Hinged casts and roller traction were used in two developing countries to treat fractured femora, most of which were due to road traffic accidents or civil violence. This method of treatment, developed by Neufeld, is particularly useful in the Third World because it uses local materials, adapted in a hospital workshop, and circumvents the difficulties and complications of standard traction and of operative treatment. The results are reported from 11 patients treated in Uganda in 1979 and from 110 treated in the Dominican Republic in 1981 and 1982. All but one fracture united without complication or significant shortening after a brief period in hospital. The method was easily taught to hospital staff and is strongly recommended.

1. A method of medullary nailing of fractures of the shaft of the tibia with a straight clover-leaf nail of large calibre is described. The nail is introduced, without exposure of the fracture, through the deep infrapatellar bursa. No external splint is used and the patient is usually allowed to walk as soon as the wound is healed. The technique is essentially that of Küntscher.

2. The results obtained in the first fifty patients so treated are described. The average period of absence from work for those twenty-five whose treatment was satisfactory was eleven weeks. There were no cases of sepsis or non-union; the only difficulties encountered were mechanical ones.

1. Linear fractures of the shaft of the long bones are divided into four basic types: 1) transverse; 2) oblique transverse; 3) spiral; and 4) oblique.

2. The mode of production of these fractures is deduced on the grounds of simple mechanical theory: 1) transverse fractures are a result of angulation; 2) oblique transyerse are the result of angulation with axial loading; 3) spiral fractures are the result of axial twists with or without axial loading; 4) oblique fractures are the result of angulation and axial twisting in the presence of axial loading.
