The purpose of this study was to investigate the effectiveness of casting in achieving acceptable radiological parameters for unstable ankle injuries.

This retrospective observational cohort study was conducted involving the retrieval of X-rays of all ankles taken over a 2 year period in an urban setting to investigate the radiological outcomes of cast management for unstable ankle fractures using four acceptable parameters measured on a single X- ray at union. The Picture Archiving and Communication System (PACS) was used, the X-rays were measured by a single observer.

From the 1st of January 2020 to the 31st of December 2021, a total of 1043 ankle fractures were treated at the three hospitals with a male to female ratio of 1:1.7. Of the 628 unstable ankle injuries, 19% of patients were lost to follow up. 190 were managed conservatively with casts, requiring an average of 4 manipulations, with a malunion rate of 23.2%. Unstable ankle injuries that were treated surgically from the outset and those who failed conservative management and subsequently converted to surgery had a malunion rate of 8.1% and 11.0% respectively.

Unstable ankle fractures pose a challenge with a high rate of radiological malunion, regardless of the treatment Casting surgery from the outset or converted to surgery, with rates of 23% and 8% and 11% respectively. In this multivariate analysis we found that conservative management was the only factor influencing the incidence of malunion, age, sex and type of fracture did not have a scientific significant influence.

Aims

The aim of this study was to determine the consensus best practice approach for the investigation and management of children (aged 0 to 15 years) in the UK with musculoskeletal infection (including septic arthritis, osteomyelitis, pyomyositis, tenosynovitis, fasciitis, and discitis). This consensus can then be used to ensure consistent, safe care for children in UK hospitals and those elsewhere with similar healthcare systems.

Methods

A Delphi approach was used to determine consensus in three core aspects of care: 1) assessment, investigation, and diagnosis; 2) treatment; and 3) service, pathways, and networks. A steering group of paediatric orthopaedic surgeons created statements which were then evaluated through a two-round Delphi survey sent to all members of the British Society for Children’s Orthopaedic Surgery (BSCOS). Statements were only included (‘consensus in’) in the final agreed consensus if at least 75% of respondents scored the statement as critical for inclusion. Statements were discarded (‘consensus out’) if at least 75% of respondents scored them as not important for inclusion. Reporting these results followed the Appraisal Guidelines for Research and Evaluation.

Rapid prototyping (RP), especially useful in surgical specialities involving critical three-dimensional relationships, has recently become cheaper to access both in terms of file processing and commercially available printing resources.

One potential problem has been the accuracy of models generated. We performed computed tomography on a cadaveric human patella followed by data conversion using open source software through to selective-laser-sintering of a polyamide model, to allow comparative morphometric measurements (bone v. model) using vernier calipers. Statistical testing was with Student's t-test.

No significant differences in the dimensional measurements could be demonstrated. These data provide us with optimism as to the accuracy of the technology, and the feasibility of using RP cheaply to generate appropriate models for operative rehearsal of intricate orthopaedic procedures.

Locking plate fixation in proximal humeral fractures has demonstrated good results tempered by a significant rate of loss of fixation. Reported rates of failure are typically around 10% of cases but can be 20% or higher. In addition large series are often made up of a diverse patient population, so we have chosen to focus solely on patients confirmed to have significantly reduced BMD who can be considered a subset at high risk of fixation failure.

Twenty-three patients (5 male, 18 female) with a proximal humeral fracture treated by locking plate fixation were confirmed on DEXA scanning to be osteopaenic (17), osteoporotic (4) or severely osteoporotic (2). Patients early in the series were reviewed retrospectively and recalled for an updated assessment where appropriate, and the remainder were followed prospectively. The average age was 66 years (range 49 to 82). Follow up was for an average of nine months following surgery (range 2 and a half to 28 months). 17 patients underwent surgery for acute injuries and 6 for established surgical neck non-unions.

Seven injuries were 2-part fractures, 12 3-part, 3 were 4-part and one a 2-part surgical neck non-union.1 plate failed due to complete loss of fixation within 2 months in a patient with severe osteoporosis and was treated with removal of metalwork. This was the only injury that failed to unite. Avascular necrosis occurred in three patients with two revised to a hemiarthroplasty. 1 patient had ongoing pain and underwent removal of the plate.

Our series demonstrated that locking plate fixation of proximal humeral fractures is associated with a low rate of fixation failure and satisfactory outcomes in patients with significantly reduced bone mineral density.
