Background. Obesity has been shown to be an independent risk factor for aseptic loosening of the tibia and smaller implant size has been correlated with increased risk of failure of tibial components in obese patients [1,2]. Many surgeons have noted that obese patients, especially females, not uncommonly will have small implant sizes. As such, we hypothesized that obesity was not directly correlated with total knee arthroplasty (TKA) implant sizes. The purpose of this study was to determine if increasing body mass index (BMI), height, and/or weight is associated with implant size in primary TKA. Methods. The institutional registry of a single academic center was reviewed to identify all primary TKAs performed between 2005 and 2016. Those without minimum 2-year follow-up or with incomplete implant data were excluded. The different manufacturer's implant designs were categorized based on anteroposterior and mediolateral dimensions of the femoral and tibial component sizes and cross sectional area was determined. BMI was categorized by the World Health Organization (WHO) obesity scale (Class I: BMI 30 to <35, Class II: BMI 35 to <40, Class III: BMI 40 kg/m. 2. or greater). Patient demographics including sex, height, weight, and BMI were analyzed to evaluate correlations with implant size using Pearson correlation coefficients. Results. There were 8,107 TKA included in the analysis with a mean age of 67.3 ± 9.5 years and mean BMI of 30.4 ± 6.3 kg/m. 2. There was a significantly higher proportion of females (63%) in the total cohort as well as in the Obese I, II, and III categories. BMI had no significant association with implant size in the femur or tibia; however, weight had a moderate association with implant size (ρ=0.39–0.48, p<0.001). Increasing height had the strongest correlation with increasing implant size of both the femoral and tibial components (ρ=0.63–0.77, p<0.001). Conclusion. Implant size was correlated with increasing height and weight, but not BMI in this cohort. Therefore, short patients who suffer from obesity, will most likely have smaller component sizes that may necessitate adjuvant fixation techniques (i.e. tibial or femoral stem). In addition, these findings may be useful in predicting TKA implant size based on a patient's height and weight. For figures, tables, or references, please contact authors directly

The significance of weight in the indications for unicompartmental knee replacement (UKR) is unclear. Our hypothesis was that weight does not affect the long-term rate of survival of UKRs. We undertook a retrospective study of 212 UKRs at a mean follow-up of 12 years (7 to 22). The patients were distributed according to body mass index (BMI; < vs ≥ 30 kg/m. 2. ) and weight (< vs ≥ 82 kg). Kaplan-Meier survivorship analysis was performed and ten-year survival rates were compared between the sub-groups. Multimodal regression analysis determined the impact of the various theoretical contraindications on the long-term rate of survival of UKR. The ten-year rates of survival were similar in the two weight subgroups (≥ 82 kg: 93.5% (95% confidence interval (CI) 66.5 to 96.3); < 82 kg: 92.5% (95% CI 82.5 to 94.1)) and also in the two BMI subgroups (≥ 30 kg/m. 2. : 92% (95% CI 82.5 to 95.3); < 30 kg/m. 2. : 94% (95% CI 78.4 to 95.9)). Multimodal regression analysis revealed that weight plays a part in reducing the risk of revision with a relative risk of 0.387, although this did not reach statistical significance (p = 0.662). The results relating weight and BMI to the clinical outcome were not statistically significant. Thus, this study confirms that weight does not influence the long-term rate of survival of UKR. We carried out a prospective, continuous study on 529 patients who underwent primary total knee replacement between January 2006 and December 2007 at a major teaching hospital. The aim was to investigate weight change and the functional and clinical outcome in non-obese and obese groups at 12 months post-operatively. The patients were grouped according to their pre-operative body mass index (BMI) as follows: non-obese (BMI < 30 kg/m. 2. ), obese (BMI . 3. 30 to 39 kg/m. 2. ) and morbidly obese (BMI > 40 kg/m. 2. ). The clinical outcome data were available for all patients and functional outcome data for 521 (98.5%). Overall, 318 (60.1%) of the patients were obese or morbidly obese. At 12 months, a clinically significant weight loss of ≥ 5% had occurred in 40 (12.6%) of the obese patients, but 107 (21%) gained weight. The change in the International Knee Society score was less in obese and morbidly obese compared with non-obese patients (p = 0.016). Adverse events occurred in 30 (14.2%) of the non-obese, 59 (22.6%) of the obese and 20 (35.1%) of the morbidly obese patients (p = 0.001)

Introduction. The National Institute for Health and Clinical Effectiveness recommends both low molecular weight heparin (LMWH) and Rivaroxaban for venous thromboembolic (VTE) prophylaxis following lower limb arthroplasty. Despite evidence in the literature that suggests Rivaroxaban reduces VTE events, there are emerging concerns from the orthopaedic community regarding an increase in wound complications following its use. Methods. Through the orthopaedic clinical directors forum, Trusts replacing LMWH with Rivaroxaban for lower limb arthroplasty thromboprophylaxis during 2009 were identified. Prospectively collected Hospital episode statistics (HES) data was then analysed for these units so as to determine rates of 90-day symptomatic deep venous thrombosis (DVT), pulmonary thromboembolism (PTE), major bleed (cerebrovascular accident or gastrointestinal haemorrhage), all-cause mortality, and 30-day wound infection and readmission rates before and after the change to Rivaroxaban. 2752 patients prescribed Rivaroxaban following TKR or THR were compared to 10358 patients prescribed LMWH. Data was analysed using odds ratios (OR). Results. There were significantly more wound infections in the Rivaroxaban group (3.85% vs. 2.81%, OR=0.72; 95% CI 0.58-0.90). There were no significant differences between the two groups for PTE (OR=1.52; 0.77-2.97), major bleed (OR=0.73; 0.48-1.12), all-cause mortality (OR=0.93; 0.46-1.87) and re-admission rate (OR=1.21; 0.88-1.67). There were significantly fewer symptomatic DVTs in the Rivaroxaban group (0.91% vs. 0.36%, OR=2.51; 1.30-4.82). Conclusion. This study is the first to describe the real impact of the use of Rivaroxaban in the NHS. When compared with LMWH in lower limb arthroplasty patients, wound infection rates were significantly higher following Rivaroxaban use whilst providing no reduction in symptomatic PTE or all-cause mortality

Aims. No predictive model has been published to forecast operating time for total knee arthroplasty (TKA). The aims of this study were to design and validate a predictive model to estimate operating time for robotic-assisted TKA based on demographic data, and evaluate the added predictive power of CT scan-based predictors and their impact on the accuracy of the predictive model. Methods. A retrospective study was conducted on 1,061 TKAs performed from January 2016 to December 2019 with an image-based robotic-assisted system. Demographic data included age, sex, height, and weight. The femoral and tibial mechanical axis and the osteophyte volume were calculated from CT scans. These inputs were used to develop a predictive model aimed to predict operating time based on demographic data only, and demographic and 3D patient anatomy data. Results. The key factors for predicting operating time were the surgeon and patient weight, followed by 12 anatomical parameters derived from CT scans. The predictive model based only on demographic data showed that 90% of predictions were within 15 minutes of actual operating time, with 73% within ten minutes. The predictive model including demographic data and CT scans showed that 94% of predictions were within 15 minutes of actual operating time and 88% within ten minutes. Conclusion. The primary factors for predicting robotic-assisted TKA operating time were surgeon, patient weight, and osteophyte volume. This study demonstrates that incorporating 3D patient-specific data can improve operating time predictions models, which may lead to improved operating room planning and efficiency. Aims. The purpose of this investigation was to determine the relationship between height, weight, and sex with implant size in total knee arthroplasty (TKA) using a multivariate linear regression model and a Bayesian model. Methods. A retrospective review of an institutional registry was performed of primary TKAs performed between January 2005 and December 2016. Patient demographics including patient age, sex, height, weight, and body mass index (BMI) were obtained from registry and medical record review. In total, 8,100 primary TKAs were included. The mean age was 67.3 years (SD 9.5) with a mean BMI of 30.4 kg/m. 2. (SD 6.3). The TKAs were randomly split into a training cohort (n = 4,022) and a testing cohort (n = 4,078). A multivariate linear regression model was created on the training cohort and then applied to the testing cohort . A Bayesian model was created based on the frequencies of implant sizes in the training cohort. The model was then applied to the testing cohort to determine the accuracy of the model at 1%, 5%, and 10% tolerance of inaccuracy. Results. Height had a relatively strong correlation with implant size (femoral component anteroposterior (AP) Pearson correlation coefficient (ρ) = 0.73, p < 0.001; tibial component mediolateral (ML) ρ = 0.77, p < 0.001). Weight had a moderately strong correlation with implant size, (femoral component AP ρ = 0.46, p < 0.001; tibial ML ρ = 0.48, p < 0.001). There was a significant linear correlation with height, weight, and sex with implant size (femoral component R. 2. = 0.607, p < 0.001; tibial R. 2. = 0.695, p < 0.001). The Bayesian model showed high accuracy in predicting the range of required implant sizes (94.4% for the femur and 96.6% for the tibia) accepting a 5% risk of inaccuracy. Conclusion. Implant size was correlated with basic demographic variables including height, weight, and sex. The linear regression and Bayesian models accurately predicted required implant sizes across multiple manufacturers based on height, weight, and sex alone. Aims. To report mid- to long-term results of Oxford mobile bearing domed lateral unicompartmental knee arthroplasty (UKA), and determine the effect of potential contraindications on outcome. Methods. A total of 325 consecutive domed lateral UKAs undertaken for the recommended indications were included, and their functional and survival outcomes were assessed. The effects of age, weight, activity, and the presence of full-thickness erosions of cartilage in the patellofemoral joint on outcome were evaluated. Results. Median follow-up was seven years (3 to 14), and mean age at surgery was 65 years (39 to 90). Median Oxford Knee Score (OKS) was 43 (interquartile range (IQR) 37 to 47), with 260 (80%) achieving a good or excellent score (OKS > 34). Revisions occurred in 34 (10%); 14 (4%) were for dislocation, of which 12 had no recurrence following insertion of a new bearing, and 12 (4%) were revised for medial osteoarthritis (OA). Ten-year survival was 85% (95% confidence interval (CI) 79 to 90, at risk 72). Age, weight, activity, and patellofemoral erosions did not have a significant effect on the clinical outcome or survival. Conclusion. Domed lateral UKA provides a good alternative to total knee arthroplasty (TKA) in the management of lateral compartment OA. Although dislocation is relatively easy to treat successfully, the dislocation rate of 4% is high. It is recommended that the stability of the bearing is assessed intraoperatively. If the bearing can easily be displaced, the fixed rather than the mobile bearing version of the Oxford lateral tibial component should be inserted instead. Younger age, heavier weight, high activity, and patellofemoral erosions did not detrimentally affect outcome, so should not be considered contraindications. Abstract. Introduction. Surgery in patients with high body mass index (BMI) is more technically challenging and associated with increased complications post-operatively. Inferior clinical and functional mid-term results for high BMI patients undergoing high tibial osteotomy (HTO) relative to normal weight patients have been reported. This study discusses the clinical, radiological and functional outcomes of HTO surgery in patients with a high BMI. Method. This is a retrospective study on patients undergoing HTO surgery using the Tomofix anatomical MHT plate between 2017 and 2022, with follow-up period of up to 5 years. The cohort was divided: non-obese (BMI <30 kg/m2) and obese (BMI>30 kg/m2). Pre and post operative functional scores were collected: Oxford Knee Score (OKS), EuroQol-5D and Tegner. Complications, plate survivorship and Mikulicz point recorded. Results. 32 HTO procedures; 19 patients BMI <30 (average 27.0) and 14 patients BMI >30 (average 36.1). In BMI<30 cohort, one readmission for investigation of venous thromboembolism, doppler negative; two complications: hinge fracture and stitch abscess. The five year survivorship of the plate was 100%. In BMI>30 cohort, one readmission for pulmonary embolism; one complication: hinge fracture. The 5 year survivorship of the plate was 93%, 1 conversion to unicompartmental knee replacement. The average OKS improvement was 17 and 18 for BMI <30 and >30 respectively. Mikulicz point change was identical. Conclusion. The Tomofix anatomical MHT plate achieves good outcomes and minimal complications irrespective of BMI. Reduced plate survivorship, thus earlier conversion may be required in the obese, however higher cohort numbers are needed to confirm this

Abstract. Introduction. Total knee replacement (TKR) in patients with skeletal dysplasia is technically challenging surgery due to deformity, joint contracture, and associated co-morbidities. The aim of this study is to follow up patients with skeletal dysplasia following a TKR. Methodology. We retrospectively reviewed 22 patients with skeletal dysplasia who underwent 31 TKRs at our institution between 2006 and 2022. Clinical notes, operative records and radiographic data were reviewed. Results. Achondroplasia was the most common skeletal dysplasia (8), followed by Chondrodysplasia punctata (7) and Spondyloepiphyseal dysplasia (5). There were fourteen men and eight women with mean age of 51 years (28 to 73). The average height of patients was 1.4 metres (1.16–1.75) and the mean weight was 64.8 Kg (34.3–100). The mean follow up duration was 68.32 months (1–161). Three patients died during follow up. Custom implants were required in twelve patients (38.71%). Custom jigs were utilised in six patients and two patients underwent robotic assisted surgery. Hinged TKR was used in seventeen patients (54.84%), posterior stabilised TKR in nine patients (29.03%), and cruciate retaining TKR in five patients (16.13%). One patient underwent a patella resurfacing for persistent anterior knee pain and another had an intra-operative medial tibial plateau fracture which was managed with fixation. No revisions occurred during the follow up period. Conclusion. Despite the technical challenges and complexity of TKR within this unique patient group, we demonstrate good implant survivorship during the study period. Cross sectional imaging is recommended preoperatively for precise planning and templating

Abstract. Introduction. Coronal plane alignment of the knee (CPAK) classification utilises the native arithmetic hip-knee alignment to calculate the constitutional limb alignment and joint line obliquity which is important in pre-operative planning. The objective of this study was to compare the accuracy and reproducibility of measuring the lower limb constitutional alignment with the traditional long leg radiographs versus computed tomography (CT) used for pre-operative planning in robotic-arm assisted TKA. Methods. Digital long leg radiographs and pre-operative CT scan plans of 42 patients (46 knees) with osteoarthritis undergoing robotic-arm assisted total knee replacement were analysed. The constitutional alignment was established by measuring the medial proximal tibial angle (mPTA), lateral distal femoral angle (LDFA), weight bearing hip knee alignment (WBHKA), arithmetic hip knee alignment (aHKA) and joint line obliquity (JLO). Furthermore, the Coronal Plane Alignment of the Knee (CPAK) classification was utilised to classify the patients based on their coronal knee alignment phenotype. Results. Mean age of the patients was 66 years (SD 9) and mean BMI 31.2 (SD 3.9). There were 27 left and 19 right sided surgeries. The Pearson's corelation coefficient was 0.722 (p=0.008) for WBHKA; 0.729 (p<0.001) for MPTA; 0.618 (p=0.14) for aHKA; 0.502 (p= 0.04) for LDFA and 0.305 (p=0.234) for JLO. CPAK classification was concordant for 53% study participants between the two groups. Conclusion. Three-dimensional CT-based modelling with computer software more accurately predicts constitutional limb alignment and JLO as defined by the CPAK classification compared to plain long-leg radiographs in pre-operative planning of total knee arthroplasty


Abstract. Objectives. To determine the effectiveness of LIA compared to ACB in providing pain relief and reducing opiates usage in hamstring graft ACL reconstructions. Materials and Methods. In a consecutive series of hamstring graft ACL reconstructions, patients received three different regional and/or anaesthetic techniques for pain relief. Three groups were studied: group 1: general anaesthetic (GA)+ ACB (n=38); group 2: GA + ACB + LIA (n=31) and group 3: GA+LIA (n=36). ACB was given under ultrasound guidance. LIA involved infiltration at skin incision site, capsule, periosteum and in the hamstring harvest tunnel. Analgesic medications were similar between the three groups as per standard multimodal analgesia (MMA). Patients were similar in demographics distribution and surgical technique. The postoperative pain and total morphine requirements were evaluated and recorded. The postoperative pain was assessed using the visual analogue scores (VAS) at 0hrs, 2hrs, 4hrs, weight bearing (WB) and discharge (DC). Results. There was no statistically significant difference in opiates intake amongst the three groups. When comparing VAS scores; there were no statistical difference between the groups at any of the time intervals that VAS was measured. However, the GA+LIA group hospital's LOS (m=2.31hrs, SD=0.75) was almost half that of GA+ACB group (m=4.24hrs, SD=1.08); (conditions t(72)=8.88; p=0.000). There was no statistical significance in the incidence of adverse effects amongst the groups. Conclusion. The LIA technique provided equally good pain relief following hamstring graft ACL reconstructions when compared to ACB, while allowing for earlier rehabilitation, mobilisation and discharge

Aims. Rates of readmission and reoperation following primary total knee arthroplasty (TKA) are under scrutiny due to new payment models, which penalize these negative outcomes. Some risk factors are more modifiable than others, and some conditions considered modifiable such as obesity may not be as modifiable in the setting of advanced arthritis as many propose. We sought to determine whether controlling for hypoalbuminaemia would mitigate the effect that prior authors had identified in patients with obesity. Methods. We reviewed the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database for the period of January 2008 to December 2016 to evaluate the rates of reoperation and readmission within 30 days following primary TKA. Multivariate logistic regression modelling controlled for preoperative albumin, age, sex, and comorbidity status. Results. Readmission rates only differed significantly between patients with Normal Weight and Obesity Class II, with a decreased rate of readmission in this group (odds ratio (OR) 0.82; 95% confidence interval (CI) 0.71 to 0.96; p = 0.010). The only group demonstrating association with increased risk of reoperation within 30 days was the Obesity Class III group (OR 1.38; 95% CI 1.05 to 1.82; p = 0.022). Hypoalbuminaemia (preoperative albumin < 35 g/L) was significantly associated with readmission (OR 1.62; 95% CI 1.41 to 1.86; p < 0.001) and reoperation (OR 1.52; 95% CI 1.18 to 1.96; p = 0.001) within 30 days. Conclusion. In this study, hypoalbuminaemia appears to be a more significant risk factor for readmission and reoperation than even the highest obesity categories. Future studies may assess whether preoperative albumin restoration or weight loss may improve outcomes for patients with hypoalbuminaemia. The implications of this study may allow surgeons to discuss risk of surgery with obese patients planning to undergo primary TKA procedures if other comorbidities are adequately controlled. Aims. The aim of this study was to screen the entire genome for genetic markers associated with risk for anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) injury. Methods. Genome-wide association (GWA) analyses were performed using data from the Kaiser Permanente Research Board (KPRB) and the UK Biobank. ACL and PCL injury cases were identified based on electronic health records from KPRB and the UK Biobank. GWA analyses from both cohorts were tested for ACL and PCL injury using a logistic regression model adjusting for sex, height, weight, age at enrolment, and race/ethnicity using allele counts for single nucleotide polymorphisms (SNPs). The data from the two GWA studies were combined in a meta-analysis. Candidate genes previously reported to show an association with ACL injury in athletes were also tested for association from the meta-analysis data from the KPRB and the UK Biobank GWA studies. Results. There was a total of 2,214 cases of ACL and PCL injury and 519,869 controls within the two cohorts, with three loci demonstrating a genome-wide significant association in the meta-analysis: INHBA, AEBP2, and LOC101927869. Of the eight candidate genes previously studied in the literature, six were present in the current dataset, and only COL3A1 (rs1800255) showed a significant association (p = 0.006). Conclusion. Genetic markers in three novel loci in this study and one previously-studied candidate gene were identified as potential risk factors for ACL and PCL injury and deserve further validation and investigation of molecular mechanisms. Aims. This study compared the cobalt and chromium serum ion concentration of patients treated with two different metal-on-metal (MoM) hinged total knee arthroplasty (TKA) systems, as well as a titanium nitride (TiN)-coated variant. Methods. A total of 63 patients (65 implants) were treated using either a MoM-coated (n = 29) or TiN-coated (n = 7) hinged TKA (GenuX mobile bearing, MUTARS; Implantcast, Germany) versus the BPKS (Brehm, Germany) hinged TKA (n = 27), in which the weight placed on the MoM hinge is diffused through a polyethylene (PE) inlay, reducing the direct load on the MoM hinge. Serum cobalt and chromium ion concentrations were assessed after minimum follow-up of 12 months, as well as functional outcome and quality of life. Results. No differences in mean age (69 years, 40 to 86), mean age adapted Charlson Comorbidity Index (3.1 (SD 1.4)), mean BMI (29.2 kg/m. 2. (SD 5.8)), or number of other implants were observed between groups. Significant improvements in outcome scores and pain levels were achieved for all groups, and there was no difference in quality of life (12-Item Short-Form Health Survey questionnaire (SF-12)). Mean cobalt and chromium ion levels were significantly higher for the GenuX versus the BPKS hinged TKA (GenuX vs BPKS: cobalt: 16.3 vs 9.4 µg/l; chromium: 9.5 vs 5.2 µg/l). The TiN-coated implants did not appear to confer improvement in the metal ion levels. Metal ion concentrations above 7 µg/l were detected in 81%(29/36) of GenuX patients versus 41% (11/27) in the BPKS group. No GenuX patients had normal levels under 2 µg/l, versus 22% of BPKS patients. No significant reduction in outcome scores was observed regardless of the metal ion levels, whereas higher work-related activity was correlated with higher chromium concentrations. Conclusion. Hinged TKA, using MoM hinges, resulted in critically high cobalt and chromium ion concentrations. The BPKS hinged TKA showed significantly lower metal ion concentrations compared with the GenuX TKA. No benefits were observed using TiN coating. The different weightbearing mechanics might influence the wear of the component materials. Introduction. Rates of readmission and reoperation following primary total knee arthroplasty (TKA) are under scrutiny due to new payment models, which penalize these negative outcomes. Many prior studies have demonstrated the risk of perioperative complications among patients with obesity. However, an elevated complication rate among patients with poor nutrition, as measured by hypoalbuminemia, has also been reported. We sought to determine whether controlling for hypoalbuminemia would mitigate the effect that prior authors had identified in patients with obesity. In addition, we hoped to identify an albumin threshold above which risk of readmission and reoperation would be minimized. Materials and Methods. We reviewed the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database for the period of 2008–2016 to evaluate the rates of perioperative complications among primary TKA patients by obesity category and albumin level. Patients were divided into groups according to World Health Organization obesity category and continuous as well as dichotomous albumin level. The primary outcomes were readmission and reoperation within 30 days of surgery. Patients without BMI or albumin data were excluded, as were patients undergoing surgery for the treatment of fracture or bone tumor. Univariate logistic regression compared predictive value of body mass index and albumin on incidence of these outcomes, using the normal weight and albumin > 3.5 g/dL groups as controls. Multivariate logistic regression modeling controlled for age, gender, and comorbidity status. Receiver operator curves (ROC) were generated to determine if an albumin threshold could be determined, above which risk for these complications would be minimized. Results. Outcome data was available 108,601 patients. Overall, 3,508 patients (3.46%) were readmitted and 1,219 patients (1.19%) underwent reoperation within 30 days. Hypoalbuminemia was present in 4,327 patients (4.11%), lowest in the Overweight group (3.16%) and highest in the Obese Type III (6.72%) and Underweight (12.62%) groups (p <0.001). Readmission and reoperation rates were highest among patients categorized as Obesity Class III (4.15 and 1.73%, respectively) compared with Normal Weight (3.59 and 1.14%, respectively, p <0.001). Readmission and reoperation rates were higher among patients with hypoalbuminemia (6.10 and 1.96%, respectively) compared with normal albumin (3.34 and 1.16%, respectively, p <0.001). After controlling for covariates of interest using multivariate regression including continuous albumin, odds ratio of readmission among the highest obesity category no longer differed from the Normal Weight control group. Odds ratio of reoperation among the highest obesity category declined from 1.52 to 1.38 after controlling for albumin and other covariates (p = 0.022). A subanalysis was performed excluding all patients with albumin < 3.5 g/dL, but results did not differ significantly from the multivariate model. ROC modeling was unable to identify a threshold for continuous BMI or albumin that would adequately mitigate risk for readmission or reoperation. Discussion. Albumin and obesity remain significant independent risk factors for reoperation following primary TKA procedures. However, after controlling for hypoalbuminemia and other important covariates, readmission rates did not significantly differ between patients with Normal Weight and any other obesity category. Controlling for albumin alone does not mitigate all risk imposed by obesity on perioperative outcomes. Future studies may assess whether preoperative albumin repletion or weight loss may improve outcomes for patients with hypoalbuminemia and obesity. Introduction. Overwhelming evidence has established obesity as a risk factor for osteoarthritis (OA) of the knee. Randomized clinical trials such as the Look AHEAD study have shown long term successful intentional weight loss with an intensive lifestyle intervention (ILI) in overweight and obese type 2 diabetics. Weight loss can also decrease knee pain in persons who have OA, but it is unknown if intentional weight loss can reduce the risk of TKR. To answer this question, data from the Look AHEAD study were examined to determine if intentional weight loss could reduce the risk of TKR. Methods. Look AHEAD is a multicenter, randomized trial which began in August 2001 and follow-up continued for a median of 11.3 years at 16 academic centers. 5145 persons aged 45–76 with diabetes were randomized to either an ILI with reduced calorie consumption and increased physical activity designed or to diabetes support and education intervention (DSE). TKR events were ascertained every 6 months. Retrospective data of reported knee pain was assessed using the WOMAC knee pain questionnaire. Participants with partial TKR or revisions were excluded. Cox proportional hazard models were used to relate baseline BMI category (obese, Class I, Class II, or Class III obesity), baseline knee pain, and treatment group with TKR. Weight change category (lost<=5%, stable, gained>=2%) from baseline to year 1 follow-up by treatment assignment was also examined as a predictor of TKR after excluding TKR occurring prior to year 1. Results. Out of the randomized participants, 2171 reported knee pain (43%) at the baseline visit (p=0.81). WOMAC knee pain score did not differ by random assignment (ILI:3.6±2.9, DSE:3.9±3.0, p=0.08). During follow up there were 631 TKRs reported by participants. TKR was more common in heavier (p<0.001), and older (p<0.001) participants and did not differ by randomization. Heterogeneity of treatment effect was observed with baseline knee pain (interaction p = 0.02), therefore analyses were stratified by presence or absence of knee pain at baseline. In persons without knee pain at baseline, there was a 29% reduced rate of TKR in the ILI group compared to the DSE group (HR[95%CI] 0.71[0.52,0.96]; Figure 1A). Whereas in persons with knee pain at baseline, there was a trend for the ILI to have an increased rate of TKR compared to DSE (1.11[0.92, 1.33]; Figure 1B). In both sets of analyses, obese participants had significantly higher hazard of TKR than overweight participants (No pain: Class I 1.78[1.04,3.05], Class II 2.27 [1.31,3.94], Class III 2.94[1.67,5.18]; With pain: Class I 1.70[1.12,2.59], Class II 2.42 [1.60,3.65], Class III 2.80[1.85,4.23]). When TKR incidence was examined by weight change at year 1 there was no difference in weight change category between randomization groups in persons with knee pain (interaction p=0.20) or without baseline knee pain (interaction p=0.87) (Figure 1C and D) or for the overall effect of year 1 weight loss category in either stratum (with knee pain p=0.26, without knee pain p=0.48). However, in persons without knee pain at baseline, ILI had a marginal reduction in hazard of TKR compared to DSE (0.71[0.49, 1.05]) but not in the group with baseline knee pain. Conclusions. In persons with no knee pain at baseline and who were overweight or had Class I or II obesity, ILI seemed to reduce the risk of TKR compared to DSE. In contrast, persons with knee pain who gained weight at 1 year and were randomized to ILI had the highest risk of TKR. This suggests that weight loss to prevent TKR may be more effective prior to the development of significant knee pain. After the onset of knee pain however, low impact or non-weight bearing activity should be considered to avoid worsening knee symptoms. For any figures or tables, please contact authors directly

Aims. We propose a state-of-the-art temporary spacer, consisting of a cobalt-chrome (CoCr) femoral component and a gentamicin-eluting ultra-high molecular weight polyethylene (UHMWPE) tibial insert, which can provide therapeutic delivery of gentamicin, while retaining excellent mechanical properties. The proposed implant is designed to replace conventional spacers made from bone cement. Methods. Gentamicin-loaded UHMWPE was prepared using phase-separated compression moulding, and its drug elution kinetics, antibacterial, mechanical, and wear properties were compared with those of conventional gentamicin-loaded bone cement. Results. Gentamicin-loaded UHMWPE tibial components not only eradicated planktonic Staphylococcus aureus, but also prevented colonization of both femoral and tibial components. The proposed spacer possesses far superior mechanical and wear properties when compared with conventional bone cement spacers. Conclusion. The proposed gentamicin-eluting UHMWPE spacer can provide antibacterial efficacy comparable with currently used bone cement spacers, while overcoming their drawbacks. The novel spacer proposed here has the potential to drastically reduce complications associated with currently used bone cement spacers and substantially improve patients’ quality of life during the treatment. Aims

The goal was to evaluate tibiofemoral knee joint kinematics during stair descent, by simulating the full stair descent motion in vitro. The knee joint kinematics were evaluated for two types of knee implants: bi-cruciate retaining and bi-cruciate stabilized. It was hypothesized that the bi-cruciate retaining implant better approximates native kinematics.

Methods

The in vitro study included 20 specimens which were tested during a full stair descent with physiological muscle forces in a dynamic knee rig. Laxity envelopes were measured by applying external loading conditions in varus/valgus and internal/external direction.

Aims. The primary objective of this study was to compare migration of the cemented ATTUNE fixed bearing cruciate retaining tibial component with the cemented Press-Fit Condylar (PFC)-sigma fixed bearing cruciate retaining tibial component. The secondary objectives included comparing clinical and radiological outcomes and Patient Reported Outcome Measures (PROMs). Methods. A single blinded randomized, non-inferiority study was conducted including 74 patients. Radiostereometry examinations were made after weight bearing, but before hospital discharge, and at three, six, 12, and 24 months postoperatively. PROMS were collected preoperatively and at three, six, 12, and 24 months postoperatively. Radiographs for measuring radiolucencies were collected at two weeks and two years postoperatively. Results. The overall migration (mean maximum total point motion (MPTM)) at two years was comparable: mean 1.13 mm (95% confidence interval (CI), 0.97 to 1.30) for the ATTUNE and 1.16 mm (95% CI, 0.99 to 1.35) for the PFC-sigma. At two years, the mean backward tilting was -0.43° (95% CI, -0.65 to -0.21) for the ATTUNE and 0.08° (95% CI -0.16 to 0.31), for the PFC-sigma. Overall migration between the first and second postoperative year was negligible for both components. The clinical outcomes and PROMs improved compared with preoperative scores and were not different between groups. Radiolucencies at the implant-cement interface were mainly seen below the medial baseplate: 17% in the ATTUNE and 3% in the PFC-sigma at two weeks, and at two years 42% and 9% respectively (p = 0.001). Conclusion. In the first two postoperative years the initial version of the ATTUNE tibial component was not inferior with respect to overall migration, although it showed relatively more backwards tilting and radiolucent lines at the implant-cement interface than the PFC-sigma. The version of the ATTUNE tibial component examined in this study has subsequently undergone modification by the manufacturer. Level of Evidence: 1 (randomized controlled clinical trial).
