We performed a randomised controlled trial comparing
computer-assisted surgery (CAS) with conventional surgery (CONV)
in total knee replacement (TKR). Between 2009 and 2011 a total of
192 patients with a mean age of 68 years (55 to 85) with osteoarthritis
or arthritic disease of the knee were recruited from four Norwegian
hospitals. At three months follow-up, functional results were marginally
better for the CAS group. Mean differences (MD) in favour of CAS
were found for the Knee Society function score (MD: 5.9, 95% confidence
interval (CI) 0.3 to 11.4, p = 0.039), the Knee Injury and Osteoarthritis
Outcome Score (KOOS) subscales for ‘pain’ (MD: 7.7, 95% CI 1.7 to
13.6, p = 0.012), ‘sports’ (MD: 13.5, 95% CI 5.6 to 21.4, p = 0.001)
and ‘quality of life’ (MD: 7.2, 95% CI 0.1 to 14.3, p = 0.046).
At one-year follow-up, differences favouring CAS were found for
KOOS ‘sports’ (MD: 11.0, 95% CI 3.0 to 19.0, p = 0.007) and KOOS
‘symptoms’ (MD: 6.7, 95% CI 0.5 to 13.0, p = 0.035). The use of
CAS resulted in fewer outliers in frontal alignment (>
3° malalignment),
both for the entire TKR (37.9% vs 17.9%, p = 0.042)
and for the tibial component separately (28.4% vs 6.3%,
p = 0.002). Tibial slope was better achieved with CAS (58.9% vs 26.3%,

The aim of this study was to analyze the results of two radiocapitellar prostheses in a large case series followed prospectively, with medium-term follow-up. A total of 31 patients with a mean age of 54 years (27 to 73) were analyzed; nine had primary osteoarthritis (OA) and 17 had post-traumatic OA, three had capitellar osteonecrosis, and two had a fracture. Overall, 17 Lateral Resurfacing Elbow (LRE) and 14 Uni-Elbow Radio-Capitellum Implant (UNI-E) arthroplasties were performed. most common hip disorder to affect adolescents. Controversy exists over
the optimal treatment of severe slips, with a continuing debate
between in situ fixation versus corrective surgery.
We present our experience in a series of 57 patients presenting
with severe unilateral SUFE (defined >
50°) managed with a subcapital
cuneiform osteotomy. Between 2001 and 2011, 57 patients (35 male, 22 female) with
a mean age of 13.1 years (9.6 to 20.3, SD 2.3) were referred to
our tertiary referral institution with a severe slip. The affected
limb was rested in slings and springs before corrective surgery
which was performed via an anterior Smith-Petersen approach. Radiographic
analysis confirmed an improvement in mean head–shaft slip angle
from 53.8. o. (standard deviation (. sd) 3.2). pre-operatively
to 9.1. o. (. sd. 3.1) post-operatively, with minimal
associated femoral neck shortening. In total 50 (88%) patients were complication
free at a mean follow-up of seven years (2.8 to 13.9 years, . sd. 3).
Their mean Oxford hip score was 44 (37 to 48) and median visual
analogue pain score was 0 out of 10 (interquartile range 0 to 4).

Low haemoglobin (Hb) at admission has been identified as a risk factor for mortality for elderly patients with hip fractures in some studies. However, this remains controversial. This study aims to analyze the association between Hb level at admission and mortality in elderly patients with hip fracture undergoing surgery. All consecutive patients (prospective database) admitted with hip fracture operated in a tertiary hospital between 2012 and 2016 were analyzed. We collected patient characteristics, time to surgery, duration and type of surgery, comorbidities, Hb at admission, nadir of Hb after surgery, the use and amount of red blood cells (RBCs) transfusion products, postoperative complications, and death. We investigated the clinical outcome of internal
fixation for pathological fracture of the femur after primary excision of
a soft-tissue sarcoma that had been treated with adjuvant radiotherapy. A review of our database identified 22 radiation-induced fractures
of the femur in 22 patients (seven men, 15 women). We noted the
mechanism of injury, fracture pattern and any complications after
internal fixation, including nonunion, hardware failure, secondary
fracture or deep infection. The mean age of the patients at primary excision of the tumour
was 58.3 years (39 to 86). The mean time from primary excision to
fracture was 73.2 months (2 to 195). The mean follow-up after fracture
fixation was 65.9 months (12 to 205). Complications occurred in
19 patients (86%). Nonunion developed in 18 patients (82%), of whom
11 had a radiological nonunion at 12 months, five a nonunion and
hardware failure and two an infected nonunion. One patient developed
a second radiation-associated fracture of the femur after internal
fixation and union of the initial fracture. A total of 13 patients
(59%) underwent 24 revision
The aim of this study was to explore risk factors for complications associated with dural tear (DT), including the types of DT, and the intra- and postoperative management of DT. Between 2012 and 2017, 12 171 patients with degenerative lumbar diseases underwent primary lumbar spine surgery. We investigated five categories of potential predictors: patient factors (sex, age, body mass index, and primary disease), surgical factors (surgical procedures, operative time, and estimated blood loss), types of DT (inaccessible for suturing/clipping and the presence of cauda equina/nerve root herniation), repair techniques (suturing, clipping, fibrin glue, polyethylene glycol (PEG) hydrogel, and polyglycolic acid sheet), and postoperative management (drainage duration). Postoperative complications were evaluated in terms of dural leak, prolonged bed rest, headache, nausea/vomiting, delayed wound healing, postoperative neurological deficit, surgical site infection (SSI), and reoperation for DT. We performed multivariable regression analyses to evaluate the predictors of postoperative complications associated with DT.Aims

A total of 187 patients with primary osteoarthritis
(OA) of the knee undergoing total knee replacement (TKR) were randomly
divided into two groups, one of which underwent synovectomy. The

Opioid prescribing has increased in the last two decades, and recently states in the USA have developed online Prescription Drug Monitoring Programs to prevent overprescribing of controlled substances. This study evaluates differences in opioid requirements between patients undergoing TKA and UKA. We retrospectively reviewed 676 consecutive TKAs and 241 UKAs. Opioid prescriptions in morphine milligram equivalents (MMEs), sedatives, benzodiazepines, and stimulants were collected from State Controlled Substance Monitoring websites six months before and nine months after the initial procedures.
