The recognition of hips at risk of displacement
in children with cerebral palsy (CP) is a difficult problem for
the orthopaedic surgeon. The Gross Motor Function Classification
System (GMFCS) and head–shaft angle (HSA) are prognostic factors
for hip displacement. However, reference values for HSA are lacking.
This study describes and compares the development of HSA in normal
hips and children with CP. We selected 33 children from a retrospective cohort with unilateral
developmental dysplasia of the hip (DDH) (five boys, 28 girls) and
50 children (35 boys, 15 girls) with CP with GMFCS levels II to
V. HSA of normal developing hips was measured at the contralateral
hip of unilateral DDH children (33 hips) and HSA of CP children
was measured in both hips (100 hips). Measurements were taken from
the radiographs of the children at age two, four and seven years.
The normal hip HSA decreased by 2° per year (p <
0.001). In children
with CP with GMFCS levels II and III HSA decreased by 0.6° (p =
0.046) and 0.9° (p = 0.049) per year, respectively. The HSA did
not alter significantly in GMFCS levels IV and V. Between the ages of two and eight years, the HSA decreases in
normal hips and CP children with GMFCS level, II to III but does
not change in GMFCS levels IV to V. As HSA has a prognostic value
for hip displacement, these reference values may help the orthopaedic
surgeon to predict future hip displacement in children with CP. Cite this article:
The purpose of this study was to determine patient-reported
outcomes of patients with mild to moderate developmental dysplasia
of the hip (DDH) and femoroacetabular impingement (FAI) undergoing
arthroscopy of the hip in the treatment of chondrolabral pathology.
A total of 28 patients with a centre-edge angle between 15° and
19° were identified from an institutional database. Their mean age
was 34 years (18 to 53), with 12 female and 16 male patients. All
underwent labral treatment and concomitant correction of FAI. There
were nine reoperations, with two patients requiring revision arthroscopy,
two requiring periacetabular osteotomy and five needing total hip arthroplasty. Patients who required further major surgery were more likely
to be older, male, and to have more severe DDH with a larger alpha
angle and decreased joint space. At a mean follow-up of 42 months (24 to 89), the mean modified
Harris hip score improved from 59 (20 to 98) to 82 (45 to 100; p

0.001). The mean Western Ontario and McMaster Universities
Osteoarthritis Index score improved from 30 (1 to 61) to 16 (0 to

0.001). Median patient satisfaction was 9.0/10 (1 to
10). Patients reported excellent improvement in function following
arthroscopy of the hip. This study shows that with proper patient selection, arthroscopy
of the hip can be successful in the young patient with mild to moderate

We report patient-reported outcomes and complications
associated with contemporary periacetabular osteotomy (PAO) surgery
in treating symptomatic acetabular dysplasia and compare these outcomes
with total hip arthroplasty (THA) in patients with similar demographic
details. Two consecutive cohorts included patients between aged
18 to 40 years who had undergone either PAO (100 hips; 24 male,
76 female) or THA (55 hips; 18 male, 37 female). At a mean follow-up
of 5.9 years (2 to 13), there was significant improvement in the
modified Harris hip pain (p <

0.001, THA),
function (p <
0.001, PAO and p = 0.001, THA), and total scores

were no significant differences in the clinical outcome scores between

The purpose of this study was to evaluate treatment
results following arthroscopic triangular fibrocartilage complex (TFCC)
debridement for recalcitrant ulnar wrist pain. According to the
treatment algorithm, 66 patients (36 men and 30 women with a mean
age of 38.1 years (15 to 67)) with recalcitrant ulnar wrist pain
were allocated to undergo ulnar shortening osteotomy (USO; n = 24),
arthroscopic TFCC repair (n = 15), arthroscopic TFCC debridement
(n = 14) or prolonged conservative treatment (n = 13). The mean
follow-up was 36.0 months (15 to 54). Significant differences in
Hand20 score at 18 months were evident between the USO group and
TFCC debridement group (p = 0.003), and between the TFCC repair
group and TFCC debridement group (p = 0.029). Within-group comparisons showed
that Hand20 score at five months or later and pain score at two
months or later were significantly decreased in the USO/TFCC repair
groups. In contrast, scores in the TFCC debridement/conservative
groups did not decrease significantly. Grip strength at 18 months
was significantly improved in the USO/TFCC repair groups, but not
in the TFCC debridement/conservative groups. TFCC debridement shows
little benefit on the clinical course of recalcitrant ulnar wrist
pain even after excluding patients with ulnocarpal abutment or TFCC
detachment from the fovea from the indications for arthroscopic

A total of 22 patients with a tibial avulsion
fracture involving the insertion of the posterior cruciate ligament
(PCL) with grade II or III posterior laxity were reduced and fixed
arthroscopically using routine anterior and double posteromedial
portals. A double-strand Ethibond suture was inserted into the joint
and wrapped around the PCL from anterior to posterior to secure
the ligament above the avulsed bony fragment. Two tibial bone tunnels
were created using the PCL reconstruction guide, aiming at the medial
and lateral borders of the tibial bed. The ends of the suture were
pulled out through the bone tunnels and tied over the tibial cortex
between the openings of the tunnels to reduce and secure the bony
fragment. Satisfactory reduction of the fracture was checked arthroscopically and
radiographically. The patients were followed-up for a mean of 24.5 months (19 to
28). Bone union occurred six weeks post-operatively. At final follow-up,
all patients had a negative posterior drawer test and a full range
of movement. KT-1000 arthrometer examination showed that the mean
post-operative side-to-side difference improved from 10.9 mm (standard

invasive and successfully restores the stability and function of

Partial knee arthroplasty (PKA), either medial
or lateral unicompartmental knee artroplasty (UKA) or patellofemoral arthroplasty
(PFA) are a good option in suitable patients and have the advantages
of reduced operative trauma, preservation of both cruciate ligaments
and bone stock, and restoration of normal kinematics within the
knee joint. However, questions remain concerning long-term survival.
The goal of this review article was to present the long-term results
of medial and lateral UKA, PFA and combined compartmental arthroplasty
for multicompartmental disease. Medium- and long-term studies suggest
reasonable outcomes at ten years with survival greater than 95% in

The use of hinged implants in primary total knee
replacement (TKR) should be restricted to selected indications and mainly
for elderly patients. Potential indications for a rotating hinge
or pure hinge implant in primary TKR include: collateral ligament
insufficiency, severe varus or valgus deformity (>
20°) with necessary
relevant soft-tissue release, relevant bone loss including insertions
of collateral ligaments, gross flexion-extension gap imbalance,
ankylosis, or hyperlaxity. Although data reported in the literature
are inconsistent, clinical results depend on implant design, proper
technical use, and adequate indications. We present our experience
with a specific implant type that we have used for over 30 years
and which has given our elderly patients good mid-term results.
Because revision of implants with long cemented stems can be very
challenging, an effort should be made in the future to use shorter stems

This paper investigates whether cortical comminution
and intra-articular involvement can predict displacement in distal
radius fractures by using a classification that includes volar comminution
as a separate parameter. A prospective multicentre study involving non-operative treatment
of distal radius fractures in 387 patients aged between 15 and 74
years (398 fractures) was conducted. The presence of cortical comminution
and intra-articular involvement according to the Buttazzoni classification
is described. Minimally displaced fractures were treated with immobilisation
in a cast while displaced fractures underwent closed reduction with
subsequent immobilisation. Radiographs were obtained after reduction,
at 10 to 14 days and after union. The outcome measure was re-displacement
or union. In fractures with volar comminution (Buttazzoni type 4), 96%
(53 of 55) displaced. In intra-articular fractures without volar
comminution (Buttazzoni 3), 72% (84 of 117) displaced. In extra-articular
fractures with isolated dorsal comminution (Buttazzoni 2), 73% (106
of 145) displaced while in non-comminuted fractures (Buttazzoni
1), 16 % (13 of 81 ) displaced. A total of 32% (53 of 165) of initially minimally displaced fractures
later displaced. All of the initially displaced volarly comminuted
fractures re-displaced. Displacement occurred in 31% (63 of 205)
of fractures that were still in good alignment after 10 to 14 days. Regression analysis showed that volar and dorsal comminution
predicted later displacement, while intra-articular involvement
did not predict displacement. Volar comminution was the strongest

Dysplasia of the hip, hypotonia, osteopenia,
ligamentous laxity, and mental retardation increase the complexity
of performing and managing patients with Down syndrome who require
total hip replacement (THR). We identified 14 patients (six males,
eight females, 21 hips) with Down syndrome and degenerative disease
of the hip who underwent THR, with a minimum follow-up of two years

Intact abductors of the hip play a crucial role
in preventing limping and are known to be damaged through the direct lateral
approach. The extent of trauma to the abductors after revision total
hip replacement (THR) is unknown. The aim of this prospective study
was to compare the pre- and post-operative status of the gluteus
medius muscle after revision THR. We prospectively compared changes
in the muscle and limping in 30 patients who were awaiting aseptic
revision THR and 15 patients undergoing primary THR. The direct
lateral approach as described by Hardinge was used for all patients.
MRI scans of the gluteus medius and functional analyses were recorded
pre-operatively and six months post-operatively. The overall mean
fatty degeneration of the gluteus medius increased from 35.8% (1.1
to 98.8) pre-operatively to 41% (1.5 to 99.8) after multiple revision
THRs (p = 0.03). There was a similar pattern after primary THR,
but with considerably less muscle damage (p = 0.001), indicating
progressive muscle damage. Despite an increased incidence of a positive
Trendelenburg sign following revision surgery (p = 0.03) there was
no relationship between the cumulative fatty degeneration in the
gluteus medius and a positive Trendelenburg sign (p = 0.26). The

The role of arthroscopy in the treatment of soft-tissue
injuries associated with proximal tibial fractures remains debatable.
Our hypothesis was that MRI over-diagnoses clinically relevant associated
soft-tissue injuries. This prospective study involved 50 consecutive
patients who underwent surgical treatment for a split-depression fracture
of the lateral tibial condyle (AO/OTA type B3.1). The mean age of
patients was 50 years (23 to 86) and 27 (54%) were female. All patients
had MRI and arthroscopy. Arthroscopy identified 12 tears of the
lateral meniscus, including eight bucket-handle tears that were
sutured and four that were resected, as well as six tears of the
medial meniscus, of which five were resected. Lateral meniscal injuries
were diagnosed on MRI in four of 12 patients, yielding an overall
sensitivity of 33% (95% confidence interval (CI) 11 to 65). Specificity
was 76% (95% CI 59 to 88), with nine tears diagnosed among 38 menisci
that did not contain a tear. MRI identified medial meniscal injuries
in four of six patients, yielding an overall sensitivity of 67%
(95% CI 24 to 94). Specificity was 66% (95% CI 50 to 79), with 15
tears diagnosed in 44 menisci that did not contain tears. MRI appears to offer only a marginal benefit as the specificity
and sensitivity for diagnosing meniscal injuries are poor in patients
with a fracture. There were fewer arthroscopically-confirmed associated

There have been differing descriptions of the
anterolateral structures of the knee, and not all have been named
or described clearly. The aim of this study was to provide a clear
anatomical interpretation of these structures. We dissected 40 fresh-frozen
cadaveric knees to view the relevant anatomy and identified a consistent
structure in 33 knees (83%); we termed this the anterolateral ligament
of the knee. This structure passes antero-distally from an attachment
proximal and posterior to the lateral femoral epicondyle to the
margin of the lateral tibial plateau, approximately midway between
Gerdy’s tubercle and the head of the fibula. The ligament is superficial
to the lateral (fibular) collateral ligament proximally, from which
it is distinct, and separate from the capsule of the knee. In the
eight knees in which it was measured, we observed that the ligament
was isometric from 0° to 60° of flexion of the knee, then slackened
when the knee flexed further to 90° and was lengthened by imposing

A number of studies have reported satisfactory
results from the isolated revision of an acetabular component. However,
many of these studies reported only the short- to intermediate-term
results of heterogeneous bearing surfaces in a mixed age group. We present our experience of using a ceramic-on-ceramic (CoC)
bearing for isolated revision of an uncemented acetabular component
in 166 patients (187 hips) who were under the age of 50 years at
the time of revision. There were 78 men and 88 women with a mean
age of 47.4 years (28 to 49). The most common reason for revision
was polyethylene wear and acetabular osteolysis in 123 hips (66%),
followed by aseptic loosening in 49 hips (26%). We report the clinical and radiological outcome, complication
rate, and survivorship of this group. The mean duration of follow-up
was 15.6 years (11 to 19). The mean pre-operative Harris hip score was 33 points (1 to 58),
and improved to a mean of 88 points (51 to 100) at follow-up. The
mean pre-operative total Western Ontario and McMaster Universities
Osteoarthritis Index score was 63.2 (43 to 91) and improved to 19.8
points (9 to 61) post-operatively. Overall, 153 of 166 patients
(92%) were satisfied with their outcome. Kaplan–Meier survivorship
analysis, with revision or radiological evidence of implant failure
(13 patients, 8%) as end-points, was 92% at 15 years (95% confidence
interval 0.89 to 0.97). Isolated revision of a cementless acetabular component using

The objective of this study was to validate the
efficacy of Takeuchi classification for lateral hinge fractures
(LHFs) in open wedge high tibial osteotomy (OWHTO). In all 74 osteoarthritic
knees (58 females, 16 males; mean age 62.9 years, standard deviation
7.5, 42 to 77) were treated with OWHTO using a TomoFix plate. The
knees were divided into non-fracture (59 knees) and LHF (15 knees)
groups, and the LHF group was further divided into Takeuchi types
I, II, and III (seven, two, and six knees, respectively). The outcomes
were assessed pre-operatively and one year after OWHTO. Pre-operative
characteristics (age, gender and body mass index) showed no significant
difference between the two groups. The mean Japanese Orthopaedic
Association score was significantly improved one year after operation
regardless of the presence or absence of LHF (p = 0.0015, p <
0.001, respectively). However, six of seven type I cases had no
LHF-related complications; both type II cases had delayed union;
and of six type III cases, two had delayed union with correction
loss and one had overcorrection. These results suggest that Takeuchi
type II and III LHFs are structurally unstable compared with type

Inherent disadvantages of reverse shoulder arthroplasty
designs based on the Grammont concept have raised a renewed interest
in less-medialised designs and techniques. The aim of this study
was to evaluate the outcome of reverse shoulder arthroplasty (RSA)
with the fully-constrained, less-medialised, Bayley–Walker prosthesis performed
for the treatment of rotator-cuff-deficient shoulders with glenohumeral
arthritis. A total of 97 arthroplasties in 92 patients (53 women
and 44 men, mean age 67 years (standard deviation ( The Bayley–Walker prosthesis provides reliable pain relief and
reasonable functional improvement for patients with symptomatic
cuff-deficient shoulders. Compared with other designs of RSA, it
offers a modest improvement in forward elevation, but restores external
rotation to some extent and prevents scapular notching. A longer
follow-up is required to assess the survival of the prosthesis and

The long term biological effects of wear products
following total hip arthroplasty (THA) are unclear. However, the indications
for THA are expanding, with increasingly younger patients undergoing
the procedure. This prospective, randomised study compared two groups of patients
undergoing THA after being randomised to receive one of two different
bearing surfaces: metal-on-polyethylene (MoP) n = 22 and metal-on-metal
(MoM) n = 23. We investigated the relationship between three variables:
bearing surface (MoP Our results demonstrated significantly higher mean cobalt and
chromium (Co and Cr) blood levels in the MoM group at all follow-up
points following surgery (p <
0.01), but there were no significant
differences in the chromosomal aberration indices between MoM and
MoP at two or five years (two years: p = 0.56, p = 0.08, p = 0.91, p
= 0.51 and five years: p = 0.086, p = 0.73, p = 0.06, p = 0.34)
for translocations, breaks, loss and gain of chromosomes respectively.
Regression analysis showed a strong linear relationship between
Cr levels and the total chromosomal aberration indices in the MoM
group (R2 = 0.90016), but this was not as strong for
Co (R2 = 0.68991). In the MoP group, the analysis revealed
a poor relationship between Cr levels and the total chromosomal
aberration indices (R2 = 0.23908) but a slightly stronger
relationship for Co (R2 = 0.64292). Across both groups,
Spearman’s correlation detected no overall association between Co and Cr
levels and each of the studied chromosomal aberrations. There remains
no clear indication which THA bearing couple is the most biocompatible,
especially in young active patients. While THA continues to be very
successful at alleviating pain and restoring function, the long-term
biological implications of the procedure still require further scrutiny. Cite this article:
We are currently facing an epidemic of periprosthetic
fractures around the hip. They may occur either during surgery or
post-operatively. Although the acetabulum may be involved, the femur
is most commonly affected. We are being presented with new, difficult
fracture patterns around cemented and cementless implants, and we
face the challenge of an elderly population who may have grossly
deficient bone and may struggle to rehabilitate after such injuries.
The correct surgical management of these fractures is challenging.
