Despite a high frequency of ligamentous ankle injuries, scant attention has been given to the morphology of the IF morphology incisura fibularis in the onset and development of these lesions. Therefore, we systematically reviewed the relation between ligamentous ankle disorders and the morphometrics of the IF. Summary. Syndesmotic malreduction or failure to restore fibular length are the leading causes for early reoperation after ankle fracture surgery. Anatomic fracture reduction and congruent ankle mortise can be achieved in the majority of cases following revision surgery. Introduction. The goal of ankle fracture surgery is to restore anatomical congruity. However, anatomic reduction is not always achieved, and residual talar displacement and postoperative malreduction predispose a patient to post-traumatic arthritis and poor functional outcomes. The present study aimed to determine the most common surgical errors resulting in early reoperation following ankle fracture surgery. Patients & Methods. We performed a chart review to determine the most common types of malreductions that led to reoperation within the first week following ankle fracture surgery. From 2002 to 2011, we identified 5123 consecutive ankle fracture operations in 5071 patients. 79 patients (1.6%) were reoperated on due to malreduction (residual fracture displacement > 2mm) detected in postoperative radiographs. The Dumfries and Galloway Royal Infirmary (DGRI) catchment area encompasses 5 of the “7 Stanes” mountain bike trails which had approximately 165000 visitors in 2010. Using our hospital coding system we identified potential mountain bike injuries in 2010. Patient postcode, injuries, operating theatre time and number of clinic appointments was recorded. We confirmed mountain bike related hospital admissions in 29 patients. 13 patients had local (DG) postcodes and 16 had non-DG post codes. The DG postcode patients required 41 bed days, 8 operations, 400 minutes theatre time, and 35 DGRI clinic appointments. The non-DG postcodes required 50 bed days, 11 operations, 730 minutes theatre time, 3 DGRI clinic appointments and 8 outpatient referrals to other hospitals. We aimed to review the outcome of Agility total ankle replacements carried out in our institution between 2002 and 2006. Follow-up consisted of clinical and radiological review pre-operatively, then at 6 weeks, 6 and 12 months, and annually until 10 years post op. Clinical review included the American Orthopaedic Foot and Ankle Score, satisfaction and pain scores. Case notes were reviewed to determine intra and post-operative complications. 30 arthroplasties were performed in 30 consecutive patients. Pre-operative diagnosis was rheumatoid arthritis(16), primary osteoarthritis(12) and post-traumatic osteoarthritis(2). After a mean follow up of 6.2 years (1.4–10.1), 4 patients had died, and 22 out of the remaining 24 were available for follow-up. Intra operative complications included lateral malleoli fracture(3) and superficial peroneal nerve injury(2). Post operative complications included 1 early death, but this was not related to the surgical procedure. Two patients developed deep infections of the prosthesis. We undertook a retrospective audit to assess quality of service provided by Nurse-Led Review Clinic at Glasgow Royal Infirmary for patients sustaining ankle fracture requiring surgical stabilisation. Nursing staff had received training from the senior author regarding clinical examination and radiograph interpretation. We retrospectively reviewed the clinical documentation and radiographs of 104 patients who attended from January 2009 to December 2009. Any clinical issues were identified and radiographs were scrutinised by two of the authors to assess accuracy of interpretation. Nurse-led management was then assessed as to its appropriateness. Finally two retrospective questionnaires were used to assess both the nurses and patients satisfaction with the clinic. Nurse-led clinic protocol: First appointment 10 days: Wound review, application of lightweight plaster. Second appointment 6 weeks: Removal of plaster, check radiographs. Final appointment 12 weeks: Clinical assessment, radiographs, discharge. Clinical assessment: ensure wound satisfactory, range of movement and weight-bearing are improving. Radiographic criteria: 6 weeks: Assess for talar shift, lucency or metal-work concerns. 12 weeks: Assess evidence of fracture union, infection, loosening or backing out. If any concerns with the patients' progress nursing staff would discuss with the consultant. First appointment: 7 wound problems. 5 managed by nurses and resolved. 2 discussed with surgeon, 1 settled, 1 required oral antibiotics. 3 radiographs discussed with surgeon. 2 conservative management. 1 re-operation. Summary. The ankle X-ray has moderate diagnostic power to identify syndesmotic instability, showing large sensitivity ranges between observers. Classification systems and radiographic measurements showed moderate to high interobserver agreement, with extended classifications performing worse. Introduction. There is no consensus regarding the diagnosis and treatment of ankle fractures with respect to syndesmotic injury. The diagnosis of syndesmotic injury is currently based on intraoperative findings. Surgical indication is mainly made by ankle X-ray assessment, by several classification systems and radiographic measurements. Misdiagnosis of the injury results in suboptimal treatment, which may lead to chronic complaints, like instability and osteoarthritis. This study investigates the diagnostic power and interobserver agreement of three classification methods and radiographic measures, currently used to assess X-ankles and to identify syndesmotic injury. Patients and Methods. Twenty patients (43.2 ± 15.3yrs) with an ankle fracture, indicated for surgery, were prospectively included. All patients received a preoperative ankle X-ray, which was assessed by several observers: two orthopaedic surgeons, one trauma surgeon and two radiologists.
