Blockade of the suprascapular nerve (SSN) with local anaesthetic is used frequently in shoulder surgery and for chronic shoulder pain. Anatomical landmarks may be used to locate the nerve prior to infiltration with local anaesthetic, but ultrasound is becoming a popular to locate the nerve.

Twelve cadaveric shoulders from 6 specimens were injected with dye using the landmark and ultrasound technique. The shoulders were scanned with computed tomography, and then dissected to ascertain the accuracy of each technique.

Using CT scan results, we found the ultrasound group to be more accurate in placing the anaesthetic needle close to the suprascapular notch, and therefore nerve, and this was significant (p = 0.0009). When analysing the ink data, although we did not observe a significant difference in amount of nerve covered by ink, we did note a correlation, and, given this study group is small, that may be considered a statistical trend.

This study, which is one of the largest cadaveric studies investigating landmark and ultrasound guided block of the suprascapular nerve and we believe the first to use CT, demonstrates that ultrasound guided block is significantly more accurate than the landmark technique, and would therefore recommend that ultrasound guidance be used when blocking the suprascapular nerve, given its higher accuracy and lower complication rate.

Total Shoulder Resurfacing (TSR) provides a reliable solution for the treatment of glenohumeral arthritis. It confers a number of advantages over traditional joint replacement with stemmed humeral components, in terms of bone preservation and improved joint kinematics.

This study aimed to determine if humeral reaming instruments produce a thermal insult to subchondral bone during TSR. This was tested in vivo on 13 patients (8 with rheumatoid arthritis and 5 with osteoarthritis) with a single reaming system and in vitro with three different humeral reaming systems on saw bone models. Real-time infrared thermal video imaging was used to assess the temperatures generated.

Synthes Epoca instruments generated average temperatures of 40.7°C (SD 0.9°C) in the rheumatoid group and 56.5°C (SD 0.87°C) in the osteoarthritis group (p = 0.001). Irrigation with room temperature saline cooled the humeral head to 30°C (SD 1.2°C). Saw bone analysis generated temperatures of 58.2°C (SD 0.79°C) in the Synthes (Epoca) 59.9°C (SD 0.81°C) in Biomet (Copeland) and 58.4°C (SD 0.88°C) in the Depuy (CAP) reamers (p=0.12).

Humeral reaming with power driven instruments generates considerable temperatures both in vivo and in vitro. This paper demonstrates that a significant thermal effect beyond the 47°C threshold needed to induce osteonecrosis is observed with humeral reamers, with little variation seen between manufacturers. Irrigation with room temperature saline cools the reamed bone to physiological levels, and should be performed regularly during this step in TSR.

The gold standard for measuring knee alignment is the lower limb mechanical axis. This is traditionally assessed by weight-bearing full length lower limb X-rays (LLX). CT scanograms (CTS) are however, becoming increasingly popular in view of lower radiation exposure, speed and supine positioning. We assessed the correlation and reproducibility of knee joint coronal alignment using these two imaging modalities.

LLX and CTS images were obtained in 24 knees with degenerate joint disease or failed TKR. Hip to ankle mechanical alignment were measured using the PACS software. Coronal knee alignment was assessed from the centre of the knee, measuring the valgus/varus angle relative to the mechanical axis. Measurements were made by two orthopaedic surgeons (Research Fellow and Consultant) on two separate occasions.

The mean alignment angles measured by observers 1 and 2 on CTS were 180.29° (SD 6.04) and 180.71° (SD 6.13) respectively, while on LLX were 181.04° (SD7.58) and 181.04° (SD 7.72). The measurements between the two observers were highly correlated for both the CTS (r = 0.97, p < 0.001) and the LLX (r = 0.99, p < 0.001). The angles measured on CTS and LLX were highly correlated (r = 0.826, p < 0.001) with high degree of internal consistency (ICC = 0.804). Malalignment of greater than 5° was seen in 19% of the CTS and 35% of the LLX.

There was good correlation between CT scanogram and weight-bearing X-ray measurements in normally-aligned knees. However, as expected, in the malaligned lower limb, the influence of weight-bearing is critical which demonstrates the significance of weight-bearing X-rays.

Patello-femoral arthritis can result in a considerable thinning of the patella. The restoration of an adequate patella thickness is key to the successful outcome of knee arthroplasty.

The objectives were (1) to establish a reproducible patella width:thickness index including chondral surface and (2) to investigate whether there is a difference between bone alone and bone/chondral construct thickness as shown by MRI.

Forty three MRI scans of young adults, mean age 27 (range 17–38), 34 male and 9 female, were studied. Exclusion criteria included degenerative joint disease, patello-femoral pathology or age under 16/over 40 (102 patients). The bony and chondral thickness of the patella and its width were measured. Inter/intra observer variability was calculated and correlation analysis performed.

We found a strong correlation between patella plus cartilage thickness and width (Pearson 0.75, P < 0.001). The mean width:thickness ratio was 1.8 (SD 0.10, 95% CI 1.77–1.83). Without cartilage the ratio was 2.16 (SD 0.15, 95% CI 2.11–2.21), correlation was moderate (Pearson 0.68, P < 0.001). The average patella cartilage thickness was 4.1mm (SD 1.1, 95% CI 3.8–4.5).

The narrow confidence intervals for the ratio of patella width:thickness suggest that patella width can be used as a guide to accurate restoration of patella thickness during total knee or patella-femoral replacement. We would recommend a ratio of 1.8:1.

We sought to validate a method of measuring the range of motion of knees on radiographs as part of a new system of “Virtual Knee Clinics”.

The range of motion of 52 knees in 45 patients were first obtained clinically with goniometers and compared to radiographs of these patients' knees in full active flexion and extension. Four methods of plotting the range of motion on the radiographs were compared.

The intra-class correlation coefficient (ICC) for inter-rater reliability using the goniometer was very high; ICC=0.90 in extension and 0.85 in flexion. The best ICC for radiographic measurement in extension was 0.86 indicating substantial agreement and best ICC in flexion was 0.95 (method 4). ICC for intra-rater reliability was 0.98 for extension and 0.99 for flexion on radiographic measurements.

Measuring range of motion of the knee has never previously been validated in the literature. This study has allowed us to set up a “Virtual Knee Clinic,” combining postal questionnaires and radiographic measurements as a surrogate for knee function. We aim to maintain high quality patient surveillance following knee arthroplasty, reduce our new to follow-up ratios in line with Department of Health guidelines and improve patient satisfaction through reduced travel to hospital outpatients.

Successful osseointegration requires the production of a mechanically competent collagenous matrix, by osteoblasts, at the implant site. Lysophosphatidic acid (LPA) is a bioactive lipid which we discovered interacts with vitamin D3 (D3) to secure human osteoblast (hOB) maturation on both titanium (Ti) and hydroxyapatite.

We therefore covalently attached LPA and a related compound, (3S) 1-fluoro-3-hydroxy-4-butyl-1-phosphate (FHBP), to both solid and porous Ti discs and seeded them with hOBs to assess their ability to support D3-induced cell maturation. Solid functionalised discs were washed and reused a further two times, whilst other discs were stored for 6 months.

Increased alkaline phosphatase (ALP) activity indicated that both LPA and FHBP-modified Ti serve as superior substrates for securing D3-induced hOB maturation compared to unmodified metal (p < 0.001). Although total ALP activity was less for cells on recycled discs and after storage, enzyme levels were still significantly greater compared to hOBs grown on control Ti. LPA and D3 co-treatment also resulted in an increase in osteocalcin (∼17ng/ml versus 6ng/ml for D3 alone, P < 0.001) and collagen synthesis (∼310pg/ml versus <10pg/ml for D3 alone, P < 0.001).

Research is ongoing to evaluate the efficacy of our modified Ti surfaces to secure hOB formation from their stem cell progenitors.

The trapeziometacarpal joint (TMJ) is the most commonly involved arthritic joint in the hand and is often injected in the outpatient setting. This study assesses the accuracy of TMJ injections.

Six pairs of thawed, fresh-frozen cadaveric upper limbs were placed in the anatomic position. The limbs were randomized to be injected by one of two clinicians (a senior and a junior orthopaedic trainee). The TMJ of these specimens was palpated and injected with 0.5mls aqueous jelly dyed with methylene blue. An independent investigator dissected the specimens and the location of the dye was recorded. A Posterior-Anterior radiograph was then taken to assess the bony anatomy of the joint and graded according to Eaton's classification.

Dye was found inside the joint capsule in 10 (83%) of the 12 specimens. Using Fishers Exact test no significant difference was found between the 2 injectors (p=0.46). The 2 joints where the dye was extra-articular had grade III and IV arthritis, whereas all other joints were graded I.

This study shows that good accuracy of TMJ injection can be achieved using palpation in the earlier stages of TMJ arthritis, when surface anatomy is accurate enough for an intra-articular injection. This is also when synovitis is more prevalent and injections are more relevant. However the failure rate of injections increases as the disease advances.

Glenohumeral arthritis is associated with eccentric posterior glenoid wear and subsequent retroversion. Total shoulder arthroplasty provides a reliable and robust solution for this pattern of arthritis but success may be tempered by malposition of the glenoid component, resulting in pain, functional impairment, prosthetic loosening and ultimately failure. Correction of glenoid retroversion through anterior eccentric reaming, prior to glenoid component implantation, is performed to restore normal joint biomechanics and maximise implant longevity.

The aim of this study was to assess whether magnetic resonance imaging (MRI) or plain axillary radiography (XR) most accurately assessed glenoid version and hence provided the optimal modality for pre-operative templating.

Glenoid version was assessed in pre-operative shoulder MRIs and axillary radiographs (XR) by two independent observers in forty-eight consecutive patients undergoing total shoulder arthroplasty.

The mean glenoid version measured on magnetic resonance imaging was −14.3 degrees and −21.6 degrees on axillary radiographs (mean difference −7.36, p=<0.001). Glenoid retroversion was overestimated in 73% of XRs. Intra-observer and inter-observer reliability coefficients for MRI were 0.96 and 0.9 respectively. Intra-observer and inter-observer reliability coefficients for XR were 0.8 and 0.71 respectively.

Axillary radiographs significantly overestimate glenoid retroversion and are less precise than shoulder magnetic resonance, which provides excellent intra- and inter-observer reliability. MRI is a useful pre-operative osseous imaging modality for total shoulder arthroplasty as it offers a more precise method of determining glenoid version, in addition to the standard assessment rotator cuff integrity.

With modern day easy access to information, many health staff may be presumptuous of patients' level of understanding of medical terms and abbreviations. A recent audit of consent forms in Orthopaedic trauma in our department showed that doctors used abbreviations in 21% of consent forms; this was seen to increase to 48% during the re-audit. The findings motivated us to conduct this study to evaluate the level of patients' understanding of commonly used abbreviations/terminologies.

This questionnaire-based study recruited patients from both our elective and Trauma Orthopaedic units. Patient age, gender, medical and educational backgrounds were all randomised. Patients' understanding of 24 abbreviations/terminologies, selected from consent forms, patients' discharge letters and verbal communication with patients, were quantitatively and qualitatively assessed. Patients' perspectives were also sought.

All 182 patients who participated were proficient in English language. Most patients(80.2%) understood the term “physio,” however only 3.8% could correctly interpret “DHS”. 10% of patients understood “TKR,” 8.2% understood “THR” and 3.8% understood “NOF”. Interestingly, although 61.5% understood “DVT,” only 8.2% understood “PE” with most interpreting it as physical education/exercise. Only 8.2% related “MI” to any form of cardiac pathology. Almost all patients confirmed the use of unfamiliar abbreviations by health staff during communication.

Our study revealed that patients were not conversant with many abbreviations used in Orthopaedics. There is a need for greater awareness amongst doctors and other health staff about the indiscriminate use of abbreviations. From patients' perspective, interpretation should be given when using abbreviations or avoid their use altogether.

Proximal interphalangeal joint arthrodesis for lesser toe deformity is commonly stabilised using a Kirschner wire passed through the tip of the toe. Recently, a number of all-inside intramedullary fusion devices have been developed, with the aim of decreasing infection risk through avoiding exposed metalwork. We report our early experience with Stayfuse™ (Nexa Orthopaedics).

Case records of patients undergoing Stayfuse™ proximal interphalangeal joint arthrodesis between July 2009 and November 2011 were examined. Twenty-eight Stayfuse™ proximal interphalangeal joint arthrodesis procedures in 14 patients were performed. Overall, 8 (29%) procedures required revision within 6 months. Of these, 3 (11%) intraoperative failures occurred, requiring immediate revision (Kirschner wire stabilisation or excision arthroplasty). All involved bending failure of the implant clip mechanism. A further 5 (18%) toes required revision procedures within 6 months of the index procedure, 3 (11%) for dissociation of the implant clip mechanism and 2 (7%) for periprosthetic fracture.

For patients perceived to be at higher risk of infective complications, all-inside stabilisation systems are an attractive concept. However, with 29% of patients undergoing revision procedures within 6 months, we question the safety of choosing Stayfuse™ implants over Kirschner wire stabilisation.

Femoral neck stress fractures (FNSF) are uncommon, representing 3.5% to 8% of all stress fractures in military recruits. The majority of displaced FNSF undergo operative fixation and are at risk of avascular necrosis (AVN) and non union with a 40–100% medical discharge rate. We aimed to review the incidence and outcome of displaced FNSF in Royal Marine Recruits.

Retrospective review identified 6 Royal Marine recruits, aged 17 to 25, who had suffered a displaced FNSF over a 6 and a half year period. Incidence was 0.93 per 1000 recruits.

Patients were treated urgently by operative fixation with a 2 hole dynamic hip screw device, in 3 cases supplemented with an anti-rotation screw. There were no cases of AVN, no surgical complications and no further procedures were required. All united with a mean time to union of 11 months. 50% had a union time greater than 1 year. 2 completed training, 2 are still in rehabilitation and 2 (33%) were discharged before completion of training.

These fractures are slow to unite compared to other fractures at this site or stress fractures elsewhere. With urgent surgical intervention and stable fixation all however went onto successful union with time and all returned to rehabilitation or training with minimal complications. Awareness of the length of time to union has been invaluable in guiding treatment and rehabilitation. It can help avoid the risks of unnecessary secondary interventions for delay to union.

Despite becoming commonplace in hospital, little research has been performed into whether tablet computers can directly impact on trauma inpatient experience.

We performed a prospective, questionnaire-based study to assess inpatient desire to view radiographs on tablet PCs and if so, whether this impacted on patient-rated outcomes of understanding and satisfaction.

We surveyed 50 consecutive trauma patients using a validated questionnaire, assessing involvement in decision-making, satisfaction with explanation of injury and management and whether viewing imaging was desirable. A further 50 patients were then offered the opportunity to view their images on a tablet during their consultant discussion. Data was collected assessing similar outcomes and also whether viewing images was beneficial.

Patients who viewed their images reported significantly higher levels of satisfaction with the explanation of their injury (p=0.01) and management plan (p=0.03) and involvement in the decision making process (p=0.0001). Additionally, 70% thought their explanation would not have been as effective had they not seen their images, with 88% stating that viewing them had a positive impact on their overall experience.

In conclusion, enabling trauma inpatients to view their images on a tablet is a worthwhile practice as it improves involvement in decision-making, patient satisfaction, perceived understanding and overall experience.

Acetabular fractures are amongst the most complex fractures. It has been suggested that pre-contouring the fixation plates may save intra-operative time, blood loss, reduce intra-operative fluoroscopy and improve the reduction. The purpose of this study was to assess if the contouring could be done reliably using the mirror image of the uninjured hemipelvis.

Using the CT data of 12 specimens with no bony abnormality 3D models were reconstructed. Using computer software (AMIRA, Visage Imaging) the mirror image of the left hemipelvis and the right hemipelvis were superimposed based on landmarks. The distances between the surfaces were then calculated and displayed in the form of colour maps.

The colour maps demonstrated that for the areas around were acetabular fixation plates would be placed the differences were small. For the anterior column plate 50% of the specimens had differences of less than 1mm, which based on the work of Letournel and Judet would represent an anatomical reduction. For the posterior column plate 58% had differences of less than 1mm.

This study demonstrates that there is considerable symmetry between both hemipelvises and that precontouring on the mirror image of the uninjured side is an accurate, quick and reliable method for precontouring.

Introduction

Pelvic and acetabular injuries are relatively rare and surgical reconstruction usually occurs only in specialist centres. As part of their work up there is a local protocol for radiological investigations including Judet oblique views for acetabular fractures, pelvic inlet and outlet for pelvic ring fractures and urethrograms for sustaining anterior pelvic injury. The aim of this service evaluation was to assess whether patients had these radiological investigations prior to transfer.

Methods

The last 50 patients transferred for surgery were evaluated (41 male, 9 female), average age 48 (range 17–86). Four were excluded as original radiology not available and one due to non-acute presentation. Regional PACS systems were accessed and radiological investigations recorded.

We present our experience of treating 57 cases of bone defects associated with chronic osteomyelitis (COM) and an algorithm for their treatment.

A retrospective analysis of our operation database revealed 377 patients treated for COM (2002–2010). 76 (20%) had bone defects, of these 57 had notes and x-rays available. The tibia was most commonly affected (63%), followed by the femur (21%). Infection control procedures included debridement, drilling and sequestrectomy. Long-term antibiotics were seldom used. Prerequisites to reconstruction surgery were; fully healed skin, absence of sequestrae on x-ray and no antibiotics for 2-months. Decision on the method of treatment of defect was made depending on; age, defect size, viability of periosteum and physes, condition of soft tissues and coexisting deformity.

Initial treatment was; plaster stabilisation (15), frame stabilisation (6), free fibula structural bone grafts (9), ipsilateral vascularised fibula graft (7), non-structural cancellous bone graft (8), bone transport (8) and amputation (4). Forty three (75%) patients were successfully treated with initial strategy. Initial treatment therefore failed in 14 (25%) patients. Successful treatment subsequently used was; structural bone grafting (6), non-structural bone grafting (4), bone transport (3) and Rush Rod stabilisation (1).

Little is known about osteomyelitis-induced bone defects, which cause massive morbidity in developing countries. Our novel research shows that these can be treated successfully, often by relatively simple methods. In the absence of ongoing infection, non-vascularised bone grafting techniques are often successful. Bone transport or vascularised grafting are more reliable but more complex solutions.

We report the clinical outcomes of children with complex congenital foot deformities treated with UMEX® mini-external fixators. This is a prospective review of our experience in patients treated in this way, from 2004. The indications for treatment were resistant/recurrent Congenital Talus Equina Varus(CTEV), cavo-varus deformity secondary to Charcot-Marie-Tooth disease, arthrogryposis, fibular hemimelia and other congenital abnormalities. In addition, one patient underwent this treatment since he was allergic to casting material.

A total of 32 children (35 feet) have been treated, with a male to female ratio of 22:10 respectively. The patient-age at operation ranged between 3 and 15 years (median age −7 years). Three patients underwent bilateral procedures and 29 patients underwent unilateral foot operations. Twenty-eight patients underwent previous soft-tissue and bony corrective operations with serial casting. The frames were removed at an average of 69 days after application with a further 6 weeks in a walking cast. Seventy-nine percent of the patients had good functional outcome. Further operations were needed in 10 patients. Complications occurred in 10 patients, including pin-site infections, bony overgrowth at pin-site and proximal tibio-fibular diastasis.

In conclusion, this is a simple fixator to use with a small learning curve. In groups of patients with complex congenital abnormalities, we achieved good functional outcome with low-complication rates.

We investigated the effect of adjuvant and neoadjuvant chemotherapy regimens on the tibial regenerate after removal of the external fixator in a rabbit model of distraction osteogenesis using New Zealand white rabbits.

Forty rabbits were randomly distributed into two groups. In the neoadjuvant group, half of the rabbits received 1mg/kg cisplatinum & 2mg/kg adriamycin at eight weeks of age followed by 1mg/kg cisplatinum & 4mg/kg adriamycin at ten weeks of age. The remaining ten received an identical volume of normal saline using the same regimen. The adjuvant group differed only in the timing of the chemotherapy infusion. Half received the initial infusion ten days prior to the osteotomy, with the second infusion four days following the osteotomy. Again, the remaining ten rabbits received an identical volume of normal saline using the same regimen. This produced an identical interval between infusions and identical age at osteotomy in both groups. All rabbits underwent a tibial osteotomy at 12 weeks of age. Distraction started 24hours after osteotomy at a rate of 0.75mm a day for 10 days, followed by 18 days without correction to allow for consolidation of the regenerate.

At week 16 there was no difference in Bone Mineral Density (BMD), Bone Mineral Content (BMC) or volumetric Bone Mineral Density (vBMD) in the adjuvant group. Neoadjuvant chemotherapy appears to have a significant detrimental effect on BMD, vBMD and BMC. Despite this there were no significant alterations in the mechanical properties of the regenerate. Histologically there was a trend for increased cortical thickness in the control groups compared to intervention however this did not prove statistically significant.

In conclusion, adjuvant chemotherapy may be more beneficial for cases where distraction osteogenesis is being considered to replace segmental bone loss after tumour excision.

Survivors of infantile meningococcal septicaemia often develop progressive skeletal deformity as a consequence of physeal damage at multiple sites, particularly in the lower limb. Distal tibial physeal arrest typically occurs with sparing of the distal fibular physis leading to a rapidly progressive varus deformity. Isolated case reports include this deformity, but to our knowledge there is no previous literature that specifically reports the development of this deformity and potential treatment options.

We report our experience of 6 patients (7ankles) with this deformity, managed with corrective osteotomy using a programmable circular fixator.

Manipulation under anaesthetic (MUA) is an established treatment for frozen shoulder. Frozen shoulder may coexist with other shoulder conditions, whose treatment may differ from MUA. One such condition is calcific tendonitis. Only one study to date documents treatment of patients with frozen shoulder and concurrent calcific tendinitis.

The objective was to demonstrate that MUA and injection is a satisfactory treatment for concurrent diagnosis of frozen shoulder and calcific tendinitis.

Patients with a clinical diagnosis of frozen shoulder and radiological evidence of calcific tendinitis were prospectively recruited from Jan 1999 – Jan 2009. Treatment by MUA and injection was performed. Clinical examination, Oxford Shoulder Scores and need for further treatment were used as outcome measures.

Fourteen patients (median age 53.5 years) were identified with frozen shoulder and concurrent calcific tendinitis. Significant improvement in both Oxford Shoulder Score and range of movement was achieved following MUA (P values < 0.001). Two patients required further treatment (not for calcific tendinitis). This improvement was maintained in the long term (median 107 wks).

It is our belief that MUA and injection is a safe and effective treatment, addressing the frozen shoulder with MUA takes priority, and as such frozen shoulder “trumps” other pathologies occurring simultaneously.

We present a prospective, randomised, single-centre, multi-surgeon, controlled trial comparing minimally invasive (MIS) and standard approach total knee arthroplasty (TKA).

86 patients undergoing 92 total knee replacements were recruited. 46 operations were randomised to the MIS treatment arm, 46 to the standard control arm. Data collected included postoperative blood loss, length of stay and complications. Patients underwent surgery via a quadriceps sparing or standard medial parapatella approach. All operations were performed using MIS instruments and an identical postoperative care pathway.

The MIS group had a significantly shorter length of stay (1.4 days, p=0.004) and fewer complications (p=0.003). Demographics, operative time blood loss and radiographic alignment were comparable between the groups. There were no deaths, pulmonary emboli or surgical site infections. 1 patient developed DVT and 1 required revision for pain and failure to regain flexion within 9 months of surgery, both in the control group. Nine percent of MIS patients versus 35% controls suffered a complication delaying discharge, of which 2% and 17% related to surgical site problems respectively.

MIS resulted in a shorter admission and fewer complications, whilst achieving satisfactory component alignment. We discuss the potential economic implications.

This study is a prospective analysis of clinical outcome in 201 consecutive patients treated with medial patellofemoral ligament reconstruction using an autologous semitendinosus graft between October 2005 and January 2011. Patients received pre and post-operative clinical evaluation, radiological assessment and outcome scoring systems.

193 patients (92 male, 119 female) underwent 211 procedures, with mean age 26 (16–49) and follow-up 16 months (6–42 months). Indications were atraumatic recurrent patella dislocation (68%), traumatic recurrent dislocation (22.8%), instability (5%), single dislocation (2.7%) and anterior knee pain (1.4%). Trochlea dysplasia was moderate in 57% and mild in 35%.

There have been no recurrent dislocations/ subluxations. 10 patients have required further surgery. The mean pre-op Kujala Scores were 55 (SE 5.21) and post-op scores improved to mean 82 (31–100) (SE 1.18)(p < 0.001). This improvement and significance is mirrored with Oxford (27 to 41), WOMAC (76 to 93), Fulkerson (53 to 83), IKDC (46 to 75), Tegner (4.1 to 5.3) and SF12 (38 to 51) scores (p < 0.005). 93% of patients were satisfied with their operation. History of prior realignment surgery was associated with significantly worse outcomes compared to patients where MPFL reconstruction was their first realignment procedure (p < 0.05).

This series is the largest reported in the literature for any technique of MPFL surgery. This technique allows for objective intra-operative evaluation of the required graft tension to optimise patella tracking.

The introduction of direct thrombin inhibitors in arthroplasty surgery has reignited the debate on the risk of wound complications when using chemical thromboprophylaxis. It has been suggested that direct thrombin inhibitors might lead to an increased risk of systemic and operative site bleeding and wound sepsis when compared to low molecular weight heparin.

In July 2009, departmental thromboprophylaxis policy for patients undergoing hip and knee replacement surgery (including revision) was changed from subcutaneous enoxaparin for the duration of inpatient stay to dabigatran for 10 days (knees) or 28 days (hips) unless contraindidated. In the 2 years prior to policy change, 1091 patients underwent hip or knee arthroplasty (Group 1), with1150 patients undergoing the same procedures in the 2 years following July 2009 (Group 2). A minority of patients were already on warfarin (2% in group 1, 3% in group 2).

This study presents a retrospective analysis of all patients who returned to theatre within 30 days of joint replacement surgery to assess whether the change in unit policy caused any discernible increase in bleeding-related complications.

In group 1, 23/1091 patients (2.1%) returned to theatre within 30 days. 8 were for reasons unrelated to thromboprophylaxis (mainly dislocated hips), 5 for gastrointestinal bleeding (mainly upper GI endoscopy) and 10 for wound complications (haematoma, wound breakdown, or washout of early infection). In group 2, 22 / 1150 patients (1.9%) returned to theatre within 30 days. 12 were for unrelated reasons, 5 for GI bleeding, and 5 for wound complications. The lower return to theatre rate in the second group was not statistically different.

This study, in a large heterogeneous group of patients, suggests that a change from enoxaparin to dabigatran does not increase the incidence of early infection, or the risk of bleeding at the operative site or the gastrointestinal tract.

Orthopaedic cobalt chromium particles and ions can induce indirect DNA damage and chromosome aberrations in human cells on the other side of a cellular barrier in tissue culture. This occurs by intercellular signalling across the barrier. We now show that the threshold for this effect depends on the metal form and the particle composition.

Ionic cobalt and chromium induced single strand breaks at concentrations equivalent to those found in the blood of patients with well functioning metal on metal hip prostheses. However, they only caused double strand breaks if the chromium was present as chromium (VI), and did not induce chromosome aberrations. Nanoparticles of cobalt chromium alloy caused DNA double strand breaks and chromosome aberrations, of which the majority were tetraploidy. Ceramic nanoparticles induced only single strand breaks and/or alkaline labile sites when indirectly exposed to human fibroblasts.

The assessment of reproductive risk from maternal exposure to biomaterials, especially those liberated by orthopaedic implants, is not yet possible with epidemiology. Whilst the barrier model used here differs from the in vivo situation in several respects, it may be useful as a framework to evaluate biomaterial induced damage across physiological barriers.

Eighty-eight consecutive patients with symptomatic rotator cuff tears were entered in to a prospect study with a novel technique of open double row repair using a ‘Capstan’ screw technique. The medial row has standard anchors, but the lateral suture row is a 35mm × 6.5mm ‘Capstan' screw. This allows up to 28 suture bridges to be taken from the medial row to the lateral row compressing the footprint and spreading the load. This creates a very robust repair or ‘bulletproof repair’. This was used for medium to large isolated supraspinatus tears

Each patient had a pre and post operative Oxford Shoulder Score (OSS), American Shoulder and Elbow Score (ASES Score). The mean pre-operative OSS was 22 (maximum 48) and the mean post-operative OSS was 45, (p < 0.0001). Flexion improved from a mean of 117° to 172° (p < 0.0001). The clinical re-tear rate was 3.4%. 95% were satisfied with the procedure. There were no deep infections. 18% had transient stiffness, 6% stiffness at one year but none severe enough to warrant release. There were no instances of deltoid dysfunction.

This demonstrates excellent results in terms of OSS, patient satisfaction and function. Clinical re-tear rate is markedly reduced in comparison to previous literature.

Massive tears of the supraspinatus of the rotator cuff lead to painful loss of movement. The literature supports repair of these tears for young healthy individuals, however they present a surgical challenge with historically poor results from both athroscopic and standard open techniques.

Prof Bunker has developed a surgical technique for massive rotator cuff tears with a Grammont Osteotomy of the spine of the acromion, when standard surgical techniques will not allow the necessary exposure: the so called “Full Monty”.

Patients were entered in to a prospective study to obtain the functional benefit of this procedure. Each patient had a pre-operative American Shoulder Elbow Score (ASES) Oxford Shoulder score (OSS), pain score, range of movement. Post-operatively these measures were repeated along with a patient questionnaire on function and satisfaction.

The mean American Shoulder score (ASS) preoperatively was 7 (out of a possible 30) and improved postoperatively to 23(P = 0.00011). The improvement in the Oxford Shoulder Score was 22 (out of a possible 48) preoperatively to 43 postoperatively (0.0001) and 80% patients stated their treatment was “successful”.

We believe this a successful surgical option for a patient with “massive” rotator cuff tear that is not amenable to standard surgical techniques.

Accelerometer based gait analysis (AGA) is a potential alternative to the more commonly used skin marker based optical motion analysis system(OMAS). The use of gyroscopes in conjunction with accelerometers (i.e. inertial sensors), enables the assessment of position and angular movements of body segments and provides ambulatory kinematic characterisation of gait.

We investigated commonly used gait parameters and also a novel parameter, Pelvic obliquity (PO) and whether they can be used as a parameter of physical function and correlate with classic clinical outcome scores

Gait was studied in healthy subjects (n=20), in patients with end stage hip OA (n=20) and in patients with end stage knee OA (n=20). Subjects walked 20 metres in an indoor environment along a straight flat corridor at their own preferred speed. A 3D inertial sensor was positioned centrally between the posterior superior iliac spines (PSIS) overlying S1.

Comparing gait parameters of end stage hip OA patients with an age and gender matched healthy control group, significantly lower walking speed, longer step duration and shorter step length was observed. After correcting for walking speed between groups, significantly less average range of motion of PO (RoMpo) was observed for patients with end stage hip OA compared to healthy subjects and patients with end stage knee OA.

IGA allows objective assessment of physical function for everyday clinical practice and allows assessment of functional parameters beyond time only. IGA measures another dimension of physical function and could be used supplementary to monitor recovery of OA patients after TJR.

Daycase surgery has advantages for patients, clinicians and trusts. The Best Practice Tariff uplift is £200/case for Minor Foot Procedures performed as daycases. Before discharge, Foot & Ankle daycase procedures in Cheltenham General Hospital require physiotherapy assessment and frequently an orthotic aid. This audit analysed length of stay of daycase patients on a Foot and Ankle list. The standard was 100% of daycase patients to be discharged the same day.

Length of stay for a consecutive series of patients was calculated for all daycase procedures from October to December 2010. An intervention was made comprising a weekly multidisciplinary bulletin from the Orthopaedic Consultant. This highlighted post-operative weight-bearing instructions and orthotic requirements for forthcoming daycase patients to physiotherapists, nursing staff and junior doctors. The data was compared with a second consecutive series of patients from October to December 2011.

The first series included 38 listed daycases of which 61% (23 patients) were daycase discharges. The second series comprised 41 listed daycases who received pre-operative physiotherapy assessment and provision of required orthotic aids; 85% (35 patients) of this group were discharged the same day. Data analysis using Fisher's exact test reveals this intervention had a statistically significant impact on the number of patients discharged the same day (p < 0.0207).

The financial implications are increased Best Practice Tariff with an £1800 uplift and reduction in the estimated cost of unnecessary overnight stays of £4640 over the 3 months. Improved multidisciplinary communication can significantly improve the patient experience, bed occupancy and cost of care.

Urinary catheterisation may be associated with a risk of infection in hip and knee arthroplasty, however post-operative urinary retention can be distressing for the patient. This study investigates the factors that might predict the need for post-operative catheterisation.

A prospective blinded study of 128 consecutive patients undergoing lower limb arthroplasty was carried out. Data obtained included sex, age, joint (hip or knee), prostate score (IPSS), previous urinary retention, co-morbidities and residual bladder volume after attempted complete voiding measured using an ultrasound bladder scanner. Anaesthetic type and requirement for urinary catheterisation postoperatively were recorded. Predictive risk factors were identified using binary logistic regression, and expressed as odds ratios (OR) and 95% confidence intervals (CI).

The mean age was 69.5 years. There were 56 males and 72 females. 38 patients (30%) required post-operative catheterisation. Logistic regression identified the following predictive risk factors: bladder scan volume per ml increase (OR 1.006; CI 1.000–1.012); male sex (OR 5.51; CI 2.01–15.16); previous catheter (OR 4.26; CI 1.53–11.82); spinal/epidural/combined spinal epidural anaesthesia (OR 6.78; CI 1.75–26.32); moderate IPSS score (OR 5.15; CI 1.73–15.37); severe IPSS score (OR 3.53; CI 0.605–20.54). Our model specificity is 91%, positive predictor value 71% and negative predictor value 82%.

We report the largest multicentre series analysing the use of bone scans investigating painful post-operative Total Knee Replacements (TKR). We questioned the usefulness of reported scintigraphic abnormalities, and how often this changed subsequent management. 127 three-phase bone-scans were performed during a two-year period. Early and late flow phases were objectively classified. Reported incidences of infection and loosening were determined. Reports were subjectively summarised and objectively analysed to establish the usefulness of this investigation. Eight cases were excluded.

Scans were classified as: 33% (39) normal, 53% (63) as possibly abnormal, 6% (7) probably abnormal, and 8% (10) as definitely abnormal. Thirteen patients (11%) underwent revision TKR surgery. Intra-operative analysis revealed loosening of one femoral component, and massive metallosis of the patella in another. Cultures were negative in all cases. The sensitivity and specificity of a definitely abnormal investigation in predicting need for revision surgery was 23% and 82% (respectively). High instances of ambiguously reported abnormalities were observed.

This investigation has no role to play in the routine investigation of a painful TKR. It is unnecessary in investigation of periprosthetic infection and should not be used in a routine assessment of a painful TKR. If used it should be limited until an experienced revision surgeon has made a full assessment.

The rate of Metastatic Spinal Cord Compression (MSCC) has been increasing over recent years with increased patient survival from improved cancer treatment. MSCC presents an increasing demand for spinal surgical resources. NICE guidance was issued in 2008 to improve diagnosis and management and to prevent unnecessary delays which may result in disability. The recent advances in management of cancers coupled with improved spinal surgical approaches have improved the outcome in MSCC. Early surgery has been shown to improve restoration of function. A recent systematic review found that surgery produced superior results to radiotherapy alone for the management of MSCC. However, the quality of evidence so far is mostly from observational studies.

We would like to use Bluespier to create a database of MSCC patients referred to our tertiary centre. Our database would include all adult patients referred to the spinal surgical service with MSCC. Information recorded would be the diagnosis, time of onset and imaging, comorbidities, previous interventions, clinical findings, ASIA score, mobility status, sphincteric status, Karnofsky, Tokuhashi, Tomita and Bauer scores. These scores have been shown by numerous studies to have the best predictive value for outcome following MSCC. The SINS and Boriani MSCC protocols will be collected and externally validated. Time to surgery, operative data and intra operative complications will be recorded. PROMs will include the Oswestry / Neck disability index, VAS and SF36 scores. Post operative complications, morbidity and mortality will be collected and the details of any other therapy received. We would score the patients on admission and at 3 months, 6 months and one year post operatively (if survival allows). This will be done in out patients and via postal and telephone questionnaires. The database will flag the time intervals.

This database will enable us to improve the quality of care given to patients with MSCC, provide evidence to highlight the importance of prompt referral and surgical intervention, audit our care against the standards set out by NICE and establish the risks, complications and outcomes of surgical intervention in this high risk group. It will be the first study to externally validate and compare several different scoring systems and protocols (above) in the same cohort. Finally, the data can be used to perform a costing analysis for the treatment of MSCC in the NHS.

Patients using a neutral rotation brace post proximal humerus fracture fixation have improved functional outcome and external rotation of the shoulder compared to patients using a standard polysling.

Patients who have proximal humerus fracture fixation with extramedullary plates and screws have a risk of reduced range of movement especially external rotation. Gerber et al showed that the average external rotation after fixation of proximal humeral fractures was 39 degrees in their patient cohort compared to a normal range of 80–100 degrees. This can lead to reduced function and poor patient related outcomes. Geiger et al showed that in a cohort of 28 patients, poor functional outcome was noted in 39.3% with an average Constant-Murley Score of 57.9.

Current practice is to utilise a polysling holding the shoulder in internal rotation post-shoulder fixation. Patients usually wear the sling for up to 6 weeks. We believe that this increases the risk of adhesion formation with the shoulder in internal rotation in the shoulder joint. Therefore this can cause loss of external rotation in the shoulder joint. We believe that holding the shoulder in a neutral alignment, with a neutral rotation brace post-fixation, will enable an increased rate of external rotation post-operatively thus improving external rotation and functional outcome

There is currently no literature comparing the different slings used post-operatively and we believe that this study would be the first of its kind. It would have a substantial change in the way clinicians manage proximal humeral fractures and will potentially reduce the numbers of re-operations to divide adhesions or perform capsular releases. Secondary benefits include a potential earlier return to full function and work and improved patient satisfaction.

Study proposal: Prospective Randomised Controlled Trial of the neutral rotation brace compared to the standard, currently used, polysling post proximal humerus fracture fixation. No blinding of either participants or clinicians. Three surgeons utilising similar fixation techniques via the deltopectoral approach and using Philos plate fixation (Synthes Ltd.). Standardised post-operative rehabilitation protocol for all patients.

Follow up: clinical review and postal outcomes for 1 year

Primary outcomes: Post operative functional outcome scores (Oxford, DASH, EQL) obtained at 6 weeks, 9 weeks, 3 months and 1 year). These will be compared to scores taken pre-operatively.

Secondary outcomes: Clinical review at 6 weeks, 3 months and 1 year with range of movement measurements. Radiographs also taken at 6 weeks and 3 months to assess union. Patient questionnaire at 1 year (with outcome scores) assessing patient return to work, complications and patient satisfaction

Inclusion criteria: Proximal humeral fractures requiring operative intervention with extramedullary plate fixation (i.e. fractures displaced by 1cm and/or angulated by 45 degrees or more). Age>18.

Exclusion Criteria: Patients having intra-operative findings of complete Pectoralis major rupture or if operative exposure requires complete Pectoralis major tenotomy. (These patients need to be held in internal rotation with a standard polysling to allow healing of the Pectoralis major tendon).
