We report the replacement of 42 hips in 34 adults with untreated congenital dislocation. We used Charnley low friction implants, cementing the cup at the level of the true acetabulum after deepening and enlarging it by our own technique of cotyloplasty. Results were evaluated in 38 hips after a mean of 5.5 years. All the patients showed marked improvement, with no infection and, as yet, no late revision. The technical difficulties of the operation and the complications are discussed

The treatment of acromioclavicular injuries is controversial; few studies document the late results. We have reviewed 53 patients about five years after conservative management, in nine for subluxation and 44 for dislocation. Subjective and objective results were satisfactory in all cases except for one with painful subluxation, who was the only patient to change her occupation because of the injury. At review, joint stability was demonstrated by improvement in position and by the very small increase in the coracoclavicular gap on stress radiographs

Three patients were reviewed seven, eight and fourteen years after delayed open reduction of traumatic posterior dislocation of the hip. The delay between injury and operation varied from twenty-eight to ninety-three days. At the time of review symptoms were minimal or absent, and in all cases the injured hip was clinically normal. Radiological examination showed loss of articular cartilage in one hip and trabecular changes in the bone of all three. On the basis of these three results the traditional pessimism about prognosis in such cases may not be justified

Total hip replacement for adults with unreduced congenital dislocation presents a difficult problem because soft-tissue contractures usually prevent sitting at the normal anatomical level. Extensive soft-tissue division or a high-level acetabulum leads to reduced function and poor fixation of the components. We describe a new technique for hip replacement in such cases. The shortened abductors and flexors are released proximally and excision of the upper third of the ilium allows them to be repaired without tension, while providing bone graft to reconstruct the acetabular roof. We report 12 such replacements in 10 patients with good results and few early complications

We treated 120 children between the ages of 12 and 31 months with 137 developmental dislocations of the hip and reviewed them at a mean follow-up of 14 years. We had used two-directional arthrography of all hips before reduction to evaluate the anterior, superior, and posterior portions of the limbus. Of the 137 hips, 64 had no interposed limbus in the AP view of the arthrogram, but 45 of these had an interposed anterior or posterior portion of the limbus. The hips with good stability and no interposed limbus in either AP or lateral arthrograms had excellent results by closed methods; in the other cases the results were less satisfactory. Our findings suggest that hips suitable for management by closed reduction can be identified by two-directional arthrography. Hips shown to have an interposed limbus are best managed by open reduction

Routine ultrasound evaluation of neonates and young infants for congenital dislocation of the hip has been recommended. We have used the methods of decision analysis to determine whether every neonate should be examined by ultrasound or just those at increased risk. We have also studied the reliability and accuracy of ultrasound in following infants during observation and treatment, using published data. We find that ultrasound is not the preferred strategy for the screening of neonates, and that its role in evaluating high-risk patients depends on the point of view. For an individual, when third parties are bearing the cost, ultrasound is useful. For society as a whole, the routine ultrasound evaluation of the high-risk patient is not advantageous. For follow-up, ultrasound using the methods of Graf has a low reliability and there are no adequate data for methods using dynamic assessment

We report a prospective study of 46 patients with acute complete dislocation of the acromioclavicular joint. They were all treated by suture of the deltoid and trapezius over the clavicle with no repair of the coracoclavicular ligaments, using only temporary fixation with two wires. At operation 43 patients (93.5%) had damage to the trapezius or deltoid or both. The coracoclavicular ligaments were intact in six (13%). Follow-up was from 2 to 7.9 years (mean 5.8), and at the latest review only five patients (10.9%) had redisplacement, due to premature removal of wires for infection in one, to migration of the wires in another and to partial failure of the muscle repair in three. We consider that the deltoid and trapezius attachments are important clinical stabilizers of the clavicle and that their repair, with reinforcement, is a useful addition to any method of surgical treatment

A prospective neonatal screening programme for congenital dislocation of the hip is reported. This covered over 20,000 live births including all the babies born in one area, with a virtually complete follow-up. All babies were examined within the first seven days of life and all hips which were dislocated or unstable after four days (1.6%) were splinted. Only two screened cases were "missed" and subsequently detected at 15 and 18 months. No later cases were discovered, and patients between 10 and 20 years of age had apparently remained asymptomatic. The detailed results show that neonatal screening is effective in detecting and preventing a crippling disorder in otherwise normal people. The examination, though brief, is difficult to perform well; it is considered that failure of the method lies with the examiner and not with the tests

Arthrodesis of the hip joint performed between the ages of twelve and fifteen years, is the most satisfactory treatment for cases of pathological dislocation after acute suppurative arthritis in infants

1. Three cases of anterior dislocation of the head of the radius associated with an undisplaced fracture of the olecranon are described. 2. A combination of pronation of the forearm and hyperextension of the elbow is suggested as a possible mechanism

The late results of early treatment of congenital dislocation of the hip in the von Rosen splint have been recorded in a follow-up study of 111 children with an average age of ten years (range eight to sixteen). The clinical diagnosis had been made within two days of birth and in most cases confirmed by radiological examination. Splintage was in all cases begun within the first few days and maintained for an average period of ten weeks. Two cases of failure of the initial treatment were encountered but had good final results. The follow-up study included a radiological examination in which were recorded the general configuration of the joint, the CE-angle described by Wiberg, the presence or absence of an os acetabuli and the roundness of the femoral head. The findings were compared with those in a control group of 222 normal children. All the treated hips developed normally except for a single case of minor dysplasia and a single case of slight enlargement of the femoral head and neck following appearances suggestive of avascular necrosis at the age of two years

1 . A case of posterior dislocation of the shoulder treated by open reduction and obliteration of the defect in the head of the humerus by implantation of the subscapularis tendon is reported. 2. It is suggested that this method is indicated in cases in which there is a deep V-shaped depression on the anterior aspect of the humeral head

1. Two cases of lateral dislocation of the radio-humeral joint with greenstick fracture of the upper end of the ulna are described. 2. One case was complicated by a radial nerve lesion. 3. Treatment was by reduction under general anaesthesia and resting the elbow in a collar and cuff sling. Full recovery was present in six weeks

We examined an39 patients (45 knees) who had undergone Elmslie-Trillat procedure for recurrent or habitual dislocation of the patella with a follow-up of more than ten years. The mean age at the time of surgery was 18.4 years; the mean follow-up was 161 months (120 to 238). Using Fulkerson’s functional knee score, 41 knees (91%) had an excellent or good result at a mean follow-up of 45 months, and this was maintained in 29 (64%) at the final review. The main cause of deterioration in the clinical results was the onset or worsening of patellofemoral joint pain, not patellar instability

The place and effectiveness of surgery for acromioclavicular dislocation is disputed. We have reviewed 29 patients all treated by an operation which holds the clavicle down to the coracoid process. This was effective in both acute and late cases, with rapid return to work, a low incidence of complications and no requirement for secondary procedures. We consider it to be the method of choice when operation is indicated

The modified Magnuson-Stack procedure of lateral and distal transfer of the insertion of the subscapularis muscle was used in 38 consecutive patients with recurrent anterior dislocation of the shoulder, all of whom have been followed-up for a minimum of three years. There was only one failure; the other 37 patients had an excellent or satisfactory result with an adequate range of movement. Bankart and Hill-Sachs lesions were not a constant finding but subscapularis laxity was detected in almost every case, an observation that helps to confirm its central role as a cause of recurrence

We measured the range of rotation in both hips of 397 normal children and in the unaffected hip of 135 children with unilateral congenital dislocation of the hip. Both groups were assessed for generalised joint laxity. Joint laxity was more common in normal children with an internally centred arc of hip rotation than in normal children with a neutral or an externally rotated arc. The children with congenitally dislocated hips had significantly more joint laxity than did the control group and significantly more of them had an internally centred arc of hip rotation. We suggest that the lax joint capsule fails to mould away the neonatal anteversion of the femoral neck during the first few months of life

The management of traumatic dislocation of the knee in 40 patients (41 knees) with a mean age of 26.3 years is described. They were treated by primary repair and reconstruction with autologous grafting of the anterior (ACL) and posterior cruciate ligaments (PCL) and repair injuries to the collateral ligament and soft-tissue. The ACL and PCL were reconstructed using the patellar tendon and the gracilis and semitendinosus tendons, respectively. Early mobilisation using a continuous-passive-movement machine and active exercises was started on the second day after operation. At a mean follow-up of 39 months no patient reported ‘giving way’ and all except one had good range of movement. Of the 41 knees, 21 were rated as excellent, 15 good, four fair and one poor. Early reconstruction of the cruciate ligaments and primary repair of the collateral ligaments followed by an aggressive rehabilitation programme are recommended for these young, active patients

We reviewed 14 patients (16 hips) treated by open reduction and upper femoral derotation osteotomy for congenital dislocation of the hip. Nine patients with 11 treated hips had growth deformities of the proximal femur; in all of them the top screw of the fixation plate lay within the cartilaginous precursor of the greater trochanter. In the five control hips the top screw was more distal. In the nine patients (mean follow-up 10.8 years) there was an increase of 14 degrees in the neck-shaft angle (p = 0.01) and of 18 degrees in the angle between the capital femoral physis and the shaft (p = 0.01) compared to the control group. This indicates that growth disturbance of the greater trochanteric apophysis as a result of plate fixation leads to long-term deformity

We describe a case of traumatic anterior dislocation of the hip in a 14-year-old boy with associated intrapelvic displacement of the femoral head and ipsilateral fractures of the shaft of the femur and greater trochanter. There was a delay in presentation of eight days. At operation the femoral head was reduced into the acetabulum after enlarging the obturator foramen by performing an osteotomy of the superior pubic ramus
