals at three (T3; n = 8), four (T4; n = 8), and seven (T7; n = 8) months. All the samples were histologically assessed by the International Cartilage Repair Society grading system and Mankin histological scores. Biopsies were taken from human patients (n = 10) with chronic articular cartilage lesions and compared with the ovine specimens. The ovine perilesional cartilage presented with macroscopic and histological signs of degeneration. At T1 the International Cartilage Repair Society ‘Subchondral Bone’ score decreased from a mean of 3.0 (sd 0) to a mean of 1.9 (sd 0.3) and the ‘Matrix’ score from a mean of 3.0 (sd 0) to a mean of 2.5 (sd 0.5). This progressed further at T3, with the International Cartilage Repair Society ‘Surface’ grading, the ‘Matrix’ grading, ‘Cell Distribution’ and ‘Cell Viability’ grading further decreasing and the Mankin score rising from a mean of 1.3 (sd 1.4) to a mean of 5.1 (sd 1.6). Human biopsies achieved Mankin grading of a mean of 4.2 (sd 1.6) and were comparable with the ovine histology at T1 and T3.

The perilesional cartilage in the animal model became chronic at one month and its histological appearance may be considered comparable with that seen in human osteochondral defects after trauma.
