Aims

Mendelian randomization (MR) is considered to overcome the bias of observational studies, but there is no current meta-analysis of MR studies on rheumatoid arthritis (RA). The purpose of this study was to summarize the relationship between potential pathogenic factors and RA risk based on existing MR studies.

Methods

PubMed, Web of Science, and Embase were searched for MR studies on influencing factors in relation to RA up to October 2022. Meta-analyses of MR studies assessing correlations between various potential pathogenic factors and RA were conducted. Random-effect and fixed-effect models were used to synthesize the odds ratios of various pathogenic factors and RA. The quality of the study was assessed using the Strengthening the Reporting of Observational Studies in Epidemiology using Mendelian Randomization (STROBE-MR) guidelines.

Aims

To explore the synovial expression of mucin 1 (MUC1) and its role in rheumatoid arthritis (RA), as well as the possible downstream mechanisms.

Methods

Patients with qualified synovium samples were recruited from a RA cohort. Synovium from patients diagnosed as non-inflammatory orthopaedic arthropathies was obtained as control. The expression and localization of MUC1 in synovium and fibroblast-like synoviocytes were assessed by immunohistochemistry and immunofluorescence. Small interfering RNA and MUC1 inhibitor GO-203 were adopted for inhibition of MUC1. Lysophosphatidic acid (LPA) was used as an activator of Rho-associated pathway. Expression of inflammatory cytokines, cell migration, and invasion were evaluated using quantitative real-time polymerase chain reaction (PCR) and Transwell chamber assay.

Introduction

Highly crosslinked ultrahigh-molecular-weight polyethylene (XLPE) reduces wear and osteolysis in total hip arthroplasty, but it is unclear if XLPE will provide the same clinical benefit in total knee arthroplasty (TKA). Adhesive and abrasive wear generally dominate in polyethylene acetabular components, whereas fatigue wear is an important wear mechanism in polyethylene TKA tibial inserts. The wear resistance of XLPE depends on the crosslink density of the material, which may decrease during in vivo mechanical loading, leading to more wear and increased oxidation. To examine this possibility, we measured crosslink density and oxidation levels in loaded and unloaded locations of retrieved tibial inserts to evaluate the short-term performance of XLPE material in TKA.

Materials and Methods

Forty retrieved XLPE tibial inserts (23 remelted, 17 annealed) retrieved after a mean time of 18 ± 14 months were visibly inspected to identify loaded (burnished) and unloaded (unburnished) locations on the plateaus of each insert using a previously published damage mapping method. For each insert, four cubes (3 mm3) were cut from loaded and unloaded surface and subsurface locations (Fig. 1). Swell ratio testing was done according to ASTM F2214 to calculate crosslink density of the cubes. With a microtome, 200 μm sections were taken adjacent to the cubes and oxidation was assessed with Fourier transform infrared spectroscopy following ASTM F2102 (Fig. 2). Surface oxidation was measured in the sections adjacent the surface cubes and subsurface oxidation was measured in sections adjacent to the subsurface cubes. The effects of location (surface vs. subsurface in the loaded and unloaded regions) and thermal treatment (annealed vs. remelted) on crosslink density and oxidation were assessed with repeated measures generalized estimating equations (GEEs), with the implant treated as the repeated factor. Results are presented as means and 95% confidence intervals and the level of significance was α=0.05.

The aim of this study was to assess a specific protocol for the treatment of patients with a parosteal osteosarcoma of the distal femur with limb salvage involving hemicortical resection and reconstruction using recycled pasteurised autograft and internal fixation. Between January 2000 and January 2010, 13 patients with a mean age of 26.5 years (17 to 39) underwent this procedure. All the tumours were staged according to Enneking’s criteria: there were eight stage IA tumours and five stage IB tumours. The mean follow-up was 101.6 months (58 to 142), and mean post-operative Musculoskeletal Tumour Society functional score was 88.6% (80% to 100%) at the final follow-up. All the patients had achieved bony union; the mean time to union was 11.2 months (6 to 18). Local recurrence occurred in one patient 27 months post-operatively. No patient had a pulmonary metastasis.

A hemicortical procedure for the treatment of a parosteal osteosarcoma is safe and effective. Precise pre-operative planning using MRI is essential in order to define the margins of resection. Although it is a technically demanding procedure, gratifying results make it worthwhile for selected patients.

Aim: To investigate whether there is any difference in regional brain volumes between AIS patients and age matched, sex matched control subjects.

Method and Materials: 20 adolescent idiopathic scoliosis (AIS) female patients (age ranged from 11 yrs to 18 yrs, mean age of 14.5 yrs) and 20 sex matched, age matched controls have undergone MRI brain examination performed with a 1.5T scanner (Sonata, Siemens Medical Solutions, Erlanger, Germany). A Magnetization Preparation Rapid Acquisition Gradient Echo (MPR) sequence was used. Volumes of neuroanatomical regions were quantitated automatically by using whole brain segmentation technique of atlas-based hybrid warping. The whole brain was classified into 100 fine anatomical regions.

The results were taken as significant when p value was less than 0.05.

Results: Significant unilateral regional differences were found in the following regions:

Left thalamus and left postcentral gyrus of AIS patients were significantly larger than the control subjects. Anterior and posterior limb of right internal capsule, right caudate nucleus, right cuneus and left middle occipital gyurs of AIS patients were significantly smaller than the control subjects. Some regions were bilaterally involved: Perirhinal and hippocampus regions were larger in AIS while inferior occipital gyrus and precuneus were smaller than the corresponding regions in the control subjects. In the midline, the volumes of corpus callosum and brainstem in AIS patients were significantly larger than the control subjects.

Conclusion: Our study found that significant differences in particular regional brain volumes exist between AIS patients and the controls. Most of these regions involved the brain unilaterally, indicating that there might be abnormal asymmetrical development of the brain in AIS. This is the first study of its kind to show the presence of L-R asymmetry of regional brain volume difference in AIS patients as compared to normal controls. The findings might also help to explain the reported poor performance in the combined visual and proprioceptive test, spatial orientation test, abnormal nystagmus response to calorie test, and impaired postural balance in AIS patients.

Nonunion of the humerus with bone loss and shortening due to osteomyelitis is rare but difficult to treat. We describe our experience with a callus distraction technique using a monolateral external fixator for the treatment of this condition.

Between October 1994 and January 2004, 11 patients were treated. There were seven males and four females, with a mean age of 14 years (10 to 17). The mean bone loss was 1.9 cm (1 to 2.7) and the mean length discrepancy in the upper limb was 5.6 cm (3.5 to 8.0).

The mean follow-up was for 106 months (54 to 166). The mean external fixation index was 34.8 days/cm (29.8 to 40.5). The mean lengthening was 9.5 cm (5.5 to 13.4). There were seven excellent results, three good and one poor. There were nine excellent functional results and two good.

The treatment of humeral nonunion with bone loss and shortening due to osteomyelitis by callus distraction is a safe and effective means of improving function and cosmesis.

We present a retrospective study of 27 patients treated by callus distraction using a unilateral external fixator of our own design for nonunion with bone loss and shortening of the femur caused by suppurative osteomyelitis. The unilateral external fixator was used either alone or in combination with an intramedullary nail. The mean age of the patients was 13.6 years (8 to 18). The fixator was used alone in 13 patients and with an intramedullary nail in 14. The bone results at a mean follow-up of 88 months (37 to 144) were excellent in 16 patients and good in 11.

The functional results were excellent in 18 patients and good in nine. However, four patients still had draining sinuses at the latest follow-up. A residual deformity greater than 7° was present in seven femora, but this did not adversely affect function or require further treatment.
