Transtrochanteric curved varus osteotomy was designed to avoid some of the disadvantages of varus wedge osteotomy, such as post-operative leg-length discrepancy. In this retrospective study we investigated the leg-length discrepancy and clinical outcome after transtrochanteric curved varus osteotomy undertaken in patients with osteonecrosis of the femoral head. Between January 1993 and March 2004, this osteotomy was performed in 42 hips of 36 patients with osteonecrosis of the femoral head. There were 15 males and 21 females with a mean age at surgery of 34 years (15 to 68). The mean follow-up was 5.9 years (2.0 to 12.5). The mean pre-operative Harris hip score was 64.0 (43 to 85) points, which improved to a mean of 88.7 (58 to 100) points at final follow-up. The mean varus angulation post-operatively was 25° (12° to 38°) and the post-operative mean leg-length discrepancy was 13 mm (4 to 25). The post-operative leg-length discrepancy showed a strong correlation with varus angulation (Pearson’s correlation coefficient; r = 0.9530, p <
0.0001), which may be useful for predicting the leg-length discrepancy which can occur even after transtrochanteric curved varus osteotomy.
We report the outcome of 39 patients who underwent
a modified Pauwels’ intertrochanteric osteotomy for nonunion of
a femoral neck fracture following failed osteosynthesis. There were
31 men and eight women with a mean age of 47.2 years (34 to 59).
By Pauwels’ classification, there were 11 Type II fractures and
28 Type III fractures. The mean follow-up was 7.9 years (2 to 19).
In the 11 patients whose initial treatment had been osteotomy, union was
achieved in nine (81.8%). In 28 patients whose initial treatment
had been with a lag screw or a dynamic hip screw, union was achieved
in 27 (96.4%). Limb lengths were equalised in 14 of 16 patients
(87.5%) with pre-operative shortening. The mean neck-shaft angle
improved significantly from 100.5° (80° to 120°) to 131.6° (120°
to 155°) (p = 0.004). The mean modified Harris hip score was 85.6
points (70 to 97) and the mean modified Merle d’Aubigné score was
14.3 (11 to 18). Good to excellent functional outcomes were achieved
in 32 patients (88.8%). A modified Pauwels’ intertrochanteric osteotomy
is a reliable method of treating ununited fractures of the femoral neck
following failed osteosynthesis: coxa vara and shortening can also

A cadaver study using six pairs of lower limbs was conducted to investigate the accuracy of computer navigation and standard instrumentation for the placement of the Birmingham Hip Resurfacing femoral component. The aim was to place all the femoral components with a stem-shaft angle of 135°. The mean stem-shaft angle obtained in the standard instrumentation group was 127.7° (120° to 132°), compared with 133.3° (131° to 139°) in the computer navigation group (p = 0.03). The scatter obtained with computer-assisted navigation was approximately half that found using the conventional jig. Computer navigation was more accurate and more consistent in its placement of the femoral component than standard instrumentation. We suggest that image-free computer-assisted navigation may have an application in aligning the femoral component during hip resurfacing.
The cam-type deformity in femoroacetabular impingement
is a 3D deformity. Single measurements using radiographs, CT or
MRI may not provide a true estimate of the magnitude of the deformity.
We performed an analysis of the size and location of measurements
of the alpha angle (α°) using a CT technique which could be applied
to the 3D reconstructions of the hip. Analysis was undertaken in
42 patients (57 hips; 24 men and 18 women; mean age 38 years (16
to 58)) who had symptoms of femoroacetabular impingement related
to a cam-type abnormality. An α° of >
50° was considered a significant
indicator of cam-type impingement. Measurements of the α° were made
at different points around the femoral head/neck junction at intervals
of 30°: starting at the nine o’clock (posterior), ten, eleven and
twelve o’clock (superior), one, two and ending at three o’clock
(anterior) position. The mean maximum increased α° was 64.6° (50.8° to 86°). The two
o’clock position was the most common point to find an increased α°
(53 hips; 93%), followed by one o’clock (48 hips; 84%). The largest α°
for each hip was found most frequently at the two o’clock position
(46%), followed by the one o’clock position (39%). Generally, raised α angles
extend over three segments of the clock face. Single measurements of the α°, whether pre- or post-operative,
should be viewed with caution as they may not be representative
of the true size of the deformity and not define whether adequate
correction has been achieved following surgery. Cite this article:
Despite a lack of long-term follow-up, there
is an increasing trend towards using femoral heads of large diameter
in total hip replacement (THR), partly because of the perceived
advantage of lower rates of dislocation. However, increasing the
size of the femoral head is not the only way to reduce the rate
of dislocation; optimal alignment of the components and repair of
the posterior capsule could achieve a similar effect. In this prospective study of 512 cemented unilateral THRs (Male:Female
230:282) performed between 2004 and 2011, we aimed to determine
the rate of dislocation in patients who received a 22 mm head on
a 9/10 Morse taper through a posterior approach with capsular repair
and using the transverse acetabular ligament (TAL) as a guide for the
alignment of the acetabular component. The mean age of the patients
at operation was 67 years (35 to 89). The mean follow-up was 2.8
years (0.5 to 6.6). Pre- and post-operative assessment included
Oxford hip, Short Form-12 and modified University of California
Los Angeles and Merle D’Aubigne scores. The angles of inclination
and anteversion of the acetabular components were measured using
radiological software. There were four dislocations (0.78%), all
of which were anterior. In conclusion, THR with a 22 mm diameter head performed through
a posterior approach with capsular repair and using the TAL as a
guide for the alignment of the acetabular component was associated

The angle of inclination of the acetabular component in total hip replacement is a recognised contributing factor in dislocation and early wear. During non-navigated surgery, insertion of the acetabular component has traditionally been performed at an angle of 45° relative to the sagittal plane as judged by the surgeon’s eye, the operative inclination. Typically, the method used to assess inclination is the measurement made on the postoperative anteroposterior radiograph, the radiological inclination. The aim of this study was to measure the intra-operative angle of inclination of the acetabular component on 60 consecutive patients in the lateral decubitus position when using a posterior approach during total hip replacement. This was achieved by taking intra-operative photographs of the acetabular inserter, representing the acetabular axis, and a horizontal reference. The results were compared with the post-operative radiological inclination. The mean post-operative radiological inclination was 13° greater than the photographed operative inclination, which was unexpectedly high. It appears that in the lateral decubitus position with a posterior approach, the uppermost hemipelvis adducts, thus reducing the apparent operative inclination. Surgeons using the posterior approach in lateral decubitus need to aim for a lower operative inclination than when operating with the patient supine in order to achieve an acceptable radiological inclination.
In patients with severe quadriplegic cerebral palsy and painful hip dislocation proximal femoral resection arthroplasty can reduce pain, but the risk of heterotopic ossification is significant. We present a surgical technique of autologous capping of the femoral stump in order to reduce this risk, using the resected femoral head as the graft. A retrospective study of 31 patients (43 hips) who had undergone proximal femoral resection arthroplasty with (29 hips) and without autologous capping (14 hips) was undertaken. Heterotopic ossification was less frequent in patients with autologous capping, and a more predictable pattern of bony overgrowth was found. For a selected group of non-ambulatory patients with long-standing painful dislocation of the hip, we recommend femoral resection arthroplasty over more complicated reconstructive operations. The risk of heterotopic ossification, which is a major disadvantage of this operation, is reduced by autologous capping.
Internal lengthening devices in the femur lengthen
along the anatomical axis, potentially creating lateral shift of
the mechanical axis. We aimed to determine whether femoral lengthening
along the anatomical axis has an inadvertent effect on lower limb
alignment. Isolated femoral lengthening using the Intramedullary
Skeletal Kinetic Distractor was performed in 27 femora in 24 patients
(mean age 32 years (16 to 57)). Patients who underwent simultaneous realignment
procedures or concurrent tibial lengthening, or who developed mal-
or nonunion, were excluded. Pre-operative and six-month post-operative
radiographs were used to measure lower limb alignment. The mean lengthening
achieved was 4.4 cm (1.5 to 8.0). In 26 of 27 limbs, the mechanical
axis shifted laterally by a mean of 1.0 mm/cm of lengthening (0
to 3.5). In one femur that was initially in varus, a 3 mm medial
shift occurred during a lengthening of 2.2 cm. In a normally aligned limb, intramedullary lengthening along
the anatomical axis of the femur results in a lateral shift of the
mechanical axis by approximately 1 mm for each 1 cm of lengthening.
There have been several studies examining the
association between the morphological characteristics seen in acetabular
dysplasia and the incidence of the osteoarthritis (OA). However, most studies focus mainly on acetabular morphological
analysis, and few studies have scrutinised the effect of femoral
morphology. In this study we enrolled 36 patients with bilateral
acetabular dysplasia and early or mid-stage OA in one hip and no
OA in the contralateral hip. CT scans were performed from the iliac
crest to 2 cm inferior to the tibial tuberosity, and the morphological
characteristics of both acetabulum and femur were studied. In addition, 200 hips in 100 healthy volunteer Chinese adults
formed a control group. The results showed that the dysplastic group
with OA had a significantly larger femoral neck anteversion and
a significantly shorter abductor lever arm than both the dysplastic
group without OA and the controls. Femoral neck anteversion had
a significant negative correlation with the length of the abductor
lever arm and we conclude that it may contribute to the development
of OA in dysplastic hips. Cite this article:
Injury to the sciatic nerve following closed manipulation of a dislocated total hip replacement is rare. We present such a case in an elderly patient with partial recovery following exploration and release of the nerve.
Fractures of the proximal femur are one of the
greatest challenges facing the medical community, constituting a
heavy socioeconomic burden worldwide. The National Hip Fracture
Audit currently provides a framework for service evaluation. This
evaluation is based upon the assessment of process rather than assessment
of patient-centred outcome and therefore it fails to provide meaningful
data regarding the clinical effectiveness of treatments. This study
aims to capture data from the cohort of patients who present with
a fracture of the proximal femur at a single United Kingdom Major
Trauma Centre. Patient-centred outcomes will be recorded and provide
a baseline cohort within which to test the clinical effectiveness
of experimental interventions.
A total of 31 patients, (20 women, 11 men; mean
age 62.5 years old; 23 to 81), who underwent conversion of a Girdlestone
resection-arthroplasty (RA) to a total hip replacement (THR) were
compared with 93 patients, (60 women, 33 men; mean age 63.4 years
old; 20 to 89), who had revision THR surgery for aseptic loosening
in a retrospective matched case-control study. Age, gender and the
extent of the pre-operative bone defect were similar in all patients.
Mean follow-up was 9.3 years (5 to 18). Pre-operative function and range of movement were better in the
control group (p = 0.01 and 0.003, respectively) and pre-operative
leg length discrepancy (LLD) was greater in the RA group (p <
0.001). The post-operative clinical outcome was similar in both
groups except for mean post-operative LLD, which was greater in
the study group (p = 0.003). There was a significant interaction
effect for LLD in the study group (p <
0.001). A two-way analysis
of variance showed that clinical outcome depended on patient age
(patients older than 70 years old had worse pre-operative pain,
p = 0.017) or bone defect (patients with a large acetabular bone
defect had higher LLD, p = 0.006, worse post-operative function
p = 0.009 and range of movement, p = 0.005), irrespective of the
group. Despite major acetabular and femoral bone defects requiring complex
surgical reconstruction techniques, THR after RA shows a clinical
outcome similar to those obtained in aseptic revision surgery for
hips with similar sized bone defects. Cite this article:
Eighteen hip fusions were converted to total
hip replacements. A constrained acetabular liner was used in three hips.
Mean follow up was five years (two to 15). Two (11%) hips failed,
requiring revision surgery and two patients (11%) had injury to
the peroneal nerve. Heterotopic ossification developed in seven
(39%) hips, in one case resulting in joint ankylosis. No hips dislocated. Conversion of hip fusion to hip replacement carries an increased
risk of heterotopic ossification and neurological injury. We advise
prophylaxis against heterotropic ossification. When there is concern
about hip stability we suggest that the use of a constrained acetabular
liner is considered. Despite the potential for complications, this procedure
had a high success rate and was effective in restoring hip function.
Revision total hip replacement (THR) for young
patients is challenging because of technical complexity and the potential
need for subsequent further revisions. We have assessed the survivorship,
functional outcome and complications of this procedure in patients
aged <
50 years through a large longitudinal series with consistent treatment
algorithms. Of 132 consecutive patients (181 hips) who underwent
revision THR, 102 patients (151 hips) with a mean age of 43 years
(22 to 50) were reviewed at a mean follow-up of 11 years (2 to 26)
post-operatively. We attempted to restore bone stock with allograft
where indicated. Using further revision for any reason as an end point,
the survival of the acetabular component was 71% ( This overall perspective on the mid- to long-term results is
valuable when advising young patients on the prospects of revision

We undertook a retrospective review of 33 patients who underwent total femoral endoprosthetic replacement as limb salvage following excision of a malignant bone tumour. In 22 patients this was performed as a primary procedure following total femoral resection for malignant disease. Revision to a total femoral replacement was required in 11 patients following failed segmental endoprosthetic or allograft reconstruction. There were 33 patients with primary malignant tumours, and three had metastatic lesions. The mean age of the patients was 31 years (5 to 68). The mean follow-up was 4.2 years (9 months to 16.4 years). At five years the survival of the implants was 100%, with removal as the endpoint and 56% where the endpoint was another surgical intervention. At five years the patient survival was 32%. Complications included dislocation of the hip in six patients (18%), local recurrence in three (9%), peri-prosthetic fracture in two and infection in one. One patient subsequently developed pulmonary metastases. There were no cases of aseptic loosening or amputation. Four patients required a change of bushings. The mean Musculoskeletal Tumour Society functional outcome score was 67%, the mean Harris Hip Score was 70, and the mean Oxford Knee Score was 34. Total femoral endoprosthetic replacement can provide good functional outcome without compromising patient survival, and in selected cases provides an effective alternative to amputation.
We have developed a CT-based navigation system using infrared light-emitting diode markers and an optical camera. We used this system to perform cementless total hip replacement using a ceramic-on-ceramic bearing couple in 53 patients (60 hips) between 1998 and 2001. We reviewed 52 patients (59 hips) at a mean of six years (5 to 8) postoperatively. The mid-term results of total hip replacement using navigation were compared with those of 91 patients (111 hips) who underwent this procedure using the same implants, during the same period, without navigation. There were no significant differences in age, gender, diagnosis, height, weight, body mass index, or pre-operative clinical score between the two groups. The operation time was significantly longer where navigation was used, but there was no significant difference in blood loss or navigation-related complications. With navigation, the acetabular components were placed within the safe zone defined by Lewinnek, while without, 31 of the 111 components were placed outside this zone. There was no significant difference in the Merle d’Aubigne and Postel hip score at the final follow-up. However, hips treated without navigation had a higher rate of dislocation. Revision was performed in two cases undertaken without navigation, one for aseptic acetabular loosening and one for fracture of a ceramic liner, both of which showed evidence of neck impingement on the liner. A further five cases undertaken without navigation showed erosion of the posterior aspect of the neck of the femoral component on the lateral radiographs. These seven impingement-related mechanical problems correlated with malorientation of the acetabular component. There were no such mechanical problems in the navigated group. We conclude that CT-based navigation increased the precision of orientation of the acetabular component and control of limb length in total hip replacement, without navigation-related complications. It also reduced the rate of dislocation and mechanical problems related to impingement.
Proximal femoral varus osteotomy improves the
biomechanics of the hip and can stimulate normal acetabular development
in a dysplastic hip. Medial closing wedge osteotomy remains the
most popular technique, but is associated with shortening of the
ipsilateral femur. We produced a trigonometric formula which may be used pre-operatively
to predict the resultant leg length discrepancy (LLD). We retrospectively
examined the influence of the choice of angle in a closing wedge
femoral osteotomy on LLD in 120 patients (135 osteotomies, 53% male,
mean age six years, (3 to 21), 96% caucasian) over a 15-year period
(1998 to 2013). A total of 16 of these patients were excluded due
to under or over varus correction. The patients were divided into
three age groups: paediatric (<
10 years), adolescent (10 to
16 years) and adult (>
16 years). When using the same saw blades
as in this series, the results indicated that for each 10° of angle
of resection the resultant LLD equates approximately to multiples
of 4 mm, 8 mm and 12 mm in the three age groups, respectively. Statistical testing of the 59 patients who had a complete set
of pre- and post-operative standing long leg radiographs, revealed

We report the results of 79 patients (81 hips)
who underwent impaction grafting at revision hip replacement using the
Exeter femoral stem. Their mean age was 64 years (31 to 83). According
to the Endoklinik classification, 20 hips had a type 2 bone defect,
40 had type 3, and 21 had type 4. The mean follow-up for unrevised
stems was 10.4 years (5 to 17). There were 12 re-operations due to intra- and post-operative
fractures, infection (one hip) and aseptic loosening (one hip).
All re-operations affected type 3 (6 hips) and 4 (6 hips) bone defects.
The survival rate for re-operation for any cause was 100% for type
2, 81.2% (95% confidence interval (CI) 67.1 to 95.3) for type 3,
and 70.8% (95% CI 51.1 to 90.5) for type 4 defects at 14 years.
The survival rate with further revision for aseptic loosening as
the end point was 98.6% (95% CI 95.8 to 100). The final clinical
score was higher for patients with type 2 bone defects than type
4 regarding pain, function and range of movement. Limp was most
frequent in the type 4 group (p <
0.001). The mean subsidence
of the stem was 2.3 mm ( The impacted bone grafting technique has good clinical results
in femoral revision. However, major bone defects affect clinical
outcome and also result in more operative complications.
Failure of total hip arthroplasty with acetabular deficiency occurred in 55 patients (60 hips) and was treated with acetabular revision using morsellised allograft and a cemented metal-backed component. A total of 50 patients (55 hips) were available for clinical and radiological evaluation at a mean follow-up of 5.8 years (3 to 9.5). No hip required further revision of the acetabular component because of aseptic loosening. All the hips except one had complete incorporation of the allograft demonstrated on the radiographs. A complete radiolucent line of >
1 mm was noted in two hips post-operatively. A good to excellent result occurred in 50 hips (91%). With radiological evidence of aseptic loosening of the acetabular component as the end-point, the survivorship at a mean of 5.8 years after surgery was 96.4%. The use of impacted allograft chips in combination with a cemented metal-backed acetabular component and screw fixation can achieve good medium-term results in patients with acetabular bone deficiency.
Radiostereometric analysis (RSA) can detect early
micromovement in unstable implant designs which are likely subsequently
to have a high failure rate. In 2010, the Articular Surface Replacement
(ASR) was withdrawn because of a high failure rate. In 19 ASR femoral
components, the mean micromovement over the first two years after implantation
was 0.107 mm ( We conclude that the ASR femoral component achieves initial stability
and that early migration is not the mode of failure for this resurfacing
arthroplasty.
