However, the role of the long form of the leptin receptor (Ob-Rb) in cartilage degeneration needs further study. The aim of this study was to determine the effect of increasing the expression of Ob-Rb on chondrocytes using a lentiviral vector containing Ob-Rb. The best marker for assessing glycaemic control prior to total knee arthroplasty (TKA) remains unknown. The purpose of this study was to assess the utility of fructosamine compared with glycated haemoglobin (HbA1c) in predicting early complications following TKA, and to determine the threshold above which the risk of complications increased markedly. This prospective multi-institutional study evaluated primary TKA patients from four academic institutions. Patients (both diabetics and non-diabetics) were assessed using fructosamine and HbA1c levels within 30 days of surgery. Complications were assessed for 12 weeks from surgery and included prosthetic joint infection (PJI), wound complication, re-admission, re-operation, and death. The Youden’s index was used to determine the cut-off for fructosamine and HbA1c associated with complications. The aim of this study was to assess the effect of multimorbidity
on improvements in health-related quality of life (HRQoL) following
total hip arthroplasty (THA) and total knee arthroplasty (TKA). Using data from a regional joint registry for 14 573 patients,
HRQoL was measured prior and one year following surgery using the
Oxford Hip Score (OHS) and Oxford Knee Score (OKS), and the 12-Item
Short-Form Health Survey Physical and Mental Component Summary scores
(PCS and MCS, respectively). Multimorbidity was defined as the concurrence
of two or more self-reported chronic conditions. A linear mixed-effects
model was used to test the effects of multimorbidity and the number

of using this stem, we have detected a small group of patients who
have presented with symptomatic metaphyseal debonding. The aim of

in the rate of wear between acetabular components positioned within
and outside the ‘safe zones’ of anteversion and inclination angle. We reviewed 100 hips in 94 patients who had undergone primary
total hip arthroplasty (THA) at least ten years previously. Patients
all had the same type of acetabular component with a bearing couple
which consisted of a 28 mm cobalt-chromium head on a highly crosslinked
polyethylene (HXLPE) liner. A supine radiostereometric analysis
(RSA) examination was carried out which acquired anteroposterior
(AP) and lateral paired images. Acetabular component anteversion
and inclination angles were measured as well as total femoral head
penetration, which was divided by the length of implantation to

Previous genome-wide association studies (GWAS) have reported significant association of the single nucleotide polymorphism (SNP) rs8044769 in the fat mass and obesity-associated gene (FTO) with osteoarthritis (OA) risk in European populations. However, these findings have not been confirmed in Chinese populations. We systematically genotyped rs8044769 and evaluated the association between the genetic variants and OA risk in a case-controlled study including 196 OA cases and 442 controls in a northern Chinese population. Obesity is a global epidemic of 2.1 billion people and a well known cause of osteoarthritis. Joint replacement in the obese attracts more complications, poorer outcomes and higher revision rates. It is a reversible condition and the fundamental principles of dealing with reversible medical conditions prior to elective total joint replacement should apply to obesity. The dilemma for orthopaedic surgeons is when to offer surgery in the face of a reversible condition, which if treated may obviate joint replacement and reduce the risk and severity of obesity related disease in both the medical arena and the field of orthopaedics.
Patients with flexion instability after total knee arthroplasty
(TKA) often present with a recurrent effusion, which may be a haemarthrosis.
While the radiographic factors contributing to flexion instability
have been elucidated, the clinical diagnosis remains challenging.
Our aim, in this study, was to determine the mean white cell count
and differential profile in pre-operative aspirations of synovial
fluid in a consecutive series of patients undergoing revision TKA
for flexion instability. Between 2000 and 2010, 60 patients undergoing aseptic revision
TKA for flexion instability were identified. The results of the
pre-operative aspiration of synovial fluid were available for 53
patients (88%). These patients were 1:2 matched to 106 patients
who underwent aseptic TKA for indications other than flexion instability.
The mean age of the patients at revision TKA was 65 years (44 to
82) and 55% were women. The mean follow-up was 4.3 years (2 to 10.2).Aims

tools from large sets of poorly controlled healthcare metadata.
Companies often sell reports at a physician or facility level based
on various flawed data sources, and comparative websites of ‘publicly
reported data’ purport to educate the public. Physicians should
be aware of concerns and pitfalls seen in such data definitions,
data clarity, data relevance, data sources and data cleaning when
