1. Specimens are described in which osteoid was seen in undecalcified bone sections prepared from a number of osteoarthritic femoral heads. 2. It was localised mostly in the pressure segments. 3. The reason for the presence of this osteoid is not well understood and the possibilities of local histochemical changes and/or cellular metaplasia are discussed.
This series, albeit consecutive and unselected, is very small; conclusions must therefore be tentative. The results do however suggest that the dangers of late manipulative reduction properly performed may have been exaggerated. Attempted without undue force it proved a harmless procedure, often successful even in cases of long duration. When it succeeds, the results in slips of unacceptable degree are at least as good as those following the more major surgical procedures, which can thus be avoided. Even when the displacement is minor its correction should on theoretical grounds at least decrease the prospects of later osteoarthritis. The only absolute contra-indication is a fused epiphysial plate. The degree of slip and above all the duration of symptoms should not preclude an attempt at manipulative reduction.
1. The development of sclerosis of the femoral head after fracture of the femoral neck has been investigated by a combined microradiographic and histological examination of twenty femoral heads removed during arthroplasty of the hip done at varying intervals after fracture. 2. Mineralisation of the bone did not differ from the normal. 3. There was a direct correlation between the density of the femoral head as judged radiologically and the width of the trabeculae. In cases of sclerosis of the femoral head the trabeculae were broader than normal and histological examination showed that this was caused by apposition of new bone upon the necrotic trabeculae. 4. Although sclerosis is a result of necrosis, it is at the same time a definite sign that revascularisation and restitution are going on.
1. Discs of bone from two fixed sites on the front of the femur were taken from ninety-one necropsy subjects and the density, width and histological appearance of the cortical bone were examined. 2. Cortical thickness, and the density of the femoralcortex, decreased with increasingage of bone. 3. There was an increase in the rate of resorption of the bone cortex from both sexes after the fifth decade. 4. The difference between the density of the metaphysial cortex and that from the diaphysis increased for both sexes after the age of fifty, because of the greater metaphysial resorption. 5. There was no change in the degree of mineralisation of the cortical bone with age. The decrease in density with age is, therefore, accounted for by resorption.
1. An apparatus was designed to determine the shearing strength of the upper tibial epiphysis in the rat. Observations were made with this instrumenton normal animals, on animals receiving growth-hormone, and on animals receiving oestrogen. 2. When the epiphysis separates from the diaphysis, the plane of cleavage is constant, passing through the third layer of the epiphysial plate. 3. Growth-hormone decreases and sex-hormone increases the shearing strength of the epiphysial plate. These changes are due to alterations produced by these two hormones in the thickness of the third layer of the epiphysial plate. 4. It is suggested that these findings may be of significance in providing an anatomical basis for slipping of the upper femoral epiphysis in man, especially when it is associated with the adiposo-genital syndrome or with rapid adolescent growth.
Aims. Conflicting clinical results are reported for the ATTUNE Total Knee Arthroplasty (TKA). This randomized controlled trial (RCT) evaluated five-year follow-up results comparing cemented ATTUNE and PFC-Sigma cruciate retaining TKAs, analyzing component migration as measured by radiostereometric analysis (RSA), clinical outcomes, patient-reported outcome measures (PROMs), and radiological outcomes. Methods. A total of 74 primary TKAs were included in this single-blind RCT. RSA examinations were performed, and PROMs and clinical outcomes were collected immediate postoperatively, and at three, six, 12, 24, and 60 months’ follow-up. We have investigated the role of the penetration of saline on the shear strength of the cement-stem interface for stems inserted at room temperature and those preheated to 37°C using a variety of commercial bone cements. Immersion in saline for two weeks at 37°C reduced interfacial strength by 56% to 88% after insertion at room temperature and by 28% to 49% after preheating of the stem. The reduction in porosity as a result of preheating ranged from 71% to 100%. Increased porosity correlated with a reduction in shear strength after immersion in saline (r = 0.839, p <
0.01) indicating that interfacial porosity may act as a fluid conduit.

Aims. Contemporary outcomes of primary total hip arthroplasties (THAs) with highly cross-linked polyethylene (HXLPE) liners in patients with inflammatory arthritis have not been well studied. This study examined the implant survivorship, complications, radiological results, and clinical outcomes of THA in patients with inflammatory arthritis. Methods. We identified 418 hips (350 patients) with a primary diagnosis of inflammatory arthritis who underwent primary THA with HXLPE liners from January 2000 to December 2017. Of these hips, 68% had rheumatoid arthritis (n = 286), 13% ankylosing spondylitis (n = 53), 7% juvenile rheumatoid arthritis (n = 29), 6% psoriatic arthritis (n = 24), 5% systemic lupus erythematosus (n = 23), and 1% scleroderma (n = 3). Mean age was 58 years (SD 14.8), 66.3% were female (n = 277), and mean BMI was 29 kg/m. 2. (SD 7). Uncemented femoral components were used in 77% of cases (n = 320). Uncemented acetabular components were used in all patients. Competing risk analysis was used accounting for death. Mean follow-up was 4.5 years (2 to 18). Results. The ten-year cumulative incidence of any revision was 3%, and was highest in psoriatic arthritis patients (16%). The most common indications for the 15 revisions were dislocations (n = 8) and periprosthetic joint infections (PJI; n = 4, all on disease-modifying antirheumatic drugs (DMARDs)). We compared two methods of reconstruction of the abductor mechanism in 15 patients after prosthetic replacement of the upper femur, to assess abductor strength and function. Six patients in group I had direct fixation of the gluteus medius tendon and a segment of the original bone to the prosthesis. Nine patients in group 2 had the abductor tendon fixed to the iliotibial band. We assessed clinical function, isometric muscle strength and muscle cross-sectional area for each patient. The patients in group 1 had better clinical and functional results (p = 0.059), with average peak torques for hip abduction of 92% of that in the non-operated leg in group 1, and of 57% in group 2. Group 1 had a mean muscle cross-sectional area of 69% and a mean value of strength per cross-sectional area of 134% when compared with the control side. The respective values for group 2 were 52% and 91%. Direct fixation of the abductor muscles to the prostheses gave improved function and higher isometric abductor muscle force.
Several authors have reported complications from screw removal after treatment of slipped upper femoral epiphysis by single screw fixation, and have attributed these to poor screw design. We have developed a simple and reliable method of screw removal which uses a cannulated 8.0 mm end-threaded cancellous screw (Smith &
Nephew Richards Medical, Memphis, Tennessee) and a specially designed cannulated trephine. The method has been successful, with minimal complications, and a limited surgical exposure.
