We performed a retrospective study to determine
the effect of osteoporosis on the functional outcome of osteoporotic
distal radial fractures treated with a volar locking plate. Between
2009 and 2012 a total of 90 postmenopausal women with an unstable
fracture of the distal radius treated with a volar locking plate
were studied. Changes in the radiological parameters of 51 patients
with osteoporosis (group 1, mean age 66.9, mean T-score –3.16 ( We found that osteoporosis does not have a negative effect on
the functional outcome and additional analysis did not show a correlation

Aseptic loosening of the femoral component is
an important indication for revision surgery in unicompartmental knee
replacement (UKR). A new design of femoral component with an additional
peg was introduced for the cemented Oxford UKR to increase its stability.
The purpose of this study was to compare the primary stability of
the two designs of component. Medial Oxford UKR was performed in 12 pairs of human cadaver
knees. In each pair, one knee received the single peg and one received
the twin peg design. Three dimensional micromotion and subsidence
of the component in relation to the bone was measured under cyclical
loading at flexion of 40° and 70° using an optical measuring system.
Wilcoxon matched pairs signed-rank test was performed to detect
differences between the two groups. There was no significant difference in the relative micromotion
(p = 0.791 and 0.380, respectively) and subsidence (p = 0.301 and
0.176, respectively) of the component between the two groups at
both angles of flexion. Both designs of component offered good strength

Epidemiological studies enhance clinical practice
in a number of ways. However, there are many methodological difficulties
that need to be addressed in designing a study aimed at the collection
and analysis of data concerning fractures and other injuries. Most
can be managed and errors minimised if careful attention is given

The outcome at ten years of 100 Freeman hip stems (Finsbury Orthopaedics, Leatherhead, United Kingdom) retaining the neck with a proximal hydroxyapatite coating in a series of 52 men (six bilateral) and 40 women (two bilateral), has been described previously. None required revision for aseptic loosening. We have extended the follow-up to 20 years with a minimum of 17 years. The mean age of the patients at total hip replacement was 58.9 years (19 to 84). Six patients were lost to follow-up, but were included up to their last clinical review. A total of 22 patients (22 hips) had died, all from causes unrelated to their surgery. There have been 43 re-operations for failure of the acetabular component. However, in 38 of these the stem was not revised since it remained stable and there was no associated osteolysis. Two of the revisions were for damage to the trunnion after fracture of a modular ceramic head, and in another two, removal of the femoral component was because of the preference of the surgeon. In all cases the femoral component was well fixed, but could be extracted at the time of acetabular revision. In one case both components were revised for deep infection. There has been one case of aseptic loosening of the stem which occurred at 14 years. This stem had migrated distally by 7.6 mm in ten years and 8.4 mm at the time of revision at which stage it was found to be rotationally loose. With hindsight this component had been undersized at implantation. The survivorship for the stem at 17 years with aseptic loosening as the endpoint was 98.6% (95% confidence interval 95.9 to 100) when 62 hips were at risk. All remaining stems had a satisfactory clinical and radiological outcome. The Freeman proximally hydroxyapatite-coated femoral component is therefore a dependable implant and its continued use can be recommended.
Heterotopic ossification (HO) of the hip after injury to the central nervous system can lead to joint ankylosis. Surgery is usually delayed to avoid recurrence, even if the functional status is affected. We report a consecutive series of patients with HO of the hip after injury to the central nervous system who required surgery in a single, specialised tertiary referral unit. As was usual practice, they all underwent CT to determine the location of the HO and to evaluate the density of the femoral head and articular surface. The outcome of surgery was correlated with the pre-, peri- and post-operative findings. In all, 183 hips (143 patients) were included of which 70 were ankylosed. A total of 25 peri-operative fractures of the femoral neck occurred, all of which arose in patients with ankylosed hips and were associated with intra-articular lesions in 18 and severe osteopenia of the femoral head in seven. All the intra-articular lesions were predicted by CT and strongly associated with post-operative complications. The loss of the range of movement before ankylosis is a more important factor than the maturity of the HO in deciding the timing of surgery. Early surgical intervention minimises the development of intra-articular pathology, osteoporosis and the resultant complications without increasing the risk of recurrence of HO.
The cortical strains on the femoral neck and proximal femur were measured before and after implantation of a resurfacing femoral component in 13 femurs from human cadavers. These were loaded into a hip simulator for single-leg stance and stair-climbing. After resurfacing, the mean tensile strain increased by 15% (95% confidence interval (CI) 6 to 24, p = 0.003) on the lateral femoral neck and the mean compressive strain increased by 11% (95% CI 5 to 17, p = 0.002) on the medial femoral neck during stimulation of single-leg stance. On the proximal femur the deformation pattern remained similar to that of the unoperated femurs. The small increase of strains in the neck area alone would probably not be sufficient to cause fracture of the neck However, with patient-related and surgical factors these strain changes may contribute to the risk of early periprosthetic fracture.

Electromagnetic fields (EMF) are widely used in musculoskeletal
disorders. There are indications that EMF might also be effective
in the treatment of osteoporosis. To justify clinical follow-up
experiments, we examined the effects of EMF on bone micro-architectural
changes in osteoporotic and healthy rats. Moreover, we tested the
effects of EMF on fracture healing. EMF (20 Gauss) was examined in rats (aged 20 weeks), which underwent
an ovariectomy (OVX; n = 8) or sham-ovariectomy (sham-OVX; n = 8).
As a putative positive control, all rats received bilateral fibular
osteotomies to examine the effects on fracture healing. Treatment
was applied to one proximal lower leg (three hours a day, five days
a week); the lower leg was not treated and served as a control.
Bone architectural changes of the proximal tibia and bone formation
around the osteotomy were evaluated using Objectives
Methods
Unstable bicondylar tibial plateau fractures
are rare and there is little guidance in the literature as to the
best form of treatment. We examined the short- to medium-term outcome
of this injury in a consecutive series of patients presenting to
two trauma centres. Between December 2005 and May 2010, a total
of 55 fractures in 54 patients were treated by fixation, 34 with
peri-articular locking plates and 21 with limited access direct
internal fixation in combination with circular external fixation
using a Taylor Spatial Frame (TSF). At a minimum of one year post-operatively,
patient-reported outcome measures including the WOMAC index and
SF-36 scores showed functional deficits, although there was no significant
difference between the two forms of treatment. Despite low outcome scores,
patients were generally satisfied with the outcome. We achieved
good clinical and radiological outcomes, with low rates of complication.
In total, only three patients (5%) had collapse of the joint of
>
4 mm, and metaphysis to diaphysis angulation of greater than 5º,
and five patients (9%) with displacement of >
4 mm. All patients
in our study went on to achieve full union. This study highlights the serious nature of this injury and generally
poor patient-reported outcome measures following surgery, despite
treatment by experienced surgeons using modern surgical techniques.
Our findings suggest that treatment of complex bicondylar tibial
plateau fractures with either a locking plate or a TSF gives similar
clinical and radiological outcomes. Cite this article:
We carried out a retrospective review over ten months of patients who had presented with a low-energy subtrochanteric fracture. We identified 13 women of whom nine were on long-term alendronate therapy and four were not. The patients treated with alendronate were younger, with a mean age of 66.9 years (55 to 82) Our study suggests that prolonged suppression of bone remodelling with alendronate may be associated with a new form of insufficiency fracture of the femur. We believe that this finding is important and indicates the need for caution in the long-term use of alendronate in the treatment of osteoporosis.
A comprehensive review of the literature relating to the pathology and management of the diabetic foot is presented. This should provide a guide for the treatment of ulcers, Charcot neuro-arthropathy and fractures involving the foot and ankle in diabetic patients.
The augmentation of fixation with bone cement
is increasingly being used in the treatment of severe osteoporotic fractures.
We investigated the influence of bone quality on the mechanics of
augmentation of plate fixation in a distal femoral fracture model
(AO 33 A3 type). Eight osteoporotic and eight non-osteoporotic femoral
models were randomly assigned to either an augmented or a non-augmented
group. Fixation was performed using a locking compression plate.
In the augmented group additionally 1 ml of bone cement was injected
into the screw hole before insertion of the screw. Biomechanical
testing was performed in axial sinusoidal loading. Augmentation significantly
reduced the cut-out distance in the osteoporotic models by about

Antegrade nailing of proximal humeral fractures
using a straight nail can damage the bony insertion of the supraspinatus
tendon and may lead to varus failure of the construct. In order
to establish the ideal anatomical landmarks for insertion of the
nail and their clinical relevance we analysed CT scans of bilateral
proximal humeri in 200 patients (mean age 45.1 years ( We therefore emphasise the need for ‘fastidious’ pre-operative

Highly active anti-retroviral therapy has transformed HIV into a chronic disease with a long-term asymptomatic phase. As a result, emphasis is shifting to other effects of the virus, aside from immunosuppression and mortality. We have reviewed the current evidence for an association between HIV infection and poor fracture healing. The increased prevalence of osteoporosis and fragility fractures in HIV patients is well recognised. The suggestion that this may be purely as a result of highly active anti-retroviral therapy has been largely rejected. Apart from directly impeding cellular function in bone remodelling, HIV infection is known to cause derangement in the levels of those cytokines involved in fracture healing (particularly tumour necrosis factor-α) and appears to impair the blood supply of bone. Many other factors complicate this issue, including a reduced body mass index, suboptimal nutrition, the effects of anti-retroviral drugs and the avoidance of operative intervention because of high rates of wound infection. However, there are sound molecular and biochemical hypotheses for a direct relationship between HIV infection and impaired fracture healing, and the rewards for further knowledge in this area are extensive in terms of optimised fracture management, reduced patient morbidity and educated resource allocation. Further investigation in this area is overdue.
We retrospectively evaluated the clinical and
radiological outcomes of a consecutive cohort of patients aged >
70 years with a displaced fracture of the olecranon, which was treated
non-operatively with early mobilisation. We identified 28 such patients
(27 women) with a mean age of 82 years (71 to 91). The elbow was
initially immobilised in an above elbow cast in 90° of flexion of
the elbow for a mean of five days. The cast was then replaced by
a sling. Active mobilisation was encouraged as tolerated. No formal
rehabilitation was undertaken. At a mean follow-up of 16 months
(12 to 26), the mean ranges of flexion and extension were 140° and
15° respectively. On a visual analogue scale of 1 (no pain) to 10,
the mean pain score was 1 (0 to 8). Of the original 28 patients
22 developed nonunion, but no patients required surgical treatment. We conclude that non-operative functional treatment of displaced
olecranon fractures in the elderly gives good results and a high

The relationship between post-operative bone
density and subsequent failure of total knee replacement (TKR) is
not known. This retrospective study aimed to determine the relationship
between bone density and failure, both overall and according to
failure mechanism. All 54 aseptic failures occurring in 50 patients
from 7760 consecutive primary cemented TKRs between 1983 and 2004
were matched with non-failing TKRs, and 47 failures in 44 patients
involved tibial failures with the matching characteristics of age
(65.1 for failed and 69.8 for non-failed), gender (70.2% female), diagnosis
(93.6% OA), date of operation, bilaterality, pre-operative alignment
(0.4 and 0.3 respectively), and body mass index (30.2 and 30.0 respectively).
In each case, the density of bone beneath the tibial component was assessed
at each follow-up interval using standardised, calibrated radiographs.
Failing knees were compared with controls both overall and, as a
subgroup analysis, by failure mechanism. Knees were compared with
controls using univariable linear regression. Significant and continuous elevation in tibial density was found
in knees that eventually failed by medial collapse (p <
0.001)
and progressive radiolucency (p <
0.001) compared with controls,
particularly in the medial region of the tibia. Knees failing due
to ligamentous instability demonstrated an initial decline in density
(p = 0.0152) followed by a non-decreasing density over time (p =
0.034 for equivalence). Non-failing knees reported a decline in
density similar to that reported previously using dual-energy x-ray
absorptiometry (DEXA). Differences between failing and non-failing
knees were observable as early as two months following surgery.
This tool may be used to identify patients at risk of failure following

We analysed the effects of commonly used medications
on human osteoblastic cell activity in vitro, specifically proliferation
and tissue mineralisation. A list of medications was retrieved from
the records of patients aged >
65 years filed in the database of
the largest health maintenance organisation in our country (>
two
million members). Proliferation and mineralisation assays were performed
on the following drugs: rosuvastatin (statin), metformin (antidiabetic),
metoprolol (β-blocker), citalopram (selective serotonin reuptake
inhibitor [SSRI]), and omeprazole (proton pump inhibitor (PPI)).
All tested drugs significantly stimulated DNA synthesis to varying
degrees, with rosuvastatin 5 µg/ml being the most effective among

Osteoporosis and fragility fractures in men constitute a considerable burden in healthcare. We have reviewed 2035 men aged over 50 years with 2142 fractures to clarify the epidemiology of these injuries and their underlying risk factors. The prevalence of osteoporosis ranged between 17.5% in fractures of the ankle and 57.8% in those of the hip. The main risk factors associated with osteoporosis were smoking (47.4%), alcohol excess (36.2%), body mass index <
21 (12.8%) and a family history of osteoporosis (8.4%). Immobility, smoking, self-reported alcohol excess, a low body mass index, age ≥72 and loss in height were significantly more common among men with fractures of the hip than in those with fractures elsewhere.
