The objectives of this study were to assess the effect of anterior cruciate ligament (ACL) resection on flexion-extension gaps, mediolateral soft tissue laxity, maximum knee extension, and limb alignment during primary total knee arthroplasty (TKA). This prospective study included 140 patients with symptomatic knee osteoarthritis undergoing primary robotic-arm assisted TKA. All operative procedures were performed by a single surgeon using a standard medial parapatellar approach. Optical motion capture technology with fixed femoral and tibial registration pins was used to assess study outcomes pre- and post-ACL resection with knee extension and 90° knee flexion. We present a retrospective study of 25 patients treated by open arthrolysis of the elbow for post-traumatic stiffness. The mean follow-up was for 7.8 years (5 to 10.8). Despite few good-quality studies on the subject, total hip arthroplasty (THA) is increasingly being performed for displaced intracapsular fractures of the neck of femur. We compared outcomes of all patients with displacement of these fractures treated surgically over a ten-year period in one institution. A total of 2,721 patients with intracapsular fractures of the femoral neck treated with either a cemented hemiarthroplasty or a THA at a single centre were retrospectively reviewed. To validate the Sydney Hamstring Origin Rupture Evaluation (SHORE), a hamstring-specific clinical assessment tool to evaluate patient outcomes following surgical treatment. A prospective study of 70 unilateral hamstring surgical repairs, with a mean age of 47.3 years (15 to 73). Patients completed the SHORE preoperatively and at six months post-surgery, and then completed both the SHORE and Perth Hamstring Assessment Tool (PHAT) at three years post-surgery. The SHORE questionnaire was validated through the evaluation of its psychometric properties, including; internal consistency, reproducibility, reliability, sensitivity to change, and ceiling effect. Pain, swelling and inflammation are expected
during the recovery from total knee arthroplasty (TKA) surgery.
The severity of these factors and how a patient copes with them
may determine the ultimate outcome of a TKA. Cryotherapy and compression
are frequently used modalities to mitigate these commonly experienced
sequelae. However, their effect on range of motion, functional testing,
and narcotic consumption has not been well-studied. A prospective, multi-center, randomised trial was conducted to
evaluate the effect of a cryopneumatic device on post-operative
TKA recovery. Patients were randomised to treatment with a cryopneumatic
device or ice with static compression. A total of 280 patients were
enrolled at 11 international sites. Both treatments were initiated
within three hours post-operation and used at least four times per
day for two weeks. The cryopneumatic device was titrated for cooling
and pressure by the patient to their comfort level. Patients were evaluated by physical therapists blinded to the
treatment arm. Range of motion (ROM), knee girth, six minute walk
test (6MWT) and timed up and go test (TUG) were measured pre-operatively,
two- and six-weeks post-operatively. A visual analog pain score
and narcotic consumption was also measured post-operatively. At two weeks post-operatively, both the treatment and control
groups had diminished ROM and function compared to pre-operatively.
Both groups had increased knee girth compared to pre- operatively.
There was no significant difference in ROM, 6MWT, TUG, or knee girth
between the 2 groups. We did find a significantly lower amount of
narcotic consumption (509 mg morphine equivalents) in the treatment
group compared with the control group (680 mg morphine equivalents)
at up to two weeks postop, when the cryopneumatic device was being
used (p <
0.05). Between two and six weeks, there was no difference
in the total amount of narcotics consumed between the two groups.
At six weeks, there was a trend toward a greater distance walked
in the 6MWT in the treatment group (29.4 meters versus 7.9
meters, p = 0.13). There was a significant difference in the satisfaction
scores of patients with their cooling regimen, with greater satisfaction
in the treatment group (p <
0.0001). There was no difference
in ROM, TUG, VAS, or knee girth at six weeks. There was no difference
in adverse events or compliance between the two groups. A cryopneumatic device used after TKA appeared to decrease the
need for narcotic medication from hospital discharge to 2 weeks
post-operatively. There was also a trend toward a greater distance
walked in the 6MWT.
We evaluated 535 consecutive primary cementless total knee replacements (TKR). The mean follow-up was 9.2 years (0.3 to 12.9) and information on implant survival was available for all patients. Patients were divided into two groups: 153 obese patients (BMI ≥ 30) and 382 non-obese (BMI <
30). A case-matched study was performed on the clinical and radiological outcome, comparing 50 knees in each group. We found significantly lower mean improvements in the clinical score (p = 0.044) and lower post-operative total clinical scores in the obese group (p = 0.041). There was no difference in the rate of radiological osteolysis or lucent lines, and no difference in alignment. Log rank test for survival showed no significant differences between the groups (p = 0.167), with a ten-year survival rate of 96.4% (95% confidence interval (CI) 92 to 99) in the obese and 98% (95% CI 96 to 99) in the non-obese. There has been a significant reduction in unicompartmental knee arthroplasty (UKA) procedures recorded in Australia. This follows several national joint registry studies documenting high UKA revision rates when compared to total knee arthroplasty (TKA). With the recent introduction of robotically assisted UKA procedures, it is hoped that outcomes improve. This study examines the cumulative revision rate of UKA procedures implanted with a newly introduced robotic system and compares the results to one of the best performing non-robotically assisted UKA prostheses, as well as all other non-robotically assisted UKA procedures. Data from the Australian Orthopaedic Association National Joint Arthroplasty Registry (AOANJRR) for all UKA procedures performed for osteoarthritis (OA) between 2015 and 2018 were analyzed. We present a series of 16 patients treated between 1993 and 2006 who had a failed total ankle replacement converted to an arthrodesis using bone grafting with internal fixation. We used tricortical autograft from the iliac crest to preserve the height of the ankle, the malleoli and the subtalar joint. A successful arthrodesis was achieved at a mean of three months (1.5 to 4.5) in all patients except one, with rheumatoid arthritis and severe bone loss, who developed a nonunion and required further fixation with an intramedullary nail at one year after surgery, before obtaining satisfactory fusion. We report a new method of ankle arthrodesis which combines an anterior approach with a dowel technique of bone grafting and screw fixation. In 20 ankles of 20 patients, ten with osteoarthritis, eight with rheumatoid arthritis and two others, we achieved 19 solid bony fusions and one painless fibrous ankylosis. There have been few reports in the literature of total elbow arthroplasty extending beyond 10 to 15 years. We reviewed 40 patients (41 elbows) with a mean age of 56 years (19 to 83) who had undergone a Coonrad/Coonrad-Morrey elbow arthroplasty by one surgeon for various diagnoses between 1974 and 1994. Distal radial fractures are the most common fracture sustained by the adult population. Most can be treated using cast immobilization without the need for surgery. The aim of this study was to assess the feasibility of a definitive trial comparing the commonly used fibreglass cast immobilization with an alternative product called Woodcast. Woodcast is a biodegradable casting material with theoretical benefits in terms of patient comfort as well as benefits to the environment. We carried out a prospective randomised study comparing medial epicondylectomy with anterior transposition for the treatment of ulnar neuropathy at the elbow. The mean follow-up period was 4.5 years and we assessed the patients neurologically and orthopaedically. Neither procedure appeared to have a significant effect on elbow function.
