The morphometry of the distal femur was largely studied to improve
bone-implant fit in total knee arthroplasty (TKA), but little is
known about the asymmetry of the posterior condyles. This study
aimed to investigate the dimensions of the posterior condyles and
the influence of externally rotating the femoral component on potential prosthetic
overhang or under-coverage. We analysed the shape of 110 arthritic knees at the time of primary
TKA using pre-operative CT scans. The height and width of each condyle
were measured at the posterior femoral cut in neutral position,
and in 3º and 5º of external rotation, using both central and medial
referencing systems. We compared the morphological characteristics
with those of 14 TKA models.Aims

The purpose of this study was to evaluate whether an innovative
templating technique could predict the need for acetabular augmentation
during primary total hip arthroplasty for patients with dysplastic
hips. We developed a simple templating technique to estimate acetabular
component coverage at total hip arthroplasty, the True Cup: False
Cup (TC:FC) ratio. We reviewed all patients with dysplastic hips
who underwent primary total hip arthroplasty between 2005 and 2012.
Traditional radiological methods of assessing the degree of acetabular dysplasia
(Sharp’s angle, Tönnis angle, centre-edge angle) as well as the
TC:FC ratio were measured from the pre-operative radiographs. A
comparison of augmented and non-augmented hips was undertaken to
determine any difference in pre-operative radiological indices between
the two cohorts. The intra- and inter-observer reliability for all
radiological indices used in the study were also calculated.Aims

Many studies have investigated the kinematics of the lumbar spine and the morphological features of the lumbar discs. However, the segment-dependent immediate changes of the lumbar intervertebral space height during flexion-extension motion are still unclear. This study examined the changes of intervertebral space height during flexion-extension motion of lumbar specimens. First, we validated the accuracy and repeatability of a custom-made mechanical loading equipment set-up. Eight lumbar specimens underwent CT scanning in flexion, neural, and extension positions by using the equipment set-up. The changes in the disc height and distance between adjacent two pedicle screw entry points (DASEP) of the posterior approach at different lumbar levels (L3/4, L4/5 and L5/S1) were examined on three-dimensional lumbar models, which were reconstructed from the CT images.Objectives
Methods
We report our experience of using a computer
navigation system to aid resection of malignant musculoskeletal tumours
of the pelvis and limbs and, where appropriate, their subsequent
reconstruction. We also highlight circumstances in which navigation
should be used with caution. We resected a musculoskeletal tumour from 18 patients (15 male,
three female, mean age of 30 years (13 to 75) using commercially
available computer navigation software (Orthomap 3D) and assessed
its impact on the accuracy of our surgery. Of nine pelvic tumours,
three had a biological reconstruction with extracorporeal irradiation,
four underwent endoprosthetic replacement (EPR) and two required
no bony reconstruction. There were eight tumours of the bones of
the limbs. Four diaphyseal tumours underwent biological reconstruction.
Two patients with a sarcoma of the proximal femur and two with a
sarcoma of the proximal humerus underwent extra-articular resection
and, where appropriate, EPR. One soft-tissue sarcoma of the adductor
compartment which involved the femur was resected and reconstructed
using an EPR. Computer navigation was used to aid reconstruction
in eight patients. Histological examination of the resected specimens revealed tumour-free
margins in all patients. Post-operative radiographs and CT showed
that the resection and reconstruction had been carried out as planned
in all patients where navigation was used. In two patients, computer
navigation had to be abandoned and the operation was completed under
CT and radiological control. The use of computer navigation in musculoskeletal oncology allows
accurate identification of the local anatomy and can define the
extent of the tumour and proposed resection margins. Furthermore,
it helps in reconstruction of limb length, rotation and overall

These rotations have a direct effect on the functional orientation
of the acetabulum. The aim of this study was to quantify changes
in pelvic tilt between different functional positions. Pre-operatively, pelvic tilt was measured in 1517 patients undergoing
total hip arthroplasty (THA) in three functional positions – supine,
standing and flexed seated (the moment when patients initiate rising
from a seated position). Supine pelvic tilt was measured from CT
scans, standing and flexed seated pelvic tilts were measured from standardised
lateral radiographs. Anterior pelvic tilt was assigned a positive

of the hip, producing real time osseous models of the pelvis and
femur being moved through flexion, adduction, and internal rotation.
The location and size of the cam deformity and its relationship
to the angle of flexion of the hip and pelvic tilt, and the position

In this paper we propose a new classification
of neurogenic peri-articular heterotopic ossification (HO) of the
hip based on three-dimensional (3D) CT, with the aim of improving
pre-operative planning for its excision. A total of 55 patients (73 hips) with clinically significant
HO after either traumatic brain or spinal cord injury were assessed
by 3D-CT scanning, and the results compared with the intra-operative
findings. At operation, the gross pathological anatomy of the HO as identified
by 3D-CT imaging was confirmed as affecting the peri-articular hip
muscles to a greater or lesser extent. We identified seven patterns
of involvement: four basic (anterior, medial, posterior and lateral)
and three mixed (anteromedial, posterolateral and circumferential).
Excellent intra- and inter-observer agreement, with kappa values
>
0.8, confirmed the reproducibility of the classification system. We describe the different surgical approaches used to excise
the HO which were guided by the 3D-CT findings. Resection was always
successful. 3D-CT imaging, complemented in some cases by angiography, allows
the surgeon to define the 3D anatomy of the HO accurately and to

This study sought to establish the prevalence of the cross over
sign (COS) and posterior wall sign (PWS) in relation to the anterior
pelvic plane (APP) in an asymptomatic population through reliable
and accurate 3D-CT based assessment. Data from pelvic CT scans of 100 asymptomatic subjects (200 hips)
undertaken for conditions unrelated to disorders of the hip were
available for analysis in this study. A previously established 3D
analysis method was applied to assess the prevalence of the COS
and PWS in relation to the APP.Aims

Our aim was to compare kinematic with mechanical alignment in
total knee arthroplasty (TKA). We performed a prospective blinded randomised controlled trial
to compare the functional outcome of patients undergoing TKA in
mechanical alignment (MA) with those in kinematic alignment (KA).
A total of 71 patients undergoing TKA were randomised to either
kinematic (n = 36) or mechanical alignment (n = 35). Pre- and post-operative
hip-knee-ankle radiographs were analysed. The knee injury and osteoarthritis
outcome score (KOOS), American Knee Society Score, Short Form-36,
Euro-Qol (EQ-5D), range of movement (ROM), two minute walk, and timed
up and go tests were assessed pre-operatively and at six weeks,
three and six months and one year post-operatively.Aims

We have observed clinical cases where bone is formed in the overlaying muscle covering surgically created bone defects treated with a hydroxyapatite/calcium sulphate biomaterial. Our objective was to investigate the osteoinductive potential of the biomaterial and to determine if growth factors secreted from local bone cells induce osteoblastic differentiation of muscle cells. We seeded mouse skeletal muscle cells C2C12 on the hydroxyapatite/calcium sulphate biomaterial and the phenotype of the cells was analysed. To mimic surgical conditions with leakage of extra cellular matrix (ECM) proteins and growth factors, we cultured rat bone cells ROS 17/2.8 in a bioreactor and harvested the secreted proteins. The secretome was added to rat muscle cells L6. The phenotype of the muscle cells after treatment with the media was assessed using immunostaining and light microscopy.Objectives

This study aimed to determine the relationship between pedicle-lengthening
distance and bulge-canal volume ratio in cases of lumbar spinal
stenosis, to provide a theoretical basis for the extent of lengthening
in pedicle-lengthening osteotomies. Three-dimensional reconstructions of CT images were performed
for 69 patients (33 men and 36 women) (mean age 49.96 years; 24
to 81). Simulated pedicle-lengthening osteotomies and disc bulge
and spinal canal volume calculations were performed using Mimics

Analysis of the morphology of the distal femur, and by extension
of the femoral components in total knee arthroplasty (TKA), has
largely been related to the aspect ratio, which represents the width
of the femur. Little is known about variations in trapezoidicity
(i.e. whether the femur is more rectangular or more trapezoidal).
This study aimed to quantify additional morphological characteristics
of the distal femur and identify anatomical features associated
with higher risks of over- or under-sizing of components in TKA. We analysed the shape of 114 arthritic knees at the time of primary
TKA using the pre-operative CT scans. The aspect ratio and trapezoidicity
ratio were quantified, and the post-operative prosthetic overhang
was calculated. We compared the morphological characteristics with
those of 12 TKA models.Aims
Methods
Penetration of the dorsal screw when treating
distal radius fractures with volar locking plates is an avoidable complication
that causes lesions of the extensor tendon in between 2% and 6%
of patients. We examined axial fluoroscopic views of the distal
end of the radius to observe small amounts of dorsal screw penetration,
and determined the ideal angle of inclination of the x-ray beam
to the forearm when making this radiological view. Six volar locking plates were inserted at the wrists of cadavers.
The actual screw length was measured under direct vision through
a dorsal approach to the distal radius. Axial radiographs were performed
for different angles of inclination of the forearm at the elbow. Comparing axial radiological measurements and real screw length,
a statistically significant correlation could be demonstrated at
an angle of inclination between 5° and 20°. The ideal angle of inclination
required to minimise the risk of implanting over-long screws in
a dorsal horizon radiological view is 15°. Cite this article:
Neuropathic changes in the foot are common with
a prevalence of approximately 1%. The diagnosis of neuropathic arthropathy
is often delayed in diabetic patients with harmful consequences
including amputation. The appropriate diagnosis and treatment can
avoid an extensive programme of treatment with significant morbidity
for the patient, high costs and delayed surgery. The pathogenesis
of a Charcot foot involves repetitive micro-trauma in a foot with impaired
sensation and neurovascular changes caused by pathological innervation
of the blood vessels. In most cases, changes are due to a combination
of both pathophysiological factors. The Charcot foot is triggered
by a combination of mechanical, vascular and biological factors
which can lead to late diagnosis and incorrect treatment and eventually
to destruction of the foot. This review aims to raise awareness of the diagnosis of the Charcot
foot (diabetic neuropathic osteoarthropathy and the differential
diagnosis, erysipelas, peripheral arterial occlusive disease) and
describe the ways in which the diagnosis may be made. The clinical

The objective of this study was to evaluate the rotation and
translation of each joint in the hindfoot and compare the load response
in healthy feet with that in stage II posterior tibial tendon dysfunction
(PTTD) flatfoot by analysing the reconstructive three-dimensional
(3D) computed tomography (CT) image data during simulated weight-bearing. CT scans of 15 healthy feet and 15 feet with stage II PTTD flatfoot
were taken first in a non-weight-bearing condition, followed by
a simulated full-body weight-bearing condition. The images of the
hindfoot bones were reconstructed into 3D models. The ‘twice registration’
method in three planes was used to calculate the position of the
talus relative to the calcaneus in the talocalcaneal joint, the
navicular relative to the talus in talonavicular joint, and the cuboid
relative to the calcaneus in the calcaneocuboid joint.Objective
Methods
The cam-type deformity in femoroacetabular impingement
is a 3D deformity. Single measurements using radiographs, CT or
MRI may not provide a true estimate of the magnitude of the deformity.
We performed an analysis of the size and location of measurements
of the alpha angle (α°) using a CT technique which could be applied
to the 3D reconstructions of the hip. Analysis was undertaken in
42 patients (57 hips; 24 men and 18 women; mean age 38 years (16
to 58)) who had symptoms of femoroacetabular impingement related
to a cam-type abnormality. An α° of >
50° was considered a significant
indicator of cam-type impingement. Measurements of the α° were made
at different points around the femoral head/neck junction at intervals
of 30°: starting at the nine o’clock (posterior), ten, eleven and
twelve o’clock (superior), one, two and ending at three o’clock
(anterior) position. The mean maximum increased α° was 64.6° (50.8° to 86°). The two
o’clock position was the most common point to find an increased α°
(53 hips; 93%), followed by one o’clock (48 hips; 84%). The largest α°
for each hip was found most frequently at the two o’clock position
(46%), followed by the one o’clock position (39%). Generally, raised α angles
extend over three segments of the clock face. Single measurements of the α°, whether pre- or post-operative,
should be viewed with caution as they may not be representative
of the true size of the deformity and not define whether adequate
correction has been achieved following surgery. Cite this article:
We reviewed seven children with torticollis due to refractory atlanto-axial rotatory fixation who were treated in a halo vest. Pre-operative three-dimensional CT and sagittal CT imaging showed deformity of the superior articular process of C2 in all patients. The mean duration of halo vest treatment was 67 days (46 to 91). The mean follow-up was 34 months (8 to 73); at the latest review six patients demonstrated remodelling of the deformed articular process. The other child, who had a more severe deformity, required C1-2 fusion. We suggest that patients with atlanto-axial rotatory fixation who do not respond to conservative treatment and who have deformity of the superior articular process of C2 should undergo manipulative reduction and halo-vest fixation for two to three months to induce remodelling of the deformed superior articular process before C1-2 fusion is considered.
In this study we randomised 140 patients who
were due to undergo primary total knee arthroplasty (TKA) to have the
procedure performed using either patient-specific cutting guides
(PSCG) or conventional instrumentation (CI). The primary outcome measure was the mechanical axis, as measured
at three months on a standing long-leg radiograph by the hip–knee–ankle
(HKA) angle. This was undertaken by an independent observer who
was blinded to the instrumentation. Secondary outcome measures were
component positioning, operating time, Knee Society and Oxford knee
scores, blood loss and length of hospital stay. A total of 126 patients (67 in the CI group and 59 in the PSCG
group) had complete clinical and radiological data. There were 88
females and 52 males with a mean age of 69.3 years (47 to 84) and
a mean BMI of 28.6 kg/m2 (20.2 to 40.8). The mean HKA
angle was 178.9° (172.5 to 183.4) in the CI group and 178.2° (172.4
to 183.4) in the PSCG group (p = 0.34). Outliers were identified
in 22 of 67 knees (32.8%) in the CI group and 19 of 59 knees (32.2%)
in the PSCG group (p = 0.99). There was no significant difference
in the clinical results (p = 0.95 and 0.59, respectively). Operating time,
blood loss and length of hospital stay were not significantly reduced
(p = 0.09, 0.58 and 0.50, respectively) when using PSCG. The use of PSCG in primary TKA did not reduce the proportion
of outliers as measured by post-operative coronal alignment. Cite this article:
