Introduction and Objective

Kinematic Alignment (KA) is a surgical technique that restores the native knee alignment following Total Knee Arthroplasty (TKA). The association of this technique with a medial pivot implant design (MP) attempts to reestablish the physiological kinematics of the knee. Aim of this study is to analyze the clinical and radiological outcomes of patients undergoing MP-TKA with kinematic alignment, and to assess the effect of the limb alignment and the orientation of the tibial component on the clinical outcomes.

Materials and Methods

We retrospectively analyzed 63 patients who underwent kinematic aligned medial pivot TKA from September 2018 to January 2020. Patient-Related Outcomes (PROMs) and radiological measures were collected at baseline, 3 months and 12 months after surgery.

Companies manufacturing total knee arthroplasty (TKA) prostheses produce a variety of tibial and femoral components of different dimensions denoted by numbers or letters. Surgeons frequently implant components that are compatible but not of the same size on the femur and tibia. Recent studies suggest that equally sized femoral and tibial components produce better outcomes compared to size-mismatched components. In our study, we aim to explore the relationship between component size and outcome measured by oxford knee score at six weeks and one year following TKA.

A cohort of twenty-four patients who underwent TKA and had well-functioning prosthesis were studied. Thirteen (54%) had equally sized TKA components implanted, seventy-four patients (42%) had components that were mismatched by one size, and one (4%) had components that were mismatched by more than one size. The Oxford Knee Score (OKS) obtained preoperatively, at six weeks and one year postoperatively were retrieved from an electronic database. All data were analysed using R software.

A significant improvement in pre-operative and one-year postoperative OKS was observed. Patients who received one-size mismatched tibial and femoral components demonstrated a less pronounced improvement in OKS as compared with patients who received equally sized components.

When possible, it may be best to utilise equally sized prosthetic tibial and femoral components when performing total knee arthroplasty. Manufacturers may be able to produce better patient outcomes by including prostheses that are between sizes as part of their production line.

Restoring native hip anatomy and biomechanics is important to create a well-functioning total hip arthroplasty (THA). Hip offset and leg length are regarded as the most important biomechanical characteristics. This study investigated their association with clinical outcomes including patient reported outcome measures (PROMs) and functional tests. This prospective cohort study was conducted in 77 patients undergoing primary THA (age=65±11 years). Hip offset and leg length were measured on anteroposterior radiographs of the hip pre- and postoperatively. Participants completed the Western Ontario & McMaster Universities Osteoarthritis Index (WOMAC) and performed functional tests (i.e. gait, single leg stance, sit-to-stand, block step-up) preoperatively, and 3 and 12 months postoperatively. A wearable motion sensor was used to derive biomechanical parameters. Associations between radiographic and functional outcomes were investigated with the Spearman's rho correlation coefficient. Subgroup comparisons were conducted for patients with more than 15% decreased or increased femoral offset after THA. Differences in postoperative offset and leg length had little impact on clinical outcomes. Femoral offset subgroups demonstrated no significantly different WOMAC function scores. In functional tests, patients with >15% decreased femoral offset after THA demonstrated more sagittal plane motion during block step-up (14.43° versus 10.66°; p=0.04) while patients with >15% increased femoral after THA demonstrated more asymmetry of frontal plane motion during block step-up (34.05% versus 14.18%; p=0.03). To create a well-functioning THA, there seems to be a reasonable safe zone regarding the reconstruction of offset and leg length.

A risk factor for patellofemoral instability is trochlear dysplasia. Trochleoplasty is a surgical procedure used to reshape the trochlear groove to improve patellar stability. This study seeks to compare pre-op MRI measurements and post operative MRI measurements for patients who have undergone trochleoplasty in correlation with their clinical outcomes scores.

Data was collected from a database of patients known to have trochlear dysplasia who underwent trochleoplasty. Radiological Data was collected pre-op and subsequent post op MRI data collected included TT-TG, Patella Tilt, IS, sulcus angle. Data score sheets pre-op and post op trochleoplasty completed by patients were also collected.

10 patients had pre and post op MRI's documented. 80%(8/10) females and 20%(2/10) males, average age of 30 years old (range 23 – 32 years old). Average MRI pre-op scores: IS ratio: 1.2, Patella tilt: 24.14, sulcus angle 160.13, and TT-TG distance of 16.94. 1 year average MRI post-op scores: IS ratio: 1.28, Patella tilt 15.56, sulcus angle 148.66 and TT-TG distance 16.78. 1 year post op Kujala and Norwich instability scores patient reported improved stability, function and confidence post op compared to pre-op.

Subjective and objective scores reflected an improvement of stability. MRI demonstrated a deeper trochlear groove post-operatively which should provide resistance against lateral patella movement and patellar dislocations. TT-TG pre and post op remained constant. Pre op and post op Kujala scores reflected improved function. The Norwich instability scores pre and post op reflected satisfaction of treatment.

There are not a lot of studies published on trochleoplasty. Based on this study it is clear that patients with patellofemoral instability with severe trochlear dysplasia will benefit from trochleoplasty. The sample size of the data analysis was only 10. However it reflected that function 1 year post procedure remained stable.

Following acute ACL rupture patients are routinely referred for rehabilitation but the timing and level of functional recovery related to rehabilitation outcome are poorly defined.

The primary aim of this study was to measure functional recovery following acute ACL rupture in the clinical setting using a two dimensional movement analysis system. A longitudinal research design was used; we aimed for three clinical movement analysis sessions over the course of rehabilitation. One hundred and fifteen patients were recruited. Sixty three uninjured matched controls were recorded once performing all the functional activities; walking, jogging, distance hop and run and stop. Participants were filmed in the sagittal plane using a digital camcorder to extract kinematic data. Average recovery over time was modelled using a least squares third order polynomial.

The secondary aim was to define the outcome measures and treatment goals used in ACL rehabilitation by specialist knee physiotherapists. A questionnaire was distributed to 300 hospitals across the UK. From the 44 responses insight was obtained about parameters physiotherapists use to plan treatment and evaluate recovery.

Repeated movement analysis showed that gait velocity took on average 85 days to recover to within the normal limits of uninjured controls. Jogging velocity took 30 days; Hop distance took 55 days for the non-injured leg and 100 days for the injured leg; Knee range during the landing phase of run and stop took 80 days to recover but demonstrated some deterioration. The questionnaire identified that specialist knee physiotherapists use 60 different outcome measures and 34 rehabilitation treatment goals, which can be sub-divided into patient reported (PR), functional activities (A) and impairments (I). The percentage usage by physiotherapists for each category of outcome measure were 55.8% (A), 62.8% (I) to 67.4% (PR) and for treatment goals 55.8% (PR), 69.8% (A) to 81.4% (I). Hopping is the most frequently evaluated functional activity but there are large differences in its utilisation. The application of functional goals and outcome measures in rehabilitation is not universal with specialist physiotherapists generally adopting an impairment approach.

Repeated movement analysis in the clinical setting provided objective data on the recovery of functional activities that progressively challenge knee stability. Gait and hop distance appear to be the most useful variables for tracking performance over time but their predictive value needs to be explored further. Adaptations in the non-injured leg indicate that its use as a control needs to be done with caution. For jogging and run and stop there appears to be a threshold after which patients can perform these activities rather than a gradual recovery. Clinical movement analysis could be used to provide objective feedback on recovery levels and help guide the rehabilitation process. However, currently functional goals and milestones are not always included in the planning and evaluation of rehabilitation. Developing better rehabilitation should involve greater integration of functional activity measures into practice. This would require a shift from an impairment rehabilitation approach to focus on functional goals.

Financial and human cost effectiveness is an increasing evident outcome measure of surgical innovation. Considering the human element, the aim is to restore the individual to their “normal” state by sparing anatomy without compromising implant performance. Gait lab studies have shown differences between different implants at top walking speed, but none to our knowledge have analysed differing total hip replacement patients through the entire range of gait speed and incline to show differences. The purpose of this gait study was to 1) determine if a new short stem femoral implant would return patients back to normal 2) compare its performance to established hip resurfacing and long stem total hip replacement (THR) implants.

110 subjects were tested on an instrumented treadmill (Kistler Gaitway, Amherst, NY), 4 groups (short-stem THR, long-stem THR, hip resurfacing and healthy controls) of 28, 29, 27, and 26 respectively. The new short femoral stem patients (Furlong Evolution, JRI) were taken from the ongoing Evolution Hip trial that have been tested on the treadmill with minimum 12months postop. The long stem total hip replacements and hip resurfacing groups were identified from out 800 patient gait database. They were only chosen if they were 12 months postop and had no other joint disease or medical comorbidities which would affect gait performance.

All subjects were tested through their entire range of gait speeds and incline after having a 5 minute habituation period. Speed intervals were at 0.5kms increments until maximum walking speed achieved and inclines at 4kms for 5, 10, 15%. At all incremental intervals of speed, the vertical component of the ground reaction forces, center of pressure and temporal measurements were collected for both limbs with a sampling frequency of 100Hz. Body weight scaling was applied to correct for mass differences and a symmetry index to compare the implanted hip to the contralateral normal hip. All variables for each subject group were compared to each other using an analysis of variance (ANOVA) with Tukey post hoc test with significance set at α=0.05.

The four experimental groups were reasonably matched for demographics and the implant groups for PROMs. Hip resurfacing had a clear top walking speed advantage, but when assessing the symmetry index on all speeds and incline, all groups were not significantly different. Push-off and step length was statistically less favourable for the short/long THR group (p=0.005–0.05) depending on speed/incline.

The primary aim of this study was determine if implant design affected gait symmetry and performance. Interestingly, irrespective of implant design, symmetry with regards to weight acceptance, impulse, push-off and step length was returned to normal when comparing to healthy controls. However individual implant performance on the flat and incline, showed inferior (p<0.05) push-off force and step length in the short stem and long stem THR groups when compared to controls. Age and gender may have played a part for the short stem group. It appears that the early gait outcomes for the short stem device are promising. Assessment at the 3 year mark should be conclusive.

The aim of this study was to perform a comprehensive evaluation of the changes in function from pre- to post-surgery in total and unilateral knee arthroplasty (UKA/TKA) patients. Twenty healthy (age 62.4 ±5.9, 11 male), 14 UKA (age 60.9 ±10.1, 8 male) and 17 TKA (age 67.2 ±8.1, 9 male) patients were studied. KA patients were assessed four weeks pre- and six months post-operation. Measures of perceived pain and function were collected using Oxford Knee Score (OKS) questionnaire. Tests of objective function included joint range of motion (RoM), ultrasound imaging, and 3-D motion analysis/inverse modelling from gait and sit-stand. An optimal set of variables was used to classify KA function using the Cardiff DST method. Pre-KA and healthy individuals were accurately classified (96%). Post-operation questionnaire measures of function improved for both UKA and TKA groups. However, observed measures of RoM, muscle atrophy and gait had only limited gains. This resulted in 57% of UKA and only 27% of TKA patients being classified as healthy post-operation. The results of this study show that 6 months post-surgery UKA patients had higher function than TKA. Using statistical approaches to combine functional assessments has provided an accurate platform to classify function and estimate changes from pre- to post-surgery. The clinical application of this tool requires further investigation and comparison to commonly used clinical techniques.

The use of a tourniquet during total knee arthroplasty (TKA) is controversial. Return to function and pain are believed to be affected by the use of a tourniquet. The hypothesis of this study was that use of a tourniquet (T) would delay postoperative functional recovery and increase pain as compared to no tourniquet use (NT). 200 patients were recruited for this prospective, double-blinded, randomized controlled trial. All surgeries were performed by one of two fellowship trained arthroplasty surgeons at our institution. Patients were randomized to either undergo TKA with T or NT and blinded to group allocation. An otherwise standardized perioperative protocol was followed. The primary outcome measures were functional assessment testing using the timed up-and-go (TUG) and stair-climb (SC) tests and visual analog scale pain (VAS-P) scores. Secondary outcome measures included blood loss and range-of-motion (ROM). Patients completed outcomes measures preoperatively, in hospital, and postoperatively at 4–6 weeks and 6–8 months. Minimal detectable change (MDC) and Student's T-test, alpha of p < 0.05, were used to determine significance. No significant differences were seen in postoperative TUG, SC, VAS-P, or ROM at any time point. NT patients were seen to have significantly more calculated blood loss (means: T 1,370.04mL, NT 1,743.85mL; p < 0.001), without a significant increase in transfusion events. Tourniquet use during TKA significantly decreases blood loss and does not adversely affect early postoperative outcomes. Tourniquet use during routine TKA is safe and effective and concerns over deleterious effects on function and pain may not be justified.

Achilles tendon rupture may lead to significant functional deficits, which mechanisms are poorly understood. The primary aim was to investigate if the Achilles tendon (AT) was longer, muscles weaker or gait changed on the injured leg 4–5 years after the injury. Secondary aim was to compare functional outcomes with patient reported Achilles Tendon Total Rupture Score (ATRS).

We invited all participants from an RCT of conservatively treated AT Rupture (ATR) with or without early weight-bearing (early-WB, non-WB), and 12 moths of follow up. Of the original 56, 37 patients participated, 19 from early-WB (1 re-rupture (RR)), and 18 from non-WB (2 RR). Time from injury to follow up was 4,5 years (4,1 to 5,1). AT length was measured using ultrasound with a validated protocol (Barfod K.W. et al.). Heel raise work was measured on a 10 degree inclining platform. The exercise lasted until the patient could not maintain frequency or height of lift. Number and height of lift was measured using reflective markers in a Vicon system, and total work calculated. Foot pressure mapping (FPM) was measured barefoot, using an EMED platform (novel, Germany). Statistics: T-test for limb to limb comparisons and linear regression for ATRS correlations was applied.

Including RR in the sample did not impact the results. We found no differences in any of the variables between the early-WB and non-WB groups. Compared to the uninjured limb, the Achilles tendon was an average of 1,8 (1,2–2,3) cm longer on the injured limb, which produced 40% less work. A smaller calf circumference (p < 0.001), larger dorsiflextion (p = 0.001), and Achilles tendon resting angle (p < 0.001) was found for the injured limb. Difference in mean medial forefoot peak pressure was approaching significance (healthy 484 (SD 165) KPa, injured: 439 (SD 160), p = 0.08). Similarly the difference in pressure / time integral of the medial forefoot was approaching significance (Healthy: 129 (SD 35)KPa, injured: 115 (SD 44)KPa, p = 0.08). Duration of contact time of the heel was extended and heel lift off was delayed in the injured limb (p = 0.02 for both). ATRS could not be linked to Achilles tendon length or total work using linear regression.

Conservatively treated Achilles tendon ruptures were approximately 1,8 cm longer. The limb was persistently weaker. A subtle change in heel contact duration and time of heel rise could be detected on the injured limb. ATRS does not appear to correlate directly with AT length or loss of total work.

Distal radial fractures are amongst the most common trauma referrals, however controversy remains regarding their optimum management. We undertook a retrospective review of the management of distal radial fractures in our department.

The prospectively maintained trauma database was used to identify patients admitted for operative management of a dorsally displaced distal radial fracture between June 2008 and June 2009. Only extra-articular or simple intra-articular fractures were included (AO classification A2/A3/C1/C2). Operation notes were reviewed to determine the method of fixation. Patients were contacted by post and asked to complete a functional outcome score - Disabilities of the Arm, Shoulder and Hand (DASH). A further 12 patients with similar fractures who had been managed conservatively were also asked to complete a DASH score to provide a comparison between operative and non-operative management.

98 patients were identified - 67 female, 31 male. Mean age was 51 years, range 15-85 years. All patients were at least 1 year post-op.

26 patients had manipulation under anaesthesia (MUA). 48 patients had MUA and K-wire fixation, which was supplemented with synthetic bone substitute in 16 cases. 3 patients had MUA and bone graft and 21 patients had open reduction and internal fixation (ORIF) with a volar plate.

34 correctly completed DASH scores were returned. A lower score equates to a better functional outcome. Mean DASH scores were: MUA 14.8; MUA+K-wire 13.1; ORIF 13.6; conservative 47.1.

This data would indicate that patients with a significantly displaced distal radial fracture have a better functional outcome with operative management to improve the fracture alignment. However, all of the methods of fixation used resulted in similar functional outcomes at one year.

Clavicular hook plates have been used over the last decade in the treatment of lateral clavicular fractures with good rates of union reported throughout the literature. Fewer studies have reported the functional outcome of these patients and some have reported potential soft tissue damage post plate removal. We aimed to review the functional outcomes alongside union rates in patients treated with hook plates for lateral clavicular fractures.

In this retrospective case series, 21 patients with traumatic lateral third clavicular fractures were included. 15 had Neer type II fractures, 4 Neer type III fractures, 1 patient had a Neer type I fracture and 1 radiograph was not able to be classified. All patients were treated with clavicular hook plates at the same district general hospital by five experienced surgeons between March 2010 and February 2015 adhering to the same surgical protocol. All patients had standard physiotherapy and post operative follow up. Plates were removed when radiological union was achieved in all but one patient who had the plate removed before union was achieved due to prolonged non-union. Patients were followed up post plate removal and evaluated clinically using the Oxford Shoulder Score. Their post plate radiographs were assessed by an independent radiologist and bony union documented.

21 patients were included. Mean age was 40 (range 14–63) with a male:female ratio of 17:4. Mean follow up was 5 months post injury (1–26 months). The hook plate remained in situ for a mean time of 4.3 months (2–16 months). One patient developed a post-operative wound infection treated with antibiotics, 2 patients developed adhesive capsulitis, one patient had not achieved bony union prior to hook plate removal at 16 months, however did achieve union 2 months post plate removal, two patients required revision plating. All patients achieved bony union eventually with good alignment and no displacement of the acromioclavicular joint seen on the most recent post operative radiographs. Post plate removal Oxford Shoulder Scores indicated good shoulder function with a mean score of 41.5 (maximum score possible 48 and the range of scores for our cohort was 30–47).

Our data would support the use of hook plates in the treatment of lateral clavicular fractures. All patients achieved union eventually with good alignment and this was reflected in the good functional outcome scores. This study is limited in its small cohort and short-term follow up. More research is required to examine the long term consequences of hook plate surgery in a larger patient population.
