We examined the mechanical properties of Vicryl (polyglactin 910) mesh Mesh fibres were visible at six weeks but had been completely resorbed by 12 weeks, with no evidence of chronic inflammation. The tendon-implant neoenthesis was predominantly an indirect type, with tendon attached to the bone-hydroxyapatite surface by perforating collagen fibres.
We report the long-term outcome of 33 patients
(37 knees) who underwent proximal tibial open-wedge osteotomy with
hemicallotasis (HCO) for medial osteoarthritis of the knee between
1995 and 2000. Among these, 29 patients with unilateral HCO were
enrolled and 19 were available for review at a mean of 14.2 years
(10 to 15.7) post-operatively. For these 19 patients, the mean Hospital

Although it is clear that opening-wedge high
tibial osteotomy (HTO) changes alignment in the coronal plane, which is
its objective, it is not clear how this procedure affects knee kinematics
throughout the range of joint movement and in other planes. Our research question was: how does opening-wedge HTO change
three-dimensional tibiofemoral and patellofemoral kinematics in
loaded flexion in patients with varus deformity?Three-dimensional
kinematics were assessed over 0° to 60° of loaded flexion using
an MRI method before and after opening-wedge HTO in a cohort of
13 men (14 knees). Results obtained from an iterative statistical
model found that at six and 12 months after operation, opening-wedge

We performed A total of 12 cadaveric lower limbs were tested with a commercial
image-free navigation system using trackers secured by bone screws.
We then tested a non-invasive fabric-strap system. The lower limb
was secured at 10° intervals from 0° to 60° of knee flexion and
100 N of force was applied perpendicular to the tibia. Acceptable
coefficient of repeatability (CR) and limits of agreement (LOA)
of 3 mm were set based on diagnostic criteria for anterior cruciate
ligament (ACL) insufficiency.Objectives

Radiographs of 110 patients who had undergone 120 high tibial osteotomies (60 closed-wedge, 60 open-wedge) were assessed for posterior tibial slope before and after operation, and before removal of the hardware. In the closed-wedge group the mean slope was 5.7° ( Posterior tibial slope decreases after closed-wedge high tibial osteotomy and increases after an open-wedge procedure because of the geometry of the proximal tibia. The changes in the slope are stable over time, emphasising the influence of the operative procedure rather than of the implant.
We present a 25-year-old patient with juvenile rheumatoid arthritis and ankylosis of both hips and both knees treated by staged bilateral hip and knee arthroplasty. She was followed up for 18 months. We discuss the pre-operative planning, surgical details and post-operative rehabilitation.
Trochlear dysplasia is a developmental condition characterised by an abnormally flat or dome-shaped trochlea and is an important contributory factor to patellofemoral instability and recurrent dislocation. We studied prospectively a series of 54 consecutive patients (59 knees) with patellofemoral instability secondary to trochlear dysplasia, who were treated by a trochleoplasty by a single surgeon between June 2002 and June 2007. Pre- and post-operative scores were assessed by the patients and a satisfaction questionnaire was completed. Of the 54 patients (59 knees) in the series, 39 (44 knees) were female and 15 (15 knees) were male. Their mean age at surgery was 21 years and 6 months (14 years 4 months to 33 years 11 months). In 40 patients (42 knees) the mean follow-up was for 24 months (12 to 58). One patient was unable to attend for follow-up. An analysis of the results of those patients followed up for at least 12 months showed a statistically significant improvement in outcome (p <
0.001 for all scores). Overall, 50 patients (92.6%) were satisfied with the outcome of their procedure. The early results of trochleoplasty are encouraging in this challenging group of patients.
In an initial randomised controlled trial (RCT)
we segregated 180 patients to one of two knee positions following total
knee replacement (TKR): six hours of knee flexion using either a
jig or knee extension. Outcome measures included post-operative
blood loss, fall in haemoglobin, blood transfusion requirements,
knee range of movement, limb swelling and functional scores. A second
RCT consisted of 420 TKR patients randomised to one of three post-operative
knee positions: flexion for three or six hours post-operatively,
or knee extension. Positioning of the knee in flexion for six hours immediately
after surgery significantly reduced blood loss (p = 0.002). There
were no significant differences in post-operative range of movement,
swelling, pain or outcome scores between the various knee positions
in either study. Post-operative knee flexion may offer a simple
and cost-effective way to reduce blood loss and transfusion requirements
following TKR. We also report a cautionary note regarding the potential risks
of prolonged knee flexion for more than six hours observed during
clinical practice in the intervening period between the two trials,
with 14 of 289 patients (4.7%) reporting lower limb sensory neuropathy

Between July 1986 and August 1996, we performed 32 total knee arthroplasties (TKA) on 32 patients with partially or completely ankylosed knees secondary to infection. Their mean age at surgery was 40 years (20 to 63) and the mean follow-up was ten years (5 to 13). The mean post-operative range of movement was 75.3° (30 to 115) in those with complete and 98.7° (60 to 130) in those with partial ankylosis. The mean Hospital for Special Surgery knee score increased from 57 to 86 points post-operatively. There were complications in four knees (12.5%), which included superficial infection (one), deep infection (one), supracondylar femoral fracture (one) and transient palsy of the common peroneal nerve (one). Although TKA in the ankylosed knee is technically demanding and has a considerable rate of complications, reasonable restoration of function can be obtained by careful selection of patients, meticulous surgical technique, and aggressive rehabilitation.
We used an A significant improvement in functional weight-bearing was observed between six and 12 weeks. The significant increase in ground reaction force through the operated limb between six and 12 weeks was greater than that reported previously with morcellised graft augmented reconstructions. Histological appearance and collagen fibre orientation with bone block augmentation more closely resembled that of an intact enthesis compared with the morcellised grafting technique. Bone block augmentation of tendon-implant interfaces results in more reliable functional and histological outcomes, with a return to pre-operative levels of weight-bearing by 24 weeks.
We developed an Kinematic gait analysis showed nearly normal function of the joint by 12 weeks. Force-plate assessment showed a significant increase in functional weight-bearing in the grafted animals (p = 0.043). The tendon-implant interface showed that without graft, encapsulation of fibrous tissue occurred. With autograft, a developing tendon-bone-HA-implant interface was observed at six weeks and by 12 weeks a layered tendon-fibrocartilage-bone interface was seen which was similar to a direct-type enthesis. With stable mechanical fixation, an appropriate bioactive surface and biological augmentation the development of a functional tendon-implant interface can be achieved.
We present the ten- to 15-year follow-up of 31
patients (34 knees), who underwent an Elmslie-Trillat tibial tubercle osteotomy
for chronic, severe patellar instability, unresponsive to non-operative
treatment. The mean age of the patients at the time of surgery was
31 years (18 to 46) and they were reviewed post-operatively, at
four years (2 to 8) and then at 12 years (10 to 15). All patients
had pre-operative knee radiographs and Cox and Insall knee scores. Superolateral
portal arthroscopy was performed per-operatively to document chondral
damage and after the osteotomy to assess the stability of the patellofemoral
joint. A total of 28 knees (82%) had a varying degree of damage
to the articular surface. At final follow-up 25 patients (28 knees)
were available for review and underwent clinical examination, radiographs
of the knee, and Cox and Insall scoring. Six patients who had no
arthroscopic chondral abnormality showed no or only early signs
of osteoarthritis on final radiographs; while 12 patients with lower
grade chondral damage (grade 1 to 2) showed early to moderate signs
of osteoarthritis and six out of ten knees with higher grade chondral
damage (grade 3 to 4) showed marked evidence of osteoarthritis;
four of these had undergone a knee replacement. In the 22 patients
(24 knees) with complete follow-up, 19 knees (79.2%) were reported
to have a good or excellent outcome at four years, while 15 knees
(62.5%) were reported to have the same at long-term follow-up. The
functional and radiological results show that the extent of pre-operatively
sustained chondral damage is directly related to the subsequent

We reviewed retrospectively the results in 211 consecutive patients who had undergone limb salvage for bone neoplasia with endoprosthetic reconstruction of the proximal femur (96), distal femur (78), proximal tibia (30) and total femur (7). Their mean age was 50 years (11 to 86) and the mean follow-up period was 37.3 months (1 to 204). A total of 35 (16.6%) prostheses failed. Overall, implant survival was 78% (95% confidence interval (CI) 0.29 to 0.54) at five years, 60% (95% CI 0.93 to 2.35) at ten years and 60% (95% CI 1.27 to 3.88) at 15 years. Survivorship of the limb was 97.6% (95% CI 1.73 to 3.35) at ten years. The gender, age, diagnosis and location of the tumour were not prognostic variables for failure. Modular endoprosthetic replacement in the lower limb is a durable long-term reconstructive option, with the implants generally outlasting the patient.
We aimed to obtain anthropometric data on Korean
knees and to compare these with data on commonly available total
knee arthroplasties (TKAs). The dimensions of the femora and tibiae
of 1168 knees were measured intra-operatively. The femoral components
were found to show a tendency toward mediolateral (ML) under-coverage
in small femurs and ML overhang in the large femurs. The ML under-coverage
was most prominent for the small prostheses. The ML/anteroposterior
(ML/AP) ratio of Korean tibiae was greater than that of tibial components. This study shows that, for different reasons, current TKAs do
not provide a reasonable fit for small or large Korean knees, and
that the ‘gender-specific’ and ‘stature-specific’ components help
for large Korean femurs but offer less satisfactory fits for small
femurs. Specific modifications of prostheses are needed for Asian
knees.
Radiological assessment of total and unicompartmental
knee replacement remains an essential part of routine care and follow-up.
Appreciation of the various measurements that can be identified
radiologically is important. It is likely that routine plain radiographs
will continue to be used, although there has been a trend towards
using newer technologies such as CT, especially in a failing knee,
where it provides more detailed information, albeit with a higher
radiation exposure. The purpose of this paper is to outline the radiological parameters
used to evaluate knee replacements, describe how these are measured
or classified, and review the current literature to determine their
efficacy where possible.
There remains a lack of data on the reliability of methods to
estimate tibial coverage achieved during total knee replacement.
In order to address this gap, the intra- and interobserver reliability
of a three-dimensional (3D) digital templating method was assessed
with one symmetric and one asymmetric prosthesis design. A total of 120 template procedures were performed according to
specific rotational and over-hang criteria by three observers at
time zero and again two weeks later. Total and sub-region coverage
were calculated and the reliability of the templating and measurement
method was evaluated.Objectives

The current indications for functional restoration
of extension of the knee following quadriceps resection or loss require
reappraisal. The contribution of pedicled and free functional muscle
transfer is likely to be over-emphasised in many studies, with good
functional outcomes predominantly reported only in the context of
cases with residual quadriceps function. In cases with total quadriceps
resection or loss, all forms of reconstruction perform poorly. Furthermore,
in smaller resections with loss of two or fewer components of the
quadriceps, minimal impairment of function occurs in the absence
of functional reconstruction, suggesting that functional restoration
may not be warranted. Thus there is a paradox in the current approach
to quadriceps reconstruction, in that small resections are likely
to be over-treated and large resections remain under-treated. This review suggests a shift is required in the approach and
rationale for reconstructing functional extension of the knee after
quadriceps resection or loss. A classification based on current
evidence is suggested that emphasises more clearly the indications
and rationale for functional transfers.
The clinical diagnosis of a partial tear of the
anterior cruciate ligament (ACL) is still subject to debate. Little
is known about the contribution of each ACL bundle during the Lachman
test. We investigated this using six fresh-frozen cadaveric lower
limbs. Screws were placed in the femora and tibiae as fixed landmarks
for digitisation of the bone positions. The femur was secured horizontally
in a clamp. A metal hook was screwed to the tibial tubercle and
used to apply a load of 150 N directed anteroposteriorly to the
tibia to simulate the Lachman test. The knees then received constant
axial compression and 3D knee kinematic data were collected by digitising
the screw head positions in 30° flexion under each test condition.
Measurements of tibial translation and rotation were made, first with
the ACL intact, then after sequential cutting of the ACL bundles,
and finally after complete division of the ACL. Two-way analysis
of variance analysis was performed. During the Lachman test, in all knees and in all test conditions,
lateral tibial translation exceeded that on the medial side. With
an intact ACL, both anterior and lateral tibial landmarks translated
significantly more than those on the medial side (p <
0.001).
With sequential division of the ACL bundles, selective cutting of
the posterolateral bundle (PLB) did not increase translation of
any landmark compared with when the ACL remained intact. Cutting the
anteromedial bundle (AMB) resulted in an increased anterior translation
of all landmarks. Compared to the intact ACL, when the ACL was fully
transected a significant increase in anterior translation of all
landmarks occurred (p <
0.001). However, anterior tibial translation
was almost identical after AMB or complete ACL division. We found that the AMB confers its most significant contribution
to tibial translation during the Lachman test, whereas the PLB has
a negligible effect on anterior translation. Section of the PLB
had a greater effect on increasing the internal rotation of the
tibia than the AMB. However, its contribution of a mean of 2.8°
amplitude remains low. The clinical relevance of our investigation
suggests that, based on anterior tibial translation only, one cannot distinguish
between a full ACL and an isolated AMB tear. Isolated PLB tears
cannot be detected solely by the Lachman test, as this bundle probably
contributes more resistance to the pivot shift.
