The purpose of this study is to assess the long term results of combined ACL reconstruction and unicompartmental knee replacements (UKR). These patients have been selected for this combined operation due to their combination of instability symptoms from an absent ACL and unicompartmental arthritis.

Retrospective review of 44 combined UKR and ACL reconstruction by a single surgeon. Surgeries included both medial and lateral UKR combined with either revision ACL reconstruction or primary ACL reconstruction. Patient reported outcomes were obtained preoperatively, at one year, 5 years and 10 years. Revision rate was followed up over 13 years for a mean of 7.4 years post-surgery.

The average Oxford score at one year was 43 with an average increase from pre-operation to 1 year post operation of 15. For the 7 patients with 10 year follow up average oxford score was 42 at 1 year, 43 at 5 years and 45 at 10 years.

There were 5 reoperations. 2 for revision to total knee arthroplasty and 1 for an exchange of bearing due to wear. The other 2 were the addition of another UKR. For those requiring reoperation the average time was 8 years.

Younger more active patients presenting with ACL deficiency causing instability and unicompartmental arthritis are a difficult group to manage. Combining UKR and ACL reconstruction has scant evidence in regard to long term follow up but is a viable option for this select group. This paper has one of the largest cohorts with a reasonable follow up averaging 7.4 years and a revision rate of 11 percent.

Combined unilateral knee replacements and ACL reconstruction can be a successful operation for patients with ACL rupture causing instability and unicompartmental arthritis.

Glenoid exposure is the name of the game in total shoulder arthroplasty. I can honestly say that it took me more than 5 years but less than 10 to feel confident exposing any glenoid, regardless of the degree of bone deformity and the severity of soft-tissue contracture. This lecture represents the synthesis of my experience exposing some of the most difficult glenoids. The basic principles are performing extensive soft-tissue release, minimizing the anteroposterior dimension of the humerus by osteophyte excision, making an accurate humeral neck cut, having a plethora of glenoid retractors, and knowing where to place them. The ten tips, in reverse order of importance are: 10.) Tilt the table away from operative side—this helps face the surface of the glenoid, especially in cases of posterior wear, toward the surgeon. 9.) Have multiple glenoid retractors—these include a large Darrach, a reverse double-pronged Bankart, one or two blunt Homans, small and large Fukudas. 8.) Remove all humeral osteophytes before attempting to retract the humerus posteriorly to expose the glenoid—this helps to decrease the overall anteroposterior dimension of the humerus and allows for maximum posterior displacement of the humerus. 7.) Make an accurate humeral neck cut—even 5mm of extra humeral bone will make glenoid exposure difficult. 6.) Optimal humeral position—it has been taught that abduction, external rotation, and extension is the optimal position. It may vary with each case. Therefore, experiment with humeral rotation to find the position that allows maximum visualization. This is often the position that makes the cut surface of the humerus parallel to the surface of the glenoid. 5.) Optimal retractor placement—my typical retractor placement is a Fukuda on the posterior lip of the glenoid, a reverse double-pronged Bankart on the anterior neck of the scapula, and a blunt Homan posterosuperiorly. Occasionally, a second blunt Homan anteroinferiorly is helpful, particularly in muscular males with a large pectoralis major. 4.) Laminar spreader for lateral humeral displacement—this can be helpful for posterior capsulorrhaphy or for posterior glenoid bone grafting. 3.) Maximal humeral capsular release—the release of the anterior capsule from the humerus must go well past the 6 o'clock position and up the posterior surface of the humerus. This aides in humeral exposure but also allows for more posterior displacement of the humerus during glenoid exposure. 2.) Anteroinferior capsular release or excision—extensive anteroinferior release or excision (my preference), allows for maximal posterior humeral displacement and also restores external rotation. 1.) Posterior or posteroinferior capsular release—release of the posteroinferior corner of the capsule from the glenoid results in a noticeable increase in posterior humeral retractability. In cases without substantial posterior subluxation, extensive release of the entire posterior capsule is performed

Adequate fixation of implant components is an important goal for all arthroplasty procedures. Aseptic loosening is one of the leading causes of revision surgery in total knee arthroplasty. Radiostereometric analysis (RSA) is an imaging technique to measure implant migration, with established migration thresholds for well-fixed, at risk, and unacceptably migrating components. The purpose of the present study was to examine the long-term fixation of a cemented titanium fixed bearing polished tibial baseplate. Patients enrolled in a previous two-year prospective trial were recalled at ten years. All patients received a cemented, posterior-stabilised total knee replacement of the same design implanted by one of three surgeons. Of the original 35 patients, 16 were available for long-term follow-up, with one patient lost to follow-up, nine patients deceased, and a further nine patients unwilling to return to the clinic. Each patient underwent RSA imaging in a supine position using a conventional RSA protocol. Migration of the tibial component in all planes as well as maximum total point motion (MTPM) was compared between all time points (baseline, six weeks, three months, six months, one year, two years) up to the ten year follow-up visits. Outcome scores including the Knee Society Score (KSS), WOMAC, SF-12, and UCLA Activity Score were recorded. At ten years, the mean migrations of the tibial component were less than 0.1 mm and 0.1 degree in all planes relative to the post-operative RSA exam. There was no significant difference in tibial component migration between time points. However, MTPM increased significantly over time (p = 0.002), from 0.23 ± 0.18 mm at six weeks to 0.42 ± 0.20 mm at ten years. At one year, 13 patients had an acceptable MTPM level, three patients had an ‘at risk’ level, and no patient had an ‘unacceptable’ level. No patients were revised at ten years. WOMAC and KSS were significantly improved (p < 0.0001) at the latest follow-up compared to pre-operatively, but there was no difference in SF-12. The median UCLA Activity Score at latest follow-up was six (range, two to eight). The tibial baseplate demonstrated solid fixation at ten years. No patients had an unacceptable MTPM level at one year and no patients were revised at ten years, supporting the use of RSA to predict long-term loosening risk. The low level of tibial baseplate migration found in the present study correlates to the low rate of revision for this implant as reported in individual studies and in joint replacement registries

Introduction. Pelvic tilt can vary over time due to aging and the possible appearance of sagittal spine disorders. Cup position in total hip arthroplasty (THA) can be influenced due to these changes. We assessed the evolution of pelvic tilt and cup position after THA and the possible appearance of complications for a minimum follow-up of ten years. Materials and methods. 343 patients received a cementless THA between 2006 and 2009. All were diagnosed with primary osteoarthritis and their mean age was 63.3 years (range, 56 to 80). 168 were women and 175 men. 250 had no significant lumbar pathology, 76 had significant lumbar pathology and 16 had lumbar fusion. Radiological analysis included sacro-femoral-pubic (SFP), acetabular abduction (AA) and anteversion cup (AV) angles. Measurements were done pre-operatively and at 6 weeks, and at five and ten years post-operatively. Three measurements were recorded and the mean obtained at all intervals. All radiographs were evaluated by the same author, who was not involved in the surgery. Results. There were nine dislocations: six were solved with closed reduction, and three required cup revision. All the mean angles changed over time; the SFP angle from 59.2º to 60º (p=0.249), the AA angle from 44.5º to 46.8º (p=0.218), and the AV angle from 14.7º to 16.2º (p=0.002). The SFP angle was lower in older patients at all intervals (p<0.001). The SFP angle changed from 63.8 to 60.4º in women and from 59.4º to 59.3º in men, from 58.6º to 59.6º (p=0.012). The SFP angle changed from 62.7º to 60.9º in patients without lumbar pathology, from 58.6º to 57.4º in patients with lumbar pathology, and from 57.0º to 56.4º in patients with a lumbar fusion (p=0.919). The SFP cup angle was higher in patients without lumbar pathology than in the other groups (p<0.001), however, it changed more than in patients with lumbar pathology or fusion at ten years after THA (p=0.04). Conclusions. Posterior pelvic tilt changed with aging, influencing the cup position in patients after a THA. Changes due to lumbar pathology could influence the appearance of complications at mid and long-term

Aim. There is a constant increase of joint arthroplasties to improve the quality of life of an aging population. Prosthetic-joint infections are rare, with an incidence of 1–2%, but they represent serious complications in terms of morbidity and mortality. The mortality was known to be approaching 8% in the elderly. The aim of this retrospective study is to reassess the two-year mortality rate over the last ten years. Method. Patients having a prosthetic joint infection at Lausanne University Hospital (Switzerland) between 2006 and 2016 were included. The two-year mortality rate depending on sex, age, type of infection and type of surgical therapy was measured. Results. 444 patients (61% hips, 37% knees) were identified with a median age of 70 years. The two-year mortality rate was 5%. There was no difference between hip and knee prosthesis but the mortality was higher for men (6%) than women (4%). The rate has not changed over the last ten years. However, the mortality rate is more than doubling (12%) for patients over eighty years old. Furthermore, chronic prosthetic-joint infections seem to have a higher mortality rate (7%) than acute ones (4%). Finally, patients treated with a one-stage or two-stage exchange seem to have a lower mortality rate (<1%) than the ones treated with debridement and retention (11%). Conclusion. The mortality seems to be less than thought before and depends on sex, age, type of infection and type of surgical therapy

Aim

In Two-Stage Revision, utilizing temporary antibiotic spacers is widely accepted. These spacers are available prefabricated or can be individually moulded intraoperatively. In this study, we analysed the efficacy of prefabricated and individual spacers in infection eradication of periprosthetic joint infection in knee and hip arthroplasties.

Method

All spacers implanted at a tertiary academic center during two-stage exchanges between June 2010 and December 2019 were retrospectively analysed. Among 249 patients, 167 cases (minimum follow-up ≥ 12 months) were included. Commercial spacers contained vancomycin and gentamycin, while individual spacers contained vancomycin alone. Subgroup analysis by manufacturers was conducted using non-parametric methods including Mann-Whitney U and Kruskal-Wallis tests. Survival analysis utilized Kaplan-Meier curves, and categorical data were analyzed using the Chi² test. Statistical significance was defined as p < 0.05.


Instability and aseptic loosening are the two main complications after revision total hip arthroplasty (rTHA). Dual-mobility (DM) cups were shown to counteract implant instability during rTHA. To our knowledge, no study evaluated the 10-year outcomes of rTHA using DM cups, cemented into a metal reinforcement ring, in cases of severe acetabular bone loss. We hypothesized that using a DM cup cemented into a metal ring is a reliable technique for rTHA at 10 years, with few revisions for acetabular loosening and/or instability.

This is a retrospective study of 77 rTHA cases with severe acetabular bone loss (Paprosky ≥ 2C) treated exclusively with a DM cup (NOVAE STICK; SERF, DÉCINES-CHARPIEU, FRANCE) cemented into a cage (Kerboull cross, Burch-Schneider, or ARM rings). Clinical scores and radiological assessments were performed preoperatively and at the last follow-up. The main endpoints were revision surgery for aseptic loosening or recurring dislocation.

With a mean follow-up of 10.7 years [2.1-16.2], 3 patients were reoperated because of aseptic acetabular loosening (3.9%) at 9.6 years [7-12]. Seven patients (9.45%) dislocated their hip implant, only 1 suffered from chronic instability (1.3%). Cup survivorship was 96.1% at 10 years. No sign of progressive radiolucent lines were found and bone graft integration was satisfactory for 91% of the patients.

The use of a DM cup cemented into a metal ring during rTHA with complex acetabular bone loss was associated with low revision rates for either acetabular loosening or chronic instability at 10 years. That's why we also recommend DM cup for all high risk of dislocation situations.

Total knee replacement (TKA) surgery is an excellent and well-proven procedure for the treatment of end stage arthritis of the knee. Many refinements have taken place over time in an attempt to improve the components, wear qualities of the polyethylene, and the surgical technique to improve accuracy of component positioning, reduce patient pain, improve postoperative range of motion, ultimately improve results and to prolong the time until revision surgery may occur. This study examines the results of a gap balancing surgical technique in which components were implanted that had a posterior cruciate substituting design. This technique is performed with exacting alignment and balancing of the flexion and extension gaps prior to implantation of the knee components. The follow up is at a minimum of ten years. 515 consecutive knee replacements were followed prospectively for a minimum of ten years. The average age at surgery was 70 years, 73% of patients were female, with an average BMI of 31. All patients carried a diagnosis of osteoarthritis and a cemented, posterior stabilized design TKA (Balanced Knee System, Ortho Development) was implanted. All cases were performed by one of two experienced joint replacement surgeons. The surgical technique demanded flexion and extension gap balancing as well as soft tissue balancing prior to finishing cuts being performed on the femoral side (See figures 1 and 2). Polyethylene spacers come in 1 millimeter increments. 28% of patients died postoperatively at an average of 7.4 years. These patients were older on average at the time of index surgery (76.6 years). None had undergone revision surgery. Of the remaining patients Knee Society scores (39 preop to 91 post op at ten years), function scores and range of motion all improved significantly. What's more, these results were not diminished at ten years. There were no component failures and less than 1% radiographic progressive lucent lines. Eleven revision surgeries (2.1 %) were performed with 2 acute superficial wound revisions, 3 late infections, one patellar tendon disruption from a fall at 7 years (BMI 45.7), 2 complete revisions performed elsewhere for unsatisfactory results, and 3 spacer exchanges for perception of postoperative laxity. For the current study we also examined subgroups of the morbidly obese, octogenarians, and those with a preoperative valgus deformity of greater than 15%. At follow-up these subgroups fared very well with the exception of the heaviest BMI's being limited in range of motion because of soft tissue impingement. Results suggest that this balancing technique gives excellent results with few complications at ten year evaluation. We believe that careful attention to bony and soft tissue balancing and equalization of gaps in flexion and in extension will prove beneficial for TKA longevity in even longer-term evaluation. Different therapeutic options exist, but their management is still poorly standardized because of the lack of data from randomized trials. The aim of this retrospective study is to assess the infection eradication success rate, over the last ten years, using different patient adapted treatment options. Method. Patients having a prosthetic-joint infection at Lausanne University Hospital (Switzerland) between 2006 and 2016 were included. The success rate depending on age, type of prosthesis, type of infection and type of surgical procedure was analyzed. Results. 444 patients (61% hips, 37% knees) were identified with a median age of 70 years. The success rate was 93% for two-stage exchange, 78% for one-stage exchange and 75% for debridement with retention of the prosthesis. The failure rate was higher for knee prosthetic-joint infections (27%) than hip infections (13%). Furthermore, chronic and in elderly prosthetic-joint infections seemed to have a worse prognosis. Conclusions. The infection eradication depends on age, type of prosthesis, type of infection and type of surgical procedure, with three times less failure in two-stage exchange surgery

Anterior cruciate ligament (ACL) reconstruction is a commonly performed operation. A variety of graft options are used with the most popular being bone-patellar-tendon-bone and hamstring autograft. There has been an increase in the popularity of hamstring autograft over the past decade. The aim of the study was to assess the ten year subjective knee function and activity level following four-strand semitendinosis and gracilis (STG) anterior cruciate ligament reconstruction. 86 patients underwent anterior cruciate reconstruction by two knee surgeons over a 12 month period (January 1999 to December 1999). 80 patients meet the inclusion criteria of arthroscopic ACL reconstruction. The same surgical technique was used by both surgeons involving four-strand STG autograft, single femoral and tibial tunnels and aperture graft fixation with the Round headed Cannulated Interference (RCI) screw. Patient evaluation was by completion of a Lysholm Knee Score and Tegner Activity Level Scale at a minimum of ten years from reconstructive surgery. This was by initial postal questionnaire and subsequent telephone follow-up. 80 patients underwent anterior cruciate reconstruction with average age 30.9 years (15 to 58 years). There was a 77.5% (62 patients) response at ten years to the questionnaire. The median Lysholm Knee Score at ten years was 94 (52 to 100). The median activity level had decreased from 9 to 5 at ten years according to the Tegner Activity Scale. 73% of patients reported a good or excellent outcome on the Lysholm score. The group of patients was further divided into those that required meniscal surgery and those that did not. The patients that did not require meniscal surgery had a median Lysholm score of 94 and those that required meniscal surgery had a similar median Lysholm score of 92.5. However it was noted that 8 patients required medial and lateral partial menisectomies at the time of original reconstruction. This group of patients had a median Lysholm Knee Score of 83.5 and Tegner Activity Scale of 4 at ten years following reconstruction. 17 of the 62 patients (27.4%) required re-operation because of further knee symptoms, with 4 patients requiring revision of the anterior cruciate following re-rupture. In conclusion anterior cruciate ligament reconstruction with four-strand STG hamstring autograft provides a reliable method of restoring knee function, with a 6% revision rate for re-rupture at ten years. Combined partial medial and lateral menisectomy at the time of the initial reconstruction is a poor prognostic indicator of function at ten years

Introduction. Despite being from different manufacturers, Exeter stem and Ogee cup are commonly used together as cemented ‘cross breed’ combination in United Kingdom. The purpose of this study was to evaluate the long-term outcome of this combination. Materials and methods. The ten years outcome of 131 primary hip replacements using an Exeter stem and an Ogee cup combination were studied retrospectively from clinical audit data and radiographs. Results. The Harris Hip score (HHS) improved significantly from 46 pre-surgery to 88 post-surgery. This had a gradual decrease to 81 over ten years. This effect was associated with a decrease in function score but a sustained pain score. Radiologically, five cups were loose and two stems had failed. One stem had migrated and another had progressive endosteolysis. There were two recurrent dislocations, of which one was revised to a bipolar socket. The ten year survival of the combination implant was 99% (CI 97 to 100) with revision as an end point. The same with impending failure as end point was 93% (CI 87 to 99). Conclusion. It might be preferable to have a multi-centric prospective study to demonstrate the performance of this commonly done combination implant at various settings. In the meantime it is concluded that the Exeter stem-Ogee cup combination arthroplasty has acceptable outcomes at ten years

The robotic-assisted system (ROBODOC) is the first active robot that was designed to reduce potential human errors in performing cementless total hip arthroplasty (THA). We have reported minimum five years follow-up clinical results. However, to our knowledge, there have been no longer follow-up reports. The purpose of this study was to prospectively compare the minimum ten years follow-up results of robotic-assisted and hand-rasping stem implantation techniques. Between 2000 and 2002, we performed 146 THA on 130 patients who were undergoing primary THA. Robot assisted primary THA was performed on 75 hips and a hand-rasping technique was used on 71 hips. Among them, 112 hips (53 hips in the robotic milling group and 59 hips in the hand-rasping group) were followed more than 10 years. Follow-up periods ranged from 120–152 months (average 135). Preoperatively, we plan the position and the size of the stem three-dimensionally for both groups. At the operation, posterolateral approach was used. We evaluated survivorship and compared clinical results. At the final follow-up, no stem was revised in either group. Plain radiographs showed bone ingrowth fixation for all the stems of both groups. There were no signs of mechanical loosening in any implant. Preoperatively, there were no significant differences in the Japanese Orthopedic Association (JOA) hip scores between the two groups. Ten years postoperatively, it was significantly better in the robotic milling group (98 points and 96 points, respectively) (Mann-Whitney U-test; p<0.05). The main difference was observed in the category of range of motion (19 points and 18 points, respectively) (p=0.01). In the previous study, we have reported that the JOA hip score was significantly better in the robotic milling group up to three years postoperatively. In the present study, we found that it was still significantly better at ten years postoperatively. In conclusion, robotic milling THA was associated with better clinical scores until ten years postoperatively

This study aims to directly compare the three widely used bearing surfaces: metal-on-highly crosslinked polyethylene (MoHXLPE), ceramic-on-ceramic (CoC) and metal-on-metal (MoM), regarding clinical and radiologic outcome parameters. METHODS. From November 1999 to November 2001, 300 primary THAs were performed using the uncemented Alloclassic Variall cup and stem (Zimmer Inc., Warsaw, Indiana). The patients were divided into three groups according to the bearing couple implanted, with 100 persons in each group (MoHXLPE, CoC, MoM). Radiographic and clinical data was collected preoperative and at the last follow-up. RESULTS. There were 71 patients (74hips) in the MoHXLPE group, 63 patients (67hips) in the CoC group and 67 patients (69hips) in the MoM group available at the time of the last follow-up. Average follow-up was 10.4 years (MoHXLPE: 10.4, CoC: 10.5, MoM: 10.3). In the MoHXLPE and the CoC group 3 hips each had to undergo revision surgery. In the MoM group 4 hips were revised. Overall survival with revision for any reason as the end point was 95.9% for MoHXLPE, 95.5% for CoC and 94.2% for MoM. The mean Harris Hip Score (HHS) for patients who were not revised was 92.5 points, 92.3 points and 94.3 points at the last follow-up, for the MoHXLPE, CoC and MoM group respectively. Radiolucent lines around the stem in Gruen zones one and/or seven occurred in 17% to 27% of THAs in the three groups without clinical relevance. CONCLUSIONS. This study shows excellent results of all three bearing couples after ten years of follow-up. Our findings support the choice of ceramic-on-ceramic or metal-on-highly crosslinked polyethylene over metal-on-metal as bearing couple in active patients receiving a total hip replacement. Contrary to other studies we could not find advantages of CoC over MoHXLPE at mid-term follow-up

Introduction. Acetabular fractures are a challenging problem. It has been published that outcome is dependent upon the type of fracture, the reduction of the fracture and concomitant injuries. The end-points of poor outcome include avascular necrosis of the femoral head, osteoarthritis. However, we lack definitive statistics and so counselling patients on prognosis could be improved. In order to achieve this, more outcome studies from tertiary referral centres are required. We present the first long term follow up from a large tertiary referral Centre in Ireland. Methods. We identified all patients who were ten years following open reduction and internal fixation of an acetbular fracture in our centre. We invited all of these patients to attend the hospital for clinical and radiographic follow-up. As part of this, three scoring systems were completed for each patient; the Short-form 36 health survey (SF36), the Merle d'Aubigné score and the Short Musculoskeletal Functional Assessment (SMFA). Results. The data represents one years activity at a new tertiary referral unit. We idenfied a total of 44 patients who were ten years following ORIF of acetabular fractures in our unit. 21 patients (48%) replied to written invitation and attended the hospital for clinical and radiographic follow-up. A further 7 patients were contacted by telephone and interviewed to guage their rehabilitation. 3 patients had passed away. The remaining 13 patients were not contactable. Of those who attended in person for follow-up; 18 were male and 3 were female. The mean age at follow-up was 40.5 years (Range 27-60). In terms of fracture pattern epidemiology, 43% of patients sustained posterior column and wall fractures, 29% posterior wall, 14% posterior column alone, 9.5% transverse with posterior wall and 9.5% bicolumnar. 2 patients in the follow-up group had total hip replacements. Of the remaining patients the overall mean SF36 score was 78.8% (SD 16.4). The mean SMFA was 14.1% (SD 5). The mean Merle d'Aubigné score was 14.9 (SD 3.2) with 63% graded as good or excellent. Comparison of outcome between sub-groups according to fracture clasification showed no significant difference. Traumatic sciatic nerve injury was sustained by four patients in the follow-up group and all patients continued to complain of ongoing weakness. Of the patients who were contacted via telephone, 2 had total hip replacements. The remaining 5 reported no significant problems with their hips and cited this as the reason for not attending follow-up. Conclusion. Overall the outcome of the patients was more favourable than expected. This was supported by the results of the clinical scoring systems. In some patients this also appeared to be despite poor radiographic findings. Our observations suggest that concomitant injuries, especially sciatic nerve injury have a profound negative influence on the patients' ability to fully rehabilitate. These data provide a valuable tool for the trauma surgeon in providing the patient with an educated prognosis

Fully constrained liners are used to treat recurrent dislocations or patients at high risk after total hip replacements. However, they can cause significant morbidities including recurrent dislocations, infections, aseptic loosening and fractures. We examine long term results of 111 patients with tripolar constrained components to assess their redislocation and failure rate.

The purpose of this study was to assess survivorship, complications and functional outcomes at a minimum 10 years after the constrained tripolar liners used in our institute.

We retrospectively identified 111 patients who had 113 revision tripolar constrained liners between 1998 and 2008. Eighty-nine were revised due to recurrent dislocations, 11 for pseudotumor with dysfunctional abductors, and 13 for periprosthetic infection with loss of soft tissue stabilizers. All patients had revision hip arthroplasty before the constrained liner was used: 13 after the first revision, 17 after the second, 38 after the third, and 45 had more than 3 revisions. We extracted demographics, implant data, rate of dislocations and incidence of other complications. Kaplan Meier curves were used to assess dislocation and failure for any reason. WOMAC was used to assess quality of life.

At 10 years, the survival free of dislocation was 95.6% (95%CI 90- 98), and at 20 years to 90.6% (95% CI 81- 95.5). Eight patients (7.1%) had dislocations of their constrained liners: 1 patient had simultaneous periprosthetic infection identified at the time of open reduction, and 1 patient sustained stem fracture 3 months prior to the liner dislocation. At 10 years, the survival to any further surgery was 89.4% (95% CI 82–93.8), and at 20 years, this was 82.5 (95% CI 71.9–89.3). Five patients (4.4%) had deep infection: 4 of these had excision arthroplasty due to failure to control infection, while 1 patient was treated successfully with debridement, exchange of mobile components and intravenous antibiotics. Two patients (1.8%) had dissociated rings that required change of liner, ring and head. Two patients (1.8%) had periprosthetic femoral fractures that were treated by revision stems and exchange of constrained liners. The mean WOMAC functional and pain scores were 66.2 and 75.9 of 100, respectively.

Constrained tripolar liners in our institute provided favourable results in the long term for recurrent dislocation hip arthroplasty with dysfunctional hip stabilizers. Infection in these patients can prove to be difficult to treat due to their poor soft tissue conditions from repeated surgeries. Comparing long terms results from other types of constrained liners is essential to evaluate these salvage liners.

Introduction

Long term results of Total Hip Arthroplasty (THA) are affected by wear of articulation. Ceramic on ceramic articulation have been used especially for young patients because of its low wear and bio-inert property. However, because of its hardness, it is concerned that ceramic fracture, chipping, or squeaking might happen with ceramic on ceramic articulation.

Objective

The purpose of this study was to investigate over 10-years clinical and radiographic results of ceramic bearing cementless THA.

Introduction. The correction of paediatric scoliosis is a surgical procedure in which substantial bleeding occurs, and which historically has been associated with high levels of blood transfusion. Transfusions are associated with risks of infection, allergic reactions, volume overload and immunosuppression. The purpose of our study was to evaluate the efficacy of a care pathway implemented at St Thomas' hospital over ten years with a focus on minimising the need for blood transfusions. A Spinal Surgery Care Pathway was developed including: nurse-led clinics facilitating pre-operative haemoglobin optimisation; intra operative cell-salvage, the use of tranexamic acid, and a transfusion criteria awareness programme. Methods. We reviewed the records of all patients 18 years of age or younger undergoing spinal corrective surgery between 2000 and 2010. Collating data from the surgical records and haematology laboratory, we identified the proportion of patients requiring transfusion and the respective volume of blood or blood products transfused. Results. 534 spinal procedures were undertaken on patients over the decade of interest. Over this time, the proportion of patients requiring transfusion reduced from 83.3% in 2000, to 6.4% in 2010 (p < 0.001); a consistent year on year reduction was witnessed between 2005 and 2010. In those requiring transfusion, the volume of packed red cells transfused fell from a mean of 7.7 units in 2000, to 1.5 units in 2010 (p < 0.001). These results were despite an increase in posterior fusions, associated with higher bleeding, from 50.0% to 79.4% over the same time period. Conclusion. Over ten years, we have demonstrated that the implementation of a multifaceted, multidisciplinary pathway together with challenging cultural attitudes to blood transfusion, can dramatically and sustainably reduce the need for blood transfusion and the risks associated therewith. These data lends weight to the adoption of such a care pathway in paediatric scoliosis surgery

INTRODUCTION. Despite a large percentage of total knee arthroplasty failures occurs for disorders at the patello-femoral joint (PFJ), current navigation systems report tibio-femoral (TFJ) kinematics only, and do not track the patella. Despite this tracking is made difficult by the small bone and by its full eversion during surgery, a new such technique has been developed, which includes a new tracker, new corresponding surgical instrumentation also for patellar resurfacing, and all relevant software. The aim of this study is to report an early experience in patients of these measurements, i.e. TFJ and PFJ kinematics. METHODS. These measurements were taken in the first ten patients, affected by primary gonarthrosis and implanted with a resurfacing posterior-stabilised prosthesis in the period July 2010 – May 2011. A standard knee navigation system was enhanced by a specially-designed patellar tracker, mounted with a cluster of three light emitting diodes. Standard procedures for femoral and tibial bone preparation were performed according to the navigation system, and the patellar was resurfaced. Relevant resection planes were taken by an instrumented verification probe. Final position of the three components and lower limb alignment were also acquired. Joint kinematics was deduced from the anatomical survey, which included anatomical landmarks on the patellar posterior aspect, and according to established recommendations and original proposals. RESULTS. In addition to the standard assessment of TFJ kinematics, patellar tracking was performed successfully in all cases without complications, resulting in a maximum of 30 min longer operations. PFJ kinematics (see figure) after replacement and resurfacing showed a mean (± standard deviation, over the patients) range of flexion, tilt and medio-lateral shift respectively of 66.9° ± 8.5° (mean of minimum flexion ÷ of maximum flexion, 15.6° ÷ 82.5°), 8.0° ± 3.1° (−5.3° ÷ 2.8°), and 5.3 ± 2.0 mm (−5.5 ÷ 0.2 mm). Statistically significant correlations were found between the internal/external rotation of the femoral component and the range of PFJ tilt (p=0.05; R=0.64); in three patients, medio-lateral PFJ shift seemed to be affected by the medio-lateral position of the femoral component. DISCUSSION AND CONCLUSIONS. Data obtained from our preliminary experience support the relevance, feasibility and efficacy of patellar tracking in navigated knee arthroplasty by means of a standard knee navigation system, suitably extended to track also the patellar motion. Patellar-based measurement provides for a more comprehensive assessment of the whole knee function, not only for the resurfacing but also for a best possible positioning of the femoral and tibial components


Background. Autologous chondrocyte implantation (ACI) and mosaicplasty (MP) are two methods of repair of symptomatic articular cartilage defects in the adult knee. This study represents the only long-term comparative clinical trial of the two methods. Methods. A prospective, randomised comparison of the two modalities involving 100 patients with symptomatic articular cartilage lesions was undertaken. Patients were followed for ten years. Pain and function were assessed using the modified Cincinnati score, Bentley Stanmore Functional rating system and visual analogue scores. ‘Failure’ was determined by pain, a poor outcome score and arthroscopic evidence of graft disintegration. Results. Patients had a mean age at index operation of 31. There was a long mean pre-op duration of symptoms of seven years and the defects had an average of 1.5 operations (excluding arthroscopy) to the articular cartilage lesion prior to the cartilage repair surgery. The aetiology of the articular cartilage defects was mainly trauma; some patients had osteochondritis dissecans or chondromalacia patellae. Five patients were lost to follow-up. A total of 23 out of 42 mosaicplasty patients failed, 10 out of 58 ACI patients failed (p<0.001). Most patients did well for the first two years when there was a steep failure of mosaicplasty patients, after which the failure rate was more constant. There was a low steady failure rate of ACI over the 10 years. Older patients treated by ACI did worse than younger patients; age was less of a prognostic indicator in MP. Patients irrespective of gender or aetiology of the defect fared better with ACI than MP. Conclusion. At ten years, patients who underwent cartilage repair using ACI fared significantly better than those who underwent mosaicplasty
