We have investigated whether shape of the knee
can predict the clinical outcome of patients after an anterior cruciate
ligament rupture. We used statistical shape modelling to measure
the shape of the knee joint of 182 prospectively followed patients
on lateral and Rosenberg view radiographs of the knee after a rupture
of the anterior cruciate ligament. Subsequently, we associated knee
shape with the International Knee Documentation Committee subjective
score at two years follow-up. The mean age of patients was 31 years
(21 to 51), the majority were male (n = 121) and treated operatively
(n = 135). We found two modes (shape variations) that were significantly
associated with the subjective score at two years: one for the operatively
treated group (p = 0.002) and one for the non-operatively treated
group (p = 0.003). Operatively treated patients who had higher subjective
scores had a smaller intercondylar notch and a smaller width of
the intercondylar eminence. Non-operatively treated patients who
scored higher on the subjective score had a more pyramidal intercondylar
notch as opposed to one that was more dome-shaped. We conclude that
the shape of the femoral notch and the intercondylar eminence is predictive
of clinical outcome two years after a rupture of the anterior cruciate

knee arthroplasty (TKA) to deal with significant coronal plane deformities,
but is also associated with significant disadvantages. We describe steps to avoid release of the collateral (superficial
medial and lateral collateral) ligaments during TKA in severely

To assess the sustainability of our institutional
bone bank, we calculated the final product cost of fresh-frozen femoral
head allografts and compared these costs with the use of commercial
alternatives. Between 2007 and 2010 all quantifiable costs associated
with allograft donor screening, harvesting, storage, and administration
of femoral head allografts retrieved from patients undergoing elective
hip replacement were analysed. From 290 femoral head allografts harvested and stored as full
(complete) head specimens or as two halves, 101 had to be withdrawn.
In total, 104 full and 75 half heads were implanted in 152 recipients.
The calculated final product costs were €1367 per full head. Compared
with the use of commercially available processed allografts, a saving
of at least €43 119 was realised over four-years (€10 780 per year)
resulting in a cost-effective intervention at our institution. Assuming
a price of between €1672 and €2149 per commercially purchased allograft,
breakeven analysis revealed that implanting between 34 and 63 allografts
per year equated to the total cost of bone banking. Cite this article:
We are currently facing an epidemic of periprosthetic
fractures around the hip. They may occur either during surgery or
post-operatively. Although the acetabulum may be involved, the femur
is most commonly affected. We are being presented with new, difficult
fracture patterns around cemented and cementless implants, and we
face the challenge of an elderly population who may have grossly
deficient bone and may struggle to rehabilitate after such injuries.
The correct surgical management of these fractures is challenging.

To date, all surgical techniques used for reconstruction
of the pelvic ring following supra-acetabular tumour resection produce
high complication rates. We evaluated the clinical, oncological
and functional outcomes of a cohort of 35 patients (15 men and 20
women), including 21 Ewing’s sarcomas, six chondrosarcomas, three sarcomas
not otherwise specified, one osteosarcoma, two osseous malignant
fibrous histiocytomas, one synovial cell sarcoma and one metastasis.
The mean age of the patients was 31 years (8 to 79) and the latest
follow-up was carried out at a mean of 46 months (1.9 to 139.5)
post-operatively. We undertook a functional reconstruction of the pelvic ring using
polyaxial screws and titanium rods. In 31 patients (89%) the construct
was encased in antibiotic-impregnated polymethylmethacrylate. Preservation
of the extremities was possible for all patients. The survival rate
at three years was 93.9% (95% confidence interval (CI) 77.9 to 98.4),
at five years it was 82.4% (95% CI 57.6 to 93.4). For the 21 patients
with Ewing’s sarcoma it was 95.2% (95% CI 70.7 to 99.3) and 81.5%
(95% CI 52.0 to 93.8), respectively. Wound healing problems were
observed in eight patients, deep infection in five and clinically
asymptomatic breakage of the screws in six. The five-year implant survival
was 93.3% (95% CI 57.8 to 95.7). Patients were mobilised at a mean
of 3.5 weeks (1 to 7) post-operatively. A post-operative neurological
defect occurred in 12 patients. The mean Musculoskeletal Tumor Society
score at last available follow-up was 21.2 (10 to 27). This reconstruction technique is characterised by simple and
oncologically appropriate applicability, achieving high primary
stability that allows early mobilisation, good functional results

We report the results at a mean of 24.3 years
(20 to 32) of 61 previously reported consecutive total hip replacements carried
out on 44 patients with severe congenital hip disease, performed
with reconstruction of the acetabulum with an impaction grafting
technique known as cotyloplasty. The mean age of the patients at
operation was 46.7 years (23 to 68) and all were women. The patients
were followed post-operatively for a mean of 24.3 years (20 to 32), using
the Merle d’Aubigné and Postel scoring system as modified by Charnley,
and with serial radiographs. At the time of the latest follow-up,
28 acetabular components had been revised because of aseptic loosening
at a mean of 15.9 years (6 to 26), and one at 40 days after surgery
because of repeated dislocations. The overall survival rate for aseptic
failure of the acetabular component at ten years was 93.1% (95%
confidence interval (CI) 86.5 to 96.7) when 53 hips were at risk,
and at 23 years was 56.1% (95% CI 49.4 to 62.8), when 22 hips remained
at risk. These long-term results are considered satisfactory for
the reconstruction of an acetabulum presenting with inadequate bone

Ankle replacements have improved significantly since the first reported attempt at resurfacing of the talar dome in 1962. We are now at a stage where ankle replacement offers a viable option in the treatment of end-stage ankle arthritis. As the procedure becomes more successful, it is important to reflect and review the current surgical outcomes. This allows us to guide our patients in the treatment of end-stage ankle arthritis. What is the better surgical treatment – arthrodesis or replacement?
Total hip replacement (THR) after acetabular
fracture presents unique challenges to the orthopaedic surgeon.
The majority of patients can be treated with a standard THR, resulting
in a very reasonable outcome. Technical challenges however include
infection, residual pelvic deformity, acetabular bone loss with
ununited fractures, osteonecrosis of bone fragments, retained metalwork,
heterotopic ossification, dealing with the sciatic nerve, and the
difficulties of obtaining long-term acetabular component fixation.
Indications for an acute THR include young patients with both femoral
head and acetabular involvement with severe comminution that cannot
be reconstructed, and the elderly, with severe bony comminution.
The outcomes of THR for established post-traumatic arthritis include
excellent pain relief and functional improvements. The use of modern
implants and alternative bearing surfaces should improve outcomes

The ‘jumbo’ acetabular component is now commonly
used in acetabular revision surgery where there is extensive bone
loss. It offers high surface contact, permits weight bearing over
a large area of the pelvis, the need for bone grafting is reduced
and it is usually possible to restore centre of rotation of the
hip. Disadvantages of its use include a technique in which bone
structure may not be restored, a risk of excessive posterior bone
loss during reaming, an obligation to employ screw fixation, limited
bone ingrowth with late failure and high hip centre, leading to increased
risk of dislocation. Contraindications include unaddressed pelvic
dissociation, inability to implant the component with a rim fit,
and an inability to achieve screw fixation. Use in acetabulae with
<
50% bone stock has also been questioned. Published results
have been encouraging in the first decade, with late failures predominantly because
of polyethylene wear and aseptic loosening. Dislocation is the most
common complication of jumbo acetabular revisions, with an incidence
of approximately 10%, and often mandates revision. Based on published results,
a hemispherical component with an enhanced porous coating, highly
cross-linked polyethylene, and a large femoral head appears to represent
the optimum tribology for jumbo acetabular revisions. Cite this article:
In developmental dysplasia of the hip (DDH),
a bone defect is often observed superior to the acetabulum after
the reconstruction at the level of the true acetabulum during total
hip replacement (THR). However, the essential amount of uncemented
acetabular component coverage required for a satisfactory outcome
remains controversial. The purpose of this study was to assess the
stability and function of acetabular components with a lack of coverage >
30% (31% to 50%). A total of 760 DDH patients underwent THR with
acetabular reconstruction at the level of the true floor. Lack of
coverage above the acetabular component of >
30% occurred in 56
patients. Intra-operatively, autogenous morcellised bone grafts
were used to fill the uncovered portion. Other than two screws inserted through
the acetabular shell, no additional structural supports were used
in these hips. In all, four patients were lost to follow-up. Therefore,
52 patients (52 hips, 41 women and 11 men) with a mean age of 60.1
years (42 to 78) were available for this study at a mean of 4.8
years (3 to 7). There were no instances of prosthesis revision or

To evaluate the applicability of MRI for the quantitative assessment
of anterior talofibular ligaments (ATFLs) in symptomatic chronic
ankle instability (CAI). Between 1997 and 2010, 39 patients with symptomatic CAI underwent
surgical treatment (22 male, 17 female, mean age 25.4 years (15
to 40)). In all patients, the maximum diameters of the ATFLs were
measured on pre-operative T2-weighted MR images in planes parallel
to the path of the ATFL. They were classified into three groups based
on a previously published method with modifications: ‘normal’, diameter
= 1.0 - 3.2 mm; ‘thickened’, diameter >
3.2 mm; ‘thin or absent’,
diameter <
1.0 mm. Stress radiography was performed with the
maximum manual force in inversion under general anaesthesia immediately
prior to surgery. In surgery, ATFLs were macroscopically divided
into two categories: ‘thickened’, an obvious thickened ligament
and ‘thin or absent’. The imaging results were compared with the
macroscopic results that are considered to be of a gold standard.Objectives

The aim of this study was to compare the operating
time, length of stay (LOS), adverse events and rate of re-admission
for elderly patients with a fracture of the hip treated using either
general or spinal anaesthesia. Patients aged ≥ 70 years who underwent
surgery for a fracture of the hip between 2010 and 2012 were identified
from the American College of Surgeons National Surgical Quality
Improvement Program (ACS-NSQIP) database. Of the 9842 patients who
met the inclusion criteria, 7253 (73.7%) were treated with general
anaesthesia and 2589 (26.3%) with spinal anaesthesia. On propensity-adjusted
multivariate analysis, general anaesthesia was associated with slightly increased
operating time (+5 minutes, 95% confidence interval (CI) +4 to +6,
p <
0.001) and post-operative time in the operating room (+5
minutes, 95% CI +2 to +8, p <
0.001) compared with spinal anaesthesia.
General anaesthesia was associated with a shorter LOS (hazard ratio
(HR) 1.28, 95% CI 1.22 to 1.34, p <

0.001), thromboembolic

The major problem with repair of an articular cartilage injury
is the extensive difference in the structure and function of regenerated,
compared with normal cartilage. Our work investigates the feasibility
of repairing articular osteochondral defects in the canine knee
joint using a composite lamellar scaffold of nano-ß-tricalcium phosphate
(ß-TCP)/collagen (col) I and II with bone marrow stromal stem cells
(BMSCs) and assesses its biological compatibility. The bone–cartilage scaffold was prepared as a laminated composite,
using hydroxyapatite nanoparticles (nano-HAP)/collagen I/copolymer
of polylactic acid–hydroxyacetic acid as the bony scaffold, and
sodium hyaluronate/poly(lactic-co-glycolic acid) as the cartilaginous
scaffold. Ten-to 12-month-old hybrid canines were randomly divided
into an experimental group and a control group. BMSCs were obtained
from the iliac crest of each animal, and only those of the third
generation were used in experiments. An articular osteochondral
defect was created in the right knee of dogs in both groups. Those
in the experimental group were treated by implanting the composites
consisting of the lamellar scaffold of ß-TCP/col I/col II/BMSCs.
Those in the control group were left untreated.Objectives
Methods
Revision total hip replacement (THR) for young
patients is challenging because of technical complexity and the potential
need for subsequent further revisions. We have assessed the survivorship,
functional outcome and complications of this procedure in patients
aged <
50 years through a large longitudinal series with consistent treatment
algorithms. Of 132 consecutive patients (181 hips) who underwent
revision THR, 102 patients (151 hips) with a mean age of 43 years
(22 to 50) were reviewed at a mean follow-up of 11 years (2 to 26)
post-operatively. We attempted to restore bone stock with allograft
where indicated. Using further revision for any reason as an end point,
the survival of the acetabular component was 71% ( This overall perspective on the mid- to long-term results is
valuable when advising young patients on the prospects of revision

We describe our technique and rationale using
hybrid fixation for primary total hip arthroplasty (THA) at the Hospital
for Special Surgery. Modern uncemented acetabular components have
few screw holes, or no holes, polished inner surfaces, improved
locking mechanisms, and maximised thickness and shell-liner conformity. Uncemented
sockets can be combined with highly cross-linked polyethylene liners,
which have demonstrated very low wear and osteolysis rates after
ten to 15 years of implantation. The results of cement fixation
with a smooth or polished surface finished stem have been excellent,
virtually eliminating complications seen with cementless fixation
like peri-operative femoral fractures and thigh pain. Although mid-term
results of modern cementless stems are encouraging, the long-term
data do not show reduced revision rates for cementless stems compared
with cemented smooth stems. In this paper we review the conduct
of a hybrid THA, with emphasis on pre-operative planning, surgical
technique, hypotensive epidural anaesthesia, and intra-operative

The aim of this review is to address controversies
in the management of dislocations of the acromioclavicular joint. Current
evidence suggests that operative rather than non-operative treatment
of Rockwood grade III dislocations results in better cosmetic and
radiological results, similar functional outcomes and longer time
off work. Early surgery results in better functional and radiological
outcomes with a reduced risk of infection and loss of reduction compared
with delayed surgery. Surgical options include acromioclavicular fixation, coracoclavicular
fixation and coracoclavicular ligament reconstruction. Although
non-controlled studies report promising results for arthroscopic
coracoclavicular fixation, there are no comparative studies with
open techniques to draw conclusions about the best surgical approach.
Non-rigid coracoclavicular fixation with tendon graft or synthetic
materials, or rigid acromioclavicular fixation with a hook plate,
is preferable to fixation with coracoclavicular screws owing to
significant risks of loosening and breakage. The evidence, although limited, also suggests that anatomical
ligament reconstruction with autograft or certain synthetic grafts
may have better outcomes than non-anatomical transfer of the coracoacromial
ligament. It has been suggested that this is due to better restoration
horizontal and vertical stability of the joint. Despite the large number of recently published studies, there
remains a lack of high-quality evidence, making it difficult to
draw firm conclusions regarding these controversial issues. Cite this article:
